ABSTRACTThe aim of this review was to present and discuss current views on the treatment of condylar fractures (CFs). The authors addressed the following issues: the etiology, epidemiology and mechanisms of CFs; strategies and methods for the treatment of CFs. Moreover, the choice of surgical approach for the open treatment of CFs as well as techniques and materials used for fixation of CF are discussed.The PubMed database was used to search for relevant articles published between 2000 and 2018. The analysis referred to both original and review papers (including meta-analyses) that concerned adult patients. There are still differences in opinions among researchers regarding the choice of appropriate treatment – closed or open. There is no consensus among open treatment supporters in choosing the right surgical approach. The important question is which material to choose for osteosynthesis of the condylar fracture.Recent studies show satisfactory results in terms of stability of condylar osteosynthesis with the use of 3D plates. Further discussion is required on the choice of material for fixation of the condylar fracture and, in particular, on the possibility of using resorbable materials.

As fraturas mandibulares são o segundo tipo de fraturas mais comum que acomete a face, e elas podem ter um impacto negativo na estética e anatomia facial, bem como causar problema em atividades cotidianas como falar e mastigar. Este artigo tem por objetivo relatar o caso clínico de um paciente vítima de agressão física, que apresentou uma fratura unilateral de côndilo associada à fratura oblíqua de sínfise. Paciente do gênero masculino, 32 anos, leucoderma, através das radiografias PA de mandíbula e Towne foram constatadas fratura da região de sínfise mandibular e côndilo do lado esquerdo. A escolha para o tratamento desta fratura, foi o tratamento cirúrgico com fixação interna da fratura condilar com sistema 2.0mm. Após sete dias foi iniciada a fisioterapia com espátulas de madeira para o restabelecimento de adequada abertura bucal, apresentando ao final uma recuperação satisfatória, sem intercorrências. O uso de placas e parafusos para o tratamento cirúrgico de fraturas de côndilo mandibular e sínfise se mostrou eficaz, resultando no sucesso cirúrgico com um pós-operatório sem complicações e no reestabelecimento funcional e da oclusão. Problem Study of correlation of recurrence of NPF with its histopathologic appearance. Methods A retrospective and prospective study of 184 NPF cases identified, treated in Government Ear Nose Throat Hospital, and followed over a period of 26 years (1981–2006) and their histopathological slides are preserved, their recurrence rate is documented. The histopathology of NPF is varied - composed of vascular and fibrous components and their proportion varied. In our study we will undertake the review of histopathology slides that are already preserved and would also like to study the new recurrent cases in the coming 6 months. Results All recurrences were observed with in one year of of treatment and the recurrence rate was 19.66%. Recurrence had no correlation with age of patient, duration of symptoms, or surgical approach but correlated with stage of tumor at presentation (p less than 0.05). Preoperative embolisation was done in 16.85% cases and did not show any statistical difference in recurrence rate between the embolised and non embolised. Conclusion The correlation between the histopathological appearance and the recurrence rate will help in choosing the right approach for surgery and also postoperative follow-up to detect the recurrence at the earliest. Significance Histopathological detail can help in knowing the prognosis in terms of chances of recurrence and also guide for follow up at close intervals and advocate the appropriate treatment at the earliest.

Mandibular condylar fractures in a growing child may cause long-term consequences. A conservative approach has been recommended to permit the remodelling of the condyles as well as to re-establishing the mandibular dynamics and oral functions. This case report presents a bilateral mandibular condylar fracture in an 11 years old girl treated with myofunctional appliances in combination with exercises. The successful results from this clinical case support a conservative approach when treating mandibular condylar fractures in a growing child instead of an open surgical approach.

