Abstract One of the current problems in the presence of COVID-19 pandemic crisis is to analyze the effects of the second wave on public health to design appropriate strategies to reduce negative effects in society. The study here analyzes first and second wave of COVID-19 pandemic in Italy because is one of European countries to experience a rapid increase in confirmed cases and deaths. Results reveal that the first wave of COVID-19 pandemic in Italy had a high negative effect on public health from February to May 2020 that declined with the approaching of summer season and with the health policy of lockdown and quarantine; instead, second wave of the COVID-19 pandemic, from October 2020, has increasing confirmed cases but admission to ICUs is, to date, below maximum capacity and total deaths have a lower level than first wave. This study can support best practice of crisis management to design effective health policies to constrain current and future waves of the COVID-19 pandemic and epidemics of similar viral agents.

Public health policies are often accused of being paternalistic, or to show the ‘Nanny State’ in action. This chapter argues that complaints about paternalism in public health policy are, for a variety of reasons, much less convincing than is often thought. First, for conceptual reasons, it is difficult to specify what it would be for a policy to be paternalistic. Second, two of the elements that make paternalism problematic at an individual level—interference with liberty and lack of individual consent—are endemic to public policy contexts in general and so cannot be used to support the claim that paternalism in particular is wrong. The chapter concludes that instead of debating whether a given policy is paternalistic, it would be better to ask whether the infringements of liberty it contains are justifiable, without placing any weight on whether or not those infringements of liberty are paternalistic.

Marijuana is the most consumed illicit drug in the world, with over 192 million users. Due to the current legalization push of marijuana in the United States, there has been a lack of oversight regarding its public health policies, as marijuana advocates downplay the drug’s negative effects. This paper’s approach is from a public health perspective, focusing specifically on the cases of violence amongst some marijuana users. Here, we present 14 cases of violence with chronic marijuana users that highlight reoccurring consequences of: marijuana induced paranoia (exaggerated, unfounded distrust) and marijuana induced psychosis (radical personality change, loss of contact with reality). When individuals suffering from pre-existing medical conditions use marijuana in an attempt to alleviate their symptoms, ultimately this worsens their conditions over time. Although marijuana effects depend on the individual’s endocannabinoid receptors (which control behavioral functions, like aggression) and the potency level of tetrahydrocannabinol (THC) in the drug, scientifically documented links between certain marijuana users and violence do exist. Wider public awareness of the risks and side effects of marijuana, as well as a more prudent health policy, and government agency monitoring of the drug’s composition, creation, and distribution, are needed and recommended.

Background: China is generally regarded internationally as an “authoritarian” state. Traditional definitions have assigned many negative connotations surrounding the term of authoritarian. We realize that it might not be considered value-neutral in other countries. But authoritarian in the Chinese context emphasizes more on centralized decision making, collectivism, coordinating all activities of the nation, and public support, which is considered a value-neutral term. Therefore, it is adopted in this paper. We would like to clarify this. Authoritarian governance is considered an important mechanism for developing China's economy and solving social problems. The COVID-19 crisis is no exception. Most of the current research on crisis management and government crises focuses on advanced, democratic countries. However, the consequences of crisis management by authoritarian governments have not been fully appreciated. Although prior research has addressed authoritarian initiatives to manage crises in China, authoritarian interventions have rarely been theorized in public health emergencies.Methods: Based on a literature review and theoretical analysis, we use a descriptive and qualitative approach to assess public health policies and mechanisms from an authoritarian perspective in China. In light of the key events and intervention measures of China's government in response to COVID-19, the strategic practices of the Communist Party of China (CPC) to construct, embody, or set political goals through authoritarian intervention in public health crisis management are discussed.Results: China's government responded to the COVID-19 pandemic with a comprehensive authoritarian intervention, notably by establishing a top-down leadership mechanism, implementing a resolute lockdown, rapidly establishing square cabin hospitals, enhancing cooperation between different government departments, mobilizing a wide range of volunteer resources, enforcing the use of health codes, imposing mandatory quarantine on those returning from abroad, and implementing city-wide nucleic acid testing. These measures ensured that China was able to contain the outbreak quickly and reflect on the unique role of the Chinese authoritarian system in responding to public health crises.Conclusions: Our paper contributes to expanding the existing understanding of the relationship between crisis management and authoritarian system. China's response to COVID-19 exemplifies the unique strengths of authoritarian institutions in public health crisis management, which is a helpful and practical tool to further enhance the CPC's political legitimacy. As a socialist model of crisis management with Chinese characteristics, it may offer desirable experiences and lessons for other countries still ravaged by the epidemic.

Abstract Background: The coronavirus disease 2019 (COVID-19) pandemic constitutes a public health emergency that threatens all of society. During this time, it is important for people to help each other by providing support related to the viral components of the pandemic while also assisting with difficulties experienced at the social level. However, the nature of the infection itself puts everyone at risk, which may prevent altruistic behaviors. In other words, the pandemic has created a dilemma in which individuals may need to choose between their personal safety and continuing to help others. As such, this study explored how public health emergencies affect altruistic behaviors in the context of COVID-19.Methods: Questionnaire surveys were distributed to 1508 residents from 31 provinces across China in February 2020 during the outbreak of COVID-19. Structural equation models were then implemented to test multiple research hypotheses using the obtained data.Results: Findings showed that the severity of the pandemic had both positive and negative effects on altruistic behaviors. Empathy mediated the positive relationship between the severity of the pandemic and altruistic behavior, while the sense of control mediated the negative effect between the severity of the pandemic and altruistic behavior.Conclusions: In the context of public emergencies such as the COVID-19 pandemic, altruistic behaviors can be enhanced by strengthening the factors of empathy and sense of control. These findings are useful for a government’s altruistic behavior promotion and disaster risk management.

