Twin anemia polycythemia sequence after fetoscopy: predictors of development

Objective. To identify predictors of post-fetoscopic twin anemia polycythemia sequence (pTAPS) after fetoscopic laser photocoagulation (FLP) of placental anastomoses for twin–twin transfusion syndrome (TTTS). Patients and methods. The study included 195 patients who underwent FLP of anastomoses for TTTS during the period from 2010 to September 2019. A group of 13 pregnant women in whom pTAPS developed in the postoperative period was selected. Results. Out of 169 pregnancies lasting for more than 2 weeks after FLP of anastomoses, pTAPS developed in 13 (7.7%). It was revealed that FLP was performed before 20 weeks in 85% of those who developed pTAPS, while the average period in the group was 22.1 weeks. Localization of the placenta along the anterior wall of the uterus with pTAPS was observed 3 times more often than along the posterior wall, while in the rest of the operated patients this ratio was approximately 1:1. The number of anastomoses ranged from 3 to 11, most frequently – 5. Conclusion. With the development of pTAPS, there is a high probability of an unfavorable outcome. Localization of the placenta along the anterior wall of the uterus, localization of the anastomoses in the amniotic cavity of the donor twin are the factors that predispose to the development of pTAPS. The presence of 1–2 large AV anastomoses together with no more than 5 small anastomoses may serve as a probable predictor of the pTAPS development. Objectives: To sonographically assess uterine leiomyoma among pregnant women. Study Design: Cross-sectional Descriptive study. Setting: Gilani Ultrasound Center, Ferozepur Road Lahore. Period: Sep to Dec 2019. Material & Methods: The sample size was all the pregnant women with fibroid. Ultrasound machine Honda 2000 HS and Toshiba xerio x4 were used. Results: Out of 73 patients, 47(64.4%) had fibroid at the anterior wall of the uterus, 14(19.2%) patients had fibroid at the posterior wall of the uterus, 5(6.8%) patients had submucosal fibroid, 2(2.7%) patients had fibroid in the lateral wall of the uterus, 2(2.7%) patients had fibroid at fundal region of the uterus and 1(1.4%) of each had fibroid in cervix, lower uterine segment and subserosal. Conclusion: The findings of this study concluded that the anterior wall of the uterus is more favorable for leiomyoma in pregnant women.

The objective: to evaluate the results of planned open surgical interventions (POSI) in pregnant women with primary symptomatic chronic vein disease (PSCVD). Materials and methods. The study included 457 pregnant women operated on a routine basis with PSCVD. The patients underwent ultrasound duplex angioscanning (USDA) of the veins of the lower extremities, inguinal canals, and iliac veins. The severity of the clinical manifestations of PSCVD was evaluated on a VCSS scale. With the help of the circadian visual-analog scale, the dynamics after the operative pain were studied. The effect of POSI on uterine tone and cardiac function of the fetus was investigated using cardiotocography. When forming the results for POSI, data from USDA, the severity of clinical and cosmetic manifestations of PCVD, the need for active prevention of thrombophlebetic, thromboembolic and hemorrhagic complications, minimization of surgical childbirth were taken into account. The results of treatment were studied in 2 groups of patients. Patients of the first group were treated with conservative therapy, the second group – POSI. Parametric indicators, which were presented as mean and standard error M(SD), were used for statistical data processing of the study, and their reliability was estimated using Student’s t-test. The species was determined to be significant at p<0.05. Results. 495 POSI were performed at 28-38 weeks gestation in an obstetric hospital (perinatal center), by a vascular surgeon from the staff of the center, who was familiarized with the peculiarities of working with this contingent of patients. Surgery was performed under local anesthesia in 346 (75.49%) pregnant women at one extremity, in 111 (24.51%) – at two in one session. According to the results of the study, it was noted that in the first group 33.7% of patients had a positive clinical result due to the reduction of pain syndrome, while 67.5% of patients had an increase and spread of varicose transformation in the area of saphenous and/or non-saphenous veins with spread of pathological venous reflux in the distal direction. 78% of patients had the need to administer prophylactic doses of low molecular weight heparins, both during pregnancy and in postpartum period. Out of 126 pregnant women with pronounced varicose veins of the external genital organs and perineum in 36 (28.6%) deliveries were performed by caesarean section. In 9 (3%) patients there was an acute thrombophlebitis of the subcutaneous veins of the lower extremities, which required 5 pregnant women to undergo urgent surgery when the inflammatory process had spread to the middle and above along the femoral vein of the large subcutaneous vein. Pregnant women of the second group with PSCVD on the basis of obstetric hospital (perinatal center) POSI were performed in the organization, which laid the ideology of the FTS program, strictly individual indications for intervention in the optimal terms of pregnancy, multidisciplinary management of patients, due to this in 93% of operated patients regression of clinical manifestations of the disease was noted, whch contributed to the correct and safe delivery of pregnancy with 100% absence of preterm birth, abnormalities in fetal development, pregnancy course, negative impact on obstetric and somatic condition of the pregnant woman. No hemorrhagic, thrombophlebic, thromboembolic complications were noted. Patients in the postoperative period did not require medical support, as during the period of pregnancy, during delivery and in the postpartum period. There were no indications for surgical delivery. 