Type 2 diabetes mellitus is one of the most common chronic health conditions around the world and is a major challenge to health care providers. Its incidence across the world is increasing. There is long-term affection of pancreas before symptoms start, allowing for a large window for early detection and prevention of the disease. We conducted this review using a comprehensive search of MEDLINE, PubMed, and EMBASE, January 1987, through March 2017. The following search terms were used: diabetes type 2, prevention diabetes type 2, effects of lifestyle on health, alcohol and diabetes, smoking and diabetes, preventive strategies for diabetes type 2. Our aim in this study is to understand the pathophysiology of diabetes mellitus type 2 with regards to the risk factors and how to prevent them. Diabetes mellitus type 2 is considered a preventable disease, therefore, studying its risk factors and trying to reduce them is very important. Even minor modifications in risk factors of the disease could achieve significant reduction in incidence and progression of diabetes. Weight loss, improved dietary intake, and increased exercise levels are the most important interventions that will lead to significant decrease in the occurrence of the disease.

Objetivo: determinar la adherencia terapéutica en personas con diabetes mellitus tipo 2 que asisten a una institución de salud, a través de la etiqueta Conducta terapéutica: enfermedad o lesión 1609 presentes. Materiales y Métodos: se realizó un estudio descriptivo de corte transversal con enfoque cuantitativo. La población de estudio estuvo conformada por 178 pacientes que cumplieron con los criterios de selección. Se obtuvo la caracterización sociodemográfica, funcionalidad y estado mental; se midió la adherencia terapéutica en escala de 1 menos adherente y 5 más adherente, con un instrumento validado basado en la etiqueta de resultado del Nursing Outcomes Classification de personas con diabetes mellitus tipo 2. Resultados: el 37.1 % de la población objeto de estudio era de sexo masculino, la ocupación predominante fue la dedicación al hogar, el principal nivel de escolaridad fue primaria, perteneciente en su mayoría a la religión católica, con una media de 11.5 de años de diagnóstico de diabetes tipo 2 y la adherencia terapéutica para la muestra estudiada mostró una media de 3,68. Conclusiones: en la muestra analizada de personas con diabetes tipo 2 se encontró una adherencia terapéutica entre en ocasiones manifestada y con frecuencia manifestada. El incumplimiento de actividad física prescrita es el factor que presentó mayores cifras de no adherencia.PALABRAS CLAVES: conductas saludables, cumplimiento de la medicación, diabetes mellitus  tipo 2, enfermería.                         Therapeutic adherence for people with diabetes mellitus type 2                                                                      ABSTRACTObjective: To determine the therapeutic adherence in people with diabetes mellitus type 2 that attend a health institution, through the therapeutic conduct protocol: disease or injury 1609 present. Materials and Methods: a cross-sectional descriptive study with a quantitative focus was performed. The population of the study was made up of 178 patients that met the criteria of selection. The socio-demographic characterization, functionality, and mental state were obtained; the therapeutic adherence was measured on a scale from 1 (less adherent) to 5 (most adherent), with a validated instrument based on the results protocol from Nursing Outcomes Classification for people with diabetes mellitus type 2. Results: 37,1% of the study target population were male, the predominant occupation was home-work, the main education level was primary school, mostly from catholic religion, with a mean of 11.5 years with diagnosis of diabetes type 2 and the therapeutic adherence for the studied sample showed a mean of 3,68 Conclusions: in the analyzed sample of people with diabetes type 2 a therapeutic adherence was found between occasionally manifested and frequently manifested. The diagnosis, treatment and rehabilitation of patients with chronic pancreatitis (CP) with concomitant diabetes mellitus (DM), whose incidence is increasing. The objective: was to investigate the state of functional capacity of the pancreas in patients with comorbidity of chronic pancreatitis and diabetes type 2. Materials and methods. 137 patients with CP with and without concomitant diabetes mellitus were studied: the main group consisted of 112 outpatients with CP in combination with diabetes mellitus in the phase of stable or unstable remission, the comparison group – 25 patients with isolated CP, and the control group – 30 healthy individuals. Verification of diagnoses and determination of the pancreas functional capacity parameters was carried out according to generally accepted world standards. Results. In comorbidity, exocrine insufficiency (ExI) was significantly more severe than in the group of patients with isolated CP in the content of fecal α-elastase, which corresponded to the medium and mild degrees of ExI respectively, and the coprogram score – respectively (5,60±0,10) and (3,89±0,16) points (p<0,05). We found a significant predominance of patients with comorbidity relative to that with an isolated course of CP among those who had mild (22,6 % vs. 11,7 %) and moderate (50,1 % vs. 3,7 %) degree of ExI in the complete absence of severe pancreas ExI in isolated CP against 5.1 % of such patients with CP and diabetes mellitus type 2. A statistically significant higher content of HbA1c in patients with CP with concomitant diabetes was found in relation to the group of patients with isolated CP (7,71±0,16 % vs. (5,66±0,10 %), glucose – respectively (28,89±0,33 mmol/l against 5,27±0,11 mmol/l), the level of the HOMA index – 3,29±0,07 against 1,63±0,09 (p<0,05), which confirmed the presence of IR in patients with pancreas endocrine insufficiency in the cohort of subjects, and also proved that these patients have diabetes type 2. Evaluation of the ultrasound score of the structure of the software proved that in comorbidity with diabetes mellitus, this figure is higher and corresponds to a severe degree, while in isolated CP of the studied patients – the average severity (6,25±0,67) points against (3,89±0,78) points. Conclusion. The distribution by index and severity of patients acording M-ANNHEIM in points statistically significantly proved that the comorbidity of CP and diabetes mellitus was dominated by patients with moderate (72,2 % vs. 24,0 %) and severe CP severity (17,0 % against 8,0 %), and in isolated CP – dominated by patients with moderate severity (64,0 % vs. 4,5 %), patients with severe were more in the group of comorbid patients (6,3 % vs. 4,0 %).