AbstractWhat is hardly known in the studies of the COVID-19 global pandemic crisis is the impact of general lockdown during the first wave of COVID-19 pandemic both public health and economic system. The main goal of this study is a comparative analysis of some European countries with a longer and shorter period of national lockdown during the first wave of COVID-19 from March to August 2020. Findings suggests that: a) countries with shorter period of lockdown have a variation of confirmed cases/population (%) higher than countries with longer period of lockdown; b) countries with shorter period of lockdown have average fatality rate (5.45%) lower than countries with longer period of lockdown (12.70%), whereas variation of fatality rate from August to March 2020 suggests a higher reduction in countries with longer period of lockdown (−1.9% vs 0.72%). However, Independent Samples Test and the Mann-Whitney test reveal that the effectiveness of longer period of lockdown versus shorter one on public health is not significant. In addition, the COVID-19 pandemic associated with longer period of lockdown has a higher negative impact on economic growth with consequential social issues in countries. Results of the impact of COVID-19 lockdowns on public health and economies of some leading countries in Europe, during the first wave of the COVID-19 pandemic, can provide vital information to design effective containment strategies in future waves of this pandemic to minimize the negative effects in society.

Public health policies might thus be thought of as the policies that guide these ‘organized efforts’ to protect and improve health. The scope of such policies depends a good deal, however, on what is considered to be entailed by ‘organized efforts’. and on how centrally ‘organized efforts’ are understood to be related to efforts that are more decentralized, more informal, less organized, perhaps even ‘spontaneous’. The relative importance and legitimacy of centralized versus decentralized uses of knowledge in protecting and enhancing health is a common underlying theme in discussion of public health policy. This chapter discusses public health policy, and differences in outcomes, across different developed countries.

In rural America, opioid use disorder (OUD) continues to adversely impacted familial, public, and economic systems, creating extraordinary societal and financial burden. This comparative analysis of state-level public health policy and practices in rural opioid use abatement promotes the development and implementation of contextualized evidence-based comprehensive policy initiatives. Policy analysis across select highly affected states (Indiana, Kansas, Kentucky, and West Virginia) and exemplar OUD policy response states (Colorado, Massachusetts, Ohio, and Vermont) was performed using a systematic review of literature, legislation, plans, and policies. Findings included close alignment between states’ OUD policies and public health best practice standards; minimized differences between exemplar and highly affected states policy responses; and resource driven gaps in opioid epidemic legislation, regulation, guidelines, strategic plans, and initiatives. Furthermore, it is advocated that public and private stakeholders committed to health equity must seek reductions in opioid related disease and mortality through increased resource allocation.

In order to achieve more ethical global health outcomes, health policies must be driven by health priorities and should take into account broader health policy requirements, including the needs of specific national health systems. It is thus important to recognize that the division of interests in key policy areas are not necessarily between the priorities of rich and poor countries, but between (1) pharmaceutical industry interests and health policy interests, and (2) national industrial and trade policy interests and public health policies. In this article I will focus on two broad common interests for health policy officials. Both have become important in the context of current global negotiations relating to access to medicines; pandemic influenza; and public health, innovation, and intellectual property rights. These are (1) ensuring access, availability, and the safety of pharmaceuticals, and (2) ensuring that research-and-development efforts respond to public health needs. I argue that these issues are not solely the concern of developing countries because the diminishing national policy space for health in pharmaceutical policies presents a challenge to all governments, including rich ones.

Abstract Background: Few theories of health behavior explicitly take the effect of social norms on behavior into account. The ones that do assume that the effect of norms on behavior operates through motivation. We use a behavior model that is new to public health to test whether norms affect behavior through motivation or ability. Methods: This study uses data from a household survey of Nigerian women, ages 14-24. The survey collected data on socio-economic and demographic characteristics of women, whether they were sexually experienced, and whether they used contraception. The survey also collected data on descriptive and injunctive norms around premarital sex and contraceptive use. Multivariate logistic regression was used to test whether the effect of unfavorable social norms on modern contraceptive use operates through motivation or ability. Results: The data was used to construct variables measuring unfavorable social norms, motivation, ability, and modern contraceptive use. After adjusting for a range of socio-economic and demographic variables, we found that unfavorable social norms had a statistically significant negative effect on contraceptive use. The analysis suggests that the effect of unfavorable social norms on contraceptive use operates through ability.Conclusion: This study sheds light on the pathway through which social norms affect contraceptive use. The findings suggest that public health interventions may be able to counter the negative effects of unfavorable social norms on modern contraceptive use by increasing women’s ability to practice contraception. These findings have important implications for the design of interventions that aim to increase contraceptive use among young women in Nigeria. The study also demonstrates the utility of the FBM, a practitioner-friendly, behavior change model, in examining the relationship between norms and behavior.

Many health policies are designed with the intention of improving health outcomes for all. Yet implementation of policies are variable across contexts, potentially limiting its impact on population health outcomes. The potential impact of a policy to advance health equity depends both on the design and its implementation, requiring ongoing evaluation and stakeholder engagement. Despite the importance of health policies in shaping public health, health care policy implementation science remains underrepresented in research. We argue that enhanced integration of policy questions within implementation science could reduce the time lag from policy to practice and improve population health outcomes to build a body of evidence on effective policy implementation. In this commentary, we argue that approaches to studying policy implementation science should reflect the dynamic and evolving policy context, analogous to the “learning healthcare system,” to better understand and respond to systematic and multilevel impacts of policy. Several example opportunities for a learning health policy system are posed in building a broader agenda toward research and practice in policy implementation science in public health.
