The author pointed out that heart noises of functional nature lose their character when the forearm is squeezed by the cuff, approaching to normal tones, while noises of organic origin intensify under these conditions. The same applies to vascular valves.

P117 A recent report by Tiret et al. (Hypertension 33, 1999) revealed that a G/T polymorphism with an amino acid substitution (Lys to Asn) at codon 198 in the exon 5 of endothelin 1 gene (ET1) is associated with blood pressure in overweight people using two epidemiological studies, ECTIM and Glasgow Heart Scan Study. They suggested that G/T polymorphism strongly interacted with body mass index (BMI) in the determination of blood pressure levels. To examine the interaction among G/T polymorphism of ET1, BMI and blood pressure, we carried out an association study using a general population. Subjects (n=1,446) were recruited from Ohasama population, which is a cohort in a rural community of northern Japan. The research protocol was approved by the Institutional Review Board of the Tohoku University. DNA was extracted from the buffy coat of the participants using QIAamp DNA Blood Kit (Qiagen Inc.). G/T polymorphism of ET1 was determined by TaqMan PCR method, which is a powerful tool for semiautomatic genotype determination in a large number of samples. The frequency of T198 allele in Japanese (26%) was significantly higher than that in Caucasians (23%). The baseline characteristic (age, BMI, SBP, DBP, antihypertensive treatment) of all subjects was not significantly different according to the genotype of G/T polymorphism. In the obese subjects (≥25kg/m2), however, SBP and DBP were significantly associated with G/T polymorphism. Blood pressure level in the subjects carrying T198 allele was 2.6 mmHg in systolic (p<0.02) and 2.3 mmHg in diastolic (p<0.005) higher than that in those with GG genotype in overweight people. That the same result was obtained from different races suggested that the T198 allele of ET1 is involved in the determination of blood pressure levels in obese subjects.

Objective: This longitudinal prospective observational type study was conceived with the aim to examine the impact on renal resistive index (RRI) of the variables that we can manipulate with therapeutic and or dietetic interventions in a chronic kidney disease population in order to known which of these variables was statistically related to changes in RRI and therefore could become the object of the greatest therapeutic effort. Material and methods: This study was undertaken between May 2016 to May 2017 in the outpatient nephrology and urology clinic of San Donato Hospital in Arezzo. The study population (84 patients: 47 males and 37 females) was randomly selected among the chronic kidney patients (with various degrees of renal impairment) affected by hypertension and or diabetes mellitus. After a comprehensive medical examination these patients were submitted to determination of serum creatinine, glycated hemoglobin, 24-hour urinary albumin excretion and finally renal Doppler ultrasonography. Then the patients were submitted to a full therapeutic and dietetic intervention to ameliorate the renal impairment by a wide range of actions and after on average a one-year interval were submitted again to a new medical examination and a second determination of serum creatinine, glycated hemoglobin, 24-hour urinary albumin excretion and a new renal Doppler ultrasonography too. Results: The comparison between basal and final data revealed a slight reduction in the mean of bilateral renal resistance indices (Delta RRI: -0.0182 ± 0.08), associated to a slight increase in the mean glomerular filtration rate (Delta GFR: 0.8738 ± 10.95 ml/min/1.73 m2), a reduction in mean body weight (Delta weight: -1.9548 ± 5.26 Kg) and mean BMI (Delta BMI: -0.7643 ± 2.10 Kg/m2) as well as a reduction in the mean systolic blood pressure (Delta systolic blood pressure: -8.8333 ± 25.19 mmHg). Statistical analysis showed statistically significant correlations (p < 0.05) between Delta RRI and Delta weight (p < 0.03), Delta BMI (p < 0.02) and Delta systolic blood pressure (p < 0.05). Conclusion: Despite the many limitations the our study clearly identifies the targets (yet widely known) to act on to prevent kidney alterations related to RRI and provides further evidence, if any, of the utility of RRI as a key parameter in monitoring patients with chronic renal failure and as a valuable tool to drive the clinical efforts to contrast the kidney disease.