Background: Diabetes mellitus and depression are highly prevalent conditions throughout the world and have significant impact on health outcomes. It has been estimated that diabetes mellitus type 2 affects about 246 million people in the world; nevertheless, incidence varies among countries. There is evidence that depression is associated with a poor metabolic control in patients with type 2 diabetes mellitus that present other health problems (such as hypertension and obesity). The aim of this study protocol is to determine if obesity increases the risk for depression in patient with diabetes type 2.Methods: The analysis will be reported following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA).The studies suitable for inclusion will be assessed by the Newcastle-Ottawa Scale (NOS) to determine their methodological quality. To identify the studies of interest, we will search on PubMed and EBSCO databases. We will use the following keyword combinations: "Diabetes Mellitus type 2 AND obesity AND depression", "depression AND Diabetes Mellitus type 2", "Diabetes Mellitus type 2 AND body mass index cross sectional study", "depression AND obesity cross-sectional study". Causes for exclusion will be publications that studied patients diagnosed with diabetes mellitus type 1; articles that focused on the treatment and complications of diabetes mellitus type 2; publications that have studied other clinical or psychiatric conditions (for instance, seizure disorder or history of schizophrenia, bipolar disorder, psychotic symptoms or dementia).Conclusion: The results of this study will form the basis for a better understanding of the association between obesity and depression in patients with diabetes mellitus type 2, and will allow development of prediction tools and better interventions. It is evident that several modifiable and non-modifiable risk factors play an important role in the pathogenesis of diabetes among population. Currently, evidence for the deleterious effects of diabetes mellitus type 2 are based on cross-sectional or other observational designs. Therefore, this study will have important implications for future research and public health guidance.

Diabetes mellitus represents a serious medico-social problem that is caused by its high prevalence, a tendency to body height of number ofpatients, a high invalidism and a mortality. The main reason for death ofpatients with a diabetes mellitus type 2 is the pathology of cardiovascular system bound to an atherosclerosis to which leads lack of adequate compensation of a disease. In this article various risk factors of cardiovascular pathology at patients with a diabetes mellitus are surveyed and modern opportunities in their correction are discussed.
