The destruction of the tooth crown should be regarded as the cause leading over time to the disruption of the morphofunctional unity of the dentition and pathological condition of the entire dentoalveolar system. Untimely treatment of defects of hard tooth tissues leads to functional disorganization of the dentoalveolar system. The most eliminated defects are those of crowns of teeth determining the quality of aesthetics, phonetics, functions of biting and chewing. The destruction of the tooth crowns, which do not cause significant disruption of the above functions, are painless and almost asymptomatic. In such cases, deformation of the dentition and occlusion, dysfunction of masticatory muscles and temporomandibular joints as well as the development of pathological changes in periodontium occur slowly. Of great importance are the early diagnosis of pathological reconstruction of dentoalveolar system and the knowledge of pathogenesis of possible complications after the extraction of the teeth that determine the indications for prophylactic orthopedic dental treatment. The article presents a clinical case of restoration of destructed tooth crown 3.6 complicated by changes in bifurcation area. The patient underwent coronary-radicular separation with the use of cast metal cores, which allowed restoration of the sufficient size of the stump, function and anatomical shape of the tooth and hence the unity of dental arch.

Amelogenesis imperfecta (AI) is a hereditary disorder expressing a group of conditions that cause developmental alterations in the structure of enamel. AI is a serious problem that reduces oral health-related quality of life and causes some physiological problems. The treatment of patients with AI may upgrade the quality of life and reinforce their self-esteem. Among the treatment options for AI, full-mouth metal reinforced porcelain restoration constitutes an important alternative because of its properties. This paper presents a case of AI of the hypoplastic rough type associated with a group of dental anomalies, and describes the prosthetic management of the patient. A 26-year-old female patient presented with a chief complaint of discolored teeth. Clinical and radiographic examination of the patient confirmed the diagnosis of rough pattern hypoplastic AI. The patient was treated with full-mouth metal reinforced porcelain fixed bridge. The adaptation of the temporomandibular joints and masticatory muscles was carefully observed periodically during 4 months and, after this period, the patient tolerated well her new vertical dimension. The patient received instructions on cleansing of the subpontic and interproximal areas. Follow-up visits were scheduled at 3 months and then at 6 months. No esthetic or functional problems were seen after the follow up period.

Endolymphatic hydrops (EH) is a pathological condition which is the final common manifestation of a variety of otologic insults. In this paper we develop a classification which, on the basis of clinical and pathological data, distinguishes symptomatic and asymptomatic forms. Clinical case histories and temporal bone studies are presented to illustrate and substantiate this classification. The symptomatic form becomes evident by the hallmark symptoms of episodic vertigo and fluctuating hearing loss. The asymptomatic form is silent. Inter-conversion from one form to another may occur over time. Each of the forms can be subdivided, according to etiology, into embryopathic, acquired, and idiopathic types. The embryopathic type comprises those cases in which some noxious influence interferes with prenatal labyrinthine development. The acquired type includes those cases in which a documented insult, be it inflammatory or traumatic, is suffered by a previously normal labyrinth. The inflammation may be viral, bacterial, or spirochetal in nature, while the traumatic event may be either accidentally or surgically sustained. The idiopathic type includes cases in which the instigating event precipitating the EH is unknown. Menière's disease represents merely one example of the EH group of disorders, namely symptomatic idiopathic EH.

Sickle cell anemia is a hereditary, monogenic blood disease characterized by chronic hemolytic anemia and vasoconstricting phenomena leading to acute painful crises and chronic and progressive tissue damage. It is the most common disease among hemoglobinopathies in Brazil and worldwide. The disease originated in Africa and was brought to the Americas by the forced immigration of slaves. Sickle cell patients have clinical conditions that can be intensified during dental treatment by precipitating vasoconstrictive seizures. In this clinical case, paleness was detected in the buccal buccal and the tongue despapilada. In the intra-oral radiographic examinations, areas suggestive of bone sclerosis and radiolucent areas between the root apices were found. Maxillary protrusion due to medullary expansion was found in lateral cephaladiography. In scintigraphy and magnetic resonance imaging, areas corresponding to bone infarcts and osteonecrosis were found. The dental surgeon performs an important function, being able to diagnose the oral manifestations through clinical and/or radiographic examination. By acting preventively, we can reduce the complications and improve the quality of lives of these patients.

Summary. Today, the population of Ukraine has a high need for dental treatment, in particular in the surgical treatment of carious lesions of the teeth by restoration of integrity of the tooth crown. Over the last 10–15 years we can note a tendency to increase the average patient's requirements for greater aesthetics of fillings and restorations of the frontal and masticatory groups of teeth. During performing modern dental work it has become almost mandatory to make the fillings and restorations as close as possible to the natural transparency of the tooth, to imitate its natural appearance by reproducing a whole palette of shades, cutting edge and microrelief. The appearance of photocomposite materials with a particle size of less than 1 μm, improvement of their grinding technique – obtaining spherical particles, improving the composition of the organic matrix of the material – allowed to develop additional optical characteristics of photocomposites – the «chameleon» effect. This is an optical mimicry of color and transparency of the material when placing it in the structure of the vital or devital tooth. An example of restoration of the frontal group of teeth in a patient with non-carious lesions of hard tissues is described. Teeth treatment was performed by the method of singlelayer restoration with photocomposite material of Ukrainian production «Jen-Radiance NEW». Objective To review the lessons learned from a longitudinal intercenter comparison study. Design Longitudinal cohort study. Setting Multidisciplinary cleft services in Northern Europe. Subjects Individuals with repaired complete unilateral cleft lip and palate. Main Outcomes Measures The first four papers in this series report amount of treatment, cephalometric form, nasolabial appearance, dental arch relationship, patient/parent satisfaction. This paper considers the consistency of outcome at the five centers over time, and other relationships between outcomes. Results Some outcomes measured in childhood can be predictive over time. The amount of treatment does not correlate with the quality of clinical outcome. Conclusions Measurement of clinical outcome in childhood is an important and valid form of clinical audit. Intercenter studies are more informative than single center reports, and will have an important future role in cleft care.

