This study aimed to estimate the quartile distribution on the cognitive assessment of normal elderly with low education as measured by the Cambridge Cognitive Examination (CAMCOG). A sample of 292 elderly (> 65 years of age), screened for dementia and depression, were assessed using the CAMCOG. The CAMCOG scores of normal subjects (n = 206) were stratified according to age (65-69, 70-74, 75-79, > 80) and schooling (illiterate, 1-4, and > 5 years of formal education). Mean age was 72.8 (± 3.5) and mean schooling was 3.5 years (± 3). The mean score on the CAMCOG was 71 (± 12.7). The scores at the first quartile for illiterate/1-4 years of schooling were 58/62 (65-69 years), 52/63 (70-74 years), 48/67 (75-79 years) and 46/64 (> 80 years), respectively. There was a significant difference in the CAMCOG quartiles according to education and age. This study provides normative data on the CAMCOG of elderly people with low educational levels which may be clinically useful.

ABSTRACT A comparison has been made between (A), a relatively simple method for the measurement of aldosterone secretion rate, based on paper chromatography and direct densitometry of the aldosterone spot and (B) a more elaborate isotope derivative method. The mean secretion rate in 9 normal subjects was 112 ± 26 μg per 24 hours (method A) and 135 ± 35 μg per 24 hours (method B). The »secretion rate« in one adrenalectomized subject after the intravenous injection of 250 μg of aldosterone was 230 μg per 24 hours (method A) and 294 μg per 24 hours (method B). There was no significant difference in the mean values, and correlation between the two methods was good (r = 0.80). It is concluded that the densitometric method is suitable for clinical purposes as well as research, being more rapid and less expensive than the isotope derivative method. Method A also measures the urinary excretion of the aldosterone 3-oxo-conjugate, which is of interest in many pathological conditions. The densitometric method is obviously the less sensitive and a prerequisite for its use is an aldosterone secretion of 20—30 μg per 24 hours. Lower values are, however, rare in adults.

Background: Low socio-economic and educational levels of parents are important risk factors for under-nutrition in children. The distribution ofhealth and diseases in childhood had been shown to be strongly influenced by the social characteristics like occupation and educational attainment. The aim of this study was to determine the socioeconomic status among children with Protein Energy Malnutrition and the Controls in Zaria usingthe modification of the method earlier used by Oyedeji. Method: This study was a case control health- based descriptive study to determine the socioeconomic and demographic variables in  undernourished children in Zaria. Using systematic sampling method, a total of 132 children (cases and controls) between 6 and 59 months of age were selected for assessment of socioeconomic and demographic variables. Results: Nineteen (28.8%) of the study group belong to social class III as compared to 12 (18.2%) for the controls. As for the educational levels of the parents, among the cases, 28 (42.4%) had Islamic education, 25 (37.9%) had secondary education, 10 (15.1%), 2 (3.0%) and 1 (1.5%) had primary,  tertiary and no formal education respectively. For the controls, 7 (10.6%) had Islamic education, 34 (51.5%) had secondary education, 13 (19.7%) and 12 (18.1%) had primary and tertiary education respectively. There was none with no formal education, The statistical analysis showed significant  difference (P= 0.0009) between cases and controls for maternal educational levels only among the demographic data. Conclusion: This study has been able to establish low socio- economic factors among undernourished children studied. Keywords: Under-nutrition, socioeconomic factors, educational levels, children

