Erectile dysfunction (ED) is defined as the inability of a man to get and maintain an erection that is sufficient for sexual intercourse, and is a common problem. ED commonly has a profound negative impact on quality of life in the patient and his partner, which can result in changes to sexual self-confidence. This article outlines strategies for identifying and managing ED in primary care, outlining what needs to be assessed and the various treatment options available to manage the condition.

Abstract: Normal sexual function is a bio-psycho-social process which is significantly related to the quality of life (QOL). Any disturbance in one of the components can be a disaster in sexual life.  Erectile dysfunction is a disability to get an erection or to maintain the erection well enough for a sexual intercourse, which persists or recurs at least three months conse-cutively due to either psychogenic or organic disturbances, or both. Some psychogenic dis-turbances especially depression, anxiety, and relational distress, play some important roles in ED. Besides that, aging, low testoteron level, physical diseases, and some certain medicines have to be included. Key words: erection, erectile dysfunction, sexual stimulus, depression.     Abstract: Fungsi seksual yang normal merupakan suatu proses biopsikososial yang berkaitan langsung dengan kualitas hidup. Adanya gangguan pada salah satu komponen dapat menjadi malapetaka bagi kehidupan seksual. Disfungsi ereksi dapat dedefinisikan sebagai suatu keti-dakmampuan untuk ereksi atau mempertahankan ereksi yang cukup untuk melakukan hu-bungan seksual yang memuaskan, yang menetap atau berulang paling tidak selama tiga bulan berturut-turut. Gangguan psikogenik khususnya sindroma depresi, ankietas, dan distres relasional berperan penting dalam hal terjadinya disfungsi ereksi. Disamping itu usia lanjut, rendahnya kadar testoteron, penyakit fisik, dan beberapa jenis obat perlu diperhitungkan. Kata kunci: fisiologi ereksi, disfungsi ereksi, stimulus seksual, depresi.

Abstract Background: Erectile dysfunction (ED) has a negative impact on ones’ relationships with poor quality of life as inevitable result. Men’s sexual health has been forgotten in most developing countries and therefore the burden of ED and associated risk factors are not known in these settings. This study aimed to determine the prevalence and factors associated with erectile dysfunction among adult men in Moshi municipality, northern Tanzania. Methodology: A community based cross-sectional study was conducted in July 2019. Multi stage sampling technique was used to enroll men aged 18 years and above infour wards of Moshi municipality. The 5-item version of the International Index of Erectile Function (IIEF-5) scale was used to assess erectile dysfunction. Multivariate logistic regression was done to get factors associated with ED. Results: The mean age of the 381 men was 39.6 (SD ±16.8) years. Theoverall prevalence of ED on this study was 29.7%. The severity of ED among study participants was 13.4 % (51), 9.7 % (37), 3.7 % (14), 2.9% (11) participants had mild, mild to moderate, moderate and severe erectile dysfunction respectively. Age, tobacco use, overweight, hypertension and diabetes all showed significance association with ED. However, in multivariate logistic analysis only age ≥40 years and hypertension remain statistically significant associated with ED [(OR 5.2, 95% CI 2.68-10.21, P<0.001), (OR 11.5, 95% CI 5.8-22.76, P<0.001) and (OR 2.5, 95% CI 1.06-5.86, P=0.035) ] respectively. Conclusion: About one in three men in Moshi municipal had ED. High prevalence of ED among hypertensive individuals suggest a need to establish ED screening program during their routine clinic for early detection and treatment. Furthermore, education should be given on lifestyle modification to prevent hypertension and diabetes in the community. The outcome will be improvement of patient’s quality of life. Multiple sclerosis (MS) is a progressive and disabling neurodegenerative disease that primarily affects young adults. Despite significant therapeutic advances in the prevention of relapses, individuals with MS experience a variety of symptoms, most notably fatigue, spasticity, depression, gait and balance difficulties, and sexual dysfunction. These symptoms may interfere with activities of daily living and have a negative impact on quality of life. This review discusses treatment options for these symptoms.

