The bone marrow of 151 normal infants, children and young adults ranging in age from birth to 20 years was studied. The data were statistically evaluated and recorded in table and graph form. The following observations were made: 1. The height of the various layers in the centrifuged specimen as obtained by aspiration of 1 cc. or fraction thereof of marrow fluid was compared in the different age groups and showed an increase with progressing age of the fat and myeloid-erythroid cell layers and a decrease of the mature red cell layer. 2. The cellular composition of the bone marrow varies with age. 3. Marked variations occur during the first year of life and particularly during the first month. Thereafter up to the end of the observation period the changes are of minor extent and average values for this age group were established for all cells. 4. The cells of the erythroid series show a sharp decrease in number during the first days of life and reach essentially stable values (23.12%) by the end of the first month with a temporary decrease around the third month of life. 5. The myeloid series shows an increasing during the first two weeks of life followed by a sharp drop around the third week and the establishment by the end of the first month of values close to the average percentage as calculated for the period from 1 to 20 years (60.59%). 6. The lymphocytes increase in number during the first weeks of life, followed by a gradual decrease during the first and second year and thereafter do not show marked variations in number. (Average:16.03%.) 7. The individual cells of the myeloid and erythroid series show a pattern similar to the one described for the entire series of which they are part. There is, however, a tendency for the younger cells to slightly decrease in number and the older cells to increase with advancing age. Numerical variations with age on cells occurring in smaller numbers are less noticeable than on cell groups occurring in higher percentages. 8. The myeloid-erythroid ratio is the expression of the numerical relationship of the myeloid and erythroid series. The values are low at birth, increase rapidly during the first two weeks and approach the 1 to 20 year average (2.95) during the first year. 9. The data reported in the literature were recorded in order to complement and compare the findings of this study. 10. The normal bone marrow pattern as described may contribute to the interpretation of the cellular composition of the bone marrow under abnormal conditions. However, the diagnostic significance of individual cell findings may be limited by the great variations occurring normally in the cellular composition of the bone marrow of healthy individuals.

A Schumacher population estimate based on 995 maskinonge taken by trap nets, between the months of May and October in the years 1951–1953, set the number of maskinonge in Nogies Creek at between 769 and 1,122 in July 1953. The mean standard length of these fish was 53.0 cm. The rate of growth (26.3 cm. S.L. in the first year) compared favourably with that for maskinonge in other waters. Fish of age-groups II, III and IV predominated. The small number of fish over four years of age was apparently due to a 70% annual mortality rate at least after the third year of life and perhaps before age III.Recaptures of tagged maskinonge demonstrated that there was little movement of maskinonge in summer, and what movement there was, was mainly upstream. In the fall there was far more movement of fish, and this was mainly in a downstream direction.The area supports a fairly large population of maskinonge but the high mortality after three years of age limits its value for raising maskinonge to legal size. The potential for rearing fish to three years of age is such that it may be very advantageous to move hatchery fish here for one or two years before liberation.

ABSTRACT Objective: to evaluate the quality of life of adult patients with hematological cancer comparing Hematopoietic Stem Cell Transplantation modalities during the first year of treatment. Method: this is an observational and longitudinal research with 55 participants. Data collection was performed in six steps: before transplantation, pancytopenia, before hospital discharge, after 100, 180 and 360 days, in a reference hospital in Brazil for this treatment. The international instruments Quality of Life Questionnaire - Core 30 and Functional Assessment Cancer Therapy - Bone Marrow Transplantation were validated and translated into Portuguese (Brazil). Results: the mean age of participants was 36 years, 65% (n = 36) had leukemia diagnosis and 71% (n = 39) had undergone allogenic transplantation. In the Quality of Life Questionnarie - Core30 instrument, the pain symptom was significant between the first and second stages, and loss of appetite between the third and fourth stages, both in the allogenic group. In the Functional Assessment Cancer Therapy - Bone Marrow Transplantation, the functional well-being domain was significant between the third and fourth stages, also in the allogenic group. Conclusions: although the aggressiveness of treatment affects quality of life, patients consider it satisfactory after the first year. There are few significant differences between autologous and allogenic patients, and both groups have recovered in the course of the process.

