Intrusive Luxation in Primary Teeth: A Case Report

Background: Intrusive luxation, also called central dislocation, is often the result of an axial impact in the apical direction, moving the tooth within the alveolar bone. This is possibly one of the most traumatic injuries, since the displacement of a tooth within its socket implies extensive and acute involvement of the periodontal ligament, bone damage and rupture of the neurovascular bundle. The close relationship between the apexes of the primary teeth and the developing permanent successor in turn can lead to multiple complications, which are greater when the permanent tooth is affected in the early stages of development. Methods: The present paper describes a case of intrusion in primary teeth and its evolution following initial diagnosis as avulsion at the time of trauma. Results: The upper right central incisor, initially diagnosed as avulsed, was lodged in the floor of the right nostril, and was surgically extracted through the nose. Conclusions: In cases of intrusion of primary teeth, it is essential to diagnose the direction of the displaced tooth to rule out injury to the successor and thus prevent the occurrence of sequelae in the permanent dentition.

Background: An injury to both the primary and permanent teeth and the supporting structures is one of the most common dental problems seen in children. Splinting is usually difficult or impossible to perform in the primary dentition (due to diminutive room size and lack of patient cooperation). Healing must, therefore, occur despite mobility at the fracture line, usually resulting in interposition of connective tissue. In some instances, infection will occur in the coronal pulp. The present study reported a case of trauma to the anterior primary teeth and alveolar bone in a four year old child. The trauma has caused fracture to the crowns and roots of the primary anterior teeth. The following case was managed in a procedure that may provide primary teeth subjected to trauma a better chance than extraction with a better prognosis. Case presentation: a 4 and a half year old child was subjected to trauma in anterior segment of maxilla. Suturing of the torn soft tissue was the first step followed by pulpotomy for the left primary lateral incisor. Fixation of the right primary central and lateral incisors was done by acid etch wire fixation. Both clinical and radiographic follow up was carried out for 6.4 years. Results: healing of the soft tissue was observed after one week and completed after two months. Fixation of the teeth continued for ten months. The fracture lines in the roots remained in position. Clinically there was no sign of any pulpal inflammation or necrosis. Radiographically, no signs of infection to the surrounding tissues could be seen, no resorption in the alveolar bone, external or internal resorption of the root did not happen also. After ten months fixation ended and the wire was removed. At that time there was normal resorption of the roots of the primary incisors in relation with the normal development of the permanent incisors. After 3 years both permanent central incisors erupted in their normal position. After 6.4 years all four permanent incisors erupted into occlusion in their normal position. Conclusion: primary teeth with root fractures and severely mobile coronal fragments can be treated by a conservative approach. The severity of the sequels is directly related to the degree of permanent tooth formation (child’s age), type of dental trauma and extent of the impact. Most frequent trauma to primary dentitions are injures to the supporting tissue i.e. luxations, but root fractures are relatively uncommon. The difference in the trauma pattern favoring luxation rather than fracture has been found to be typical for the primary dentition, since the elasticity of the alveolar bone surrounding the primary teeth is high and primary tooth held in alveolar socket is less strong. Incidence of foot fractures increases at the age of 3-4 years where physiologic root resorption has begun, thereweakening the root. Traumatic injuries to the primary dentition present special problems and the management is often different as compared with the permanent dentition. Because of potential sequelae of trauma to primary teeth, a treatment method that minimizes any additional risks of further damage to the permanent successors should be selected. The report presents a case of 3.5 year-old boy after traumatic injury – extrusion of tooth 51, which required surgical treatment, and apical third horizontal root fracture of tooth 61 treated conservatively. Written consent from subject’s parents/subject’s legal guardians was obtained. Root-fractured primary tooth has been followed clinically and radiographically for 3.5 year, till advanced root resorption. Presented case and cases described in cited literature demonstrate that conservative treatment of root-fractured primary teeth is a favorable method of the treatment that allows the teeth to function naturally until the term of physiological resorption and exfoliation.

