STRUCTURE OF ACUTE RESPIRATORY VIRAL INFECTIONS AMONG HOSPITALIZED CHILDREN OF KHABAROVSK IN EPIDEMIC SEASON OF 2015-2016

The research presents results of the analysis by polymerase chain reaction of clinical material obtained from 170 children hospitalized into an infectious department of the Khabarovsk children’s territorial clinical hospital named after A.K.Piotrovich with a diagnosis of acute viral infection in epidemic season of 2015-2016. The most numerous group consisted of children under 1st year of age – 74 children (43.5%). The overall number of detected respiratory viruses was equal to 179. Most prevalent viruses were human rhinoviruses (25.1%), human adenoviruses (19.6%), and human parainfluenza viruses (16.8%). Among influenza viruses (11.7%) type A(H1N1) pdm09 was predominant. In 92 children (54.1%) the disease was caused by a mono-infection, in 40 cases (23.5%) there was identified mixed-infection. Infections of lower respiratory tract were identified in 100 patients (58.8%), stenosing laryngotracheitis was diagnosed most frequently (20.6%).

SUMMARYHuman bocavirus (HBoV) was recently identified in respiratory samples from patients with acute respiratory infections and has been reported in different regions of the world. To the best of our knowledge, HBoV has never been reported in respiratory infections in Brazil. Nasopharyngeal aspirates were collected from patients aged <5 years hospitalized in 2005 with respiratory infections in Ribeirão Preto, southeast Brazil, and tested by polymerase chain reaction (PCR) for HBoV. HBoV-positive samples were further tested by PCR for human respiratory syncytial virus, human metapneumovirus, human coronaviruses 229E and OC43, human influenza viruses A and B, human parainfluenza viruses 1, 2 and 3, human rhinovirus and human adenovirus. HBoV was detected in 26/248 (10·5%) children of which 21 (81%) also tested positive for other respiratory viruses. Despite the high rates of co-infections, no significant differences were found between HBoV-positive patients with and without co-infections with regard to symptoms.

Influenza viruses cause significant morbidity and mortality in adults each winter. At the same time, other respiratory viruses circulate and cause respiratory illness with influenza-like symptoms. Human respiratory syncytial virus (HRSV), human parainfluenza viruses (HPIV) and human metapneumovirus have all been associated with morbidity and mortality in adults, including nosocomial infections. This study evaluated 154 respiratory specimens collected from adults with influenza-like/acute respiratory illness (ILI) seen at the Edward Hines Jr VA Hospital, Hines, IL, USA, during two successive winters, 1998–1999 and 1999–2000. The samples were tested for ten viruses in two nested multiplex RT-PCRs. One to three respiratory viruses were detected in 68 % of the samples. As expected, influenza A virus (FLU-A) infections were most common (50 % of the samples), followed by HRSV-A (16 %). Surprisingly, HPIV-4 infections (5.8 %) were the third most prevalent. Mixed infections were also relatively common (11 %). When present, HPIV infections were approximately three times more likely to be included in a mixed infection than FLU-A or HRSV. Mixed infections and HPIV-4 are likely to be missed using rapid diagnostic tests. This study confirms that ILI in adults and the elderly can be caused by HRSV and HPIVs, including HPIV-4, which co-circulate with FLU-A.

Influenza and other acute respiratory viral infections are the most common contemporary infectious diseases resulting in prominent harm to human health and great economic damage. At least five groups of viruses including more than 300 subtypes are currently referred to ARVI pathogens. Such infectious agents are characterized by variability resulting in their altered antigenic characteristics, increased contagiousness, "evasion from immune response and resistance to antivirals. Relevance of influenza and other ARVIs is also accounted for by rapid development of bacteria-associated respiratory diseases. Continuous variability of influenza viruses and emergence of new ARVI pathogens pose a serious threat. In recent years, a simultaneous circulation of subtype A (H1N1) and A (H3N2) influenza viruses with a predominance of a pandemic strain as well as type B viruses have been observed. Among the causative agents of non-influenza ARVIs, respiratory syncytial virus, rhino- and adenoviruses, and I/III parainfluenza viruses are recorded most often. Here we present the data of virology and serological examination of clinical samples collected during the 2018 – 2019 epidemic season in the Republic of Kazakhstan. For this, 2794 clinical samples (2530 nasopharyngeal swabs and 264 blood serums) were collected from patients diagnosed with ARVI, ARI, bronchitis, and pneumonia. Analysis of nasopharyngeal swabs for detection of influenza by RT-PCR demonstrated that mixed etiology influenza viruses with predominance of A/H1N1pdm virus circulated in Kazakhstan. The genetic fingerprints of influenza virus were found in 511 swabs (20.20% of total examined samples). Influenza A virus RNA was detected in 508 biological samples: A/H1N1 – in 289, A/H3N2 – in 209, and unidentified virus subtype in 10 samples. Type B influenza virus was detected in 3 samples. Study of 264 serum samples by HAI assay and ELISA showed emergence of antibodies against influenza A/H1N1, A/H3N2, and B viruses in residents from various regions of Kazakhstan that indirectly confirmed co-circulation of these viruses. 42 influenza virus strains were isolated in chicken embryos, from which 28 were assigned to A/H1N1pdm virus, 13 to A/H3N2 virus, and one isolate was identified as influenza B virus. Laboratory diagnostics of clinical samples for ARVIs established that among identified non-influenza agents respiratory syncytial virus dominated, while rhinoviruses and adenoviruses were less common. Metapneumoviruses, bocaviruses, coronaviruses, and type I parainfluenza viruses were detected in few cases. Comparison of study data with those obtained after examining circulation of influenza viruses during the 2017 – 2018 epidemic season showed that in 2018 – 2019 in Kazakhstan similar to the previous epidemic season, influenza A and B viruses continued to circulate, with prevalence of A/H1N1pdm virus. Identification of non-influenza viruses causing respiratory infections in 2018 – 2019 showed predominance of respiratory syncytial virus, which correlated with data on the 2017 – 2018 epidemic season.

