Each sport has its specific rules, which determine what is allowed (or not) impacting directly on the sport demands. Studies involving physiological and time-motion measurements have shown that soccer is a highly demanding sport. The new coronavirus disease 2019 (COVID-19) has been a world health crisis. Soccer seasons were interrupted worldwide to avoid spreading the virus. Leagues resumed the season (no fans at the arenas) after several weeks of interruption, causing overlay of schedule. This overlay (e.g., games every Sunday and Wednesday) will cause accumulated fatigue on players, raising the risk of injuries. Considering this condensed calendar, the Fédération Internationale de Football Association (FIFA) has changed (temporarily) up to five substitutions during elite games (instead of three as the regular rule allows). Considering the already published scientific evidence, clearly, the change in the soccer substitution rule due to COVID-19 is insufficient. Implementing unlimited substitutions may benefit soccer players' health, coaches’ jobs, more entertainment for fans and sponsors (e.g., keeping intensity during all game, including on the second half) and eventually prolonging the useful life of the players. A real game-changer!

Characteristics and Specialization of the Immune Response in COVID-19 Abstract The outbreak of coronavirus pneumonia in Wuhan, China, became a pandemic on March 11, 2020. It has caused almost 4 million confirmed cases worldwide, with more than 270,000 deaths. Coronavirus is an enveloped RNA virus of the β-coronavirus genus distributed in birds, humans, and other mammals. The World Health Organization has named the new disease COVID-19. The scientific community is look http://doi.org/10.22201/fm.24484865e.2020.63.4.02 8 8 Revista de la Facultad de Medicina de la UNAM | ing for evidence that can lead to a better understanding of the infection and the immune response (IR), prognostic and therapeutic predictors, effective treatments and vaccines. The objective of this review was to compile updated scientific evidence of the IR to COVID-19, in order to guide professionals with solutions that have a clinical impact. The most important elements involve innate immunity with failures in the interferon system in the early stages of the infection and a sustained increase in proinflammatory interleukins. This can end in a potentially fatal cytokine storm. The infiltration of neutrophils and macrophages at the alveolar level, accompanied by neutrophilia, is very characteristic. Lymphopenia is evident at the adaptive immunity level, that, depending on the degree, can indicate the severity of the disease. Understanding the temporal sequence of the IR is crucial for choosing the appropriate and effective therapies, especially when selecting which type of anti-inflammatory drugs can be used and the frequency of the dosage. Due to the fact that it is difficult to determine when they will be clearly beneficial, not harmful to the IR and not too late, due to the irreversibility of the process. The December 2019 COVID-19 outbreak in China has led to worldwide quarantine, as recommended by local governments and the World Health Organization. Particularly affected are older adults (i.e., those aged ≥ 65 years) who are at elevated risk for various adverse health outcomes, including declines in motor ability and physical activity (PA) participation, increased obesity, impaired cognition, and various psychological disorders. Thus, given the secular increases in the older adult population, novel and effective intervention strategies are necessary to improve physical activity behaviors and health in this population. Virtual reality (VR)-integrated exercise is a promising intervention strategy, which has been utilized in healthcare fields like stroke rehabilitation and psychotherapy. Therefore, the purpose of this editorial is to synthesize recent research examining the efficacy and effectiveness of VR exercise in the promotion of favorable health outcomes among the older adults. Results indicate the application of VR exercise to facilitate improved physical outcomes (e.g., enhanced motor ability, reduced obesity), cognition and psychological outcomes. VR exercise has also been observed to be an effective intervention strategy for fall prevention in this population. Future research should employ more rigorous research designs to allow for a more robust quantitative synthesis of the effect of VR exercise on the preceding outcomes to elucidate which type(s) of VR-based PA interventions are most effective in promoting improved health outcomes among older adults. Findings from this study will better inform the development of technology-savvy PA programs for wellness promotion in older adults who practice social distancing and exercise from home under the unprecedented global health crisis.