Injuries to the teeth and surrounding structures are relatively common. Although traumatic injuries caused by falls or activities related to sports are widely discussed, the same cannot be said regarding accidents arising from non-professional extraction of primary teeth. The present study reports a 6-year-old male child who underwent mandibular alveolar bone fracture during non-professional extraction of his central lower left incisor at home, performed by his 30-year-old aunt. The root of the tooth was with an irregular physiological resorption, which acted as a lever component for the mechanical force applied, leading to bone fracture. Although not common, the possibility that dental roots with irregular resorption can act as a possible risk factor for accidents if the parents or guardians of children during the period of transitional dentition try to perform intentional extraction of primary teeth should be highlighted. Parents should always consult a professional, preferably a pediatric dentist, for monitoring this period of transitional dentition.

Preschool children are frequently affected by traumatic dental injuries, most of them during the first three years of life. Due to the resiliency of the alveolar bone surrounding the primary teeth, most of these injuries fit into tooth luxations category. In this paper, two different cases of early loss of primary incisors resulting from luxations are described. Removable esthetic space maintainers were placed using the natural crown of the traumatized teeth and the children were evaluated over a 12 month period. The treatment given could be considered an excellent rehabilitation option since it restores the esthetics and masticatory function,allows speech development, benefits oral hygiene and prevents the establishment of tongue habits and malocclusions.

AbstractA growing number of individuals using online forums are attempting to abstain from pornography (colloquially termed “rebooting”) due to self-perceived pornography-related problems. The present qualitative study explored phenomenological experiences of abstinence among members of an online “rebooting” forum. A total of 104 abstinence journals by male forum members were systematically analyzed using thematic analysis. A total of four themes (with a total of nine subthemes) emerged from the data: (1) abstinence is the solution to pornography-related problems, (2) sometimes abstinence seems impossible, (3) abstinence is achievable with the right resources, and (4) abstinence is rewarding if persisted with. Members’ primary reasons for initiating “rebooting” involved desiring to overcome a perceived addiction to pornography and/or alleviate perceived negative consequences attributed to pornography use, especially sexual difficulties. Successfully achieving and maintaining abstinence was typically experienced to be very challenging due to habitual behavior patterns and/or cravings triggered by a multiplicity of cues for pornography use, but a combination of internal (e.g., cognitive-behavioral strategies) and external (e.g., social support) resources made abstinence attainable for many members. A range of benefits attributed to abstinence by members suggest that abstaining from pornography could potentially be a beneficial intervention for problematic pornography use, although future prospective studies are needed to rule out possible third variable explanations for these perceived effects and to rigorously evaluate abstinence as an intervention. The present findings shed light on what the “rebooting” experience is like from members’ own perspectives and provide insights into abstinence as an approach for addressing problematic pornography use.

Abstract An impacted or missing permanent tooth can add significant complications to an otherwise straightforward case. When multiple impacted teeth are present, the case complexity increases further. Developing a treatment sequence, determining appropriate anchorage, and planning and executing sound biomechanics can be a challenge. The following case report illustrates a patient reportedly diagnosed with mild scleroderma as an adolescent. He presented for orthodontic treatment as an adult with multiple retained primary teeth and multiple impacted teeth. Diagnosis, treatment planning, and various methods of managing guided eruption of impacted teeth will be discussed. Following orthodontic treatment that required extraction of multiple primary and permanent teeth as well as exposure and ligation of multiple permanent teeth by an oral surgeon, the patient finished with a significantly improved functional and esthetic result.

Traditional Chinese medicine (TCM), an alternative medicine, focuses on the treatment of human disease via the integrity of the close relationship between body and syndrome analysis. It remains a form of primary care in most Asian countries and its characteristics showcase the great advantages of personalized medicine. Although this approach to disease diagnosis, prognosis and treatment has served the medical establishment well for thousands of years, it has serious shortcomings in the era of modern medicine that stem from its reliance on reductionist principles of experimentation and analysis. In this way, systems biology offers the potential to personalize medicine, facilitating the provision of the right care to the right patient at the right time. We expect that systems biology will have a major impact on future personalized therapeutic approaches which herald the future of medicine. Here we summarize current trends and critically review the potential limitations and future prospects of such treatments. Some characteristic examples are presented to highlight the application of this groundbreaking platform to personalized TCM as well as some of the necessary milestones for moving systems biology of a state-of-the-art nature into mainstream health care.

