Abstract Background The consequences for youth cardiometabolic risk might depend on whether sedentary time and physical activity are accumulated sporadically (in shorter bouts) or in a sustained pattern (in longer bouts). This study aimed to: 1) describe daily time-use compositions of youth, including time spent in shorter and longer bouts of sedentary behaviour and physical activity; and 2) examine associations between time-use compositions with cardiometabolic biomarkers.Methods Accelerometer and cardiometabolic biomarker data (adiposity, blood pressure, lipids) from 7–13 year olds (mean ± SD: 10.4 ± 1.7) from two Australian studies were pooled (complete cases adiposity n = 772). A time-use composition of nine components was formed using compositional data analysis: time in shorter and longer bouts of sedentary behaviour, light-, moderate- and vigorous-intensity physical activity, and “other time” (i.e., non-wear/sleep). Shorter and longer bouts of sedentary time were defined as < 5 and ≥ 5 min, respectively. Longer light-, moderate- and vigorous-intensity physical activity bouts were defined as ≥ 1 min. Linear regression models examined associations between overall time-use composition and cardiometabolic biomarkers. Then, associations between ratios of longer relative to shorter activity patterns, and each intensity relative to more intense activities and/or “other time”, with cardiometabolic biomarkers were derived.Results Confounder-adjusted models showed that the overall time-use composition was associated with zBMI, waist circumference, systolic blood pressure, high-density lipoprotein cholesterol, triglycerides, and a combined cardiometabolic risk score. Specifically, more time in longer relative to shorter bouts of light-intensity physical activity was associated with greater zBMI (β = 1.79, SE = 0.70, p = 0.010) and waist circumference (β = 17.28, SE = 4.87, p < 0.001). More time in longer relative to shorter bouts of vigorous-intensity physical activity was also associated with higher waist circumference (β = 2.54, SE = 1.14, p = 0.026). More relative time in total light- and vigorous-intensity physical activity (including longer and shorter bouts) was associated with lower waist circumference. In contrast, more relative time in sedentary behaviour and moderate-intensity physical activity was detrimental for waist circumference.Conclusions Accumulating physical activity in frequent short bursts may be beneficial for adiposity compared to engaging in the same amount of these intensities in longer bouts.Trial registration: 'Lifestyle Of Our Kids’ (ACTRN12615000066583 [23/01/2015]) and ‘Transform-Us!’ (ACTRN12609000715279 [19/08/2009], ISRCTN83725066 [30/06/2010]).

Sedentary and physical activity patterns (bouts/breaks) may be important for cardiometabolic health in early life. This study aimed to examine cross-sectional associations of total daily volume and patterns across the activity spectrum with cardiometabolic risk factors in youth aged 7–13 years. Objectively measured accelerometer and cardiometabolic risk factor data were pooled from two studies (n = 1219; 69% valid accelerometry). Total daily volume of sedentary time and light-, moderate-, and vigorous-intensity physical activity was determined. Time in sustained bouts and median bout lengths of all intensities and breaks in sedentary time were also calculated. Outcomes included body mass index, waist circumference, blood pressure, blood lipids, and a cardiometabolic summary score. Regression models revealed beneficial associations between total daily volumes of moderate- and vigorous-intensity physical activity and cardiometabolic risk. Time spent in ≥1 min vigorous-intensity physical activity bouts was beneficially associated with cardiometabolic risk, yet this disappeared after adjusting for total vigorous-intensity physical activity and confounders. Time accumulated in light- (≥1 min; ≥5 min) and moderate-intensity (≥1 min) physical activity bouts was detrimentally associated with cardiometabolic risk. Total daily volume and activity patterns may have implications for cardiometabolic risk early in life. Sporadic physical activity may be more beneficial for health than sustained physical activity.