This systematic review sheds light on the effectiveness of auditory–verbal therapy (AVT) outcomes in children with cochlear implants (CIs). The presented outcome is based on research findings from the last 10 years. The systematic review was designed based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and Critical Appraisal of Treatment Evidence (CATE) checklist. Specific keywords were chosen based on the research question and searched on the PubMed database. All searched papers were analysed based on specific exclusion criteria and classified into four evidence levels. The results revealed that children who participated in AV therapy can achieve linguistic skills at the same level as their hearing peers. Voice quality seemed positively affected, placing young children with CIs in the normal range for receptive vocabulary development. In contrast, reading skills seemed less benefited. AV therapy seems to contribute to integration into mainstream society. Despite the recorded speech and language improvements of young children with CIs, the aim of AV therapy is still not fulfilled. AV therapy can be seen as the best clinical practice for young children with CIs till now, but the lack of well-controlled studies is undermining.

Introduction: Helicobacter pylori (H. pylori) infection is positively associated with cardiovascular diseases, but the involvement of lipids in this association remains unclear. The present study reviewed the changes in circulating lipid levels following H. pylori eradication. Methods: A PubMed database was searched until December 2020 to identify randomized control trials (RCTs) and non-RCTs investigating the effect of H. pylori eradication on the lipid levels in inverse variance-weighted, random-effects meta-analyses. Results: A total of 24 studies (four RCTs and 20 non-RCTs) with 5270 participants were identified. The post-eradication levels were increased for high-density lipoprotein cholesterol (HDL-C; mean difference (MD) 2.28 mg/dL, 95% confidence interval (CI) 1.90 to 2.66) and triglyceride (TG; MD 3.22 mg/dL, 95% CI 1.13 to 5.31) compared with the pre-eradication levels. H. pylori eradication resulted in little to no difference in the low-density lipoprotein-cholesterol levels (MD −2.33 mg/dL, 95% CI −4.92 to 0.26). In the analyses of RCTs only, the findings for elevated HDL-C levels, but not TG, were robust. Conclusions: H. pylori eradication increases the HDL-C levels. Further studies are needed to elucidate the effects of lipid changes following H. pylori eradication on cardiovascular diseases.

Abstract Objective The aim of this study was to investigate the effects of an induced incomplete ossification of the humeral condyle (IOHC) lesion on ex vivo canine humeral condylar biomechanics. Study Design Nine paired cadaveric elbows were collected from mature dogs weighing between 20 and 25 kg. Left and right limbs were randomized to IOHC or normal groups. Limbs were prepared for mechanical testing; ligaments were preserved and an IOHC lesion was created. Elbows were potted, positioned into a biomaterials testing system at an angle of 135 degrees and axially loaded to failure at a rate of 30 mm/minute. Results Induced IOHC lesions reduced peak load (p = 0.02) when compared with an intact humerus. There was no difference between stiffness (p = 0.36) of intact humeri or humeri with an induced IOHC lesion. An induced IOHC lesion increased (p = 0.012) the probability of intracondylar fracture under load. Conclusion Cadaveric humeri are weakened by the creation of an intracondylar osteotomy and fractures secondary to induced IOHC are similar to spontaneous humeral condylar fractures. These findings support the hypothesis that naturally occurring IOHC weakens the humeral condyle and may predispose to humeral condylar fracture.

AbstractYoung adults exhibit a small asymmetry of visuospatial attention that favours the left side of space relative to the right (pseudoneglect). However, it remains unclear whether this leftward bias is maintained, eliminated or shifted rightward in older age. Here we present two meta-analyses that aimed to identify whether adults aged ≥50 years old display a group-level spatial attention bias, as indexed by the line bisection and the landmark tasks. A total of 69 datasets from 65 studies, involving 1654 participants, were analysed. In the meta-analysis of the line bisection task (n = 63), no bias was identified for studies where the mean age was ≥50, but there was a clear leftward bias in a subset where all individual participants were aged ≥50. There was no moderating effect of the participant’s age or sex, line length, line position, nor the presence of left or right cues. There was a small publication bias in favour of reporting rightward biases. Of note, biases were slightly more leftward in studies where participants had been recruited as part of a stand-alone older group, compared to studies where participants were recruited as controls for a clinical study. Similarly, no spatial bias was observed in the meta-analysis of the landmark task, although the number of studies included was small (n = 6). Overall, these results indicate that over 50s maintain a group-level leftward bias on the line bisection task, but more studies are needed to determine whether this bias can be modulated by stimulus- or state-dependent factors.