Background: Hypertension is the silent disease which a person will not know if he has increased blood pressure. The incidence rate of hypertension is very high especially in the elderly population over 60 years. Hypertension management can be done in pharmacological ways such as medicines and can use natural ingredients in lowering blood pressure that is by utilizing the noni fruit.Objectives: This study aimed to analyze the effectiveness of Noni fruit tea in lowering elderly blood pressure with hypertensionMethod: This research was quasy experimental throug with Pretest-Postest Control Group Design design. Determination of research sample by using inclusion and exclusion criteria. The sample of the study was 34 respondents divided into two groups. Intervention of Noni fruit tea given as much as 5 gram for 30 days with frequency of giving 2 times a day in the morning and afternoon. Blood pressuremeasurement was performed at the beginning of the study and at the end of the study. Data were analysed using wilcoxon test and Mann Whitney test.Result: Mann Whitney test results showed that there was significant effect after the intervention of noni fruit tea in treatment group compared with control group with systolic blood pressure value (p =0.000171) and diastolic blood pressure (p = 0.000480) with α = 0.05Conclusions: Blood pressure in the treatment group after the intervention of noni fruit tea decreased blood pressure compared with the control group. It is Suggested of research, need to do periodic nutrition education and giving noni fruit tea as alternative medicine that effectively decrease hypertension, scheduled.ABSTRAKLatar Belakang : Hipertensi merupakan the silent disease dimana seseorang tidak akan mengetahui jika dirinya mengalami peningkatan tekanan darah. Angka insiden hipertensi sangat tinggi terutama pada populasi lanjut usia diatas 60 tahun. Penatalaksaan hipertensi dapat dilakukan dengan cara farmakologis seperti obat-obatan serta dapat menggunakan bahan alami dalam menurunkan tekanan darah yaitu dengan memanfaatkan buah mengkudu.Tujuan : Penelitian ini bertujuan menganalisis efektifitas teh buah mengkudu dalam menurunkan tekanan darah lansia dengan hipertensiMetode : Penelitian ini bersifat quasy eksperimental dengan desain Pretest-Postest Control Group Design. Penentuan sampel penelitian dengan menggunakan kriteria inklusi dan eksklusi. Sampel penelitian sebanyak 34 responden yang dibagi menjadi dua kelompok. Intervensi teh buah mengkudu diberikan sebanyak 5 gram selama 30 hari dengan frekuensi pemberian 2 kali sehari pada pagi dan sore hari. Pengukuran tekanan darah darah dilakukan pada awal penelitian dan di akhir penelitian. Analisis data penelitian menggunakan uji wilcoxon dan uji Mann Whitney.Hasil : Hasil uji Mann Whitney menunjukkan bahwa terdapat pengaruh yang signifikan sesudah intervensi teh buah mengkudu pada kelompok perlakuan dibandingkan kelompok kontrol dengan nilai tekanan darah sistolik (p=0.000171) dan tekanan darah diastolik (p=0,000480) dengan α=0.05Kesimpulan : Tekanan darah pada kelompok perlakuan susudah intervensi teh buah mengkudu mengalami penurunan tekanan darah dibandingkan dengan kelompok kontrol. Saran penelitian, perlu dilakukan pendidikan gizi secara berkala dan pemberian teh buah mengkudu sebagai obat alternatif yang efektif menurunkan hipertensi, secara terjadwal.

Introduction. Erectile dysfunction (ED) combines many conditions leading to erectile dysfunction and disability to perform sexual intercourse. It was confirmed that the most common cause of ED is vascular disorders, primarily associated with atherosclerosis that leads to cardiovascular diseases. Objective. To analyze the available literature data about the methods for diagnosis vascular ED, including according to cardiovascular system status. Materials and methods. The results of the search in scientific databases Medline, EMBASE, Cochrane, Chinese BioMedical Literature Database, China National Knowledge Infrastructure, Elibrary.ru were analyzed. After a detailed verification of the reliability of sources, impact factors of journals and the sequence of presentation of the material, 63 most relevant scientific publications were selected directly for citation. Results. The first step of the vascular ED diagnosis is the monitoring of nocturnal penile tumescences. Next, the risk factors for cardiovascular diseases and ED are evaluated: anamnesis of elevated blood pressure, physical activity, smoking, body weight, measurement of blood pressure, blood analysis with the lipid and glycemic profiles. For the diagnosis of endothelial dysfunction (END), the determination of flow-dependent vasodilation of the brachial artery is used, with the possible determination of the rate of blood filling after decompression. After the END is confirmed in patients with ED, it is useful to determine the vascular age using the SCORE scale. For further management of the patient with ED and END, stratification of the associated with sexual activity cardiovascular complications risk is recommended. Conclusion. The proposed diagnostic algorithm will allow not only identify the vascular component of ED at an early stage, but also to avoid fatal cardiovascular complications by prescribing and recommending therapeutic and preventive measures to patients with asymptomatic cardiovascular diseases.

Abstract Background: Homocysteine (Hcy) is considered to be an independent risk factor for cardiovascular and cerebrovascular diseases. No study has evaluated the distribution of Hcy on a large-scale health examination. Accordingly, this study aimed to investigate the level and distribution of Hcy in the healthy physical examination population and the correlation with other biomarkers, and analyzed for cardiovascular and other diseases. The prevention provides an important scientific basis.Methods: From February 2017 to April 2020, 8063 medical examination populations were selected for analysis. Determination of serum Hcy, TC, TG, LDL-c, HDL-c, ALT, ALP, γ-GT, TBIL, GLU, urea, Cr, UA and related metabolic risk factors. According to the multivariate regression model of age, gender, smoking, drinking, body mass index (BMI), systolic blood pressure (SBP) and diastolic blood pressure (DBP), the relationship between Hcy and other biochemical indicators was evaluated. Results: Among 8063 cases, the age, BMI, SBP and DBP of the high-Hcy group were higher than those of the low-Hcy group, the difference was statistically significant (P<0.05), and the proportion of males, smoking and drinking were higher than the low In the Hcy group, the difference was statistically significant (P<0.05); the ALT, ALP, γ-GT, TBIL, Urea, Cr, UA, and TG in the high Hcy group were higher than those in the low Hcy group, and the difference was statistically significant (P<0.05 ); HDL-c in the high-Hcy group was lower than that in the low-Hcy group, and the difference was statistically significant (P<0.05). There was no statistically significant difference in TC, LDL-c, and GLU between the high- and low-Hcy groups (P>0.05). In multivariate analysis, lnHDL-C was negatively correlated with lnHcy (β=-0.038, SE=0.016, P<0.05), lnCr was positively correlated with lnHcy (β=0.055, SE=0.016, P<0.05), lnUA and lnHcy were positive correlation (β=0.043, SE=0.019, P<0.05). Conclusion: Hcy is closely related to HDL-c, Cr and UA, which indicates that Hcy may affect the metabolism of HDL-c and UA, and can also be used as an auxiliary diagnostic index for kidney injury.