Abstract Background Daily time spent in sleep, sedentary behaviour (SED), light intensity physical activity (LIPA), and moderate-to-vigorous intensity physical activity (MVPA) are compositional, co-dependent variables. The objectives of this study were to use compositional data analysis to: (1) examine the relationship between the movement behaviour composition (daily time spent in sleep, SED, LIPA and MVPA) and all-cause mortality risk, and (2) estimate the extent to which changing time spent in any given movement behaviour (sleep, SED, LIPA, or MVPA) within the movement behaviour composition was associated with changes in risk of all-cause mortality. Methods 2838 adult participants from the 2005–2006 cycle of the U.S. National Health and Nutrition Examination Survey were studied using a prospective cohort design. Daily time spent in SED, LIPA and MVPA were determined by accelerometer. Nightly time spent sleeping was self-reported. Survey data were linked with mortality data through to the end of December 2015. Compositional data analysis was used to investigate relationships between the movement behaviour composition and mortality. Results The movement behaviour composition was significantly associated with mortality risk. Time spent in MVPA relative to other movement behaviours was negatively associated with mortality risk (HR = .74; 95% CI [.67, .83]) while relative time spent in SED was positively associated with mortality risk (HR = 1.75; 95% CI [1.10, 2.79]). Time displacement estimates revealed that the greatest estimated changes in mortality risk occurred when time spent in MVPA was decreased and replaced with sleep, SED, LIPA or a combination of these behaviours (HRs of 1.76 to 1.80 for 15 min/day displacements). Conclusions The daily movement behaviour composition was related to mortality. Replacing time in MVPA or SED with equivalent time from any other movement behaviour was associated with an increase and decrease in mortality risk, respectively.

The aim of the present study was to investigate the relationships of physical activity types and sedentary behaviour with BMI and waist circumference (WC). The sample comprised 6215 adults (2775 men, 3440 women) aged 16 years and over living in Scotland. Self-reported physical activity of moderate to vigorous intensity (MVIA) included domestic activity, walking, and sports and exercises. MVIA levels were classified as being inactive, being insufficiently active, being sufficiently active for general health benefits and being sufficiently active for obesity prevention. Sedentary time was defined as television and other screen-based entertainment time (TVSE). Dependent variables were BMI-defined obesity (BMI-OB) and WC-defined obesity (WC-OB). TVSE was positively related to both WC-OB (adjusted OR 1·69 (95 % CI 1·39, 2·05) for ≥ 4 h of TVSE per d compared with < 2 h/d) and BMI-OB (OR 1·88; 95 % CI 1·51, 2·35) independently of MVIA. Those classified as most active who reported ≥ 4 h/d of TVSE had higher prevalence of BMI-OB (18·9 v. 8·3 %; P < 0·05) and WC-OB (28·0 v. 10·0 %; P < 0·01) than those equally active with < 2 h/d of TVSE. Sports and walking were related inversely to WC-OB (OR for no time compared with ≥ 30 min/d: 1·55 (95 % CI 1·24, 1·94); 2·06 (95 % CI 1·64, 2·58)), but only walking was related to BMI-OB (OR 1·94; 95 % CI 1·58, 2·37). Domestic physical activity was not related to BMI-OB or WC-OB. In conclusion, physical activity and sedentary behaviour are independently related to obesity. Public health recommendations should both promote physical activity and discourage engagement in sedentary pursuits.

