Problematic use issues military medical terminology in the disaster medicine service ministry of defense of the Russian Federation

The health care of the Russian Federation and its medicine of accidents industry were undergoing systemic changes that required, inter alia, reappraisal of approaches to the work of specialist doctors in federal ministries, agencies and services in the implementation of joint activities during the elimination of medical and sanitary consequences of emergencies of various genesis. It is noted that during interpersonal communication, medical specialists often encounter various understandings and paths of special professional terms, which often leads to a decrease in the quality of organization of interaction and management. A particular feature of the use of military-medical terminology in the Disaster Medicine Service of the Russian Federation was its mixing with the terminological definitions of specialists from other federal ministries, agencies and services, which in some cases significantly complicates management communication. A scientific search has been carried out using content analysis, rank-based dispersion analysis of data, sociological and linguistic analysis to study the most used terms that characterize such concepts as wounded, sick and affected, injured, patient, medical and sanitary care, treatment and evacuation support, medical care, and terms characterizing types of care. The most significant, according to experts, terms that reflect the practical activities of medical specialists and allow them to master at the proper level the training programs for personnel of higher qualifications in the field of disaster medicine were the terms affected in an emergency, patient (3,770,82 points), as well as treatment and evacuation support (4,670,55 points) and medical support (4,21,1 points). Meaningful terminological contradictions that can lead to a violation of organizational integrity are also identified of the system of medical and evacuation support of troops (forces) and the population during the elimination of medical and sanitary consequences of emergency situations by the forces and means of the medical service of the Armed Forces of the Russian Federation as not always reflecting the specific conditions of activity and organization of interaction of medical specialists of federal ministries, agencies and services. All this requires harmonization (bringing to a common understanding) of the relevant terminology for the application of military-medical terminology in the Disaster Medicine Service of the Russian Federation.

The results of study and analysis of organizational and functional changes in the Disaster Medicine Service of the Ministry of Health of Russia at the regional level, organizational models of the Disaster Medicine Service functioning in the subjects of the Russian Federation are presented. The study was conducted at the All-Russian Center for Disaster Medicine “Zashchita” (since 2015) and at the Federal Center for Disaster Medicine of the National Medical and Surgical Center named after N.I. Pirogov of the Ministry of Health of Russia. The purpose of the study is to analyze organizational models of the Disaster Medicine Service functioning at the regional level. Materials and research methods. The following methods were used in the study: expert evaluation, statistical, analytical, field observation, etc. The method of expert evaluation was applied through questionnaire survey of experts – Russian experts in the field of disaster medicine and emergency medical care. The questionnaires were filled in twice – questionnaire №1 – in 2015 and questionnaire №2 – in 2019-2020; a total of 529 people took part. Questionnaires № 1 and № 2 included questions and provisions relating to organizational models of the Disaster Medicine Service functioning at the regional level and to organizational and functional changes in the Disaster Medicine Service of the subjects. Results of the study and their analysis. The results of the research helped to formulate a number of principle statements on further improvement of the Disaster Medicine Service, including development of organizational structure, implementation of its organizational models at the regional level.

The article presents the history of creation, formation and development of the State Scientific Center of the Russian Federation — Federal Medical Biophysical Center named after A.I. Burnazyan of the Federal Medical and Biological Agency of Russia (A.I. Burnazyan Federal Biophysical Center, the Center). The Institute of Biophysics of USSR Ministry of Health and Clinical Hospital № 6, predecessors of the Center, were engaged in the elimination of medical and sanitary consequences of Chernobyl Radiation Accident (1986). The main directions of activities of the A.I. Burnazian Federal Medical Biophysical Center — the flagship institution of Russian health care in the field of biophysics, radiation and nuclear medicine are considered. The perspectives of scientific activity of the Center related to solving actual problems of modern radiobiology, radiation safety and biomedical technologies are outlined. It is concluded that it is expedient to create the Disaster Medicine Service of the Federal Medical and Biomedical Agency of Russia.

This article reveals the problem of the role of the religious factor in the formation of the value orientations of the military personnel of the Russian army from its inception to the present state. In the article, the author reveals the significance of the Church in the formation of the value orientations of military personnel. The problem of religious situation in foreign armies is considered. The article presents data from sociological studies confirming the increase in the number of religious servicemen in the modern Armed Forces. The concept of “religious factor” is revealed. The author suggests considering the influence of the religious factor on the formation of value orientations through the functions of religion. The article provides examples of the influence of religion on the formation of value orientations of military personnel from the time of the Baptism of Russia to the present. Examples of writers of Russian classical literature about the influence of religion on the morale of troops are given. Examples of religious participation in major battles and wars of the past years are shown. The significance of the religious factor in uniting the people and the army is shown. The work of officials of the Ministry of defense of the Russian Federation in strengthening values among military personnel in modern conditions is demonstrated. The role of the Minister of defense of the Russian Federation, General of the army S.K. Shoigu in strengthening the faith of the Russian army is outlined. Issues related to the construction of the Main Temple of the Armed Forces and its impact on the public masses were discussed. In this study, the author aims to show the significant role of religion in the formation of value orientations in Russian military personnel. The analysis shows an increasing role of religion in the minds of military personnel in modern conditions.

