Survey of ultrasound-guided peripheral intravenous practices: a report of supply usage and variability between clinical roles and departments

Background The purpose of this study was to investigate ultrasound-guided peripheral intravenous (UGPIV) supply usage practices by clinicians working in vascular access, in emergency departments (EDs), or in other roles. Methodology In 2019, a voluntary cross-sectional descriptive survey was conducted via SurveyMonkey. Data collected included demographics, practice-oriented information, procedural activities, and supplies used for UGPIV insertions. Frequency distributions and results of Fisher's exact test and one-way analysis of variance were reported using R v.3.5.2. Results A total of 26,649 surveys were distributed with a response rate of 5.5% (n = 1475). Forty-eight percent of respondents (n = 709) indicated that they worked in a vascular access role, 310 (21%) worked in an ED, and 455 (31%) categorized their role as other. Clinically meaningful differences existed in all variables for UGPIV procedures and supplies between departments (P < 0.0001) and in all care settings. Using an investigatorconstructed overall metric of supplies used, important differences were demonstrated between personnel supply use in vascular access roles and other roles (P < 0.0001) and personnel in EDs and other roles (P < 0.0001). Conclusions Use of supplies for UGPIV insertions varies by department. The variability in supply usage for UGPIV insertions revealed by this survey suggests a need for clinical education on guideline application and evaluation of compliance with policies to promote standardization of supplies for UGPIV insertion.

Background: Government of India is trying to popularize generic medicines, still most Registered Medical Practitioners (RMPs) doubt its quality and efficacy. We conducted a cross sectional survey to study the barriers that hinders the practice of writing generic prescriptions. The aim was to study doctor’s knowledge, beliefs and actual practices regarding generic medicines. This study is a KAP survey model (Knowledge, attitude, practices) to study beliefs, barriers, awareness and actual practices regarding use of generic medicines amongst private practioners and RMPs of tertiary care Government hospital in Nanded city.Methods: A cross sectional survey was conducted on a sample of 300 randomly selected RMPs practicing in Nanded, Maharashtra. A 26-item questionnaire was designed, validated, and data collected through personal visits. Fisher’s exact test was applied to see associations between variables using Graphpad Prism 7.Results: 234 RMPs responded to the questionnaire i.e. a response rate of 78% was achieved. 107 (45.7%) participants claimed to be actively prescribing generic medicines. 122 (52.1%) of the participants were unaware of any generic medical shop in their locality. 83 (35.5%) participants believed generics to be duplicate/ substandard. However, 16 (19.3%) of them still prescribed generics. One-third of them actually preferred its use for family.Conclusions: RMPs do not accept the use of generic medicines for concerns about its quality and efficacy. Also, there was unawareness regarding availability of generic medicines in the locality. RMPs need to be educated about manufacture, sale and quality aspects of generic medicines in India.

BackgroundSouth Africa is one of the few countries to produce emergency care graduates in Africa. However, a large number of these graduates are emigrating from Africa. Although academic success has been studied in various contexts in Africa, none have looked into the Bachelor of Emergency Medical Care (BEMC). Maximising academic success in the BEMC might ensure a sustainable skilled workforce in an area which has a shortage of skilled personnel.AimsThis study aimed to describe the socio-demographic variables of current South African BEMC students. We also aimed to describe whether any associations exist between socio-demographic variables, various racial groupings and repeating a year during the course of study.MethodsA cross-sectional descriptive survey was conducted among students enrolled for the BEMC in 2016. Continuous and categorical variables were analysed using descriptive statistics. The Fisher’s exact test and Pearson chi-square test were used to test associations between the survey variables and repeating a year.ResultsA total of 176 participants responded to the survey. Having a pre-existing emergency care qualification was associated with not repeating a year on the BEMC (p=0.02). The statistical difference between race and not repeating a year in the BEMC was not significant (p=0.07). However, when the black-African and minority cohort are grouped together, it is significant (p=0.05).ConclusionA number of socio-demographics were described in this study. Students with pre-existing emergency care qualifications were least likely to repeat a year.

The aim of the study was to investigate the organization of asthma education for children and parents in children’s wards in Norway over a ten-year period and how Ane and Bronky teaching material has been implemented. A prospective cross-sectional study using a questionnaire was conducted in 28, 22 and 29 children’s wards in 2004, 2006 and 2014. The response rate varied between 86 percent and 97 percent. Statistical analysis by means of Fisher’s exact test was used. The findings revealed that organized asthma education for children increased between 2004 and 2006, while it decreased in 2014. Dialogue and demonstration were common teaching methods. User participation in teaching sessions increased between 2004 and 2006. Only half of the wards reported that asthma education was the responsibility of management to a large/very large extent. This presents future challenges in establishing, prioritizing and ensuring stability in health education for children and their families.

