Background: During pregnancy, a lot of changes do occur and affect every organ system in the body. Objective of the study was to find out expected variations, if any, in liver function tests (LFT) in normal pregnancy during the third trimester.Methods: A total of 75 women aged between 20 to 35 years were enrolled in the study of which 25 were non-pregnant women (control group) and 50 were pregnant women in their third trimester (study group).Results: In the study group, there were no changes in SGPT, serum albumen and serum globulin values in any of the subjects. However out of 50 subjects, a rise in serum alkaline phosphatase could be demonstrated in as many as 44 (88%) subjects. In 10 (20%) subject Serum Bilirubin was high and in 6 (12%) subjects each a rise in SGOT and fall in Serum Proteins could be demonstrated.Conclusions: Except for a significant change in serum phosphatase levels, there is no significant variation in any of the liver function tests in third trimester of pregnancy.

Background: Change of liver biochemical profiles is normal during pregnancy. It is almost impossible to understand disease processes that can threaten women during pregnancy without understanding normal physiological change. Aim of this study is to evaluate changes in serum liver function tests in normal pregnant women in first, second and third trimester. Material and Methods: The cross-sectional comparative study was carried out in the Department of Obstetrics & Gynaecology of Rajshahi Medical College Hospital, Bangladesh in 2019. This study consists of 90 pregnant women and 90 matched control. Among the 90 pregnant women, 30 were in first trimester, 30 were in second trimester and 30 were in third trimester. Blood samples were taken for routine liver function and protein profiles Results: Serum total and direct bilirubin concentrations were significantly lower in second and third trimester. The mean ALP level was slightly increased in 2nd trimester and drastically increased in 3rd trimesters during pregnancy. Serum ALT and AST activity was significantly increased in third trimester. No significant change in serum total proteins concentration, but serum albumin concentration was significantly lower and serum globulin concentration was significantly higher in all three trimester. Serum albumin/globulin ratio was significantly reduced in second and third trimester. Conclusion: Relative values of various liver function tests during gestational trimesters appear to be the best guide to confirm the diagnosis and treatment strategies. Thus, gynecologists should routinely monitor liver function tests in all gestational trimesters to avoid the future complications to mother and offspring. TAJ 2020; 33(1): 17-24

Background: Incidence of Antitubercular Therapy (ATT)-induced hepatotoxicity is higher in India when compared to Western countries. As the occurrence of ATT-induced hepatotoxicity is unpredictable, serial intensive monitoring of hepatic function is now being recommended by the American Thoracic Society in individuals at high risk. This study was done to evaluate the risk factors for the development of ATT induced hepatotoxicity in India. Methodology: In this prospective, observational study, patient characteristics of microbiologically/ radiologically/ histopathologically confirmed tuberculosis were prospectively compiled. Serial liver function tests were done once a month in all patients. Patients who developed ATT-induced hepatotoxicity were considered as the study group and those who did not develop the event as a control group. The primary outcome measure was to estimate the hazard ratios associated with risk factors for the development of ATT induced hepatotoxicity. Cox Regression Analysis was done using SPSS 20. Results: : A total of 200 patients were enrolled in the study, of them, 14% developed ATT-induced hepatotoxicity and 86% did not develop the event. The baseline liver function tests in the study group and control group were within normal limits. Female gender, alcoholism, HIV co-infection and age >35 yrs were identified to have a higher risk for development of ATT-induced hepatotoxicity, while cases with pulmonary tuberculosis were found to be at lower risk of developing event. Conclusion: Intensive liver function monitoring needs to be done in patients with these risk factors, female gender, alcoholism, HIV co-infection, extra-pulmonary tuberculosis and age >35 yrs.

