Analysis of Nursing Cooperation in Microscopic Pituitary Tumor Resection via Transsphenoidal Approach

To analyze the clinical effect of nursing cooperation in transsphenoidal approach microscopic hypophysectomy. From January 2017 to January 2020, 80 patients who underwent transsphenoidal microscopic hypophysectomy in our hospital were selected to participate in the analysis and study. They were divided into two groups according to the randomized allocation, namely the observation group and the control group. Among them, 40 patients in the observation group and 40 patients in the control group were given routine nursing care for the patients in the control group, and comprehensive nursing intervention was adopted for the patients in the observation group, and the overall nursing effect of the two groups of patients was compared. After taking different nursing methods, the condition of patients in both groups was effectively controlled, and the effective rate of patients in the observation group with comprehensive nursing intervention was significantly better than that of patients in the control group with conventional nursing methods, and the difference had certain statistical significance (P < 0.05); The satisfaction degree of patients in the study group was significantly better than that of patients in the control group, and the difference was statistically significant (P < 0.05). The degree of negative emotions of patients in the study group was significantly better than that of patients in the control group after receiving comprehensive nursing intervention, and the difference was statistically significant (P < 0.05), and the difference in the incidence of adverse events between the two groups was not statistically significant (P > 0.05); The scores of each index of SF-36 questionnaire of patients in both groups were higher than those before nursing, and the scores of each index of patients in observation group were higher than those of patients in control group, and the difference was statistically significant (P > 0.05). With adequate preoperative preparation and mastery of the use of mechanical equipment, comprehensive nursing intervention can effectively improve the treatment effect of patients, make patients more satisfied with the nursing work, and can soothe patients’ negative psychological mood, eliminate panic, improve patients' life confidence, enhance intraoperative cooperation, and ensure that the operation can be completed smoothly.

BACKGROUND Mobile apps are becoming increasingly relevant to health care. Apps have been used to improve symptoms, quality of life, and adherence for oral drugs in patients with cancers, pregnancy, or chronic diseases, and the results were satisfying . OBJECTIVE This study aims to develop an information platform with the help of a mobile app and then evaluate whether information platform-based nursing can improve patient’s drug compliance and reduce the incidence of VTE in patients with hip fractures. METHODS We retrospectively analyzed hip fracture patients performed with traditional prevention and intervention of VTE (control group) between January 2008 and November 2012, and prospectively analyzed hip fracture patients conducted with nursing intervention based on the information platform (study group) between January 2016 and September 2017. The information platform can be divided into medical and nursing care end and the patient’s end. Based on the information platform, we could implement risk assessments, monitoring management and early warnings, preventions and treatments, health educations, follow-up and other aspects of nursing interventions for patients. We compared basic characteristics, outcomes including drug compliance, VTE occurrence, and mean length of hospitalization between the two groups. Besides, a subgroup analysis was performed in the study group according to different drug compliances. RESULTS Regarding baseline data, patients in the study group had more morbidities than those in the control group (P<0.05). The difference of drug compliance between the two groups was statistically significant (P<0.001): 64.7% of the patients in the control group had poor drug compliance and only 6.1% patients had poor drug compliance in the study group. In terms of VTE, 126 patients (10.7%) in the control group had VTE, while only 35 patients (7.1%) in the study group had VTE, and the difference was statistically significant (P=0.024). Moreover, the average length of hospitalization in the study group was also significantly lower than that in the control group (10.4 d vs. 13.7 d, P=0.000). Subgroup analysis of the study group showed that the incidence of VTE in patients with non-compliance, partial compliance, and good compliance was 56.7%, 5.8% and 2.8%, respectively (P=0.000). CONCLUSIONS Poor drug compliance leads to higher VTE occurrence. The information platform-based nursing can effectively improve the compliance of patients with hip fracture and thus significantly reduce the incidence of VTE.

