Expression of OLFM4 and Lysozyme During Necrotizing Enterocolitis and Volvulus in Newborns: a Comparative Observational Research Study

Abstract Background: In neonatal patients with necrotizing enterocolitis (NEC) and volvulus the inflammatory response is mediated by a plurality of different proteins. The proteins olfactomedin 4 (OLFM4) and lysozyme (LYZ) are part of the intestinal mucosal defense and especially OLFM4 has rarely been evaluated in neonatal gastrointestinal diseases. The aim of this study was to compare the expression levels of OLFM4 and lysozyme during NEC and volvulus in neonates. Methods: Intestinal tissues of patients with NEC and patients with volvulus were examined using immunohistochemical staining of OLFM4 and lysozyme of formalin-fixed and paraffin-embedded sections of resected tissue. Staining-positive tissues were semi-quantitatively scored from 0 (no staining), 1 (weak staining), 2 (moderate staining) to 3 (highly intense staining) by two individual investigators.Results: Both applied antibodies against OLFM4 showed different staining patterns with higher staining intensity of the antibody OLFM4 (D1E4M). OLFM4 (median score of the antibody OLFM4 (D1E4M): 3.0) and lysozyme (median score: 3.0) are highly expressed in intestinal and immune cells during NEC. The expression of OLFM4 and lysozyme in tissue with intestinal volvulus was also observable (median score of the antibody OLFM4 (D1E4M): 1.25) and median score of the antibody against LYZ: 2.0), but lower levels could be seen in comparison to tissue with NEC (p=0.033 and p=0.037, respectively).Conclusions: Both proteins, OLFM4 and lysozyme, may play a role in the pathogenesis of NEC and volvulus in neonatal patients, but the exact mechanisms of OLFM4 and lysozyme function and their role in immunological responses have not yet been resolved. These observations add new insights as basis for further large-scale population research.

Large-scale population movements can turn local diseases into widespread epidemics. Grasping the characteristic of the population flow in the context of the COVID-19 is of great significance for providing information to epidemiology and formulating scientific and reasonable prevention and control policies. Especially in the post-COVID-19 phase, it is essential to maintain the achievement of the fight against the epidemic. Previous research focuses on flight and railway passenger travel behavior and patterns, but China also has numerous suburban residents with a not-high economic level; investigating their travel behaviors is significant for national stability. However, estimating the impacts of the COVID-19 for suburban residents’ travel behaviors remains challenging because of lacking apposite data. Here we submit bus ticketing data including approximately 26,000,000 records from April 2020–August 2020 for 2705 stations. Our results indicate that Suburban residents in Chinese Southern regions are more likely to travel by bus, and travel frequency is higher. Associated with the economic level, we find that residents in the economically developed region more likely to travel or carry out various social activities. Considering from the perspective of the traveling crowd, we find that men and young people are easier to travel by bus; however, they are exactly the main workforce. The indication of our findings is that suburban residents’ travel behavior is affected profoundly by economy and consistent with the inherent behavior patterns before the COVID-19 outbreak. We use typical regions as verification and it is indeed the case.

ObjectivesTo report the situation of maternal micronutrient supplementation before and during pregnancy in Northwest China and to examine the rates of and factors related to the adherence to micronutrient supplementation among pregnant women in this region, where dietary micronutrient intake is commonly insufficient.DesignA large-scale population-based cross-sectional survey.SettingTwenty counties and ten districts of Shaanxi Province.ParticipantsA sample of 30 027 women were selected using a stratified multistage random sampling method. A total of 28 678 women were chosen for the final analysis after excluding those who did not provide clear information about nutritional supplementation before and during pregnancy.Main outcome measuresMaternal adherence to micronutrient supplementation (high and low) were the outcomes. They were determined by the start time and duration of use according to Chinese guidelines (for folic acid (FA) supplements) and WHO recommendations (for iron, calcium and multiple-micronutrient (MMN) supplements).ResultsIn total, 83.9% of women took at least one kind of micronutrient supplement before or during pregnancy. FA (67.6%) and calcium (57.5%) were the primarily used micronutrient supplements; few participants used MMN (14.0%) or iron (5.4%). Adherence to supplementation of all micronutrients was low (7.4% for FA, 0.6% for iron, 11.7% for calcium and 2.7% for MMN). Higher educational levels, higher income levels, urban residence and better antenatal care (including pregnancy consultation and a higher frequency of antenatal visits) were associated with high adherence to micronutrient supplementation.ConclusionMaternal micronutrient supplementation before and during pregnancy in Northwest China was way below standards recommended by the Chinese guidelines or WHO. Targeted health education and future nutritional guidelines are suggested to improve this situation, especially in pregnant women with disadvantaged sociodemographic conditions.