Obstetric cholestasis (OC) is a liver disorder that occurs in the late second and early third trimester of pregnancy characterized by pruritus with increased serum bile acids and other liver function tests. The pathophysiology of OC is still not completely understood. The symptoms and biochemical abnormality rapidly resolve after delivery. OC is associated with an increased risk of adverse obstetrical outcomes. The aetiology of obstetric cholestasis of pregnancy is poorly understood and is thought to be complicated and multifactorial.  OC typically occurs in the late second trimester when the oestrogen levels are the highest in pregnancy. The most common complaint is generalized intense pruritus, which usually starts after the 30th week of pregnancy. Pruritus can be more common in the palms and soles and is typically worse at night. Other symptoms of cholestasis, such as nausea, anorexia, fatigue, right upper quadrant pain, dark urine, and pale stool, can be present. Clinical jaundice is rare but may present in 14% to 25% of patients after 1 to 4 weeks of the onset of pruritus. Some patients also complain of insomnia as a result of pruritus. Generally, physical examination is unremarkable except for scratch marks on the skin from pruritus. Pruritus is a cardinal symptom of intra-hepatic cholestasis of pregnancy (ICP) and may precede biochemical abnormalities. The diagnosis of intrahepatic cholestasis of pregnancy is via the presence of clinical symptoms pruritus in the third trimester with elevated maternal total serum bile acids and excluding other diagnoses, which can cause similar symptoms and lab abnormalities. Fasting blood samples should be used to check for the total bile salt acid level as it can become elevated in the postprandial state. Once the diagnosis of OC of pregnancy is confirmed, immediate treatment is necessary, and the primary goal of therapy is to decrease the risk of perinatal morbidity and mortality and to alleviate maternal symptoms. Maternal pruritus can be alleviated with use of moisturisers and oral antihistamines. Ursodeoxycholic acid (UDCA) is the drug of choice for the treatment of ICP. Many authors have advocated elective early delivery of women with intrahepatic cholestasis of pregnancy to reduce the risk of sudden foetal death. The Royal College of Obstetricians and Gynaecologists recommends induction of labour after 37+0 weeks of gestation. Obstetric cholestasis of pregnancy is not an indication for Caesarean delivery. Postpartum pruritus typically disappears in the first 2 to 3 days following delivery, and serum bile acid concentrations will normalize eventually. ICP is not a contraindication to breastfeeding, and mothers with a history of ICP in pregnancy can breastfeed their infants. Postpartum monitoring and follow up of bile acids and liver function tests should be done in 4-6 weeks to ensure resolution. Women with the persistent abnormality of liver function test after 6 to 8 weeks require investigation for other aetiologies.

Abstract Objectives To observe and compare the difference in retinal thickness using optical coherence tomography (OCT) between patients with high myopia (HM) during the third trimester of pregnancy and age-matched HM non-pregnant women. Methods A case-control study. A total of 39 eyes from 39 HM women in the third trimester (study group) and 50 eyes of 50 age-matched non-pregnant women with HM (control group) were included. All subjects underwent SD-OCT examination. The built-in software was used to measure the retinal thickness in macular region. The data from two groups were compared using independent-samples t test. Results Among the 89 subjects in this study, the mean gestational age of the study group was 35.09 ± 2.44 weeks, and the average age was 32.24 ± 3.75 years. The average age of the control group was 34.04 ± 7.19 years old. Compared with the control group, the average thickness of parafoveal area, and the average thickness of parafoveal superior, inferior, temporal quadrants of the superficial retina and the average thickness of the foveal and parafoveal of the superficial retina were significantly decreased in the study group (P < 0.05). Compared with the control group, the average thickness of all quadrants of the retina in the parafoveal area except the nasal quadrant were significantly decreased in the study group (P < 0.05). Conclusions In this observational study, the retinal thickness of patients with high myopia during the third trimester of pregnancy was thinner than that of non-pregnant women with age-matched high myopia.

Background: Aspirin and ibuprofen are the most frequently prescribed non-steroidal anti-inflammatory drugs in the world. However, both are associated with a variety of toxicities. We applied serum metabonomics and Fisher discrimination for the early diagnosis of its toxic reaction in order to help diagnose these toxicities. Methods: A total of 45 rats were randomly divided into Control group, Aspirin group, and Ibuprofen groups. The experiment groups were given intragastric aspirin (15 mg/kg) or ibuprofen (15 mg/kg) for 3 weeks. Liver function tests were performed and blood metabonomics were analyzed by gas chromatography-mass spectrometry. Results: The most important compounds altered were trihydroxybutyric acid and l-alanine in the aspirin group, and acetoacetic acid, l-alanine, and trihydroxybutyric acid in the ibuprofen group. With respect to metabolic profiles, all 3 groups were completely distinct from one another. Fisher discrimination showed that 91.1% of the original grouped cases were correctly classified by the third week. However, only 55.6% of liver function tests were able to classify grouped cases correctly. Conclusion: Trihydroxybutyric acid, l-alanine, and acetoacetic acid were the most significant indicators of altered serum metabolites following intragastric administration of aspirin and ibuprofen in rates. These metabolomic data may be used for classification of aspirin and ibuprofen toxicity.

