Women's views on the visiting restrictions during COVID-19 in an Irish maternity hospital

Background Due to the coronavirus infection, visitors to all hospitals were greatly restricted in the UK. In maternity hospitals, only partners of women in labour were permitted to attend the hospital. Aims This study aimed to gain an understanding of women's experiences of visiting restrictions imposed due to COVID-19. Methods Women who attended the hospital for outpatient appointments and who were inpatients on the antenatal or postnatal ward during a two-week period were asked to complete an anonymous survey. Findings A total of 422 surveys were completed. The majority of women (97.6%) agreed that the hospital made adequate preparations for them to feel safe. Most women reported that the restrictions are a good thing and several advantages were identified. Women cited not having their partner with them as the main negative consequence to the restrictions. Conclusions Although women miss having their partner for support during scans and to help after the baby is born, during the COVID-19 pandemic, the safety aspect of the restrictions and the support received from staff is considered by women when making recommendations to a maternity hospital about whether, or how, to ease restrictions on visiting.

Повышение качества оказания помощи больным новой коронавирусной инфекцией COVID-19 возможно только на основе мультидисциплинарного подхода. Оценка пищевого статуса и при необходимости его коррекция являются важной составляющей комплексных программ лечения и реабилитации таких больных. Значительное число пациентов с COVID-19 составляют люди пожилого и старческого возраста, у которых уже есть признаки нутритивной недостаточности. Нарушения питания и сопутствующая им саркопения – факторы риска неблагоприятного течения COVID-19. Прогрессирование саркопении при COVID-19 объясняют возникновением вирусного миозита, миопатией, спровоцированной цитокинами, и иммобилизацией. Уменьшение мышечной массы ассоциировано с плохим прогнозом заболевания и снижением качества жизни. Коррекция нарушений пищевого статуса больных COVID-19 должна проводиться на всех этапах лечения. Важность проблемы нутритивной поддержки отражена во множестве публикаций, посвященных лечению больных COVID-19. Необходимо заметить, что Европейское общество клинического питания и метаболизма (European Society for Clinical Nutrition and Metabolism, ESPEN) в кратчайшие сроки выпустило рекомендации по нутритивной поддержке больных новой коронавирусной инфекцией, что лишний раз подчеркивает актуальность проблемы. Это стало логичным продолжением рекомендаций по коррекции пищевого статуса больных, находящихся на лечении в отделении реанимации и интенсивной терапии. Аналогичные клинические рекомендации были разработаны в Великобритании, Бразилии и целом ряде других стран. Использование препаратов для перорального дополнительного питания в большинстве случаев позволяет обеспечить потребности пациентов в необходимых нутриентах, уменьшить выраженность саркопении и повысить эффективность реабилитационных мероприятий. Improving the quality of care for patients with the new coronavirus infection COVID-19 is possible only on the basis of a multidisciplinary approach. Evaluation of nutritional status and, if necessary, its correction are an important component of comprehensive treatment and rehabilitation programs for such patients. A significant number of patients with COVID-19 are elderly and senile people who already have signs of nutritional deficiency. Eating disorders and accompanying sarcopenia are risk factors for the adverse course of COVID-19. The progression of sarcopenia in COVID-19 is explained by the occurrence of viral myositis, myopathy provoked by cytokines, and immobilization. Decreased muscle mass is associated with poor disease prognosis and reduced quality of life. Correction of nutritional disorders in patients with COVID-19 should be carried out at all stages of treatment. The importance of the problem of nutritional support is reflected in many publications devoted to the treatment of patients with COVID-19. It should be noted that the European Society for Clinical Nutrition and Metabolism (ESPEN) promptly issued recommendations on nutritional support for patients with new coronavirus infection, which once again emphasizes the urgency of the problem. This was a logical continuation of the recommendations for correcting the nutritional status of patients undergoing treatment in the intensive care unit. Similar clinical guidelines have been developed in the UK, Brazil and a number of other countries. The use of drugs for oral supplementary nutrition in most cases allows to meet the needs of patients for the necessary nutrients, to reduce the severity of sarcopenia and to increase the effectiveness of rehabilitation measures.

