The objective of this article was to describe the contribution of clinical nursing care to the environmental comfort of women with Acute Myocardial Infarction, based on the Comfort Theory and mediated by the research-care approach. Data collection took place from April to June 2011 with nine women admitted with an infarction to the Heart Hospital of Sobral, Ceará, Brazil. Four meetings were held; the first being four starting at the admission to the hospital. The participants were informed about the study objectives and the proposed method, and following care was provided based both on method and theory. Data collection was performed using individual semi-structured interviews, field diary, participant observation and thematic category content analysis. The clinical care implemented in the environmental context aimed at promoting adaptation to the coronary care unit and also providing an enabling environment for comfort and relaxed atmosphere. Clinical care targeted at promoting environmental comfort contributes to the wellbeing of women with infarction.

<p><strong>Objective:</strong> To investigate and improve the clinical nursing care method of acute myocardial infarction. <strong>Methods: </strong>Make a retrospective analysis of the clinical data for 69 cases of acute myocardial infarction from January 2009 to December 2012 in our hospital and summarize the clinical nursing method. <strong>Results:</strong> 63 from 69 patients are improved after being rescued and nursed through the above-mentioned method, and improvement rate was 92.0%. 5 patients were died (all of them were died from the heart failure), which the mortality is 8.0%. Wherein the complicated arrhythmia cases were 24, the complicated cardiogenic shock cases were 12, hospitalization time: 4‒17 days, and the average hospitalization time is 9.2 days.<strong> Conclusion:</strong> The strengthened nursing cooperation of acute myocardial infarction for patients may enhance clinical improvement rate and improve the prognosis.</p>

ABSTRACT Objective: to analyze the knowledge and practices of nursing professionals in caring for people with colostomy bag. Method: a qualitative and descriptive study conducted with 21 nursing professionals from a General Surgery Unit. Data collection used triangulation of techniques, based on non-participant observation, analysis of nursing records, and semi-structured interviews. Analysis followed the spiral technique assumptions. Results: it was possible to comprehend the theoretical and scientific knowledge that subsidize practice; identify contradictions related to statement and care in the profession’s daily work and the intervening factors, which can facilitate and/or make the nursing care process difficult. Final Considerations: knowledge and practices of professionals in caring for ostomates occur within work experiences, in which socialization of knowledge makes it possible to expand the perspectives of care.

O Infarto Agudo do Miocárdio é uma das condições de saúde/doença mais prevalentes nos atendimentos de urgência e emergência, tornando-se necessário conhecer as respostas de cada indivíduo e seus principais diagnósticos de enfermagem existentes. Este estudo objetiva identificar um perfil de diagnósticos de enfermagem presente em pacientes com Infarto Agudo do Miocárdio atendidos na urgência/emergência. Trata-se de estudo descritivo, operacionalizado a partir de uma revisão integrativa de literatura. A coleta de dados foi realizada em junho de 2019, obteve 11 artigos, traçando 12 diagnósticos de enfermagem, a partir de 20 sinais e sintomas identificados. Concluiu-se que pacientes acometidos com infarto possuem diversos diagnósticos de enfermagem, relacionados principalmente aos processos fisiológicos, sistema respiratório, cardiovascular, hidratação, além do psicológico, devendo o enfermeiro atentar-se para esses aspectos, subsidiando um cuidar holístico.Descritores: Diagnósticos de enfermagem. ObjectivesTo understand identity tensions experienced by health professionals when patient partners join a quality improvement committee.DesignQualitative ethnographic study based on participatory observation.SettingAn interdisciplinary quality improvement committee of a Canadian urban academic family medicine clinic with little previous experience in patient partnership.ParticipantsTwo patient partners, seven health professionals (two family physicians, two residents, one pharmacist, one nurse clinician and one nurse practitioner) and three members of the administrative team.Data collectionData collection included compiled participatory observations, logbook notes and semi-structured interviews, collected between the summer of 2017 to the summer of 2019.Data analysisGhadiri’s identity threats theoretical framework was used to analyse qualitative material and to develop conceptualising categories, using QDA Miner software (V.5.0).ResultsAll professionals with a clinical care role and patient partners (n=9) accepted to participate in the ethnographic study and semi-structured interviews (RR=100%). Transforming the ‘caregiver–patient’ relationship into a ‘colleague–colleague’ relationship generated identity upheavals among professionals. Identity tensions included competing ideals of the ‘good professional’, challenges to the impermeability of the patient and professional categories, the interweaving of symbols associated with one or the other of these identities, and the inner balance between the roles of caregiver and colleague.ConclusionThis research provides a new perspective on understanding how working in partnership with patients transform health professionals’ identity. When they are called to work with patients outside of a simple therapeutic relationship, health professionals may feel tensions between their identity as caregivers and their identity as colleague. This allows us to better understand some underlying tensions elicited by the arrival of different patient engagement initiatives (eg, professionals’ resistance to working with patients, patients’ status and remuneration, professionals’ concerns toward patient ‘representativeness’). Partnership with patients imply the construction of a new relational framework, flexible and dynamic, that takes into account this coexistence of identities.

