Mumps currently is a vaccine-preventable infections. However, mumps still remains an urgent problem all over the world, due to periodic increases in the incidence in older children (including the vaccinated ones against this infection), in adults (the disease often does not go smoothly with involving various organs in the pathological process and systems). The literature describes the various adverse effects of male mumps, including infertility, testicular atrophy, testicular tumors, impotence, gynecomastia, etc. The article provides information on the incidence of mumps in the Russian Federation and St. Petersburg, the clinical manifestation of the infection. It was noted that in adolescent boys and men, the presence of orchitis (orchoepididymitis) is a frequent symptom of the disease. It is shown in the article, that testicular inflammation rate increases with age of patients. A brief description of modern methods of laboratory diagnostics and therapy is given in accordance with clinical recommendations and treatment standards. A clinical case of mumps, combined severe form (mumps + orchoepididymitis), in a 17-year-old boy is presented in this article. The features of the course of the disease, the results of laboratory and instrumental examination and treatment have been analyzed in this item. Conservative therapy (antiviral and antibacterial) of mumps, as well as timely surgical treatment of orchoepididymitis, led to a complete clinical and laboratory recovery of the patient.

Primary hyperparathyroidism is one of the common endocrine diseases, which is an actual medical and social problem. Bone disorders are the most common manifestations of primary hyperparathyroidism and are characterized by increased bone metabolism with a progressive decrease in mineral density and a high risk of fractures. In the Russian Federation, most cases are manifest forms of the disease (70%), which is mainly due to the low level of diagnosis, including due to the lack of awareness of doctors. At the same time, timely detection of the disease is based on relatively accessible laboratory studies, characterized by elevated levels of calcium and parathyroid hormone in the blood, and ultrasound and scintigraphy in most cases provide visualization of the paraadenoma. A clinical case of a severe form of primary hyperparathyroidism, the late diagnosis of which caused the development of severe osteoporosis with multiple fractures, is considered. The presented case indicates that an important role in the timely detection of the disease belongs to the analysis of the clinical picture.

Objective of the research: to analyze clinical and laboratory manifestations of COVID-19 with a predominant lesion of the gastrointestinal tract (GIT) in children. It was revealed that incidence of this form of COVID-19 in children is underestimated. Based on the analysis of literature data and original clinical observations, the article describes current ideas about the frequency of detecting the form of COVID-19 with a predominant lesion of the GIT, its clinical and laboratory characteristics, the mechanisms of infectious process development in the digestive organs and approaches to rational laboratory diagnostics. The feasibility of PCR in feces in patients with suspected COVID-19 is confirmed. A description of a clinical case of COVID-19 severe form, which required differential diagnosis with acute bacterial gastroenterocolitis, is presented.

The article describes a clinical case of malaria in patient X., who was admitted to the medical institution of the Ministry of Defense. The patient was diagnosed with acute leukemia based on complaints and the clinical picture, followed by transferring the patient to the hematology centre of the Main Military Clinical Hospital n. a. N. N. Burdenko. In a short time, the patient was diagnosed with laboratory tests of the Department of Clinical and Hematological Research, and the diagnosis was reported to the specialists of the hematological centre. The patient was promptly provided with the necessary medical care. Due to the rare occurrence of this parasitic disease (the last case of malaria we diagnosed 10 years ago), we conducted a comprehensive study using various laboratory methods and modern hematological analyzers. All this made possible to recognize this disease, which is rare in the Moscow region, as well as in other regions of the Russian Federation, in a short period of time.

Purpose. To evaluate the clinical and functional results of the technique of transcleral fixation of the artificial iridochrustalic diaphragm during its decentration in a patient with aniridia and aphakia. Material and methods. Under observation was a 32-year-old patient K. with a diagnosis on the right eye: iridochrustalic diaphragm decentralization, posttraumatic aniridia, posttraumatic aphakia, who underwent a method of transcleral fixation of a displaced artificial iridochrustalic diaphragm developed at the Academician S. N. Fedorov Eye Microsurgery of the Moscow Ministry of Health of the Russian Federation. Results. Patient K. is subjectively satisfied with the visual functions obtained in the right eye. Conclusion. Thus, this clinical case demonstrates a very successful implementation of the fixation of the iridochrustalic diaphragm with its displacement to obtain satisfactory visual functions in the patient. (1) Background: to reveal the prevalence of non-motor disorders in Parkinson’s disease (PD), we analyzed both Russian and international studies on the issue of PD-associated non-motor disorders in Caucasian patients; (2) Methods: We have carried out a search for full-text Englishand Russian-language articles published during the last ten years (from 2010 to 2020) in PubMed, Scopus, Web of Science, Springer, Clinical case, and E-library databases using multiple versions of keywords and their combinations. (3) Results: General prevalence of PD-associated non-motor disorders proved to be high. At the same time, we did not find significant differences between the prevalence of cognitive, affective, or behavioral disorders in PD patients. However, depression was found to be more common in PD patients in the Russian Federation; (4) Conclusions: According to the results of our review, cognitive and affective disorders in PD represent the issues of major concern.

