ESTIMATION BY PARENTS OF ACCESSIBILITY AND QUALITY OF MEDICAL AID PROVIDED TO CHILDREN IN OUT-PATIENT CONDITIONS

Introduction. In modern conditions, the population’s satisfaction with medical care, including the urban and rural population, is one of the main criteria for the quality and accessibility of medical care. The goal. Subjective assessment of the organization of medical care for children in the conditions of the children’s polyclinical department of a medical ambulatory. Materials and methods. The study was conducted by random sampling using an anonymous questionnaire of 312 parents of children receiving medical care on an outpatient basis in the children’s department of the New Devyatkino village clinic in the Vsevolozhsky district of the Leningrad Region. Results. Preventive examinations, treating acute diseases, and testing when being under observation were established to be the main reasons for contacting the pediatric ward of a polyclinic are. Most of the parents spent an average of 24.5 ± 0.07 minutes on the way to the medical organization. Most often, parents choose to make an appointment with a doctor through a call center, the Internet and through an electronic record. The majority of parents (73.6%) believe the schedule of work of doctors to be convenient. More than half of parents (53.6%) believe that in the children’s outpatient department there are no specialists of the profile they need. In the opinion of parents, the most difficult is to arrange a visit to an ophthalmologist, otolaryngologist, and surgeon. In general, 65.3% of parents were satisfied with the organization of the children’s outpatient department, 28.9% were not completely satisfied, 2.3% were not completely satisfied. The main reasons for parents dissatisfaction are long queues at the registry (20.5 ± 0.08 minutes), to the pediatrician (average 22.7 ± 0.09 minutes) and highly specialized doctors (average 23.5 ± 0.09 minutes), difficulties in obtaining expert advice, and passing the examination. Conclusion There is a need to improve the availability and quality of medical care for children provided on an outpatient basis.

The article is devoted to the problems of providing medical care in outpatient settings to patients who have undergone organ transplantation. The authors analyzed the economic efficiency of the outpatient transplantation center and concluded that a new payment mechanism for this type of medical services is needed. In addition, the article describes the system of social measures necessary for recipients of organs that are performed in a medical organization due to the lack of a rehabilitation program for such patients.

In the context of the Moscow healthcare reform that has been carried out in recent years, the issues of evaluating the effectiveness of financing the healthcare system and the performance of medical organizations in providing medical care to the population are particularly relevant. Given the limited public resources allocated to the health sector, the quality of management of available financial, human and material resources is becoming more important. The article considers the application of the method of assessing the quality of resource management, introduced in Moscow since 2016, and its results in terms of evaluating the effectiveness of financial resources. It is revealed that the effectiveness of financial resources management is influenced by certain indicators that characterize the organization of management of the main activities of a medical organization.

The problem of assessing and improving the quality of medical care in a medical organization is considered. Analyzed the management of medical organization processes. A scheme for managing these processes, a list of regulatory documents, types and methods for monitoring the effectiveness of their implementation are presented. Formed groups of criteria used to assess the quality of the medical organization. It is argued that the introduction of a quality management system for the provision of medical care in a medical organization increases the efficiency of its activities.

Currently, work in any medical organization carries a risk of coronavirus infection, and, first of all, this applies to medical organizations dealing with the treatment of patients infected with coronavirus. Medical workers are a group at increased risk of infection with the SARS-CoV-2 virus in the provision of medical care in modern conditions, which determines their incidence of COVID-19 [1-3]. The aim of the study is to explore the prevalence of cases of COVID-19 infection in medical workers providing medical care on an outpatient basis. Materials and methods. The analysis of the incidence of COVID-19 medical workers in one of the large medical organizations of the Samara region during the period of the pandemic was carried out. The medical staff of the medical organization includes 207 doctors. There are 11 subdivisions in the structure of the medical and prophylactic institution. Research methods: content analysis of the modern regulatory framework (2020-2021), statistical, expert. For the period from May 2020 to March 2021 71 cases of infection of medical workers on an outpatient basis with SARS-CoV-2 were identified, of which 32 doctors (45%) and 39 people (55%) nurses. Based on the results of the expert assessment, the insurance nature of the infection case was established in 50 people (70%). Among the medical and nursing categories of medical workers, the largest number of cases was made by doctors and nurses of primary contact - specialists of the district service: general practitioners (40%), pediatricians (32%), doctors - obstetricians-gynecologists (12%), nurses adults (76%) and children (20%) polyclinic departments. A third of health workers received inpatient treatment for severe COVID-19, an average of 60 days. Conclusion. Medical workers are a risk group of COVID-19 who need support measures, since the possibility of occupational infection has been sufficiently proven, and the risk probably depends on the work performed and the conditions of direct contact of medical personnel, which requires further study in the current persisting epidemic conditions.

Primary health care for children is the mainstay of the system of medical care for children and includes measures for the prevention, diagnosis, treatment of diseases, medical rehabilitation, the development of healthy lifestyles and sanitary and hygienic education for the children. Medical organizations located in rural areas have particularities in the system of providing medical care to the population. This concerns the type, capacity, disposition of medical organizations, their provision with qualified medical personnel, and the possibility of obtaining qualified medical care. In order to assess the state of outpatient care for children under the conditions of municipal health care, data was taken out of the federal statistical reporting “Information on medical organization” form 30 “Information about the number of diseases registered in patients living in the area of service of the medical organization” form 12 for 2015-2017. The research was based on the Toksovo District Hospital, a state budgetary health care institution in the Leningrad Region. In the course of the study, pediatric staff were evaluated for pediatric staff, indicators of outpatient care for the children, indicators of the burden of pediatricians and morbidity rates for the treatment of children. It was found that in the years under study there was a high level of provision of pediatricians with an attached pediatric population, an increase in the number of district pediatricians and a decrease in the part-time ratio. The increase in the workload of pediatricians conducting outpatient admissions, including district pediatricians, associated with an increase in visits for diseases in 2015-2017, indicates a deterioration in the health of the children’s population, as evidenced by an increase in the overall morbidity rate for outpatient using the child population.