One year after the declaration of the COVID-19 pandemic by the World Health Organization (WHO) and despite the implementation of mandatory physical barriers and social distancing, humanity remains challenged by a long-lasting and devastating public health crisis. Non-pharmacological interventions (NPI) are efficient mitigation strategies. The success of these intense NPI is dependent on the approval and commitment of the population. The launch of a mass vaccination program in many countries in late December 2020 with mRNA vaccines, adenovirus-based vaccines, and inactivated virus vaccines has generated hope for the end of the pandemic. Current issues: The continuous appearance of new pathogenic viral strains and the ability of vaccines to prevent infection and transmission raise important concerns as we try to achieve community immunity against SARS-CoV-2 and its variants. The need of a second and even third generation of vaccines and the possibility of potentially harmful side-effects of the vaccines (i.e. venous thromboembolism ) have already been acknowledged. Perspectives: There is a critical and urgent need for a balanced and integrated strategy for the management of the COVID-19 outbreaks organized on three axes: (1) Prevention of the SARS-CoV-2 infection, (2) Detection and early diagnosis of patients at risk of disease worsening, and (3) Anticipation of medical care (PDA). Conclusion: The “PDA strategy” integrated into state policy for the support and expansion of health systems and introduction of digital organization (i.e. telemedicine, artificial intelligence and machine learning technology) is of major importance for the preservation of citizens’ health and life world-wide.

Coronavirus disease 2019 (COVID-19) is a pandemic disease of modern time with unique and rapid transmission rate and affected almost all the nations without respecting any border. Coronavirus disease 2019 (COVID-19) is arguably the biggest health crisis the world has faced in 21st century. It is an infectious disease and declared pandemic by the World Health Organization. The coronavirus disease 2019 (COVID-19) outbreak, which originated in Wuhan, China, has now spread to 192 countries and administrative regions infecting nearly 800,000 individuals of all ages as of 31 March 2020. Though most infected individuals exhibit mild symptoms including fever, upper respiratory tract symptoms, shortness of breath, and diarrhoea, or are asymptomatic altogether, severe cases of infection can lead to pneumonia, multiple organ failure, and death. Globally, at least 7900 deaths have been directly attributed to COVID19, and this number is expected to rise with the ongoing epidemic. This is particularly crucial as the current outbreak involves a new pathogen (SARS-CoV-2), on which limited knowledge exists of its infectivity and clinical profile. Research is in progress on therapeutic efficacy of various agents including anti-malarials (Chloroquine and Hydroxychloroquine), antiviral drugs, and convalescent serum of recovered patients. Unani system of medicine is one of the traditional systems of medicine which is being explored for providing preventive, supportive and rehabilitative care to patients. Unani system of medicine has a detailed description of drugs that are utilized in many infectious diseases, including respiratory infections. Immune response is essential to eliminate virus and to preclude disease progression to severe stages. Therefore, it is important to summarize the evidence regarding the preventive measures, control options such as immune-stimulator and prophylactic treatment in Unani medicine against Covid19. This review summarizes various pharmacological actions of Unani formulation Tiryaq-e-Arba in Unani literature and various reported pharmacological activities which can possibly provide prevention, control and reduction of complications of this deadly disease.

ArgumentScientific uncertainty is fundamental to the management of contemporary global risks. In 2009, the World Health Organization (WHO) declared the start of the H1N1 Influenza Pandemic. This declaration signified the risk posed by the spread of the H1N1 virus, and in turn precipitated a range of actions by global public health actors. This article analyzes the WHO's public representation of risk and examines the centrality of scientific uncertainty in the case of H1N1. It argues that the WHO's risk narrative reflected the context of scientific uncertainty in which it was working. The WHO argued that it was attempting to remain faithful to the scientific evidence, and the uncertain nature of the threat. However, as a result, the WHO's public risk narrative was neither consistent nor socially robust, leading to the eventual contestation of the WHO's position by other global public health actors, most notably the Council of Europe. This illustrates both the significance of scientific uncertainty in the investigation of risk, and the difficulty for risk managing institutions in effectively acting in the face of this uncertainty.