In the first year and a half of the pandemic, the excess mortality in Hungary was 28,400, which was 1,700 lower than the official statistics on COVID-19 deaths. This discrepancy can be partly explained by protective measures instated during the COVID-19 pandemic that decreased the intensity of the seasonal flu outbreak, which caused on average 3,000 deaths per year. Compared to the second wave of the COVID-19 pandemic, the third wave showed a reduction in the differences in excess mortality between age groups and regions. The excess mortality rate for people aged 75+ fell significantly in the third wave, partly due to the vaccination schedule and the absence of a normal flu season. For people aged 40-77, the excess mortality rate rose slightly in the third wave. Between regions, excess mortality was highest in Northern Hungary and Western Transdanubia, and much lower in Central Hungary, where the capital is located. The excess mortality rate for men was almost twice as high as that for women in almost all age groups.

The third chapter traces the beginning of the partnership through the first year of Queen Anne’s reign, as Marlborough persuades Godolphin to return to office as Lord Treasurer and his ministerial partner, with the declared aim of ‘moderation’, that is, holding the balance between the Tories and Whigs on the basis of their support of the war. The role of Queen Anne’s husband Prince George is examined and Marlborough’s and Godolphin’s separate roles are explored, along with the significance of their extensive correspondence. Marlborough is unexpectedly successful in his first campaign, but his determination to obtain a grant from Parliament to support his dukedom jeopardizes Godolphin’s project for war-supply, and their rival Rochester contests control of the Treasury and therefore the war. Marlborough forces Rochester’s resignation and the partnership is confirmed when Marlborough’s only son dies shortly before he leaves for the Continent and he adopts Godolphin’s son as his heir.

Abstract The severity of COVID-19 is age-related, with the advantage going to younger age groups. Five reasons are presented. The first two are well-known, are being actively researched by the broader medical community, and therefore are discussed only briefly here. The third, fourth, and fifth reasons derive from evolutionary life history theory, and potentially fill gaps in current understanding of why and how young and old age groups respond differently to infection with SARS-CoV-2. Age of onset of generalized somatic aging, and the timing of its progression, are identified as important causes of these disparities, as are specific antagonistic pleiotropic tradeoffs in immune system function.

Objective To construct deep learning (DL) models to improve the accuracy and efficiency of thyroid disease diagnosis by thyroid scintigraphy. Methods We constructed DL models with AlexNet, VGGNet, and ResNet. The models were trained separately with transfer learning. We measured each model’s performance with six indicators: recall, precision, negative predictive value (NPV), specificity, accuracy, and F1-score. We also compared the diagnostic performances of first- and third-year nuclear medicine (NM) residents with assistance from the best-performing DL-based model. The Kappa coefficient and average classification time of each model were compared with those of two NM residents. Results The recall, precision, NPV, specificity, accuracy, and F1-score of the three models ranged from 73.33% to 97.00%. The Kappa coefficient of all three models was >0.710. All models performed better than the first-year NM resident but not as well as the third-year NM resident in terms of diagnostic ability. However, the ResNet model provided “diagnostic assistance” to the NM residents. The models provided results at speeds 400 to 600 times faster than the NM residents. Conclusion DL-based models perform well in diagnostic assessment by thyroid scintigraphy. These models may serve as tools for NM residents in the diagnosis of Graves’ disease and subacute thyroiditis.

AbstractHere we describe a new technique to deal with alar retraction, a highly undesirable imperfection of the nose. The procedure involves placing a caudal extension graft below the vestibular portion of the lower lateral cartilage (LLC) after its detachment from the vestibular skin. The graft is fixed to the cartilage and, subsequently, to the vestibular tissue. The present retrospective study included 20 patients, 11 females and 9 males, with a mean age of 28.90 years. Follow-up ranged from 1 to 18 months. Surgery improved alar notching to a smoother dome shape and nostril exposure was reduced in every patient. The caudal extension graft of the LLC contributed to rise in overall patient satisfaction, as revealed by the postoperative increase of the Rhinoplasty Outcomes Evaluation (ROE) mean score from 40.0 to 79.17 (p < 0.0001). It also contributed to and improved functional outcomes, as indicated by the decrease of the Nasal Obstruction Symptom Evaluation (NOSE) mean score from 52.75 to 13.25 (p = 0.0001). Sex did not affect the mean ROE and NOSE scores. Thus, increased patient satisfaction measured by the ROE is present in both sexes and at both age groups but it is better detected in the first year after surgery. Functional improvements analyzed with NOSE are best detected in patients aged ≥ 30 years and in follow-ups of 11 months. The caudal extension graft of the LLC technique described herein effectively and safely corrects alar retraction and the collapse of the nasal valve while filling the soft triangle.