Copeptin is currently understood as a quantitative marker of endogenous stress. It rises rapidly in multiple acute disorders including acute myocardial infarction. As a single variable, it has only modest diagnostic accuracy for acute myocardial infarction. However, the use of copeptin within a dual-marker strategy together with conventional cardiac troponin increases the diagnostic accuracy and particularly the negative predictive value of cardiac troponin alone for acute myocardial infarction. The rapid rule-out of acute myocardial infarction is the only application in acute cardiac care mature enough to merit consideration for routine clinical care. However, the dual-marker approach seems to provide only very small incremental value when used in combination with sensitive or high-sensitivity cardiac troponin assays. This review aims to update and educate regarding the potential and the procedural details, as well as the caveats and challenges of using copeptin in clinical practice.

Objective: to understand family members’ perceptions of nursing care for people with psychiatric symptoms. Methods: qualitative study, with 13 family members, in five clinical inpatient units linked to the clinical nursing service of a general hospital. Data were collected through semi-structured interviews. Results: the challenges perceived by family members were turnover of professionals in the work schedules and difficulty in specialized management. The participants suggested training of the team, greater multi-professional integration, improved management and reduced turnover of professionals during care. Conclusion: family members verbalized difficulties and suggestions to support reflection on the care offered to people with psychiatric symptoms hospitalized in clinical units, in order to improve work practices and qualify care.

Introduction: Nurses who work in an emergency department regularly care for acute patients in a fast-paced environment, being at risk of suffering high levels of burnout. This situation makes them especially vulnerable to be accountable for decisions they did not have time to consider or have been pressured into. Research objective: The objective of this study was to find which factors influence ethical, legal and professional accountability in nursing practice in an emergency department. Research design: Data were analysed, codified and triangulated using qualitative ethnographic content analysis. Participants and research context: This research is set in a large emergency department in the Midlands area of England. Data were collected from 186 nurses using participant observation, 34 semi-structured interviews with nurses and ethical analysis of 54 applicable clinical policies. Ethical considerations: Ethical approval was granted by two research ethics committees and the National Health Service Health Research Authority. Results: The main result was the clinical nursing accountability cycle model, which showed accountability as a subjective concept that flows between the nurse and the healthcare institution. Moreover, the relations among the clinical nursing accountability factors are also analysed to understand which factors affect decision-making. Discussion: The retrospective understanding of the factors that regulate nursing accountability is essential to promote that both the nurse and the healthcare institution take responsibility not only for the direct consequences of their actions but also for the indirect consequences derived from previous decisions. Conclusion: The decision-making process and the accountability linked to it are affected by several factors that represent the holistic nature of both entities, which are organised and interconnected in a complex grid. This pragmatic interpretation of nursing accountability allows the nurse to comprehend how their decisions are affected, while the healthcare institution could act proactively to avoid any problems before they happen.