The coronavirus disease 2019 (COVID-19) pandemic is a general health crisis and has irreversible impacts on human societies. Globally, all people are at risk of being exposed to the novel coronavirus through transmission of airborne bioaerosols. Public health actions, such as wearing a mask, are highly recommended to reduce the transmission of infectious diseases. The appropriate use of masks is necessary for effectively preventing the transmission of airborne bioaerosols. The World Health Organization (WHO) suggests washing fabric masks or throwing away disposable masks after they are used. However, people often use masks more than once without washing or disposing them. The prolonged use of a single mask might—as a result of the user habitually touching the mask—promote the spread of pathogens from airborne bioaerosols that have accumulated on the mask. Therefore, it is necessary to evaluate how long the living components of bioaerosols can be viable on the masks. Here, we evaluated the viability of airborne Bacillus subtilis (B. subtilis) in bioaerosols filtered on woven and anti-droplet (non-woven) face masks. As a simulation of being simultaneously exposed to sand dust and bioaerosols, the viability rates of bioaerosols that had accumulated on masks were also tested against fine dust and airborne droplets containing bacteria. The bioaerosols survived on the masks immediately after the masks were used to filter the bioaerosols, and the bacteria significantly proliferated after one day of storage. Thereafter, the number of viable cells in the filtered bioaerosols gradually decreased over time, and the viability of B. subtilis in bioaerosols on the masks varied, depending on the mask material used (woven or non-woven). Despite the reduction in viability, bioaerosols containing living components were still found in both woven and anti-droplet masks even after six days of storage and it took nine days not to have found them on masks. The number of viable cells in bioaerosols on masks significantly decreased upon exposure of the masks to fine dust. The results of this study should provide useful information on how to appropriately use masks to increase their duration of effectiveness against bioaerosols.

In the past 100 years, the world has faced four distinctly different pandemics: the Spanish flu of 1918-1919, the SARS pandemic of 2003, the H1N1 or “swine flu” pandemic of 2012, and the ongoing COVID-19 pandemic. Each public health crisis exposed specific systemic shortfalls and provided public health lessons for future events. The Spanish flu revealed a nursing shortage and led to a great appreciation of nursing as a profession. SARS showed the importance of having frontline clinicians be able to work with regulators and those producing guidelines. H1N1 raised questions about the nature of a global organization such as the World Health Organization in terms of the benefits and potential disadvantages of leading the fight against a long-term global public health threat. In the era of COVID-19, it seems apparent that we are learning about both the blessing and curse of social media.

On June 11, 2009, the Director General of the World Health Organization, Dr. Margaret Chan, announced that the scientific evidence indicated that the criteria for an influenza pandemic had been met: pandemic H1N1/09 virus, the first in nearly 40 years, was officially upon us. The World Health Organization has estimated that as many as 2 billion or between 15 and 45 percent of the population globally will be infected by the H1N1/09 virus. Scientists and governments have been careful to walk a line between causing mass public fear and ensuring people take the risks seriously. The latest information indicates that the majority of individuals infected with the H1N1/09 virus thus far have suffered mild illness, although very severe and fatal illness have been observed in a small number of cases, even in young and healthy people (World Health Organization 2009c). There is no evidence to date that the virus has mutated to a more virulent or lethal form; however, as we enter the second wave of the pandemic, a significant number of people in countries across the world are susceptible to infection. Most importantly, certain subgroups have been categorized as high risk given the clinical evidence to date. One of these subgroups is Indigenous populations (World Health Organization 2009c).

Nutrition is an undeniable part of promoting health and performance among football (soccer) players. Nevertheless, nutritional strategies adopted in elite football can vary significantly depending on culture, habit and practical constraints and might not always be supported by scientific evidence. Therefore, a group of 28 Portuguese experts on sports nutrition, sports science and sports medicine sought to discuss current practices in the elite football landscape and review the existing evidence on nutritional strategies to be applied when supporting football players. Starting from understanding football’s physical and physiological demands, five different moments were identified: preparing to play, match-day, recovery after matches, between matches and during injury or rehabilitation periods. When applicable, specificities of nutritional support to young athletes and female players were also addressed. The result is a set of practical recommendations that gathered consensus among involved experts, highlighting carbohydrates periodisation, hydration and conscious use of dietary supplements.

Abstract Covid-19, the greatest global health crisis for a century, brought a new immediacy and urgency to international bio-medical research. The pandemic generated intense competition to produce a vaccine and contain the virus, creating what the World Health Organization referred to as an ‘infodemic’ of published output. In this frantic atmosphere, researchers were keen to get their research noticed. In this paper, we explore whether this enthusiasm influenced the rhetorical presentation of research and encouraged scientists to “sell” their studies. Examining a corpus of the most highly cited SCI articles on the virus published in the first seven months of 2020, we explore authors’ use of hyperbolic and promotional language to boost aspects of their research. Our results show a significant increase in hype to stress certainty, contribution, novelty and potential, especially regarding research methods, outcomes and primacy. Our study sheds light on scientific persuasion at a time of intense social anxiety.