OBJECTIVE: To evaluate the active health Ombudsman service as an instrument to evaluate the quality of delivery and birth care in the Cegonha Network of the Federal District of Brazil. METHODS: This is a cross-sectional study of the telephone survey type carried out with 1,007 mothers with deliveries between October 15, 2013 and November 19, 2013 in the twelve public maternity hospitals that make up the Cegonha Network of the Federal District of Brazil. The instrument has 25 multiple choice or Likert scale questions, including sociodemographic data and acceptability evaluation in five domains: accessibility, relationship between the patient and health professionals, conditions of the structure of the service, information to the patient, and equity and opinion of the patient. We have studied qualitative or categorical variables according to the frequency and distribution of proportions. We have used the score transformed into a scale from zero to 100 for the analysis of the Likert-type scale questions. Results have been expressed as mean and standard deviation. RESULTS: Access to prenatal appointments was evaluated as good or excellent by 86.1% of the participants and laboratory tests was evaluated as good or excellent by 85.2% of them. The access to imaging tests was evaluations as good or excellent by 45.7% of the women; 79.5% of the interviewees had their delivery in the maternity hospital where they sought initial care and 18.3% received a home visit by a community health agent after discharge. Most women reported that newborns were placed skin-to-skin immediately after birth, 48.9% had a companion at the time of the delivery, 76.3% were advised about the first appointment of the newborn, and 94.8% were advised on breastfeeding in the maternity hospital. Regarding the evaluation of health professionals, 85.9% of the women considered reception and cordiality as good or excellent at the prenatal care and 94.8% considered it as good or excellent at the maternity hospital. CONCLUSIONS: The active health Ombudsman service has contributed to evaluate the quality of public management by allowing the incorporation of the perspective of users of the health service in the evaluation of the acceptability of the Cegonha Network in the Federal District of Brazil.

Background. A follow-up program for high risk infants was initiated in Alzahra Maternity Hospital in Tabriz city, Iran, in 2013. The aim of this paper is to give a brief report of the program.Material and Methods. Two groups of high risk neonates were studied. The first group comprising 509 infants received services in Alzahra Maternity Hospital implemented by the follow-up program. This included a full package for family to look after high risk infant and periodic clinical evaluation at two and four weeks after birth and then two, three, four, five, and six months later again. The second group including 131 infants in Taleqani Maternity Hospital received routine services after birth with no specific follow-up care.Results. Some anthropometric indices showed a significant improvement in the intervention hospital compared to control group. These included the following: head circumference at first and second months; weight in the first, fourth, fifth, and sixth months; and height in sixth month only. Clinical evaluation of infants showed an improvement for some of the medical conditions.Conclusion. Follow-up care program for a minimum of six months after discharge from maternity hospitals may help to avoid adverse and life threatening consequences in high risk infants.

Coronavirus infection caused by a new strain of SARS-CoV-2 virus contributed to an increase in the number of infectious patients. The Ministry of Health of the Russian Federation and Rospotrebnadzor took over the organization of work to combat the pandemic. In Moscow, medical and preventive events were organized by the Government of the capital and the Department of Health of the city. As soon as possible, it was decided to reassign medical institutions to infectious hospitals for the treatment of patients with COVID-19 or with suspicion of it. “Maternity hospital No. 8” – the branch of State Clinical Hospital named after V. P. Demikhov was redesigned as a hospital on March 13, 2020. The decision was due to the presence of isolated boxes and an intensive care unit in the institution. The task was solved in one day: the first patients with coronavirus infection were admitted to the hospital for medical care at 17:00 on March 13, 2020.

