To find the association between intrapartum amniotic fluid lactate level and labour outcome

Background: Inspite of good monitoring in labour, there are still adverse labour outcomes. There is a need for supplementary test for early prediction of prolonged labour and fetal hypoxia. This study was undertaken to find to find the association between intrapartum amniotic fluid lactate level and labour outcome.Methods: 103 antenatal patients low risk at term, who entered into active labour either spontaneously or induced between September 2012 to August 2013 were included. Amniotic fluid was collected after doing ARM and lactate level was measured. Chi square test was used to find correlation between lactate level and duration of labour, mode of delivery and Apgar score.Results: In our study, patients with duration of labour between 8-<20hours 88 % had lactate level <10mmol/l, thus intra partum amniotic fluid lactate levels had a good negative predictive value when it was compared with the duration of labour. When the mode of delivery was analysed using Chi square test there was no statistically significant correlation (p<0.825) with amniotic fluid lactate. Spearman Rank correlation was done to observe correlation between lactate levels and Apgar which showed a correlation of 0.20 with p<0.05.Conclusions: Amniotic fluid lactate levels had better correlation with duration of labour and Apgar score. The lower the amniotic fluid lactate level, the better is the Apgar at birth. Better correlation could have been observed if amniotic fluid lactate levels were repeated every 3-4 hours during active labor. However, this was not possible due to practical difficulty in collection of sample.

Background and Aim: Acetate or lactate buffered, balanced isotonic rehydration fluids are commonly used for fluid therapy in dogs and may influence acid-base and electrolyte status. This study aimed to assess acid-base status, electrolyte levels, and lactate levels in dehydrated dogs after receiving acetate or lactate-containing intravenous rehydration fluids. Materials and Methods: In this prospective, randomized study, 90 dehydrated dogs were included and randomized to receive acetate [Sterofundin® ISO B. Braun Vet Care (STERO), Germany) or lactate (Ringer-Lactat-Lösung nach Hartmann B. Braun Vet Care (RL), Germany] containing intravenous fluids for rehydration. The exclusion criteria were as follows: Age <6 months, liver failure, congestive heart failure, and extreme electrolyte deviation. Physical examination, venous blood gas, and lactate levels were analyzed before and after rehydration. The two groups were compared using t-test and Chi-square test. The significance level was set at p≤0.05. Results: Post-rehydration heart rate decreased in the STERO group (p<0.001) but not in the RL group (p=0.090). Lactate levels decreased in both groups STERO (p<0.001) and in group RL (p=0.014). Sodium and chloride levels increased during rehydration in group STERO (p<0.001; p<0.001) and group RL (p=0.002; p<0.001). There was a larger decrease in lactate levels in group STERO compared to group RL (p=0.047). Conclusion: Both solutions led to a mild increase in sodium and chloride levels and decreased lactate levels. The acetate-containing solution had an inferior effect on the decrease in lactate level.

Abstract Background Eclampsia, considered as serious complication of preeclampsia, remains a life-threatening condition among pregnant women. It accounts for 12% of maternal deaths and 16–31% of perinatal deaths worldwide. Most deaths from eclampsia occurred in resource-limited settings of sub-Saharan Africa. This study was performed to determine the optimum mode of delivery, as well as factors associated with the mode of delivery, in women admitted with eclampsia at Riley Mother and Baby Hospital. Methods This was a hospital-based longitudinal case-series study conducted at the largest and busiest obstetric unit of the tertiary hospital of western Kenya. Maternal and perinatal variables, such as age, parity, medications, initiation of labour, mode of delivery, admission to the intensive care unit, admission to the newborn care unit, organ injuries, and mortality, were analysed using the Statistical Package for the Social Sciences software version 20.0. Quantitative data were described using frequencies and percentages. The significance of the obtained results was judged at the 5% level. The chi-square test was used for categorical variables, and Fisher’s exact test or the Monte Carlo correction was used for correction of the chi-square test when more than 20% of the cells had an expected count of less than 5. Results During the study period, 53 patients diagnosed with eclampsia were treated and followed up to 6 weeks postpartum. There was zero maternal mortality; however, perinatal mortality was reported in 9.4%. Parity was statistically associated with an increased odds of adverse perinatal outcomes (p = 0.004, OR = 9.1, 95% CI = 2.0–40.8) and caesarean delivery (p = 0.020, OR = 4.7, 95% CI = 1.3–17.1). In addition, the induction of labour decreased the risk of adverse outcomes (p = 0.232, OR = 0.3, 95% CI = 0.1–2.0). Conclusion There is no benefit of emergency caesarean section for women with eclampsia. This study showed that induction of labour and vaginal delivery can be successfully achieved in pregnant women with eclampsia. Maternal and perinatal mortality from eclampsia can be prevented through prompt and effective care.