Case law in Russia is Advisory in nature and is not mandatory for law enforcement practice. Courts use the signs of case law in their decisions in the reasoned part. Signs of case law is a ruling of the constitutional court of the Russian Federation and regulations of the armed forces of the Russian Federation.

In the context of the reform of the Armed Forces of the Russian Federation and a significant increase in the proportion of military personnel doing military service under contract, the issues of manning the troops with healthy, physically developed citizens with high moral and business qualities are of paramount importance. Of particular importance in the selection of candidates for military service under the contract is the conduct of laboratory and instrumental studies, professional and psychological selection, determination of the level of citizens’ physical fitness. The Federal Law «On Military Duty and Military Service» defines a two-stage system for medical examination of citizens entering military service under contract, which is necessary as a barrier in order to prevent citizenswho do not meet the necessary requirements for military personnel from entering the Russian Armed Forces. At the first stage (preliminary examination), the military and medical examination of citizens was carried out by specialist doctors working in medical organizations of the outpatient-polyclinic link of municipalities at the place of citizens’ permanent residence. Medical specialists of the regular military medical commission of the military commissariat of the constituent entity of the Russian Federation participated in the second stage (final examination) of the military medical examination. During the five-year period under study, 5,133 citizens (72.9 %) were selected out of 7,043 candidates for military service under contract, who fully met all the criteria for defenders of the Fatherland.

The article raises the issue of state sovereignty in a federal state and reveals its legal nature. The authors draw attention to the diversity of approaches to the concept and essence of sovereignty, reveal its correlation with related categories, describe the concepts of unity and divisibility of state sovereignty. The paper proves that sovereignty is not a quantitative, but a qualitative characteristic of a state, which is either present or not. The authors substantiate the exclusive possession of state sovereignty by the Russian Federation. Based on the analysis of the doctrinal, regulatory sources and the practice of the Constitutional Court of the Russian Federation, the authors show that the Russian constitutional model explicitly outlines the principle of solid and indivisible state sovereignty spreading throughout the whole territory of the Russian Federation. Recognition of the principle of state sovereignty of Russia presupposes a clear definition of the scope of rights that the Federation should possess in order for its sovereignty to be ensured. The article examines the main features of the state sovereignty of Russia enshrined in the Constitution of the Russian Federation, among which are the supremacy of federal law over the law of the subjects of the Federation, the inviolability of borders and territorial integrity, the unity of the economic space, fiscal, banking and monetary systems, common army (Armed Forces), the right of the state to protect its sovereignty and rights of citizens. Despite the unequivocal decision on the integrity of state sovereignty of the Russian Federation expressed the Constitution of the Russian Federation and by the Constitutional Court of the Russian Federation, this fundamental principle is not completely ensured since the idea of the sovereignty of the republics as components of Russia continues to retain its potential threat to Russian federalism, taking into account the provisions of Art. 73 of the Constitution of the Russian Federation that provide for the full state power of the constituent entities of the Russian Federation

Relevance. One of the important tasks in the provision of emergency medical care to victims of the mass-causalty incident is a medical triage in order to determine the priority of medical care and the priority of medical evacuation.Intention. To work out issues of interaction between the medical service of the Armed Forces of Russia, the Disaster Medicine Service and the Ambulance Service.Methodology. The program of the 16th All-Russian Congress “Ambulance 2017”, tactical and special exercises on the deployment of airfield evacuation center were included as a training and demonstration event . According to the plan of the exercise, several mass-causalty areas resulted from terrorist attacks along the border of the Leningrad Region.Results and discussion. To accomplish the tasks, an evacuation center was deployed with a capacity of up to 200 wounded and sick per day, evacuation capability of 32 stretcher wounded, and 4 h deployment time. During the exercise, non-invasive screening studies to proactively diagnose intracranial hematomas (portable infrared scanner), tension pneumothorax, hemopericardium, intrapleural and intra-abdominal bleeding (portable US device using FAST protocol), acute blood loss (portable laboratory analyzer) were performed in addition to standard examinations. To determine the priority of medical evacuation of victims, an improved Revised Trauma Score (RTS) scale with user-friendly software for Android mobile devices (smartphone, tablet) was used. In case of mass-causalty event and a shortage of specialized sanitary transport, those who needed urgent specialized medical care were transferred from an evacuation center via radial evacuation to specialized medical organizations subordinated to the Ministry of Defense, the Ministry of Health and the subjects of Russia.Conclusion. The feasibility of deploying an airfield evacuation center to eliminate the health effects of emergencies was determined by potential discrepancy between evacuation capacities of air transport and ambulances, on one hand, and the need to temporarily accommodate (including isolation) and provide emergency health care to victims.