Objective: The clinical effect of arthroscopic debridement combined with rehabilitation training in the treatment of knee arthritis was studied and analyzed. Methods: A total of 90 patients with knee osteoarthritis treated in our hospital from August 2017 to August 2018 were selected as the research objects. All patients were divided into the observation group and the control group by using the random number method. The control group was treated by arthroscopic cleaning operation, and the observation group was treated by combined rehabilitation training on the basis of the treatment in the control group. The total effective rate and simple McGill pain of the two groups were compared Score and lyshoim score. Result: The total effective rate of the observation group was significantly higher than that of the control group (P < 0.05), the difference was statistically significant; after treatment, the simple McGill pain score of the observation group was significantly lower than that of the control group (P < 0.05), the difference was statistically significant, the lyshoim score of the observation group was significantly better than that of the control group (P < 0.05), the difference was statistically significant. Conclusion: In the treatment of knee osteoarthritis, arthroscopic debridement combined with rehabilitation training has a significant effect, which can significantly reduce the pain and improve the prognosis of patients. It is worth popularizing in clinical treatment.

Objective: To explore the impact of comprehensive nursing intervention on the quality of intestinal preparation in the process of intestinal preparation for hospitalized patients undergoing colonoscopy. Methods: A total of 320 patients underwent colonoscopy in our department from January to June 2021. They were grouped by random number table. There were 160 cases in the control group and 160 cases in the observation group. The adverse reactions of intestinal preparation and the cleanliness of intestinal preparation were analyzed. Results: Through comprehensive nursing intervention, it can be found that the incidence of adverse reactions in the preparation of colonoscopy in the control group was 24.38%, and that in the observation group was 13.48%. The results showed that the incidence of adverse reactions in the preparation of colonoscopy was significantly lower than that in the control group (P < 0.05), which indicates that the difference was statistically significant. In the control group, 105 people qualified in intestinal cleanliness score (> 5 points) during enteroscopy preparation, and the cleanliness qualification rate was 65.63%. In the observation group, 139 people qualified in intestinal cleanliness score (> 5 points) during enteroscopy preparation, and the cleanliness qualification rate was 86.88%. The cleanliness qualification rate of enteroscopy preparation was significantly higher than that of the control group, and P < 0.05, which indicates that the difference was statistically significant. Conclusion: Comprehensive nursing intervention can promote the quality of preparation of patients for colonoscopy, improve the compliance of patients and improve the accuracy of examination effect.