Sociological research is an important tool for assessing the social effectiveness of medical care, it allows you to analyze the quality of the organization of medical care and social satisfaction of consumers. This study is an analysis of the results of "Selective observation of the quality and availability of services in education, health and social services, employment promotion." This statistical observation has several advantages: complex procedure for forming a sample, coverage of the population in all constituent entities of the Russian Federation, including urban and rural population, ensure that the data is representative, and allow for complex analysis in the dynamics. As a result of sociological surveys of the population of the Russian Federation in 2015, 2017, 2019, it was determined that the share of respondents satisfied with the work of health center and diagnostic services remains low (less than half of the respondents). When interviewing respondents, the reasons for dissatisfaction with the work of the health center were highlighted, the “leader” among which is “the length of waiting in queues”. Moreover, the share of respondents who choose “the work of medical specialist” and “the lack of necessary equipment and drugs” as negative aspects in the provision of medical care on an outpatient basis is increasing. About half of the respondents are satisfied with the comfort of their stay, the availability of drugs and modern equipment in the hospital. Analysis of the main negative phenomena observed in the provision of medical care to the population made it possible to determine the leading trends, which remain the high cost of drugs and an increase in the volume of paid medical care. More than half of respondents noted a decline in the quality of medical services, insufficient equipment of health center and hospitals.

The study of the patient’s temporary losses when receiving medical services in an outpatient clinic and their minimization contribute to an increase in patient satisfaction with the quality of medical care. Objective of the stud y: to assess the temporary losses of patients when visiting outpatient clinics of the city municipality. Methods and materials. The temporary losses of patients were studied by analyzing route maps compiled using a special technique. The analysis of the questionnaires made it possible to assess the patient’s satisfaction with the conditions of providing medical services, the information received in the healthcare institution and the attitude of the nursing staff to the visitor. Statistical processing of the results was carried out using the programs Statistica 5.0 and Microsoft Office Excel. Results. As a result of the conducted research, significant time costs of patients were identified when receiving medical care in polyclinics of the district center: when visiting a district doctor, a registry, a treatment room and a laboratory, which directly affected the results of assessing their satisfaction with the temporary criteria for visiting a medical organization. Scope of the results. The data obtained during the study can be used as basic information when developing measures to optimize key processes in outpatient health care institutions.

The increase in the number of patients with pathology of the maxillofacial region necessitates improving the quality of medical care provided to the population in the field of "Maxillofacial surgery". Anatomical and physiological features of the maxillofacial region, the prevalence of pathology determine the implementation of organizational measures, the involvement of narrow specialists, the need for medical organizations in modern material and technical equipment. The indicators of medical care mainly depend on a full-fledged diagnosis, and are also determined by the availability, quality and speed of specialized medical care. In modern conditions of the development of the health care system, an integrated approach to the organization, the introduction of new methods of providing medical care and rehabilitation of patients is required. Optimization of the maxillofacial surgery service within a multidisciplinary medical organization, the introduction and development of an outpatient polyclinic link, the training of highly qualified personnel contribute to improving the quality of diagnosis, treatment and timely detection of pathology. This article presents the results of optimizing the service for providing medical care to patients with pathology of the maxillofacial region. In the Penza region, the full-fledged, high-quality functioning of the service is realized through the organization of the Center for Maxillofacial Surgery on the basis of the State Budgetary Healthcare Institution «Penza Regional Clinical Hospital named after N.N. Burdenko». The structure of the Center for Maxillofacial Surgery ensures the continuity and complementarity of specialists of a medical organization in the treatment and diagnostic process. Optimal conditions have been created for the diagnosis, treatment and rehabilitation of patients with pathology of the maxillofacial region, in order to provide the population with affordable and high-quality medical care, regardless of the category of citizens. The creation of the Center for Oral and Maxillofacial Surgery contributes to an increase in the structural efficiency and medical efficiency of the health care system of the Penza region. The results of the measures and innovations carried out can be used to eliminate the existing shortcomings in the organization of medical care for patients with pathology of the maxillofacial region in other regions.

High quality of medical care is the primary goal of a health system. At the moment it cann’t be considered high. The results of sociological surveys presented that quite a large proportion of patients isn’t satisfied with the care provided by medical assistance. The causes of this are: absence of funding for health care (and related problems of material and technical base, staffing, staff interest and other), as well as the absence of commonly agreed (and unified with the world) approaches to the definition criteria (indicators), means of verification and a unified system of quality assurance. The moral and ideological factors (destructive changes in the medical profession, the divergence of moral and professional principles, a dominance of the economic aspects of rela-tions, reduced social activity and responsibility) are important. In addition, managers of medical institutions are the following problems of theoretical and methodological nature: the need for a clear distinction between the concepts of "quality of care", "quality of medical services", "quality organization" (including accessibility, ethi-cal aspects, sanitary conditions, which significantly affect the level of subjective assessment). In the context of globalization and international cooperation, it is very important to bring domestic approaches to understanding, assessing, managing the quality of health care in the world.