Abstract The immediate postextractional implant insertion is a specific way of surgical approach to implanto-prosthetic treatment. This article addresses the indications, advantages, and surgical techniques of immediate postextraction insertion, illustrated with a clinical case. Objectives: presentation of the indications, advantages and surgical procedure of immediate postextractional implant insertion. Materials and methods: The present study considered the presentation of a case in which we opted for the insertion of implants immediately postextractionally as a therapeutic attitude. Results and discussions: The advantages and disadvantages of this therapeutic technique were represented as follows. The advantage of this technique is given by the shortening of the time necessary to complete the treatment until the prosthetic phase, the disadvantage being represented by the insertion of the implant in positions sometimes less advantageous to the prosthesis. Also, the fact that we could not preserve the alveolar ridge, keeping the bone volume necessary to insert the implant in the best position of the prosthesis by complying with the biomechanical principles, made over time, a series of complications that occurred due to vicious insertion of the implant and non-compliance with those biomechanical principles. Conclusions: The analysis of the case as a whole, represents the main premise from which any dental treatment must be started, so that the functionality of the prosthetic and aesthetic reconstruction will have to be taken into account when we choose the best treatment option.

Background Temporomandibular disorders (TMD) is an umbrella term for pain and dysfunction involving the masticatory muscles and the temporomandibular joints (TMJs). TMD is the most common orofacial pain condition. Its prominent features include regional pain in the face and preauricular area, limitations in jaw movement, and noise from the TMJs during jaw movements. TMD affects up to 15% of adults and 7% of adolescents. Chronic pain is the overwhelming reason that patients with TMD seek treatment. TMD can associate with impaired general health, depression, and other psychological disabilities, and may affect the quality of life of the patient. Assessment Evaluations indicate that the recently published Diagnostic Criteria for TMD (DC/TMD) are reliable and valid. These criteria cover the most common types of TMD, which include pain-related disorders (e.g., myalgia, headache attributable to TMD, and arthralgia) as well as disorders associated with the TMJ (primarily disc displacements and degenerative disease). As peripheral mechanisms most likely play a role in the onset of TMD, a detailed muscle examination is recommended. The persistence of pain involves more central factors, such as sensitization of the supraspinal neurons and second-order neurons at the level of the spinal dorsal horn/trigeminal nucleus, imbalanced antinociceptive activity, and strong genetic predisposition, which also is included in DC/TMD. Conclusion The etiology is complex and still not clearly understood, but several biological and psychosocial risk factors for TMD have been identified. Several studies indicate that patients with TMD improve with a combination of noninvasive therapies, including behavior therapy, pharmacotherapy, physical therapy, and occlusal appliances. More stringently designed studies, however, are needed to assess treatment efficacy and how to tailor treatment to the individual patient.

Möbius syndrome is a rare congenital condition that is characterized clinically by paralysis of a group of cranial nerves. It presents diverse manifestations that can have negative repercussions on the quality of life of affected individuals. This article presents a clinical case of a child with the syndrome and addressed the clinical manifestations and difficulties experienced during dental treatment. Knowledge of the etiopathological aspects and clinical manifestations of the condition is essential to perfecting treatments that aim to improve the quality of life of such individuals and their families.

Transjugular intrahepatic portosystemic shunt is one of the few available options for treating complications of portal hypertension in patients with chronic diffuse liver diseases. Over time, shunt patency can become compromised, causing a recurrence of such complications. The clinical case presented below demonstrates the potential of TIPS and illustrates the use of an endovascular reintervention for re-establishing shunt patency, which improves the life expectancy of patients, as well as their quality of life.

Abstract Introduction: Cephalea from orofacial origin is a diffuse, mild to moderate pain that appears as a bandage around the head. There are many different etiological causes of dental origin that contribute to the appearance of cephalea. Purpose: The purpose of our research is dental treatment of patients with traumatic occlusion, bruxism and loss of occlusal support, which have a pathological condition - cephalea from orofacial origin. Material and methods:For the purposes of this paper, 15 patients with cephalea from orofacial origin were analysed, diagnosed and treated. The control group consisted of 15 patients without etiological factors. A butterfly deprogrammer and a stabilization splint were made depending on the indication. Results and discussion: According to our patient study results, it appears to be a link between inadequately made prosthetic devices, bruxism and loss of occlusal support with cephalea. The therapy should help the patient urgently and continue to relieve the cephalea symptoms from orofacial origin. The results show an improvement in 86.7% of the examinees. Conclusion: The butterfly deprogrammer and stabilization splint are new methods that can contribute along with other therapeutic modalities in improving the quality of life in a patient with cephalea from orofacial origin.
