Background: Bitewing radiography is a technique that depicts the crown of the maxillary and mandibular teeth and the crest of the alveolar bone in the same receptor. The use of film holders and paper loops in bitewing techniques is very helpful in standardising radiographs, but it has some disadvantages, including the lack of efficiency and discomfort. Therefore, silicone has been widely used in the medical field as a replacement for paper loops. Purpose: This study was conducted to describe the compatibility of the silicone material as an alternative for bitewing radiography. Methods: This research is experimental and a one-shot case study. It used the Mann–Whitney (P < 0.05) test for statistical analysis to compare the results of the radiographs using silicone loops and paper loops and to analyse the quality of each radiograph: object coverage, density, contrast, sharpness, geometry, and overlapping. Results: The images where silicone loops were used show adequate results in six radiograph quality assessments. There was no significant difference between the radiographs that were obtained using the silicon loop and the paper loop (p > 0.05). Conclusion: Silicone loops can be an alternative tool for bitewing radiography because they result in optimal image quality.

Introduction After extensive mediastinal dissection fails to achieve adequate intra-abdominal esophageal length, a Collis gastroplasty(CG) is recommended to decrease axial tension and reduce hiatal hernia recurrence. However, concerns exist about staple line leak, and long-term symptoms of heartburn and dysphagia due to the acid-producing neoesophagus which lacks peristaltic activity. This study aimed to assess long-term satisfaction and GERD-related quality of life after robotic fundoplication with CG (wedge fundectomy technique) and to compare outcomes to patients who underwent fundoplication without CG. Outcomes studied included patient satisfaction, resumption of proton pump inhibitors (PPI), length of surgery (LOS), hospital stay, and reintervention. Methods This was a single-center retrospective analysis of patients from January 2017 through December 2018 undergoing elective robotic hiatal hernia repair and fundoplication. 61 patients were contacted for follow-up, of which 20 responded. Of those 20 patients, 7 had a CG performed during surgery while 13 did not. There was no significant difference in size and type of hiatal hernias in the 2 groups. These patients agreed to give their feedback via a GERD health-related quality of life (GERD HRQL) questionnaire. Their medical records were reviewed for LOS, length of hospital stay (LOH), and reintervention needed. Statistical analysis was performed using SPSS v 25. Satisfaction and need for PPIs were compared between the treatment and control groups using the chi-square test of independence. Results Statistical analysis showed that satisfaction with outcome and PPI resumption was not significantly different between both groups ( P > .05). There was a significant difference in the average ranks between the 2 groups for the question on postoperative dysphagia on the follow-up GERD HRQL questionnaire, with the group with CG reporting no dysphagia. There were no significant differences in the average ranks between the 2 groups for the remaining 15 questions ( P > .05). The median LOS was longer in patients who had a CG compared to patients who did not (250 vs. 148 min) ( P = .01). The LOH stay was not significantly different ( P > .05) with a median length of stay of 2 days observed in both groups. There were no leaks in the Collis group and no reoperations, conversions, or blood transfusions needed in either group. Conclusion Collis gastroplasty is a safe option to utilize for short esophagus noted despite extensive mediastinal mobilization and does not adversely affect the LOH stay, need for reoperation, or patient long-term satisfaction.

Eight New York nursing homes, 4 open and 4 closed, were chosen at random for analysis. They were examined to determine if a relationship between age of assets, fiscal viability and quality of care existed. Three years of data for each nursing home was selected Several financial variables were used to construct a fiscal viability index; and a patient care index was created from selected measures that are used to measure specific aspects of institutional care. The premise was that the constructed indices will demonstrate a significant difference between closed nursing homes and homes remaining open. The analysis found that fiscal viability index could be a significant factor to differentiate the two groups of nursing homes, but the quality index showed no significant difference between the two groups.

Purpose: In an era of heightened competition to sustain patient loyalty, quality of interpersonal experiences with the staff, operational quality of hospital operations and overall satisfaction and quality of the healthcare services, in general are to be understood in-depth in both public as well as private organisations. The concept of service quality has led to the growing research on various concepts such as total quality management, market orientation, learning orientation, strategic orientation along with invigorating focus on the customer loyalty and relationship management. The outcomes of service quality (customer relationship management and loyalty), end means for the organisational success, are most significant performance measurement tools in the present competitive market. The present study is an effort in this regard to identify the factors in sustaining customer longevity. Till today few studies in the developing settings were conducted to understand the types of relationship that exists between patient-loyalty and service quality. This study analyses the suitability of customer loyalty concept in the government hospitals through using a case study of one of the biggest hospitals operating in India. Methodology and Tools Applied: The research work is the case study of Civil Hospital Ahmedabad. The data is collected from 205 indoor patients of four departments namely general medicine, orthopedic, pediatrics, obstetric and gynecology. Inter and intra relationship among the measures of service quality and patient loyalty were analysed by using relevant statistical tools to draw out inferences. Findings: Among the three patient loyalty components, using provider again for the same services is found to be more significant followed by using provider again for different services and recommending providers to others in relation to overall service quality as dependent variable, and beta values are figured out as 0.15, 0.12 and 0.09 respectively The 35% R Square value for the model service quality – patient loyalty model indicate weak predictive power of this model. However at the same juncture the study found that no significant difference in the patients' perceptions with respect to patient loyalty and quality and per se, may be concluded that both are identical measures. Alternatively the more satisfied the patients are with the quality of their interactions with staff, the more likely they are going to take treatments for similar and different medical problems and would recommend the provider to their relatives and friends.

With regard to the outpatient areas of a hospital, the smoothness of the route is now taken into consideration in the process of configuring the wayfinding system. As patients often spend time on ineffective wayfinding processes, and there is limited manpower at hospitals and a lack of clarity in the information provided by the wayfinding system, it is difficult to provide effective and timely consultation services for patients. This study was conducted at Cheng Ching Hospital, Chung Kang Branch (CCH/CKB) in Taiwan. This study attempts to investigate the relationships between the wayfinding system of the outpatient areas and the patients’ behaviors in the hospital. Depthmap software based on space syntax is adopted to assist in the route analysis and wayfinding behaviors. It integrates axial mapping analysis and isovist analysis and gives suggestions on the location, format and content of the wayfinding system. The final results of the study show that in the wayfinding task experiment gender has no significant impact on the effect of wayfinding efficiency, while a significant difference is found for age. Older people need more time to complete the wayfinding task, which means that they have poorer performance in wayfinding efficiency. The analysis of the results of space syntax shows that a good wayfinding system should be a symmetric tree-branch structure rather than circular structure in a medical building, that areas where it is easy to become lost should have a clear signage guiding system planning and configuration, and that clear guidance information should be provided to the patients to achieve the goal of saving consultation time and improving the quality of the medical environment.