Background: Urinary incontinence (UI) is a common problem among females and has been associated with significant decreased quality of  life. Few  women  seek  help  for  this  condition  with  only  a  few  who consult physiotherapy treatment.Purpose: To determine the prevalence of urinary incontinence and its impact on quality of life among the university women in South Africa.Method:  A  quantitative  cross-sectional  study design  with  145  women  ran-domly selected from the university. A questionnaire was used to determine UI Diagnosis; Impact on Qol and treatment seeking tendencies. BMI was meas-ured objectively. ethical clearance was obtained from University. Data was analysed using SPSS 17.0Results:  Forty  six(32%)  women  reported  to  having  UI.  Risk  factors  associated  with  UI  included  age,  race,  and  obesity. UI had a significant negative impact on quality of life and only 4.4% of participants with UI consulted physio-therapy for this condition.Conclusion: There is a high prevalence of UI among the women at this university with a significant impact on quality of life.The role of Physiotherapy in management of UI has been demonstrated and there is therefore a need to empower women with non-invasive treatment options, like physiotherapy.

Enlarged pores of facial skin always remain a matter of concern from both dermatologic and cosmetologic points of view. There were many conditions that are difficult to treat in which facial pores enlarge among them most common condition is acne vulgaris. This condition put a negative impact on a person’s quality of life and as well as also socially devastating for many individuals as it affects the physical appearance of a person. Although enlarged pores are not of medical concern it is considered as a more cosmetic con- cern. There are many available possible treatment options for enlarged pores but this case report is based on clinical evaluation of radiofrequency therapy in treating enlarged skin pores. In this report, three ses- sions of therapy every week for 3 weeks were applied. The results show a gradual decrease in the size of pores with almost clear skin. Introduction: The ageing of the population is associated with an increase in the number of dependent people, with an estimate of 15% living with a disability. The provision of care to family members entails an added responsibility associated with care-related problems, which can have a negative impact on caregivers. Therefore, we propose to design and evaluate a caregiving programme based on caregiver education, measuring knowledge, pain perception, emotional burden and health-related quality of life. Objective: The main objective is to improve the quality of life of informal caregivers through training interventions, in order to decrease the disability caused by caregiving tasks.Method: 99 caregivers belonging to the Aljarafe-Northern Seville Primary Care Health District participated, divided into control and intervention groups. We designed two educational workshops on care programmes for informal caregivers. They completed a knowledge questionnaire, and different validated scales related to pain, health states and quality of life.Results: Most of the caregivers presented back problems, for which they received pharmaceutical treatment. The implemented intervention was associated with a decrease in pain during basic activities of daily living, care and rest, as well as a reduction in strain index in both groups.Conclusions: Intervention programmes for informal carers of dependent patients are effective in improving the state of health and quality of life of this group, and their implementation should therefore be encouraged in primary care. Introducción: El envejecimiento de la población está asociado a un incremento de personas dependientes, estimándose que el 15% vive con alguna discapacidad. La prestación de cuidados a familiares supone una responsabilidad añadida asociada a problemas relacionados con el cuidado, que pueden repercutir negativamente en los cuidadores. Por ello, planteamos diseñar y evaluar un programa de cuidados, basado en la educación de los cuidadores, midiendo los conocimientos, la percepción del dolor, la carga emocional y la calidad de vida relacionada con la salud. Objetivo: El objetivo principal es mejorar la calidad de vida de los cuidadores informales mediante intervenciones formativas, con el fin de disminuir la incapacidad causada por las tareas del cuidado.Método: Participaron 99 cuidadores pertenecientes al Distrito Sanitario Aljarafe-Sevilla Norte de Atención Primaria, divididos en grupo control e intervención. Diseñamos dos talleres educacionales sobre programas de atención a cuidadores informales. Cumplimentaron un cuestionario de conocimientos, y diferentes escalas validadas relacionadas con el dolor, los estados de salud y la calidad de vida.Resultados: La mayor parte de los cuidadores presentaban problemas de espalda tomando medicación para ello. La intervención implementada se asoció a un descenso del dolor durante actividades básicas de la vida diaria, los cuidados y el descanso, y disminución del índice de esfuerzo en ambos grupos.Conclusiones: Los programas de intervención en cuidadores informales de pacientes dependientes resultan eficaces para mejorar el estado de salud y la calidad de vida de este colectivo por lo que se debe fomentar su realización desde la Atención Primaria.