Background Community-Acquired Pneumonia (CAP) is one of the highest health burden conditions in Australia. Disease notifications and other data from routine diagnosis suffers from selection bias that may misrepresent the true contribution of various aetiological agents. However existing Australian prospective studies of CAP aetiology have either under-represented elderly patients, not utilised Polymerase Chain Reaction (PCR) diagnostics or been limited to winter months. We therefore sought to re-evaluate CAP aetiology by systematically applying multiplex PCR in a representative cohort of mostly elderly patients hospitalised in Melbourne during non-winter months and compare diagnostic results with those obtained under usual conditions of care. Methods Seventy two CAP inpatients were prospectively enrolled over 2 ten-week blocks during non-winter months in Melbourne in 2016-17. Nasopharyngeal and oropharyngeal swabs were obtained at admission and analysed by multiplex-PCR for 7 respiratory viruses and 5 atypical bacteria. Results Median age was 74 (interquartile range 67-80) years, with 38 (52.8%) males and 34 (47.2%) females. PCR was positive in 24 (33.3%), including 12 Picornavirus (50.5% of those with a virus), 4 RSV (16.7%) and 4 influenza A (16.7%). CAP-Sym questionnaire responses were similar in those with and without viral infections. Most (80%) pathogens detected by the study, including all 8 cases of influenza and RSV, were not otherwise detected by treating clinicians during hospital admission. Conclusion One third of patients admitted with CAP during non-winter months had PCR-detectable respiratory viral infections, including many cases of influenza and RSV that were missed by existing routine clinical diagnostic processes. Equines are susceptible to respiratory viruses such as influenza and parainfluenza. Respiratory diseases have adversely impacted economies all over the world. This study was intended to determine the presence of influenza and parainfluenza viruses in unvaccinated horses from some regions of the state of São Paulo, Brazil. Blood serum collected from 72 equines of different towns in this state was tested by hemagglutination inhibition test to detect antibodies for both viruses using the corresponding antigens. About 98.6% (71) and 97.2% (70) of the equines responded with antibody protective titers (≥ 80 HIU/25µL) H7N7 and H3N8 subtypes of influenza A viruses, respectively. All horses (72) also responded with protective titers (≥ 80) HIU/25µL against the parainfluenza virus. The difference between mean antibody titers to H7N7 and H3N8 subtypes of influenza A viruses was not statistically significant (p > 0.05). The mean titers for influenza and parainfluenza viruses, on the other hand, showed a statistically significant difference (p < 0.001). These results indicate a better antibody response from equines to parainfluenza 3 virus than to the equine influenza viruses. No statistically significant differences in the responses against H7N7 and H3N8 subtypes of influenza A and parainfluenza 3 viruses were observed according to the gender (female, male) or the age (≤ 2 to 20 years-old) groups. This study provides evidence of the concomitant presence of two subtypes of the equine influenza A (H7N7 and H3N8) viruses and the parainfluenza 3 virus in equines in Brazil. Thus, it is advisable to vaccinate equines against these respiratory viruses.

Background: Influenza virus and other respiratory viruses have been identified as an essential cause of acute respiratory infections (ARIs) in children worldwide. However, there are few data on its frequency and clinical presentation in Jordan. Objectives: We aimed to identify the viral etiology of acute respiratory infections and the various clinical presentations in hospitalized children, especially those with influenza viruses compared to other respiratory viruses. Methods: A retrospective study that was conducted at the Jordan university hospital. All the positive nasopharyngeal aspirates that were collected from hospitalized children aged 0-19 years from January 2017 to January 2019 were reviewed. Results: A total of 338 nasopharyngeal aspirates (NPAs) with positive viral serology results were reviewed. Among the patients younger than four years, the RSV virus was the most frequently detected. However, the Influenza B virus was the most commonly seen in patients older than 5 years, H1N1 was more frequent in autumn (29.5%), and RSV was the most frequent virus in winter. Bronchopneumonia was the most frequent diagnosis among all hospitalized patients, followed by bronchiolitis. Out of 338 patients, 50.3 % had tachypnea, 70.7% of patients were admitted to the pediatric floor, while 18.6% presented with a severe illness and required admission to the pediatric intensive care unit (PICU). Infants under the age of one were more likely to have higher co-infection rates with other viruses compared to children over five years that had influenza. Conclusion: Presentations of influenza and other respiratory viruses vary between different age groups, such as sepsis in children younger than one year.

Viral infections in patients with post-kidney transplantation are often difficult to diagnose as well as treat. We herein report three cases with severe viral infections after kidney transplantation. All their causative pathogens could be detected promptly by polymerase chain reaction and flow cytometry during the early stages of infection. These examinations would also be of great use to monitor therapeutic responses and disease activity. It is indeed true that no specific treatment is available for most of the viral infections, but we should be aware that some infections, such as Epstein-Barr virus infection, can be treatable with prompt and specific treatment, such as rituximab.