Although the issue of corporate culture has been taken over and addressed in the literature from various perspectives, there are very few researchers about the role of leadership and motivation in it, respectively very few researchers have addressed them as important components of the international company’s corporate culture. The present paper aims to point out that leadership and motivation can be perceived as important aspects of the international company’s corporate culture. The object of the investigation was an international company (situated in Italy) and its five subsidiaries (situated in Italy, Czech Republic, Germany, and Turkey). As the main research method, there was chosen the method of the questionnaire survey, which was attempted by all the company’s employees (totally 270 respondents). The questionnaire was divided into three separate, but logically related parts—leadership, motivation, and corporate culture, and submitted to two groups of respondents—the company’s management and its employees. In total 11 hypotheses were formulated and further evaluated by the methods of Pearson Chi-square Test, Fisher’s Exact Test, Cramer’s V coefficient, Kendall rank correlation coefficient, Eta coefficient, Spearman coefficient, Mann–Whitney U test and Wilcoxon W statistics, Kruskal–Wallis test, and Friedman’s test. The results of the research have proven that leadership and motivation are important parts of the corporate culture.

Introduction. The use of age- and risk-adjusted caries prevention requires up-to-date knowledge on the cariostatic effects of fluoride, as well as the methods and safety of fluoride prophylaxis. Aim. The aim of the study was to assess dentists’ knowledge of the safety and mechanism of anticaries effects of fluoride. Material and methods. An anonymous questionnaire was conducted among 212 dentists participating in dental training. The questions included in the questionnaire related to knowledge about water fluoridation, fluoride cariostatic mechanisms and the safety of fluoride prophylaxis. The chi-square test and the Spearman’s rank correlation coefficient were used for statistical analysis. A significance level of 0.05 was used. Results. Incorrect knowledge about water fluoridation was demonstrated by 26.9% of respondents in the country and 16.0% of respondents in Europe. Most respondents (80.7%) reported higher efficacy of exogenous vs endogenous anticaries approaches, and more than half of respondents (59.0%) considered the processes of remineralisation and demineralisation as the most important anticaries activity. Almost all respondents (95.7%) agreed that the use of fluoride for caries prevention is controversial, mainly due to its dose-dependent therapeutic or toxic effects (85.4%). According to about 20% of respondents, cariostatic fluoride doses have adverse effects on the general health, mainly in the form of bone fragility (10.4%). Conclusions. Lack of correct knowledge about the dominant cariostatic effects of fluoride may result in the choice of an inappropriate preventive method or avoiding local application of fluoride preparations for fear of adverse systemic effects, which may in turn lead to increased caries prevalence in a given population.

Introduction: Appropriate intrapartum conduct in a twin delivery remains a challenging aspect of obstetric practice. The objective of this study was to compare neonatal and maternal outcomes in twin pregnancies according to mode of delivery.&nbsp; Materials and methods: This is a single centre retrospective cohort study of all consecutive spontaneously-conceived twin deliver-ies (&ge; 24 weeks, estimated fetal weight &ge; 500 grams) over a nine-year period between 01/01/2007 - 31/12/2016 at a tertiary-level centre. Neonatal outcomes included survival, APGAR score, prematurity-associated pathology (PAP), admission to the neonatal intensive care unit (NICU) and length of stay (LOS). Maternal outcomes included postpartum complications and LOS. Statistical analysis comprised Chi-square test with subsequent p-value and odds-ratio with 95% confidence interval. Statistical significance was set at p<0.05. Results: A total of 173 consecutive women with spontaneously-conceived twin deliveries were enrolled in this study, 129 (74.6%) women delivered by caesarean section (CS). The success rate of vaginal delivery (VD) was 93.6% (44/47). A strong statistical correlation was identified between CS and NICU admission; 53.2% vs. 1.5% (p=0.0001). Neonatal LOS in the NICU was significantly longer in the CS group. Prematurity-associated pathology (PAP) was noted in 75 pairs of twins (75/173); 61 pairs were delivered by CS, bearing strong statistical significance (p<0.0001). Postpartum complications occurred in 14.7% of CS compared to 13.6% of VDs.&nbsp; Conclusion: Neonates delivered by CS had a higher rate of PAP, NICU admission, lower birth weight and longer LOS. This study showed that VD is safe, especially when the first twin is in cephalic presentation

Background: It has been since antiquity that the importance of amniotic fluid and fetal growth with perinatal outcome is being documented. But the lacunae lies in studying the relationship between borderline amniotic fluid and perinatal outcome. The following study was undertaken to provide recent data that would help predict perinatal outcome in borderline AFI pregnancies.Methods: About 144 patients were considered in the study OPD/IPD patients in obstetrics and gynecology department in Bhabha Atomic Research Centre and Hospital, with about 72 cases with borderline amniotic fluid index (5-8 cm) and controls with amniotic fluid index ≥9-25 cm. Patients were selected and subjected to history taking, examination, ultrasound test with doppler studies and perinatal outcome documented over a period of one year.Results: The incidence of borderline AFI in my study was 16%. 58% were primigravidas. Meconium stained liquor was found in 18% cases compared to 7% controls. Low birth weight was found in 12.5% cases and 2.7% in controls. On applying statistical test analysis chi square test, it was found that borderline amniotic fluid index in relation to presence of meconium stained amniotic fluid and low birth weight, p value was found to be statistically significant (<0.05).Conclusions: Borderline amniotic fluid and perinatal outcome had significant relationship in terms of meconium stained liquor and birth weight while rest had no significance. Thus, borderline amniotic fluid patients require vigilant fetal surveillance.