<p>The genus Cyprideis is one of the most widespread ostracod representative of the Pleistocene brackish palaeoenvironments. Especially <em>Cyprideis torosa </em>is often found in great numbers and even in monospecific taphocoenoses and for this reason its study is very useful for palaeoenvironmental reconstructions.  The identification of different species of Cyprideis is often complicated and needs careful morphology inspection. This becomes even more difficult in the case of endemic species which present significant similarities with each other.  In this work, we have studied and analyzed several  Cyprideis species (<em>C.torosa, C. frydaci, C.dictyoti, C. pannonica, C. elisabeta, C. seminulum, C. heterostigma</em>) deriving from brackish palaeoenvironments of a Lower Pleistocene marl sequence in Sousaki Basin (Northeastern Corinth Graben, Greece). More specifically size measurements and geometric morphometrics (lateral valve outline of both right and left valves as well as females and males) were used in order to attest the similarities and dissimilarities between the different species and draw conclusions about their origin.  According to the valve outline and the multivariate analysis a close relationship between the valve shape of all Cyprideis species can be noticed. <em>C. torosa</em> is commonly grouped with <em>C. pannonica</em> except in the male right valve where the two species show some differences.  The endemic species <em>C. frydaci</em> and <em>C. dictyoti</em> can be identified by the differences in the right valve of the male and female respectively.  The other species could not be substantially differentiated using just the outline analysis which possibly denotes their common genetic origin.  The valve outline has proved to be a very useful character for recognizing the different species especially when the two valves of both females and males are considered. More analyses of representative species of Miocene and Pliocene Cyprideis are needed in order to establish their phylogenetic relationships and draw conclusions about their common ancestor.</p>

С помощью гистологических методов окраски толуидиновым синим, гематоксилином и эозином и иммуногистохимических реакций на белок PGP 9.5, тирозингидроксилазу (ТГ), белок Iba-1, изучены клеточные изменения в разных отделах сердца крыс линии Wistar в возрасте 18- 23 мес. В соединительной ткани основания сердца обнаружены очаговые воспалительные инфильтраты, внутри которых выявлены PGP 9.5 и ТГ сплетения, состоящие из парасимпатических и симпатических нервных волокон. В области клапанного аппарата, на границе фиброзного кольца и миокарда правого предсердия, обнаружены патологические изменения нервных структур - дегенерация нервных пучков и зернистый распад варикозных аксонов терминального сплетения. Установлены тесные взаимоотношения аксонов терминальной нервной сети с клетками воспалительных инфильтратов и кровеносными сосудами. Определены закономерности встречаемости в различных отделах миокарда у старых животных нейроклеточных воспалительных комплексов, состоящих из нервных волокон, кровеносных капилляров и клеток-участников местного воспалительного процесса (тучных клеток, макрофагов, фибробластов, плазмоцитов). Установлен хронический характер нейрогенного воспаления в сердце при старении. Using histological methods of staining with toluidine blue, hematoxylin-eosin and immunohistochemical reactions for the PGP 9,5 protein, tyrosine hydroxylase (TH), Iba-1 protein, cellular changes in different parts of the heart of Wistar rats at the age of 18-23 months were studied. In the connective tissue of the heart base, focal inflammatory infiltrates were found, near which PGP 9.5 and TH plexuses, consisting of parasympathetic and sympathetic nerve fibers, were detected. In the area of the valvular heart apparatus, at the border of the anneau fibreux and the myocardium of the right atrium, pathological changes in nerve structures were found: degeneration of nerve fibers and granular destruction varicose axons of the terminal plexus. A close relationship has been established between axons of the terminal nervous network and cells of inflammatory infiltrates and blood capillaries. The features of the localization of neurocellular inflammatory complexes consisting of nerve fibers, blood capillaries and cells participating in the local inflammatory process (mast cells, histiocytes, monocytes, fibroblasts, plasma cells) in various parts of the myocardium in old animals are described. The chronic nature of neurogenic inflammation in the heart during aging has been established.
