Abstract Introduction: Temporomandibular joint disorders (TMD or TMJD) involve clinical problems and symptoms affecting the temporomandibular joint (TMJ) and associated structures. The temporomandibular joints are anatomically connected to the cervical region, where cervical spine movements occur simultaneously to masticatory muscle activation and jaw movements. Objective: Our study sought to assess the relationship between the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD), surface electromyography (sEMG) of the masticatory muscles, posture and cervical flexibility in women with TMD. Method: Fifty women with an average age of 27.0 ± 6.37 years, diagnosed with TMD according to RDC/TMD, were assessed for craniocervical posture, cervical flexibility and sEMG of the masticatory muscles. Results: There were no differences in jaw function limitations, depression, pain level and its interference in work ability and daily activities, posture and sEMG between TMD diagnoses or between muscle classification (p > 0.05). Depression scores were higher among participants with biarticular dysfunction (p = 0.023). The group with bruxism exhibited a higher pain level at assessment (p = 0.001) and a greater reduction in work ability (p = 0.039). Subjects with muscular and mixed TMD showed less cervical rotation to the right when compared with those with articular TMD. Conclusion: There was no difference in posture or sEMG values for TMD diagnoses, joint and muscle dysfunctions and the presence of bruxism. Muscle dysfunction is associated with reduced cervical rotation to the right. Jaw function limitations did not interfere in posture or sEMG and depression was associated with pain.

Background: Temporomandibular disorders (TMD) are disorders of multifactorial origin that affect the temporomandibular joint and/or masticatory muscles. The TMD can generate compensations where physical therapy acts through myofascial releases and joint mobilizations to restore functionality. Objective: To verify the effects of manual therapy techniques on pain and electromyographic activity (EMG) of masticatory muscles in TMD patients. Methods: This is a quasi-experimental study composed of 10 subjects with TMD and 10 healthy subjects who underwent a session of myofascial release, joint mobilization and digit-pressure on the masticatory and cervical muscles. Pain levels and electromyographic activities of the temporal and masseter muscles were bilaterally assessed during the following situations: (1) Maximum Occlusion and (2) Chewing in the right and the left side with chewing gum. Data analysis was performed using BIOMEC-SAS software, in which the data were filtered with a 4th order Butterworth bandpass filter with 20-400hz and normalized as a percentage of maximum occlusion. Data were divided into homolateral and contralateral muscles alongside chewing. EMG variables were compared by factorial ANOVA, and pain by dependent t-test. Results: TMD patients had pain level reduction during maximum occlusion (Pre:4.55±2.2; Post:2.55±1.6; p=<0.001), right chewing (Pre:2.3±2.16; Post:0.4±0.69; p = 0.02) and left chewing (Pre: 1.0±1.15; Post:0.10±0.31; p = 0.05). There was no effect of manual therapy techniques on the EMG activity bilaterally of the temporal (p = 0.617) and masseter (p = 0.926) muscles during chewing. Patients with TMD had higher EMG activities during chewing on the homolateral temporal (p = 0.001) and masseter (0.004) muscles compared to subjects without injury. Conclusion: Manual therapy techniques reduced pain level in TMD patients. No changes in EMG activity were observed after manual therapy techniques. Finally, the study provides evidence of the influence of manual therapy techniques on short-term pain.

Temporomandibular disorder (TMD) encompasses a set of disorders involving the masticatory muscles, the temporomandibular joint and associated structures. It is a complex biopsychosocial disorder with several triggering, predisposing and perpetuating factors. In the etiology of TMD, oral parafunctions, namely bruxism, play a relevant role. The study of bruxism is complicated by some taxonomic and diagnostic aspects that have prevented achieving an acceptable standardization of diagnosis. The aim of this study was to analyze the prevalence of temporomandibular disorders and bruxism in a Portuguese sample.