Objective: To analyze the changes of hemorheology, haemodynamics and immune function of patients during propofol intravenous anesthesia in the radical resection of colorectal carcinoma and its significance. Methods: The study included 112 patients who underwent radical resection of colorectal carcinoma in our hospital between August 2016 and December 2017, and they were divided into an observation group (N=56) and a control group (N=56) using random number table. Patients in the observation group were given propofol intravenous anesthesia, while patients in the control group received inhalation anesthesia of sevoflurane. Hemorheological and haemodynamical indexes were compared and analyzed before anesthesia (T0), 90 min after induction (T1), 150 min after induction (T2) and 30 min after entering post-anesthesia care unit (T3), and the changes of immune function before and after surgery was also observed. Results: The whole blood viscosity under high, medium and low shear rates of the observation group declined significantly compared to that of the control group at T1, T2 and T3 (P<0.05). The heart rate (HR) and systolic pressure (SPB) of the observation group significantly decreased at T2 compared to those at T1 (P<0.05), but recovered to the level observed at T0 at T3. The diastolic blood pressure (DBP) of the two groups at T1, T2 and T3 was not significantly different with that at T0 (P>0.05). The levels of CD45RA+ and CD45RO+ of both groups had a significant decrease at the end of the surgery compared to before anesthesia (P<0.05); the levels of the observation group recovered at the postoperative 72nd h, and the differences with the levels before anesthesia had no statistical significance (P>0.05); the level of CD45RA+ of the control group also recovered at the postoperative 72nd h, but the difference with the level before anesthesia had no statistical significance (P>0.05); the level of CD45RO+ of the control group had a significant decrease, and the difference with the level before anesthesia was statistically significant (P<0.05). The level of CD45RA+/CD45RO+ of the observation group at the end of surgery and the postoperative 72nd h was not significantly different with those before anesthesia (P>0.05). The level of CD45RA+/CD45RO+ of the control group at the postoperative 72nd h showed a significant increase compared to before anesthesia (P<0.05). Conclusion: Propofol intravenous anesthesia has a significant improvement effect on hemorheology before radical resection of colorectal carcinoma and has a small influence on haemodynamics. Objective: To explore the emotional impact of psychological nursing intervention on patients with malignant tumor radiotherapy, and further verify the specific application value of this nursing model in the oncology department. Methods: A total of 129 patients with tumor radiotherapy admitted to our hospital from June 2018 to June 2021 were selected as subjects, and they were divided into control group and observation group by random number method. Among them, 64 cases in the control group received routine clinical nursing. 65 cases in the observation group received psychological nursing intervention. Then, the clinical nursing effect, self-rating anxiety scale (SAS) score, self-rating depression scale (SDS) score, immune function level change condition and clinical nursing satisfaction were compared under the two nursing modes, in order to fully verify the clinical application value of psychological nursing intervention mode for patients with cancer radiotherapy. Results: Comparing the clinical indexes of the two groups, SDS and SAS scores of the observation group were significantly lower than those of the control group. In terms of nursing satisfaction, the observation group is 96.92% and the control group is 90.62%, the difference was significant, and had clinical statistical significance (P < 0.05); After the comprehensive evaluation of the immune function of the two groups, it was found that the observation group was significantly better than the control group, and the difference between the two groups was statistically significant (P < 0.05). Conclusion: In the mode of psychological nursing, medical staff can carry out precise psychological nursing intervention and nutritional nursing for patients according to the specific conditions and treatment process of patients, which can greatly promote the psychological stability of patients, significantly reduce patients’ negative emotions. In addition, the scientific implementation of psychological nursing intervention in the treatment of tumor radiotherapy patients can effectively strengthen the improvement of the immune function of patients, which has a very important application value to improve the physical function of patients, promotes the patient’s body rehabilitation, and is worth in the field of widespread application.

Abstract Objectives Since polycystic ovarian syndrome (PCOS) is prevalent in reproductive women with obesity and insulin resistance, adipocytokines are often accused and investigated for pathophysiology. The aim of this study was to evaluate the adiponectin and leptin levels in normal-weight women with PCOS. Methods Forty women with PCOS and 40 age and body mass index (BMI) matched controls were included in the study. Adiponectin and leptin levels in addition to other biochemical parameters were measured. Results Leptin levels were statistically significantly higher in the study group compared to the control group (6.53 ± 2.670 vs 3.37 ± 2.002 ng/mL, p < 0.001 respectively). Although Adiponectin levels were lower in the study group compared to the control group (28.89 ± 16.124 μg/mL vs 31.05 ± 20.507, p = 0.714 respectively) the difference did not reach statistical significance. Leptin levels were positively correlated with fasting glucose, fasting insulin, free testosterone levels and homeostatic model assessment of insulin resistance (HOMA-IR) values. Adiponectin levels were negatively correlated with BMI. Conclusions Adiponectin and leptin have been suggested to play a crucial role in the pathogenesis of PCOS. Different adipocytokine levels in the normal weight PCOS group compared to age and BMI matched controls support the idea that adipose tissue in this group of women has some distinctive features not only in high BMI subgroup but also in normal weight subgroup.