BACKGROUND. By defining the cornerstone of sepsis as the “systemic” of the inflammatory response in the form of cytokine violations, the authors of the corresponding concept are forced to recognize the etiological factors almost all pathogens of infectious diseases, which naturally go with the increased activity of cytokines in serum. Without diminishing the importance of these components of inflammation, one should pay attention to the postulate that not only viruses and protozoa never cause sepsis, but also all non-bacterial pathogens are not etiological factors of this disease, although they lead to severe systemic reactions accompanied by an increase in the level of proinflammatory cytokines. OBJECTIVE. The purpose of the work is to draw the attention of physicians to the discrepancy between the new notions about sepsis imposed on the medical community and accepted not only by anesthetists, resuscitative surgeons and surgeons, but also by many infectionists. MATERIALS AND METHODS. A detailed analysis of the clinical case of sepsis in a young woman is presented, which, due to adequate surgical and long-term conservative therapy, has safely recovered. CONCLUSIONS. Sepsis is a generalized acyclic infectious disease of the bacterial and/or fungal etiology that develops in an immunodeficient organism with characteristic pathomorphological and pathologist changes in organs and tissues.

Colonoscopy is one of the most widely used procedures in medical practice for the diagnosis and treatment of many benign and malignant diseases of the colorectal tract. Colonscopy has become the reference procedure for screening and surveillance of colorectal cancer. The overall rate of adverse events is estimated to be about 2.8 per 1,000 procedures, while complications requiring hospitalization are about 1.9 per 1,000 colonoscopies. Mortality from all causes and colonoscopy-specific mortality are estimated to be 0.07 and 0.007%, respectively. An exceptional fearsome postcolonoscopy complication is colon ischemia (CI); only few cases have been reported worldwide. We present the case of a 43-year-old woman who presented to the emergency department complaining of abdominal pain; fever and rectal bleeding appeared 12 h after a voluntary ‘screening’ colonoscopy. She had no risk factors for CI. Her laboratory tests showed alterations in inflammatory markers and a computed tomography scan showed a circumferential thickening in the left colon and free fluid in the abdomen. After 12 h of observation and conservative therapy, the clinical state of the patient worsened with the rising of signs of peritonitis. Laparoscopy showed that colon infarction extended from the distal third of the transverse colon to the proximal rectum. Laparotomy, resection of the pathological colon and terminal colostomy were performed. The specimen examined confirmed an extended ischemic colitis and transmural infarction on the antimesocolic side, in the absence of a vasculitis. The patient underwent recanalization after 8 months. CI after colonoscopy is a rare and alarming complication that must be known and taken into account in the differential diagnosis of symptomatic cases after colonoscopy, particularly in patients with known risk factors. The diagnosis is mainly based on clinical data, imaging and especially endoscopy. Treatment is almost always conservative but, in some cases in which the pathological process appears irreversible, surgery becomes mandatory.

Traditionally the Cushing’s disease (CD) is considered a rare disease. Since 2012 СD has the official status of an orphan disease in the Russian Federation. Now prevalence of this disease in the world are from 1,5 to 3,9 cases of CD on 1 million people of the population. The reason of CD is the adenoma of a hypophysis (сortiсotropinoma). The “gold” standard of therapeutic tactics of patients with CD is the transsphenoidal surgery. However in 20% of cases after the radical treatment  fail to achieve remission of pathological process. Now in an arsenal of the neuroendocrinologist we have various methods of  therapy, including various methods of drug therapy. The focus of  preparations are central type action:  analogs of a somatostatin. The representative of this group of preparations is Pasireotid who has proved the efficiency by results of the multicenter clinical trials. This review  focuses of the current   state of knowledge about methods of treatment of CD, about new therapeutic opportunities with application of analogs of a somatostatin.

Phlegmon of a neck represents the serious illness which is characterized by development of the poured pyoinflammatory process in the cells spaces with fast distribution of purulent zatek that is caused by anatomic features. The clinical picture of a disease doesn’t allow to establish prevalence of pathological process in the preoperative period very often, and a large number of important anatomic formations don’t allow to sanify the center single-stages. The clinical supervision with option of treatment of the patient and a positive outcome is given in article.
