This article looks at the principles and guidelines of the Unified Health System as well as the current situation and the limitations and possibilities of Brazilian mental health policy. Based on a review of national and international government documents and the scientific literature from 2015-2020, the study observed positive advances in mental health. However, the psychiatric reform has experienced setbacks, and the balance of mental health care has swung towards hospital-centered treatment. These changes have impeded the implementation of the Psychosocial Care Network, as well as the development of therapeutic practices and strategies focused on the person’s experience, their daily life and their relations with the health promotion network. By questioning the supremacy of medical-psychiatric knowledge in the treatment of “mental illness” in the public health care system, the psychiatric reform cleared a path for the construction of new ways of addressing psychological suffering. These gains are currently at risk, making a wider debate on the current trends in mental health care in Brazil essential.

ABSTRACT Objective: To know the experience of nurses of the Psychosocial Care Networks on the development of Nursing Appointment in Mental Health. Method: Qualitative study, with interview of 20 nurses, through the analysis of the statements in the search of the meaning core. Results: was unveiled as meaning core: lack of preparation for Mental Health Care, Non-identification of the work of the nurse in Mental Health; and Performance of Nursing Appointment as bureaucratic fulfillment. Final considerations: Nurses understand Nursing Appointment as an individualized and bureaucratic activity, to consider professional regulations, and not as a work process in the multiprofessional work, thus, the Projeto Terapêutico Singular (freely translated as Unique Therapeutic Project) is not cited as a possibility of work process. It was identified necessary space for discussion of actuation and updates of the nurse in Mental Health Care in the health services studied, with emphasis on their performance in a multiprofessional team in consonance with the Mental Health Policy.

The COVID-19 pandemic heralded challenges that were both significant and unfamiliar, placing inordinate burdens on health care systems, economies, and the collective psyche of citizens. The pandemic underscored the tenuous intersections between public mental health care, politics, economics, and psychosocial capital. In South Africa, the inadequacies of the public health system have been laid bare, and the disproportionate privileges of the private health care system exposed. This article critically considers government responses to the COVID-19 pandemic, the psychosocial correlates of lockdown, politics, corruption, and public mental health policy in South Africa.

IntroductionBudgets allocated for mental health make up a relatively small proportion of total health expenditures, although there is an increasing burden of mental disorders.ObjectivesTo review the mental health situation in Greece with regards to mental health policy through review of relevant literature.AimsTo explore the basic implications of the economic crisis from a health policy perspective, reporting constraints and opportunities.MethodsA narrative review in PubMed/Medline along with a hand search in selected Greek biomedical journals was undertaken, relevant to mental health policy.ResultsGreece is among the OECD countries with high health expenditure as a percentage of Gross Domestic Product (9.7% of GDP in 2008) but it doesn’t have a specified budget for mental health and is mostly depended in out-of-pocket expenditure (48%). The system is plagued by problems, including geographical inequalities, overcentralization, bureaucratic management and poor incentives in the public sector. The lack of cost-effectiveness and the informal payments comprise a major source of inequity and inefficiency. Uneven regional distribution of psychiatrists exists and rural areas are mostly uncovered by mental health care facilities, as well as extramural mental health units and rehabilitation places, despite the current reorganization of the whole mental health care delivery system.ConclusionsThe core problem with mental health services in Greece is the shrinking budget with poor financial administration consistent with inadequate implementation of mental health policy. A clear authority with defined responsibility for overall mental health policy and budgetary matters is needed.

Objective To aprehend the social representations about the solvability in mental health care with users of the Family Health Strategy and professionals of family health teams and of the Center for Psychosocial Care. Method A qualitative study using semi-structured interviews for data collection, and the Alceste software for analysis. This software uses the Hierarchical Descending Classification based on the examination of lexical roots, considering the words as units and providing context in the corpus. Results The representations emerge in two opposing poles: the users require satisfaction with care and the professionals realize the need for improvement of health actions. Although the matricial support in mental health and the home visits are developed, the barriers related to investment in health, continuing education and organization of care persist. Conclusion The different representations enable improvements in customer service, solvability of care and aggregate knowledge and practices in the expanded perspective of health needs in the family, social and therapeutic context.

Many people experience a traumatic event at least once in their lifetime. But only a fraction of those traumatized and in need of mental health care receive psychosocial care or treatment. This may be due to barriers people experience within the help-seeking process. The individual help-seeking process is consequently highly relevant for any mental health care for trauma survivors. Understanding why people refrain from asking for help or delay the help-seeking process is central to understanding help-seeking after traumatization. Based on empirical data and theoretical models, an integrative model of individual mental health help-seeking is developed. This integrative model delineates parameters relevant for seeking psychosocial care or refraining from it.