2.4% of patients experienced complications of post-operative wounds in the form of cheese, which had no effect on pregnancy and was eliminated before delivery. In the postoperative period, if necessary, it was recommended to use elastic compression class garments 1–2. Conclusion. POSI made at PSCVD in optimal terms of pregnancy in a specialized obstetric hospital by a vascular surgeon in strictly individual indications is safe, both for the fetus and the pregnant woman. It is promising to further study the results of POSI in pregnant women with PSCVD to introduce it into the arsenal of treatment of this pathology. The objective: depression of frequency of perinatal pathology at women with decompensation form of placental dysfunction by improvement of the main diagnostic and treatment-and-prophylactic actions. Patients and methods. 154 pregnant women in gestation term from 22 to 40 weeks were surveyed. Depending on features of course of pregnancy and families of all surveyed it was divided into 4 groups. The group of the retrospective analysis was made by 45 pregnant women with decompensation placental dysfuction, the group of prospective research included 109 pregnant women of whom the main group was made by 38 women with decompensation form of placental dysfunction, the group of comparison included 47 pregnant women with the compensated form of placental dysfunction. The control group was made by 24 pregnant women with the uncomplicated course of pregnancy and labors. The complex of the conducted researches included clinical, ehografical, dopplerometrical, laboratory, morphological and statistical methods. Results. Use of advanced algorithm of diagnostic and treatment-and-prophylactic actions allows to increase efficiency of diagnostics of decompensation form of placental dysfunction for 33.3%, and rational tactics of a delivery leads to depression of perinatal pathology for 22.7%. Conclusion. Decompensation placental dysfuction is one of the main reasons for perinatal mortality and a case rate at the present stage. Use of the algorithm of diagnostic and treatment-and-prophylactic actions improved by us allows major factors of risk of this complication and the indication for change of tactics and delivery times. Background: No specific physical activity guidelines are available for women in multiple pregnancy. Aim of the study was to assess the knowledge and experience of women regarding physical activity during their latest twin pregnancy. Methods: A cross-sectional study including women after a twin delivery was conducted in Poland. A questionnaire was distributed in 2018 via web pages and Facebook groups designed for pregnant women. Results: 652 women filled out the questionnaire completely. Only 25% of women performed any physical exercises during twin gestation. The frequency of preterm delivery was similar in physically active and non-active participants. 35% of the respondents claimed to have gained information on proper activity from obstetricians during antenatal counselling while 11% claimed to be unable to identify the reliable sources of information. 7% of women admitted to feel discriminated by social opinion on exercising during a twin pregnancy. Conclusions: The population of women with a twin gestation is not sufficiently physically active and is often discouraged from performing exercises during gestation. Therefore, it is crucial to inform obstetricians to recommend active lifestyle during a twin gestation and to provide reliable information on physical activity to pregnant women. Further research on this topic is necessary in order for obstetric providers to counsel women on appropriate exercise with a twin pregnancy.

The objective: to study features of obstetric and perinitalny pathology at women with undifferentiated dysplasia of a connecting tissue. Patients and methods. 100 patients with clinical-laboratory signs of an undifferentiated dysplasia of connecting tissue are surveyed. On the basis of the received results of the patient were divided into two groups: the main – 50 women (expression of і6 points) whom regarded as patients with the expressed undifferentiated dysplasia of connecting tissue, and group of comparison – 50 women (expression <6 points) whom regarded as patients without the expressed undifferentiated dysplasia of connecting tissue. Results. Results of the conducted researches testify to the high frequency of obstetric and perinatal pathology at pregnant women with clinical-laboratory and functional signs of an undifferentiated dysplasia of connecting tissue. Conclusion. The received results are the basis for algorithm improvement the diagnostical and treatment-and-prophylactic actions at women with an undifferentiated dysplasia of connecting tissue. The objective: to improve tactics of pregravidare training of women with hysteromyoma. Patients and methods. Under observation there were 100 patients of active and late genesial age (from 22 to 44 years), with the main diagnosis hysteromyoma established on the basis of complex inspection. To all patients the conservative myomectomy by laparotomny that laparoscopic accesses out of pregnancy was made, a course of complex rehabilitational therapy in the early and late postoperative period is conducted, the next and remote results are tracked. Results. Efficiency of pregravidarny training of patients with a hysteromyoma made 67.0%, thus authentically larger frequency of offensive of pregnancy is noted in group of the patients receiving in the postoperative period therapy by agonists of GNRG (73.0%), including at women with genesial problems (52.0%). Besides, purpose of agonists of GNRG after operation allows to reduce number of recurrence of a disease in the remote period. Conclusions. The advanced technique of pregravidarny preparation allows to improve the genesial forecast of women with hysteromyoma. Key words: hysteromyoma, pregravidarny preparation, optimization.