Background: Hemoglobin E disease are the most common structural haemoglobinopathies, reaching its highest frequency in South East Asia and also prevalent in Bangladesh. There are very few data for effective screening of HB E trait. The aim of the study was to analyze hematological profile and RBC indices as a marker of screening for Hb E trait.Methodology: A cross sectional observational study was conducted among 150 subjects at Dhaka Shishu (Children) Hospital from December 2008 to November 2009. On the basis of Hb electrophoresis, all subjects were divided into three groups. Group I comprised of 50 subjects with Hb E trait, group II comprised 51 subjects with ß thalassemia trait and Group III comprised of 49 normal subjects. Result of RBC indices and blood count were compared between Hb E trait and normal study population.Results: In group I the mean age was 29.53 years and the age range was 2-47 years. In group II the mean age was 30.31 years and the range was 1-55 years. In group III the mean age was 21.81 years and the range was 1-52 years. The present study revealed the mean±SD haemoglobin level (g/dl) in group I (E trait) and group III (normal subjects) were 11.76±1.91 and 10.77±1.4 respectively. Statistically no significant difference was found between these 2 groups (P>.05). The mean corpuscular volume (MCV) in mean±SD in group I and group III were 72.89±12.24 fl and 77.35±10.27 fl respectively. No statistical significance was found as P>0.05. The mean corpuscular haemoglobin (MCH) and mean corpuscular haemoglobin concentration (MCHC) in group I were 24.84±2.71pg & 32.97±2.27 g/dl and in group III were 28.28±2.71 pg & 32.97±2.38 g/dl respectively. There were no significant difference between group I and III regarding MCH and MCHC.The mean RBC count in E trait and normal subject group were 5.01±71 million/cmm and 4.61±0.65 million/cmm respectively. There was no significant difference was found P>0.05. Mean red cell distribution width (RDW) in group I and III were 14.32 ±2.34 and 14.83±1.28 without any statistical difference (P>0.05)Conclusion: In our study there were no diagnostic significance of Blood count and RBC Indices for detection of Hb E trait. Abstract We developed a simple and highly sensitive RIA for glycated protein (GP), and used it to measure GP in serum and urine from 15 normal controls and 30 diabetics (14 with urinary excretion rate of albumin, Ualb less than 15 micrograms/min, group A; nine with 15 less than or equal to Ualb less than or equal to 150 micrograms/min, group B; and seven with Ualb greater than 150 micrograms/min, group C). The mean serum concentration of GP was above normal in all groups of diabetics, and the mean glycation ratios of serum protein (SGP) were higher in groups B and C than in normal subjects. Urinary concentrations of GP also were increased in groups B and C, although the glycation ratio of urinary protein (UGP) was decreased in group C. Consequently, the selectivity of urinary excretion of GP (UGP/SGP) was significantly decreased in group C. Moreover, there was a significant difference in the mean values of selectivity between groups of patients with various degrees of retinopathy. We suggest that measurements of serum and urinary GP are useful to evaluate the progression of diabetic complications.

Abstract We report a simple, convenient, and reproducible method, involving the use of radiolabeled cholesterol dispersed in Tween 20 as a tracer and endogenous lipoproteins as a substrate, for measuring the rate of serum cholesterol esterification in vitro. The reaction of lecithin acyltransferase (EC 2.3.1.43) was enhanced by the presence of Tween 20, which probably accelerates the exchange between radiolabeled cholesterol and endogenous lipoprotein cholesterol. In sera from 65 normal subjects, the in vitro cholesterol esterification rate was significantly correlated (r=0.47,P=0.001) with age. The mean rate of esterification of 31 subjects 30 years old or younger was significantly lower than that of 34 subjects 31 to 64 years of age. We found no significant difference in the rate of esterification between men and women. The rate of cholesterol esterification (nmol/ml per h) was significantly correlated with the concentration of endogenous free cholesterol in serum, but the fractional rate (the percentage of radiolabeled cholesterol esterified per hour) was inversely proportional to the endogenous free cholesterol. The fatty acid composition of the cholesteryl esters formed by the acyltransferase reaction may provide an index in recognizing some specific disorder.