Americans idealize personal freedom. Every citizen recognizes the cries for life, liberty, and the pursuit of happiness demanding individual choice be upheld in the United States. Our ideals are ingrained in our culture and manifest in our national documents, governances, and traditions. Our healthcare system is no exception. For years, medical schools have taught bioethics courses to student physicians. Schools design courses to train providers in biomedical ethics. Despite developing more patient centered treatments, mental health care -- a major component of health care -- lags behind in socially just care. Throughout my thesis I will discuss the discrepancy between somatic health care and mental health care, as well as the risks of compromising basic personal autonomy. In order for physicians to provide socially just health care, they must respect the principle of autonomy and abstain from treating a patient against their will. For the purposes of this paper, I am not writing an anti- nor a pro-suicide position. I do not suggest we stand idly by while watching someone gravely injure themselves or suffocate within their pain. What I am saying is that suicidal ideation is not mitigated by confinement and social isolation. Even if you believe suicide is wrong, overreacting to suicidal ideation is an ineffective and potentially harmful way to combat suicide. In this paper, I am questioning the policies of involuntary-psychiatric hospitalization.

The study aimed to understand how to conform the Support Matrix and Individual Therapeutic Project and its relation with the production of mental health care. This is qualitative research conducted in Fortaleza-CE, Center for Psychosocial Care and Center for Family Health. 17 people participated, between professionals and patients. It was used as for dates collecting semi-structured and systematic observation. The findings were analyzed by narrative analysis, grounded theory by Ricoeur. The results reveal themselves excessive referrals, medical-centered, dependence Health Center to support the specialized care, deficiency of physical space, network disconnection, outsourcing and professional unpreparedness of the Health Center as the production difficulties of care. The Support Matrix and Individual Therapeutic Project therefore happen in everyday services, but with difficulties that affect the organization and production of care.

Objetivo: Descrever a construção de uma cartilha virtual como tecnologia de cuidado em saúde mental, sendo aplicada ao contexto de distanciamento social em decorrência dos efeitos da pandemia COVID-19. Metodologia: Estudo descritivo, do tipo relato de experiência, realizado no contexto de atuação de equipe multidisciplinar de residentes no Centro de Atenção Psicossocial, no período de março a abril de 2020. Participaram profissionais da Residência Integrada em Saúde da Escola de Saúde Pública com ênfase em Saúde Mental Coletiva no município de Guaiuba, CE. Resultados: Tendo como título “Esperançar em tempos de medo”, o processo de construção da cartilha se deu em três momentos: planejamento, levantamento de conteúdos e produção da cartilha. A partir de uma linguagem dialogada, buscou-se promover reflexões para o tempo de distanciamento social e direcionamento nos modos de enfrentamento ao medo com criatividade, cuidado de si e promoção da esperança. Conclusões: A construção desse trabalho responde ao desafio de reinvenção das práticas de cuidado e das tecnologias empregadas em saúde mental no contexto de distanciamento social em decorrência da pandemia COVID-19. Conclui-se que a cartilha desenvolvida é fruto e semente para pensar e promover suporte para cuidado e autocuidado em saúde mental no contexto atual.Descritores: Infecções por Coronavirus; Saúde Mental; Tecnologia. VIRTUAL BOOKLET CONSTRUCTION FOR MENTAL HEALTH CARE IN THE TIME OF COVID-19: EXPERIENCE REPORTObjective: To describe the construction a virtual booklet as a mental health care technology, being applied to the context of social distancing due to the effects of the COVID-19 pandemic. Methodology: Descriptive study, of the experience report type, conducted in the context of work off a multidisciplinary team of residents at the Psychosocial Care Center, from March to April 2020. Professionals from the Integrated Residency in Health of the School of Public Health with emphasis on Collective Mental Health in the city of Guaiuba, CE, participated. Results: With the title “Hoping in times of fear”, the process of construction the booklet took place in three moments: planning, content survey and production of the booklet. From a language dialogue, we sought to promote reflections for the time of social distancing and directing in the ways of facing fear with creativity, self-care and promotion of hope. Conclusion: The construction of this work responds to the challenge of reinvention of care practices and technologies employed in mental health in the context of social distancing due to the COVID-19 pandemic. It is concluded that the developed booklet is fruit and seed to think and promote support for care and self-care in mental health in the current contexto. The aim of this study was to identify mental health care technologies for treating crack users in a Psychosocial Care Center for Alcohol and other Drugs (CAPsad, as per its acronym in Portuguese). A qualitative, evaluative case study was developed in a CAPSad, using fourth generation evaluation. Data collection occurred from January to March 2013 by means of semi-structured interviews applied to 36 subjects, these being health care professionals, patients, patients' relatives and managers. Data analysis identified the category strategies in mental health work. Results showed that recovery programs should provide spaces for dialogue, aiming to clarify the process of psychiatric internment to the user and family, and involve these in the therapy, implementing educational practices and ongoing consideration of mental health activities. In conclusion, it is important to discuss the technologies used in everyday care services, in light of the complexity of crack use.