Summary Objective: The current observational research literature shows extensive publication bias and contradiction. The Observational Health Data Sciences and Informatics (OHDSI) initiative seeks to improve research reproducibility through open science. Methods: OHDSI has created an international federated data source of electronic health records and administrative claims that covers nearly 10% of the world’s population. Using a common data model with a practical schema and extensive vocabulary mappings, data from around the world follow the identical format. OHDSI’s research methods emphasize reproducibility, with a large-scale approach to addressing confounding using propensity score adjustment with extensive diagnostics; negative and positive control hypotheses to test for residual systematic error; a variety of data sources to assess consistency and generalizability; a completely open approach including protocol, software, models, parameters, and raw results so that studies can be externally verified; and the study of many hypotheses in parallel so that the operating characteristics of the methods can be assessed. Results: OHDSI has already produced findings in areas like hypertension treatment that are being incorporated into practice, and it has produced rigorous studies of COVID-19 that have aided government agencies in their treatment decisions, that have characterized the disease extensively, that have estimated the comparative effects of treatments, and that the predict likelihood of advancing to serious complications. Conclusions: OHDSI practices open science and incorporates a series of methods to address reproducibility. It has produced important results in several areas, including hypertension therapy and COVID-19 research.

Abstract Background The Counselling in Primary Care service (CIPC) is the first and only nationally available public counselling service in the Republic of Ireland. This study provides initial data for the effectiveness of short-term psychotherapy delivered in a primary care setting in Ireland for the first time. Method A practice-based observational research approach was employed to examine outcome data from 2806 clients receiving therapy from 130 therapists spread over 150 primary care locations throughout Ireland. Pre-post outcomes were assessed using the CORE-OM and reliable and clinically significant change proportions. Binary logistic regression examined the effect of pre therapy symptom severity on the log odds of recovering. Six and 12 month follow up data from a subsample of 276 clients were also analysed using growth curve analysis. Results Of 14,156 referred clients, 5356 presented for assessment and 52.3% (N = 2806) consented to participate. Between assessment and post-therapy a large reduction in severity of symptoms was observed- Cohen’s d = 0.98. Furthermore, 47% of clients achieved recovery,a further 15.5% reliably improved, 2.7% reliably deteriorated and34.7% showed no reliable improvement. Higher initial severity was associated with less chance of recovering at post-therapy. Significant gains were maintained between assessment and12 months after therapy- Cohen’s d = 0.50. Conclusions Outcomes for clients in the CIPC service compared favourably with large scale counselling and psychotherapy services in jurisdictions in the U.K., the U.S.A., Norway and Sweden. This study expands the international primary care psychotherapy research base to include the entire Republic of Ireland jurisdiction.

The large- and small-scale vortical motions produced by ‘delta tabs’ in a two-stream shear layer have been studied experimentally. An increase in mixing was observed when the base of the triangular shaped tab was affixed to the trailing edge of the splitter plate and the apex was pitched at some angle with respect to the flow axis. Such an arrangement produced a pair of counter-rotating streamwise vortices. Hot-wire measurements detailed the velocity, time-averaged vorticity (Ωx) and small-scale turbulence features in the three-dimensional space downstream of the tabs. The small-scale structures, whose scale corresponds to that of the peak in the dissipation spectrum, were identified and counted using the peak-valley-counting technique. The optimal pitch angle, θ, for a single tab and the optimal spanwise spacing, S, for a multiple tab array were identified. Since the goal was to increase mixing, the optimal tab configuration was determined from two properties of the flow field: (i) the large-scale motions with the maximum Ωx, and (ii) the largest number of small-scale motions in a given time period. The peak streamwise vorticity magnitude [mid ]Ωx−max[mid ] was found to have a unique relationship with the tab pitch angle. Furthermore, for all cases examined, the overall small-scale population was found to correlate directly with [mid ]Ωx−max[mid ]. Both quantities peaked at θ≈±45°. It is interesting to note that the peak magnitude of the corresponding circulation in the cross-sectional plane occurred for θ≈±90°. For an array of tabs, the two quantities also depended on the tab spacing. An array of contiguous tabs acted as a solid deflector producing the weakest streamwise vortices and the least small-scale population. For the measurement range covered, the optimal spacing was found to be S≈1.5 tab widths.