Abstract Background: Blood cells are deficient in membrane-bound glycosyl-phosphatidylinositol (GPI)-anchored proteins in paroxysmal nocturnal hemoglobinuria (PNH) because of an acquired disorder of the pluripotent stem cell. CD55 and CD59 antigens are the most common GPI-anchored proteins that are used for the diagnosis of the presence of PNH clone. The association of MDS with PNH is rather controversial. There are few published individual cases of PNH arrived from a previous MDS or MDS following PNH. Aim: Recent studies have demonstrated the existence of PNH clone in some MDS patients. These studies were performed mostly on erythrocytes and to the best of our knowledge they are only 4. Granulocytes appear to be more sensitive markers of PNH clone existence than erythrocytes and therefore we investigate the expression of PNH clone on granulocytes of patients with MDS. Material-Methods: A total number of 95 patients 25–80 yrs old with MDS {A:19 with refractory anaemia (RA), B:9 with refractory anaemia with ring sideroblasts (RARS), C:17 with refractory anaemia with excess blasts (RAEB), D: 12 with refractory anaemia with excess blasts in transformation (RAEB-t) and E: 19 with chronic myelomonocytic leukemia (CMLL)} and 19 healthy donors were included in our study. The presence of GPI-anchored proteins (CD55, CD59)-deficient granulocytes was examined by flow cytometry. For the detection of the PNH clone the commercial kit by Beckman Coulter (cellquant CD55/CD59) was used. Statistical analysis was made by ANOVA, while Robust test was performed because there was no homogeneity of variances by ANOVA. Results: Table 1 shows the percentages of CD55 and CD59 deficient granulocytes in the 5 groups of pts with MDS comparing with the control group. a) Although the mean values of CD55 deficient granulocytes in the groups RAEB, RAEB-t and CMLL were higher enough than those of the normal subjects, there were no statistically significant differences (p>0.10) in the comparison of the mean values of CD55 deficient granulocytes between each group of patients with the control group. b) On the contrary there was a very statistically significant difference in the comparison of the mean value of CD59 deficient granulocytes between the three last groups (C, D and E) of pts with the control group (p< 0.086, p< 0.001 and p<0.001 respectively). On the contrary the two low risk forms (RA, RARS) revealed no such populations. Non-expression of CD59 antigen on granulocytes is a more sensitive marker than that of CD55 antigen for the presence of a possible PNH clone in MDS patients.

Background: Preoperative apprehension is a widespread global problem. This anxiety and apprehension can significantly affect all aspects of anesthesia and surgery. Thus, it is essential to detect and address the patient's anxiety to assist in recovery after surgery and improve the patient's outcome. Materials and Methods: This is a cross-sectional study conducted over one year in the allied hospitals of Rawalpindi Medical University (RMU). Our sample size was 380 preoperative patients, who were selected by convenience sampling. The Amsterdam Preoperative Anxiety and Information Scale (APAIS) was used.Results: The overall prevalence of anxiety was 30.5%, while the prevalence of information requirement was 33.2%. The mean total APAIS was 12.73±8.56, the mean anxiety subscore was 8.25±5.58, and the mean need for information subscore was 4.48±3.16. A statistically significant difference was found in the scores of the patients based on age, gender, education, type of anesthesia, and type of surgery (p< 0.05). The previous history of anesthesia and the presence of comorbidities did not bring a significant difference in the scores of the patients (p>0.05). Postoperative pain was the most important concern, found in 32% of the patients. Conclusion: Younger age group, female gender, having no formal education, surgery under regional anesthesia, and undergoing C-section, IOL implantation and septoplasty are associated with higher preoperative anxiety, while the presence of comorbidities and previous history of anesthesia are factors that do not significantly affect preoperative anxiety in our population.

This study was conducted to determine the attitudes of men regarding violence against women. This was a cross-sectional descriptive study. Research data were collected using a Men’s Information Form and a Scale of Attitude towards Violence against Women. The mean age of the participating men was 39.54 ± 13.55 years. The men’s general mean score for attitude regarding violence was 52.02 ± 10.81, which showed that such attitudes had a patriarchal tendency. In the univariate analyses, there was a significant difference between age group, education status, place of residence, situations of domestic violence and violent behaviour against mothers or wives, years of marriage and wife’s education status and the scores of the Scale of Attitudes towards Violence against Women (p < 0.05). Based on our findings, it is suggested that socioeconomic factors such as witnessing and experiencing violence in childhood, female unemployment, low education level, unemployment and poverty, and place of residence affect and increase violence against women.