The main data on general issues of epidemiology, etiology, pathogenesis and classification of chronic glomerulonephritis (CGN) in Ukraine were described in the article. The main information on the peculiarities of this clinic disease in pregnant women, on the course and complications of pregnancy, features of fetal development in pregnant women suffering from chronic pyelonephritis were highlighted. Separately, the questions devoted to the optimal tactics of diagnosis and treatment of CGN in pregnant women with characteristics of medicines and their groups applicable in this category of patients are disclosed. Patients and methods. 100 patients with clinical-laboratory signs an undifferentiated dysplasia of connecting tissue are surveyed. On the basis of the received results of the patient were divided into two groups: the main – 50 women (expression і6 points) regarded as patients with the expressed undifferentiated dysplasia of connecting tissue and group of comparison – 50 women (expression <6 points), regarded as patients without the connecting tissue expressed by an undifferentiated dysplasia. Results. Results of the conducted researches testify to an important role of the established features of the somatic and genesial anamnesis at pregnant women with clinical-laboratory and functional signs of an undifferentiated dysplasia of a connecting tissue. Conclusion. The received results need to be used for carrying out pregravidarny preparation and forecasting of possible obstetric and perinatal complications at pregnancy. The objective: generalization of the experience of using chlorhexidine drugs in obstetric and gynecological practice. Materials and methods. The efficacy of using Depantol and Hexicon for non-specific colpitis in patients of adolescent, reproductive age, pregnant women, women in perimenopause, as well as in patients after cervical, vaginal and perineal operations was evaluated and generalized. The surveyed groups contained 30 to 50 patients. The diagnosis of nonspecific vaginitis was verified by clinical and laboratory examination. Criteria for recovery were complete clinical and bacterial sanation. The effectiveness of therapy was evaluated immediately after treatment and after 1 and 3 months. The data were compared with the appropriate control groups or with the condition prior to treatment. Results. High efficiency of chlorhexidine preparations in the treatment of nonspecific colpitis in patients of different age categories and conditions was noted. The experience of using Depantol and Gecikon in obstetrics and gynecology is generalized, priorities are determined in the application of these drugs. Conclusion. Preparations of chlorhexidine Geksikon and Depantol are highly effective against the main pathogens of inflammatory nonspecific vulvovaginitis. They do not interfere with the functional activity of lactobacilli, which helps maintain a normal vaginal biocenosis. The drug Geksikon with success can be recommended both for the treatment of gynecological patients of reproductive age, and pregnant women. The use of Depantol, due to the regenerative activity of the dexpanthenol component, is a priority in the treatment of infectious processes in adolescence, perimenopausal age and in patients after operations on the cervix, vagina and perineum. The objective: depression of obstetric and perinatal complications at pregnant women with pathological increase of body weight during pregnancy on the basis of studying of clinical-functional, endocrinologic, metabolic, morphological features of condition of fetoplacental complex and development of advanced algorithm of treatment-and-prophylactic actions. Materials and methods. We conducted examination of 264 patients who consisted on the account concerning pregnancy in female consultations of Kiev. By a method of selection of 178 women were included in retrospective research and 86 – in prospective. The main group was made by 39 pregnant women with whom led individual discussions, control – 142 women, that received traditional references in female consultation. Results. Use of the algorithm improved by us allows to improve obstetric (decrease of frequency of a preeclampsia by 12.3%; anomalies of a patrimonial deyalnost for 8.4%) and perinatal outcomes (depression of level of an asphyxia at a delivery for 9.4% and post-natal adaptation for 7.8%) deliveries of women with an excessive increase of body weight during pregnancy. Conclusion. The received results grant the right to recommend the algorithm improved by us for wide use in practical health care.