The article presents the results of the treatment of 60 patients with premature ejaculation in combination with erectile dysfunction. Patients were divided into three groups, in which patients were prescribed non-drug treatment, or sildenafil 50 mg on demand, or sildenafil 50 mg on demand in combination with melatonin 3 mg at night. Duration of treatment was 6 months. The psychoemotional status, quality of life, quality and duration of intercourse, erectile function and chrono-rhythms of patients were assessed. The results of the study showed that treatment with sildenafil in combination with melatonin was the most clinically effective, which manifested in prolongation of sexual intercourse, improvement of the quality of erection and normalization of the chronobiological status of patients.

This study deals with how pain characteristics in conjunction with other factors affect quality of life (QoL) in a vulnerable primary care population. We recruited vulnerable older people (75+, n = 825) living in south-eastern Sweden. A postal questionnaire included pain aspects, QoL (EQ-5D-3L, RAND-36 physical functioning, attitudes toward own aging, and life satisfaction), functional status, social networks, and basic demographic information. Pain extent and localization was obtained by digitalization of pain drawings reported on standard body charts. Most respondents were experiencing pain longer than 3 months (88.8%). Pain frequency varied mostly between occasionally (33.8%) and every day (34.8%). A minority reported high pain intensity (13.6%). The lower back and lower legs were the most frequently reported pain locations (>25%). Multiple linear regression model revealed three characteristics of pain (intensity, frequency, and extent) remained inversely associated with the EQ-5D-3L index score (R2 = 0.57). Individually, each of these pain characteristics showed a negative impact on the other three dimensions of QoL (R2 = 0.23–0.59). Different features of pain had impact on different dimensions of QoL in this aging population. A global pain assessment is useful to facilitate individual treatment and rehabilitation strategies in primary care.

Objectives: To investigate how the symptoms of mucus and cough impact adults living with laryngotracheal stenosis, and to use this information to guide future research and treatment plans. Methods: A survey was developed with the support of patient advisors and distributed to people suffering with laryngotracheal stenosis. The survey comprised 15 closed and open questions relating to mucus and cough and included the Leicester Cough Questionnaire (LCQ). Descriptive statistics, X2 and thematic analyses were completed. Results: In total, 641 participants completed the survey, with 83.62% (n = 536) reporting problems with mucus; 79% having daily issues of varying severity that led to difficulties with cough (46.18%) and breathing (20.90%). Mucus affected voice and swallowing to a lesser degree. Respondents described a range of triggers; they identified smoky air as the worst environmental trigger. Strategies to manage mucus varied widely with drinking water (72.26%), increasing liquid intake in general (49.35%) and avoiding or reducing dairy (45.32%) the most common approaches to control symptoms. The LCQ showed a median total score of 14 (interquartile range 11-17) indicative of cough negatively affecting quality of life. Thematic analysis of free text responses identified 4 key themes—the Mucus Cycle, Social impact, Psychological impact, and Physical impact. Conclusion: This study shows the relevance of research focusing on mucus and cough and its negative impact on quality of life, among adults with laryngotracheal stenosis. It demonstrates the inconsistent advice and management strategies provided by clinicians for this issue. Further research is required to identify clearer treatment options and pathways.

Stretch marks (striae distensae) are linear areas of cutaneous atrophy that appear in regions of greatest stretch of the skin. Their formation is a complex multifactorial process, its etiopathogenesis has not been fully understood yet. Aesthetic correction of stretch marks is an urgent problem of modern dermatocosmetology due to their widespread prevalence and pronounced negative impact on the patient’s quality of life. The aim of this paper was to provide a review of the literature on current reported treatment options for stretch marks. The principles of the effect of various methods of treatment, their classification and data from clinical trials are considered.