Abstract Background Studies have seldom used Compositional Data Analysis (CoDA) to map the effects of sleep, sedentary behaviour, and physical activity on older adults’ cardio-metabolic profiles. This study therefore aimed to illustrate how sleep, sedentary behaviour, and physical activity profiles differ between older adult groups (60–89 years), with ‘low’ compared to those with ‘high’ concentrations of endocrine cardio-metabolic disease risk markers, using CoDA. Method Ninety-three participants (55% female) wore a thigh-mounted triaxial accelerometer for seven consecutive free-living days. Accelerometer estimates of daily average hours of engagement in sedentary behaviour (SB), standing, light-intensity physical activity (LIPA), sporadic moderate-vigorous physical activity (sMVPA, accumulated with bouts between 1 and 10 min), 10-min moderate-vigorous physical activity (10MVPA, accumulated with bouts ≥10 min), in addition to self-reported sleeping hours were reported. Fasted whole blood concentrations of total cholesterol, triglyceride, glucose, and glycated haemoglobin, and serum lipoprotein lipase (LPL), interleukin-6 (IL-6), and procollagen III N-terminal propeptide were determined. Results Triglyceride concentration appeared to be highly dependent on 10MVPA engagement as the ‘low’ and ‘high’ concentration groups engaged in 48% more and 32% less 10MVPA, respectively, relative to the geometric mean of the entire study sample. Time-use composition of the ‘low’ LPL group’s engagement in 10MVPA was 26% less, while the ‘high’ LPL group was 7.9% more, than the entire study sample. Time-use composition of the ‘high’ glucose and glycated haemoglobin groups appeared to be similar as both engaged in more Sleep and SB, and less 10MVPA compared to the study sample. Participants with a ‘low’ IL-6 concentration engaged in 4.8% more Sleep and 2.7% less 10MVPA than the entire study sample. Time-use composition of the Total Cholesterol groups was mixed with the ‘low’ concentration group engaging in more Standing and 10MVPA but less Sleep, SB, LIPA, and sMVPA than the entire study sample. Conclusion Older adults should aim to increase 10MVPA engagement to improve lipid profile and decrease SB engagement to improve glucose profile.

Introduction Sedentary behaviour, especially TV viewing, has been identified as a possible risk factor for cardiovascular disease, whereas physical activity seems to protect against major cardiovascular risk factors: obesity, dyslipidemia and hypertension. The aim of this study was to explore the association between both time spent on TV viewing and vigorous intensity physical activity in relation to body mass index, waist circumference, waist/hip ratio, serum lipids and blood pressure. Methods A total of 1693 men and women, 33-64 years of age, from the 3-year follow-up of a population-based intervention study, The Inter 99 study, were included in this cross-sectional design. Information on TV viewing, physical activity and other lifestyle habits was obtained by self-report questionnaire. Associations between TV viewing/physical activity and biological cardiovascular risk factors were explored by multiple linear regression analyses. Results Data from 835 (51%) men and 805 (49%) women were included. Mean age was 50.8 years (33.0-64.6). Men had a higher overall physical activity score than women ( P < 0.0001). TV viewing was significantly, positively associated with waist/hip ratio ( P = 0.005), body mass index ( P = 0.03), triglycerides ( P = 0.005), low density lipoprotein cholesterol ( P = 0.03), total cholesterol ( P = 0.01), systolic ( P = 0.05) and diastolic blood pressure ( P = 0.03), but not with waist circumference and high density lipoprotein cholesterol (HDL). Engaging in vigorous intensity physical activity was associated with higher HDL ( P = 0.0006) and lower waist circumference ( P < 0.0001). Conclusion TV viewing and physical activity should be regarded as separate types of behaviour that relate to different cardiovascular disease risk factors. BackgroundWe aimed to study the independent associations of 10-year change in sedentary behaviour (SB), moderate-to-vigorous physical activity (MVPA) and objectively measured cardiorespiratory fitness (CRF), with concurrent change in clustered cardiometabolic risk and its individual components (waist circumference, fasting glucose, high-density lipoprotein (HDL) cholesterol, triglycerides and blood pressure). We also determined whether associations were mediated by change in CRF (for SB and MVPA), waist circumference (for SB, MVPA and CRF) and dietary intake (for SB).MethodsA population-based sample of 425 adults (age (mean±SD) 55.83±9.40; 65% men) was followed prospectively for 9.62±0.52 years. Participants self-reported SB and MVPA and performed a maximal cycle ergometer test to estimate peak oxygen uptake at baseline (2002–2004) and follow-up (2012–2014). Multiple linear regression and the product of coefficients method were used to examine independent associations and mediation effects, respectively.ResultsGreater increase in SB was associated with more detrimental change in clustered cardiometabolic risk, waist circumference, HDL cholesterol and triglycerides, independently of change in MVPA. Greater decrease in MVPA was associated with greater decrease in HDL cholesterol and increase in clustered cardiometabolic risk, waist circumference and fasting glucose, independent of change in SB. Greater decrease in CRF was associated with more detrimental change in clustered cardiometabolic risk and all individual components. Change in CRF mediated the associations of change in SB and MVPA with change in clustered cardiometabolic risk, waist circumference and, only for MVPA, HDL cholesterol. Change in waist circumference mediated the associations between change in CRF and change in clustered cardiometabolic risk, fasting glucose, HDL cholesterol and triglycerides.ConclusionsA combination of decreasing SB and increasing MVPA, resulting in positive change in CRF, is likely to be most beneficial towards cardiometabolic health.