Purpose: The purpose of this study was to assess the knowledge of pharmacists practicing in Palestine of issues related to using psychotropic medications in older people. Methods: The study was conducted with a cross-sectional observational design using a questionnaire. A total of 400 pharmacists responded to a 19-statement knowledge test related to the use of psychotropic medications in older people. The study was conducted from July 2016 to February 2017. The reliability and internal consistency of the study tool was assessed using the test-retest method and the Cronbach alpha. Categorical groups were compared using the chi-square test and the Spearman rank correlation. Results: On the 19-statement knowledge test, the median score was 55.3% with an interquartile range of 21.9%. In a comparison of the demographic and practice-related variables of the pharmacists who scored ≥ 50% on the 19-statement knowledge test with those who scored < 50%, age, gender, and having taken a course on psychotropic medications were found to be significantly associated with performance, as shown by the chi-square test and Spearman correlation. Conclusion: Pharmacists practicing in Palestine possess less than optimal knowledge of issues related to the use of psychotropic medications in older people. Continuing educational interventions and/or training might be helpful in improving pharmacists’ knowledge of issues related to using psychotropic medications in older people.

Background: Entanglement of the umbilical cord around the foetal neck (nuchal cord) is a common finding at delivery, but its clinical relevance is not entirely clear. Nuchal cords have generally been considered to be rather benign. However, the insufficiency in data regarding the role of nuchal cord in foetal morbidity and mortality is a source of anxiety and frustration to both parturient and healthcare professionals. Considering the above facts the present study was taken up to establish the incidence of nuchal cord at delivery in the existing setup.Methods: This was a cross sectional comparative study conducted over a period of one year from May 2013 to April 2014. A total of 362 women were selected for the study. These women were briefed about the nature of the study and a written informed consent was obtained. The demographic data and obstetrical history were documented on predesigned proforma. The data obtained was analysed using percentages and comparison done using chi-square test for discrete variables.Results: Incidence of Nuchal Cord was 17.7% among study cases. Based on this study gestational age may have an effect on the presence of nuchal cord (p<0.05). Out of 64 cases with nuchal cord’, 11.4% of cases had meconium stained liquor present. 9.8% of cases with nuchal cord had FHR irregularity, which was significantly less as compared to 20.0% of cases who did not show FHR irregularity. This was statistically significant. (p=0.032). 18.7% of mother who showed presence of nuchal cord had delivery by caesarean section. 22.7% of cases with nuchal cord had NICU admissions, which was insignificantly more as compared to 17.0% of cases who did not have NICU admissions.Conclusions: Intrapartum events such as meconium staining of liquor and foetal heart rate irregularities were more commonly associated with nuchal cord but it did not affect the perinatal outcome. However, the mode of delivery was independent of the presence of nuchal cord, thereby reducing the morbidity to the mother. Hence nuchal cord is not associated with adverse perinatal outcomes and the presence of a nuchal cord per se is not found to be an indication of operative delivery.

Background: Treatment compliance can not only prevent complications among patients with heart failure but also improve their health, quality of life, decrease hospitalization, and mortality rate. Objective: To examine the treatment compliance among patients with heart failure, and identify factors related to treatment compliance. Methods: The descriptive correlational study was carried out in 112 participants by a random sampling technique in the Cardiovascular Department, C Hospital, Vietnam. Data were collected during June to August 2019. Four instruments were used: 1) Demographic and clinical conditions; 2) the Revised Heart Failure Compliance Scale; 3) Mini Mental State Examination (MMSE); and 4) the Japanese Heart Failure Knowledge Scale (JHFKS). Data were analyzed by chi-square test and Spearman rank correlation. Results: Among 54.5% of participants were demonstrated treatment compliance. Compliance with the medication and routine checkup were high (> 80%), whereas doing exercise, fluid restriction and weighing daily were low (43.8%, 33.8%, and 16.1%, respectively). Factors included gender, education level, numbers of hospitalization during the past year, comorbidities, heart failure knowledge, and cognitive function were significantly related to treatment compliance (P < .05). Conclusions: Patients should enhance heart failure knowledge in order to improve treatment compliance. By doing this, heart failure patients were supplied information about the disease when they are hospitalized. Patients with cognitive impairment were also paid attention to improve treatment compliance.