Background:Ustekinumab has been efficacy and safety for psoriatic artritis in clinical trials.Objectives:To assess effectiveness, by means of drug persistence analisys, and safety of ustekinumab in patients with psoriastic arthritis in Biobadaser.Methods:BIOBADASER is the Spanish registry of biological drugs of the Spanish Society of Rheumatology and the Spanish Medicines Agency. We identified patients aged 18 years or more with psoriatic arthritis on Ustekinumab. A descriptive analysis was performed.The persistence of ustekinumab therapy was calculated with a Kaplan-Meier curve and was compared with the persistence of anti-TNF, according to line treatment. Log Rank test was used to establish a comparison. Adverse events occurring with ustekinumab are described according to year treatment.Results:One hundred and twelve patients were on ustekinumab. Most of them were on their second or third line treatment: 53.57% more than one biological therapy (BT), 19.64% second BT, 26.79% were naïve for BT. Most of them were on 45 mg dose: 88.24%. Median duration of disease at Ustekinumab initiation was 10.1 SD 7.2 years; 69.23% had peripheral arthritis; 45.24% had obesity and 39.29% were overweight; 40,6% were on prednisone and 59.82% on DMARD. The cause of discontinuation of treatment was mainly inefficacy (82.61%) and less common an adverse event (6.52%). The probability of persistence of treatment with ustekinumab was 0.83 (95% CI 0.63-0.92) at year 1, 0.79 (0.58-0.90) at year 2 and 0.79 (0.58-0.9) at year 3 when ustekinumab was prescribed as the first line treatment. The persistence decrease when ustekinumab was prescribe as a second and third treatment: being 0.53 (0.27-0.73) the first year, 0.46 (0.22-0.67) the second year and 0.46 (0.22-0.67) as a second line treatment and 0.58 (0.44-0.70) the first year, 0.33 (0.17-0.50) the second year and 0.33 (0.17-0.50) the third year as a third line treatment.The persistence was similar to anti-TNF treatment, according to line treatment. Adverse events were mainly mild (97.83%) and occurred the first year of treatment. Most of the adverse events were classified as “infections and infestations” (36.96%).Conclusion:The persistence of ustekinumab was high, being 83% at the end of the first year on treatment and 79% the second and the third year of treatment. The persistence of ustekinumab was higher when if it was the first line treatment compared as if it was used as the second o third BT option. The persistence of Ustekinumab is similar to the persistence of anti-TNF treatments in all the analyzed treatment lines (no statistically differences were found). Adverse events occurred mainly during the first year treatment. BACKGROUNDDendritic cells (DCs) in the mucosa of the respiratory tract might be involved in the early development of pulmonary allergy or tolerance. To date, little is known about when the first DCs occur in human airways.METHODSSpecimens of the distal trachea from patients who had died from sudden death in the first year of life (n=29) and in older age groups (n=59) as well as from those who had died from respiratory tract infections in the first year of life (n=8) were examined by immunohistochemistry. Transmission electron microscopy was performed in additional samples from two adults.RESULTSIn the sudden death subgroup DCs were absent in 76% of those who died in the first year of life but were present in 53 of the 59 older cases. All infants who had died of respiratory infectious diseases had DCs in the tracheal mucosa.CONCLUSIONSMature DCs are not constitutively present in the human tracheobronchial mucosa in the first year of life, but their occurrence seems to be triggered by infectious stimuli. These data support the hypothesis that DCs play a crucial role in immunoregulation in early childhood.