BACKGROUND: Preservation and strengthening of the health of civil aviation flight personnel is an extremely urgent problem and is associated with ensuring flight safety. AIM: This study aimed to determine the structure and analyze the organs and tissues of the masticatory apparatus among civil aviation flight personnel. MATERIALS AND METHODS: To determine the incidence and intensity of main dental diseases and the morbidity structure of organs and tissues of the masticatory apparatus in civil aviation pilots, an in-depth examination was carried out in 120 men aged 2545 years (study group 1) working on various modern passenger and transport aircrafts. The duration of flight operations ranged from 2 to 17 years, with a total flight duration of 140 to 5580 h. The comparison group consisted of 146 men (aged 2545 years) of non-flying specialties (study group 2). The generally accepted methods for assessing the prevalence and intensity of the course of major dental diseases were used. RESULTS: The incidence and intensity of dental disorders, periodontal disease, temporomandibular joint disorders, and parafunctions of the masticatory muscles were defined. The incidence and intensity of major dental diseases among civil aviation flight personnel were clearly directly proportional to their flight time (r=0.547) and duration of work in civil aviation (r=0.482) but to a lesser extent to their age (r=0.416). CONCLUSIONS: Non-carious lesions of the teeth, inflammatory and dystrophic diseases of the periodontal and oral mucosa, painful dysfunction of the temporomandibular joint, and bruxism are more common among civil aviation flight personnel than among non-flying personnel, which is due to the unfavorable effect of air flight factors on organs and tissues of the mastication apparatus and the body of the pilots as a whole.

The article presents a clinical case of the diagnostics and treatment of the temporomandibular joint dysfunction (TMJ) in patient after orthodontic treatment. The hypertonia of masticatory muscles, limit of mouth opening and deviation of the jawbone to the right were observed during physical examination. Adhesion of the disk to the articular tubercle of the right temporal bone was detected on MRI. Hyperrotation of the articular head and the deviation of the jawbone to the right were observed on the charts during axiography. Disappearing of the feeling of heaviness in the right TMJ, free mouth opening, absence of deviation symptom, and the reduction of the rigidity of the masseter muscles were observed on the basis of diagnostic examinations after treatment. Presented clinical experience lets us make a conclusion that it is necessary to prescribe taking axiography and MRI during diagnosing and treating temporomandibular joint dysfunction.

OBJECTIVES The aim is to assess the knowledge, attitude and practice of dental practitioners concerning physiotherapy management of Temporomandibular joint dysfunctions (TMDs.). METHOD 61 dentists were included from Tertiary, secondary and private dental clinics of Karachi, and the study design was cross sectional. RESULTS Prior to survey 45.9% of did not had knowledge about physical therapy management TMD. Maxillofacial surgeons and master’s degree holders had more knowledge about physical therapy (PT) management. 45.9% dentists reported that PT is non-effective treatment approach. Lack of knowledge on dentist’s behalf is the foremost cause of less referral to PT 88.5%. Jaw exercises were prescribed by 83.6% of dentists. In the end of the survey 52.5% dentists were reluctant to refer, 47.5% were maybe they will refer and 96.7% dentists were eager to gain more knowledge about the benefits of PT and collaborative treatment. CONCLUSIONS This study concluded that the dental practitioners of Karachi lack knowledge, attitude and practice regarding physiotherapy treatment of TMD. The benefits of multidisciplinary approach while treating TMDs were not very well known to dental practitioners and its benefits to patients. Temporomandibular disorders usually represented as a group of painful as well as altered conditions involving the muscles of mastication and the muscles around the temporomandibular joint. Usually the patient are un aware of the condition. Temporomandibular joint disorders affects twenty five percent of the population. Temporomandibular disorders represent with musculoskeletal degenerative conditions of the joint which results in functional as well as morphological deformities of the temporomandibular joint. As temporomandibular disorders cases are complex with unique nature with respect to each case, so the diagnosis and treatment modality is quite different for each type of case.