Aims Previous prospective studies of the association between mortality and physical activity have generally not fully accounted for the interplay between movement behaviours. A compositional data modelling approach accounts for relative scale and co-dependency in time-use data across physical activity behaviours of the 24-hour day. Methods A prospective analysis of the National Health and Nutrition Examination Survey 2005–2006 on N = 1468 adults ( d = 135 deaths) in ages 50–79 years was undertaken using compositional Cox regression analysis. Daily time spent in sedentary behaviour, light intensity (LIPA) and moderate-to-vigorous physical activity (MVPA) was determined from waist-mounted accelerometer data (Actigraph 7164) and supplemented with self-reported sleep data to determine the daily time-use composition. Results The composition of time spent in sedentary behaviour, LIPA, MVPA and sleep was associated with mortality rate after allowing for age and sex effects ( p < 0.001), and remained significant when other lifestyle factors were added ( p < 0.001). This was driven primarily by the preponderance of MVPA; however, significant changes are attributable to LIPA relative to sedentary behaviour and sleep, and sedentary behaviour relative to sleep. The final ratio ceased to be statistically significant after incorporating lifestyle factors. The preponderance of MVPA ceased to be statistically significant after incorporating health at outset and physical limitations on movement. Conclusions An association is inferred between survival rate and the physical activity composition of the day. The MVPA time share is important, but time spent in LIPA relative to sedentary behaviour and sleep is also a significant factor. Increased preponderance of MVPA may have detrimental associations at higher levels of MVPA.

Background: Time spent in different intensity-specific physical activities is codependent, but the substitution effect of different activities on weight status changes in children remains unclear. This study aims to investigate the prospective association between reallocating time in different intensities of physical activity and weight status changes among Chinese children. Methods: A national sample of 15,100 normal-weight children aged 7–18 years (46.7% boys) were recruited in September 2013 and followed up for nine months. Vigorous-intensity physical activity (VPA), moderate-intensity physical activity (MPA), walking, and sedentary time were obtained by International Physical Activity Questionnaire Short Form (IPAQ-SF). Height and weight were objectively measured, by which body mass index (BMI) and BMI z-score were calculated. Weight status was classified by the Chinese criteria for 7- to 18-year-old children. Isotemporal substitution analyses (including single-factor model, partition model, and isotemporal substitution model) were applied to examine the association of time allocation with weight status changes. Results: Each 30 min/day of increase in VPA was favorably associated with a 13.2% reduced risk of incident overweight/obesity in a single-factor model and a 15.6% reduced risk in a partition model. Negative associations were found between VPA, MPA, walking and the risk of being underweight in the single-factor model, but not in the partition model. In substitution models, replacing 30 min/day sedentary time with an equal amount of VPA was favorably associated with a 16.1% reduction of the risk of being overweight/obese. Conclusion: These findings highlight the need for promoting vigorous-intensity physical activity in children.