Objective: To measure the distribution of TPO-Ab positivity and to observe the effect of thyroid peroxidase positivity on thyroid function during first trimester in normal pregnancy. Method: A cross sectional among 120 subjects were taken in this study and divided into control and study groups. Control group (Group A) consisted of 60 healthy non pregnant women age ranged between 20 to 35 years. Study group (Group B) consisted of 60 normal pregnant women of same age range. Group B was further subdivided into group B1 and group B2according to the level of TPO-Ab. Group B1 consisted of TPO-Ab positive pregnant women and group B2 consisted of TPO- Ab negative pregnant women. Control group was selected from personal contacts and study group from Out Patient Department (OPD) of Obstetrics and Gynecology of Sir Salimullah Medical College and Mitford Hospital. For assessment of thyroid function, serum free thyroxine (FT4), thyroid stimulating hormone (TSH) were measured. Serum FT4, TSH were measured by Enzyme link immunosorbant (ELISA) method. Again, serum TPO-Ab of total study population and hCG of all the pregnant women were measured. Serum TPO-Ab by Micro particle Enzyme Immunoassay (MEIA) method and hCG was estimated by ELISA. Statistical analysis was done by SPSS version 17. Results: In this study, serum FT4 and was significantly (P<0.001) higher and TSH level was significantly (P<0.001) lower in normal pregnant women during 1st trimester in comparison to those of non pregnant women. Again, 18% of pregnant women showed TPO-Ab positivity. However, serum FT4 level was significantly (P<0.001) lower whereas, TSH level was significantly (p<0.001) higher in TPO-Ab positive pregnant women in comparison to those of TPO-Ab negative pregnant women. Conclusion: TPO-Ab positivity increases during 1st trimester of normal pregnancy which decreases the hyper functional state of thyroid hormones. Objective: The present study was focussed on evaluating the hepatoprotective capacity of a combination of leaf extracts of Murraya koenigii and Plumeria rubra on drug-induced toxicity in mice liver.Methods: Hydromethanolic and aqueous extracts were prepared from the leaves of Murraya koenigii and Plumeria rubra. A combination of these extracts was tested as a dual formulation (DF). The extracts were evaluated for their antioxidant activity and phytochemical content. Healthy adult male mice were treated in five groups, for 30 d: Control (Group I-Untreated), Group II (Methotrexate at 10 mg/kg Body weight, intraperitoneally), Group III (Methotrexate+Silymarin), Group IV (Methotrexate+hydromethanolic extract of DF-250 mg/kg b.w., orally), Group V (Methotrexate+aqueous extract of the DF-250 mg/kg b.w. orally). After treatment, the animals were necropsied. Blood was collected for serum parameters such as alanine transaminase (ALT), Aspartate transaminase (AST) and γ glutamyl transpeptidase (GGT). Further liver function tests like Alkaline phosphatase (ALP), Phosphorylase as well as Oxidative stress parameters like Lipid peroxidation (LPO), Superoxide dismutase (SOD), Catalase (CAT), along with the liver protein and cholesterol content were analyzed.Results: Administration of both the DF extracts significantly ameliorated the methotrexate-induced toxic effects on all the aforementioned biochemical parameters. The hydromethanolic extract showed significant recovery in the liver function tests like ALT (p<0.05) and AST (p<0.001). Antioxidant stress enzymes like SOD (p<0.001) and Catalase (p<0.001) also showed significant recovery after administration of both the extracts. The other parameters assayed also followed the same trend. Although both the extracts showed remarkable hepatoprotection, the hydromethanolic extract was found to be significantly more potent than the aqueous extract.Conclusion: The present study concludes that the Dual formulation of the extracts of Murraya koenigii and Plumeria rubra could be an effective hepatoprotective agent due to the synergistic action of the phytoconstituents of both the plants.