Objective: To study the changes of several inflammatory index mechanism factors in the clinical efficacy of tablet and methotrexate in rheumatoid arthritis. Methods: 20 patients with rheumatoid arthritis were randomly divided into observation (n =10) and control (n =10), then normal people as normal (n=10), all three groups were given methotrexate and the observation group were treated with Qin interest pain tablets. Expression of inflammatory index mechanism factors in each group. Results: After treatment, inflammatory index mechanism factors were detected; a significant decrease of IFN-g, IL-1b, IL-10, IL-37, TNF-a and other factors was found before and after combination treatment (P <0); IL-8 and IL-13 expression (P <0); the CRP, ESR score of the study group was lower than the control group (P <0.05); and the inflammatory index mechanism factors were affected by drug combination. Conclusion: Tablets combined with methotrexate in rheumatoid arthritis patients is better than methotrexate. It obviously changed the expression of the patient’s inflammatory index mechanistic factors, which has a regulatory effect on the inflammatory mechanistic factors.

Objective: To investigate the effect of evidence-based nursing intervention in elderly patients with arrhythmia after acute myocardial infarction. Methods: 146 elderly patients with arrhythmia after acute myocardial infarction treated in our hospital from February 2016 to March 2017 were selected as research objects. According to the random number method, patients were divided into control group (73 cases, traditional nursing) and observation group (73 cases, evidence-based nursing intervention). The clinical effects and nursing satisfaction of the two groups were compared. Results: The hospital stay, average bed rest time, hospitalization cost, and incidence of arrhythmia complications in the observation group were significantly lower than those in the control group. The difference was statistically significant (P<0.05). After nursing, the four grades of heart function and satisfaction of nursing in the observation group were significantly higher than those in the control group. The difference was statistically significant (P<0.05). Conclusions: In elderly patients with arrhythmia after acute myocardial infarction, evidence-based nursing intervention was adopted. The patient's health has been greatly improved and the doctor-patient relationship has been significantly improved. This method is worth promoting.

Objective: To explore the nursing effect of standardized nursing interventions in endoscopic examination of patients with gastric ulcer complicated with diabetes. Methods: 90 patients with gastric ulcer complicated with diabetes who were treated in our hospital from May 2018 to May 2019 were selected and randomly divided into 2 groups, each containing 45. The control group received routine nursing, and the observation group performed standardized nursing intervention on the basis of the control group. The control group received conventional nursing, and the observation group performed standardized nursing intervention on the basis of the control group. After nursing, the two groups of patients were compared for their psychological state and nursing satisfaction. Results: After nursing, the HAMD and HAMA scores of the two groups were lower than before the nursing, and the observation group was lower than the control group, the difference was statistically significant(P<0.05). Conclusion: Standardized nursing intervention can alleviate the negative emotion of endoscopic examination of patients with gastric ulcer complicated with diabetes, which is conducive to the smooth progress of the examination.

<strong>Objective: </strong>To explore the effectiveness of the early nursing intervention in preventing postoperative deep vein thrombosis (DVT) of patients with lower limb fractures in the mining area. <strong>Methods</strong>: One hundred (100) patients with lower limb fracture in our hospital were evenly and randomly divided into two groups, namely the observation group and the control group respectively. The early nursing intervention was applied on patients in the observation group, and conventional orthopedic postoperative nursing was applied on patients in the control group.<strong> Results</strong>: Among 100 patients selected, 15 patients were elderly, accounting for 15%, 26 patients suffered from the obesity, accounting 26%, there were 25 patients with hypertension, accounting for 25%, 16 patients suffered from diabetes, accounting for 16%, 8 patients suffered from anemia, accounting for 8%, 38 patients had hobbies on the alcohol and tobacco, accounting for 38%, 9 patients had lower limb varicose veins, accounting for 9%, and 35 patients suffered from lower extremity edema, accounting for 35%. The incidence of DVT of patients in the observation group was obviously lower than that of patients in the control group, and the difference was of statistical significance.<strong> Conclusions</strong>: Early nursing intervention can effectively reduce the incidence of postoperative DVT of patients with lower limb fractures, and it contributes to early recovery of patients.