Breastfeeding is considered to be the cornerstone of child health. In Europe however, overall breastfeeding rates remain low. The present study aimed at estimating the frequency of breastfeeding in Greece during the COVID-19 pandemic period and comparing findings with the latest national study in order to identify a potential impact of the pandemic. Additionally, possible correlations of socio-cultural and demographic characteristics with breastfeeding indicators were investigated. This prospective cohort study included 847 women from five tertiary maternity hospitals, between January and December 2020. Data were collected by a structured questionnaire via interview during hospitalization and via telephone in the first, third and sixth month postpartum. Results showed that all breastfeeding indicators improved over the last three years. Full breastfeeding reached 7.2%, contrary to 0.78% of the latest national study at six months postpartum. Employment, marital status, educational level, mode of delivery, type of maternity hospital, body mass index before pregnancy, previous breastfeeding experience of the mother and infant’s birth weight correlated significantly with breastfeeding indicators at different time periods. The COVID-19 pandemic seems to have favorably influenced breastfeeding initiation and duration in Greece due to lockdowns, home confinement and teleworking.

The research tasks are: a) to study the situation of water consumption in Russia; b) to review the experience of water resources consumption in the UK during the pandemic; c) to analyze the experience of environmental regulation in the countries. The relevance of the proposed topic is due to the present changes in human life due to quarantine restrictions.

Women’s experiences of childbirth have a significant impact on mother and child health and well-being as well as quality of care. A valid and reliable tool is needed to assess women's experience. This study aim was to adapt the Childbirth Experience Questionnaire to the Mongolian context and assess its psychometric properties. The study conducted between October and January 2019 at the public maternity hospital in Ulaanbaatar by recruiting a total of 828 low-risk postpartum women. The response rate was 92% (n = 761). Confirmatory factor analysis indicated a good fit for the 4-factor model. Reliability was good for the overall instrument (Cronbach's alpha = 0.83) and for individual domains (Cronbach's alphas ranging from 0.45 to 0.80). The overall and domain scores were significantly higher among women who had previous birth experience, who did not receive oxytocin augmentation, and who experienced a shorter labor duration (<12 h).The Mongolian version of the Childbirth Experience Questionnaire is a valid and reliable tool for exploring the childbirth experience. It can be used to promote women-centered, respectful care in maternity hospitals.

Obstetrician-gynaecologist (Ob-Gyn) medical and nursing personnel usually have minimal in action training on infection control (IC). Limited information exists also about the epidemiology of healthcare-associated infections (HAIs) in maternity hospitals. The aim of this study was to determine in a 30-day survey prevalence of HAIs among hospitalised patients and neonates in a maternity hospital in Athens, Greece, and to offer to the IC office a practical IC manual. Patients hospitalized for more than 24 h in the clinics and the neonatal intensive care unit (NICU) were enrolled. An IC guide was created and distributed to the medical and nursing staff through educational seminars. Through the survey, among Ob-Gyn patients 16 HAIs were recognized during hospitalization and 14 HAIs after patients’ discharge; the overall prevalence of infected patients was 2.9% and the prevalence of HAIs was 3.2%. Among NICU patients, the prevalence of HAIs was 3.9%. The IC manual was found easily implemented in daily use improving staff’s compliance to IC practices. The results of the survey can be used as a baseline for future comparisons between maternity hospitals (benchmarking). The implementation of steady IC guideline protocols for a maternity hospital may update staff education and promote staff compliance on IC practices.

Background: Management capability index presents the management assessment of any organizations. Therefore, we aimed to compare Mongolian maternity hospitals with the ones that have and have not implemented the quality management system.   Methods: This study was performed at the three main maternity hospitals, in Mongolia, between July 2019 and September 2019 employing the cross-sectional study method. The study involved 480 employees. We used 9 chapters and 90 criteria that were used in over 30 Mongolian Governmental Organizations for capability assessment to determine management capability index of Maternity Hospitals. The organizational management capability was 71.8, 73.6 and 93 at Urguu Maternity Hospital, Khuree Maternity Hospital and Amgalan Maternity Hospital, respectively. It is obvious that there is a need to improve organizational knowledge, innovation, resource utilization, behavior, culture and activate their organization. In the results, there is a positive correlation between organizational capability and employee’s satisfaction.  Conclusion: Employee’s satisfaction increases when organizational management capability improves.