Background: Temporomandibular disorders have been considered as a common orofacial pain condition. The term temporomandibular pain dysfunction (TMPD) is used synonymously with myofacial pain dysfunction disorder/syndrome, temporomandibular disorder, craniomandibular disorder and many other terms. Objective: To evaluate the prevalence of signs and symptoms of temporo-mandibular joint disorder (TMD). Study Design: Descriptive cross-sectional study Place and Duration of Study: Department of Oral and Maxillofacial Surgery, Faryal Dental College, Sheikhupura , Lahore, Pakistan from 1st February 2019 to 31st May 2021. Methodology: One hundred adolescents aged 15 to 60 years were enrolled. A detailed history about the chief complaint was taken and clinical examination was done. Temporomandibular joint examination performed included Auscultation for temporomandibular joint sounds like clicking and crepitus and palpation of both TMJs and associated muscles for evaluation of pain. Results: The most common signs of temporomandibular joint disorders were temporomandibular joint pain 78%, temporomandibular joint clicking 53% and trismus 29%. The most prevalent predisposing factors of temporomandibular joint disorders were parafunctional habits 40%, unknown factors 23% and history of road traffic accident/history of difficult extractions 9%. Male to female ratio showed female predominance (P = 0.001). Conclusion: Signs and symptoms of temporomandibular joint disorders were prevalent in Pakistani population with a clear female predominance. <p><strong>Objective:</strong> To verify the level of agreement among different indexes used to achieve the prevalence of Temporomandibular Disorders (TMD). <strong>Material and Methods</strong>: One hundred one dental students were selected by a randomized process. TMD were evaluated by the Fonseca’s Anamnestic Index (FAI), Helkimo’s Clinical Index (HCI), and the Research Diagnostic Criteria for TMD (RDC/TMD). Data was analyzed using Chi-square and Kappa tests, considering a significance level of 5%. <strong>Results:</strong><em> </em>HCI showed the highest prevalence of TMD, and the comparison between RDC/TMD, FAI, and HCI showed low agreement (k=0.17 and k=0.35 respectively). Most individuals presented mild TMD for both FAI and HCI indexes. A moderate correlation for TMD severity was obtained (k<sub>w</sub>=0.53) between FAI and HCI, and a high sensitivity and low specificity were observed for both diagnosis when compared to RDC/TMD. <strong>Conclusion</strong><em>:</em> The prevalence of TMD may vary significantly depending on the index used for its diagnosis, which may lead to a large number of false positives and overtreatments.</p><p><strong> </strong></p><p><strong>Keywords: </strong>Epidemiology; Prevalence; Temporomandibular joint; Temporomandibular joint disorders; Young adult.</p>

Background and Objective. Intra-articular temporomandibular disorders are often related to pain in the area of the temporomandibular joint, ear, and temple. The aim of the study was to investigate the efficiency of anterior repositioning splints in decreasing pain related to temporomandibular joint disc displacement with reduction. Methods. The research material consisted of 112 patients, aged 24 to 45 years, of both genders, who reported for treatment at the Consulting Room of Temporomandibular Joint Dysfunctions at the Jagiellonian University in Cracow between 2014 and 2016 due to pain in the area of the temporomandibular joint(s) and noise(s) of temporomandibular joint(s) present during jaw movements with comorbid contracture of masticatory muscles. Subjects were examined according to the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) protocol and, after diagnosis of painful disc displacement with reduction and masticatory muscle contracture, they were assigned randomly to either the study or control groups (56 patients in each). In the study group, we used an anterior repositioning splint on the full lower arch for about 20 hours usage over a 4-month period. In the control group, a noninvasive therapy was applied using a biostimulation laser over 12 sessions performed every second day on the area of both temporomandibular joints with mouth open and while performing muscle self-exercises with a dominant protrusive position of the mandible. Pain intensity was evaluated using the Verbal Numerical Rating Scale (VNRS) immediately before the treatment and then after 4 and 16 weeks. The obtained data were analyzed using the Mann–Whitney U test p≤0.005. Results. The VNRS values reported during the final examination for the study group were significantly lower than for the control group p=0.0004. Conclusions. The anterior repositioning splint is an efficient tool in decreasing pain related to disc displacement with reduction. This trial is registered with Clinicaltrials.gov NCT03057262.

Purpose:This study aimed to establish the prevalence of pain in the craniomandibular and cervical spine region in individuals with Temporomandibular Disorders (TMD) and to analyze the effects of these disorders on the bilateral activation of anterior temporalis (AT) and masseter (MA) muscles during the masticatory cycle.Methods:The participants were 55 female volunteers aged 18–30 years. The presence of TMD and craniomandibular and cervical spine pain was evaluated by applying the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) questionnaire and using a combination of tests for the cervical region. The muscle activity of AT and MA during the masticatory cycle was assessed using the symmetry and antero-posterior coefficient indices.Results:The AT activity during the masticatory cycle is more asymmetric in individuals with TMD. The craniomandibular pain, more prevalent in these individuals, influenced these results.Conclusion:Individuals with TMD showed changes in the pattern activity of AT. The craniomandibular nociceptive inputs can influence the increase in asymmetry of the activation of this muscle.