The purpose of the study was to evaluate the level of job satisfaction among the nurses of Makueni District Hospital. This was necessitated by the high nurse staff turnover from the facility and a myriad of complains from various departments. The study was a cross sectional descriptive survey involving about 50 nurses that was carried out between 15 and 19<sup>th</sup> July 2013. The study employed mixed method approach. Tests of significance were through use of Chi-square, Fishers exact test and logistic regression. The key results were that overall job satisfaction was low (36%). There were significant relationships between job satisfaction and cordial relationship with the nurse manager (c<sup>2</sup> 12.131 df 4 p=0.016&lt;0.05. Logistic regression p=0.018&lt;0.05). The plans to quit the hospital and work elsewhere indicates a relationship with job satisfaction (c<sup>2</sup> 12.749 df 4 p=0.013&lt;0.05). The findings of this study suggest that there is no enabling working environment for the nurses and this could be a barrier to service delivery in the Hospital. These findings suggest that the Nurse Managers should build up effective relationship with the staff and other departments and should identify negative working conditions which affect staff and appropriately delegate authority to them.

Background: Clinical education placements provide the opportunity for students to gain practical skills and apply theoretical knowledge not otherwise available in many instances. Objectives: This research explored how undergraduate paramedic students perceive their clinical placement learning environments. Methods: A prospective cross-sectional study using a paper-based survey, the Clinical Learning Environment Inventory (CLEI), was conducted on undergraduate paramedic students studying at a large Australian university in semester 1, 2010. A total of 190 students were invited to participate in the survey. The CLEI is a standardised tool that contains 84-items and six subscales. Findings: Sixty students completed the CLEI (31% response rate). It was found that on both the actual and preferred CLEI forms, Satisfaction was found to be the most important domain having a mean score of 29.68 (SD+3.81) and 31.37 (SD+4.35) respectively.Individualisation was found to be the least important domain in both the actual and preferred form, with mean scores of 20.93 (SD+3.80) and 24.03 (SD+3.82) respectively. This indicated that paramedic students as a group sensed that theSatisfaction aspect of clinical placements are an integral aspect of their preferred clinical learning environment. Results also indicated that all six subscales were statistically different at the p<0.001 level. Conclusion: There were significant differences in the perceptions of paramedic students actual and preferred clinical learning environments. This provides educators with strategies in promoting positive learning experiences for students.

Background: Nursing educators should train nursing students to pursue physical, psychological, spiritual, and social health promotion. The purpose of this study was to explore relationships between nursing students’ meaning of life, positive beliefs, and well-being. Methods: A cross-sectional correlational study with a quantitative approach was adopted. Purposive sampling was used. A total of 170 nursing students voluntarily participated in this study. A 56-item questionnaire was used to examine nursing students’ meaning of life (1-25 items), positive beliefs (1-11 items), and well-being (1-20 items). The content validity index (CVI) of the study questionnaire was established as 0.95 by seven expert scholars. The reliability values for the three parts of the measure were as follows: meaning of life, Cronbach’s α 0.96; positive beliefs, Cronbach’s α 0.93; and well-being, Cronbach’s α 0.95. Percentages, frequencies, means, SDs, Kruskal-Wallis one-way analysis of variance by rank, Spearman’s rank correlation, one-way analysis of variance, Spearman’s rho correlation, and regression analysis were used for the data analysis. Results: Nursing students had the following mean scores: meaning of life with 4.02 (SD 0.56); positive beliefs with 3.92 (SD 0.62); and well-being with 3.95 (SD 0.57). The results indicate that for all nursing students, meaning of life was positively correlated with positive beliefs, r=0.83 (P<.01); similarly, all nursing students had positive beliefs that were positively correlated with meaning of life, r=0.83 (P<.01). In the results of the study, the nursing students’ background, meaning of life and positive beliefs explained 63% of the variance in well-being (Adjusted R2 squared =0.63, F=33.41, P<.001). Conclusions: Nursing students’ sense of meaning of life and positive beliefs may impact their well-being. Therefore, nursing educators can promote meaning of life and positive beliefs to nursing students as a way to increase their well-being for physical, psychological, spiritual, and social health promotion.

Background: A long-standing association exists between elevated triglyceride levels and coronary heart disease (CHD). High consumption of fat and carbohydrate influences enhancement of triglycerides level. Sport can reduce cholesterol and triglycerides level. Objective: This research aims to analyze relationship between carbohydrates and fat intakes with triglycerides level on female aerobic gymnasts. Method: This is an observational research using cross sectional research design which was held in Miracle Gymnasium on February17- 18th 2017. 30 subjects were selected as subject because fulfill the requirement (older than 20 years and have willingness to be a subjects). Research variable areintakes of carbohydrate, fat, and triglyceride levels. Result: Result shows 53.3% subjects have high level of carbohydrate and fat intakes followed  by 20% subjects with high level       of triglycerides. Subjects with high level of carbohydrate, fat intakes and high triglyceride level are 28%. Subjects withsufficient carbohydrate, fat intakes and high triglyceride level are 12.5%.Fisher’s exact test was done to prove hypothesis with results there is no significant relationship between carbohydrate and fat intake with triglyceride level. Conclusions: Subjects with high carbohydrate and fat intake is 53.3% and 20% subjects have high triglyceride level. Relationship between carbohydrate and fat intake with triglyceride level on female aerobic gymnasts is not significant (p>0.05).   Keywords: Intake, Carbohydrate, Fat, Aerobic Gymnasts, Triglyceride