Abstract The Addenbrooke's Cognitive Examination-Revised (ACE-R) is a highly sensitive and specific tool for the detection of mild dementia. It is particularly useful in differentiating Alzheimer's disease from frontotemporal dementia. While the first version of the test battery has been adapted in many countries, its revised version has not, probably because it was published very recently. Objective: To translate and adapt the ACE-R for use in the Brazilian population. Methods: Two independent translations were made from English into Portuguese, followed by two independent back-translations. Few adaptations in accordance to the Brazilian culture and language were made and a first version of the instrument produced. This former version of the ACE-R was administered to 21 cognitively healthy subjects aged 60 years or more, with different educational levels. Results: The mean age of the studied sample of healthy elderly was 75.4 years (ranging from 60 to 89 years). Small additional modifications were necessary after the evaluation of the first ten subjects in order to improve comprehension of the test. The final Portuguese version of the ACE-R was produced and was found to be well understood by the remaining 11 subjects, taking an average of 15 minutes to be administered. Conclusions: The Brazilian version of the ACE-R proved to be a promising cognitive instrument for testing both in research and clinical settings. With this regard, additional studies are currently being carried out in our unit in order to investigate the diagnostic properties of the ACE-R in our milieu.

Background: The acoustic stapedial reflex (ASR) test provides useful information about the function of the auditory system. While it is frequently used with adults and children in a clinical setting, its use with young infants is limited. Presently, there are few data for neonates and inadequate research into the test-retest reliability of the ASR test. Purpose: This study aimed to establish normative data and evaluate the test-retest reliability of the ASR test in healthy neonates. Research Design: A cross-sectional experimental design was used to establish ASR normative data and assess the test-retest reliability of ASR thresholds obtained from healthy neonates. Study Sample: Sixty-eight full-term neonates with mean chronological age of 2.5 days (SD = 1.8 day), who passed the automated auditory brainstem response, transient evoked otoacoustic emission, and high frequency (1 kHz) tympanometry (HFT) tests. Data Collection and Analysis: One randomly selected ear from each neonate was tested using TEOAE (transient evoked otoacoustic emission), HFT, and ASR tests using a 1 kHz probe tone. ASR thresholds were elicited by presenting pure tones of 0.5, 2, and 4 kHz and broadband noise (BBN) separately to the test ear in an ipsilateral stimulation mode. The ASR procedure was repeated to acquire retest data within the same testing session. Descriptive statistics, χ2, and analysis of variance with repeated measures tests were used to analyze ASR data. Results: All neonates exhibited ASR when stimulated by tonal stimuli or BBN. The mean ASRTs (acoustic stapedial reflex thresholds) for the 0.5, 2, and 4 kHz tones were 81.6 ± 7.9, 71.3 ± 7.9, and 65.4 ± 8.7 dB HL, respectively. The mean ASRT for the BBN was estimated to be smaller than 57.2 dB HL, given the limitation of the equipment. The 95th percentiles of the ASRT were 95, 85, 80, and 75 dB HL for the 0.5, 2, and 4 kHz and BBN, respectively. The test-retest reliability of the ASR test for all stimuli was high, with no significant difference in mean ASRTs across the test and retest conditions. Test-retest differences were within 10 dB for more than 91% of ASRT data across all stimuli. There was a slight trend of ASRTs being more repeatable in the medium ASRT range than in the higher or lower range. Conclusions: This study demonstrated that ASRTs obtained from healthy neonates were highly repeatable across test and retest sessions. Given the availability of normative data and the high test-retest reliability, the ASR test will be useful as a diagnostic tool in a battery of tests to evaluate the auditory function of neonates.
