Abstract Background: Emerging evidence suggests perceptions of being overweight account for many of the psychosocial consequences commonly associated with obesity, yet limited research has explored weight perception in associations between body mass index (BMI) and academic performance. The current study examined how BMI classification and weight perception relate to academic performance in a large cohort of youth.Methods: We used cross-sectional survey data from 61,866 grade 9-12 students attending the 122 Canadian schools that participated in Year 6 (2017/2018) of the COMPASS study. Mixed effect regression models were used to examine associations between students’ BMI classification and weight perceptions and their math and English/French course grades, stratified by sex.Results: For English/French grades, males and females with overweight or underweight perceptions were less likely to achieve higher grades than their peers with perceptions of being at “about the right weight”, controlling for BMI and covariates. For math grades, females with overweight perceptions, and all students with underweight perceptions, were less likely to achieve higher grades than their peers with “about the right weight” perceptions. All students with BMIs in the obesity range were less likely to report grades of 60% or higher than their peers with “normal-weight” BMIs. Overweight BMIs were predictive of lower achievement in females for English/French grades, and in males for math grades, relative to “normal-weight” BMIs. Results for students that did not respond to weight and weight perception items resembled those for obesity BMI and overweight/underweight perceptions, respectively.Conclusions: Overall, this study demonstrates that an obesity achievement gap remains when controlling for students’ perceptions of their weight, and that both underweight and overweight perceptions predict lower academic performance, regardless of BMI classification. Results suggest barriers to academic success exist among youth with larger body sizes, and those with perceptions of deviating from “about the right weight”.Plain Language Summary: An obesity achievement gap has been suggested as an early contributor to later socioeconomic disparities found by weight status. To date, limited research has examined how sociocultural weight norms contribute to potential links between weight status and academic performance. Emerging evidence suggests weight perception—individual’s subjective appraisal of their body weight—accounts for many of the psychosocial consequences commonly associated with obesity. We sought to determine if body weight and weight perceptions predicted grades in a large sample of Canadian secondary school students. It was hypothesized that perceptions of being at “about the right weight” would provide a protective effect. Overall, this study demonstrates that an obesity achievement gap remains when controlling for students’ perceptions of their weight, and that weight perceptions—both underweight and overweight—predict academic performance, regardless of BMI classification. Results contribute to a body of research that encourages the consideration of both overweight and underweight perceptions and their potential impact on adolescent emotional and physical health. Further research is needed to determine the mechanisms underlying these relationships, in order to remove barriers to academic success among youth with larger body sizes, and those with perceptions of deviating from “about the right weight”.

Purpose: To examine the association between weight status duration and weight perception accuracy. Design: Retrospective study. Setting: National Health and Nutrition Examination Survey 2003 to 2006. Sample: A total of 4029 adults aged 20 years or older. Measures: To assess weight status duration, participants’ past (10 years ago) and present body mass indexes were used to create 4 groups: normal weight in the past and present (group 1), normal weight in the past but overweight/obese in the present (group 2), overweight/obese in the present but not in the past (group 3), and overweight/obese in the past and present (group 4). We classified individuals as having an accurate/inaccurate weight perception based on present body mass index and whether they considered themselves as overweight, underweight, or about the right weight. Analysis: Logistic regression. Results: Our findings suggest that participants who were overweight/obese in the past, present, or during both time periods had increased odds of possessing an inaccurate weight perception. Furthermore, being normal weight in the past but obese in the present increased the odds of having an inaccurate weight perception nearly 10-fold and almost 5-fold for those obese in both the past and present. Conclusion: Studies such as this can aid clinicians in identifying individuals who may have increased odds of inaccurately identifying their weight status, thereby providing additional opportunities to facilitate weight loss.

Abstract Objectives Overweight in mothers and children in sub-Saharan Africa is rapidly increasing and may be related to body size preferences. The objective of this study was to measure mothers’ preferences for their own and their child's body size and how they relate to food choices. Methods We enrolled 271 mothers and their children (6–59 months) in Lilongwe and Kasungu Districts. Based on standard body-mass index and weight-for-height z-score cutoffs, 78 mothers (29%) were normal weight and 193 (71%) were overweight; 120 children (44%) were normal weight and 151 (56%) were overweight. Interviewers used a set of 7 adult female and 7 child body silhouette drawings and a semi-structured question guide to measure mothers’ perceptions of their own and their child's preferred and healthy body sizes and how their preferences affected food choices. We performed chi-squared tests comparing body size perceptions and grouped open-ended responses by weight status. Results Mothers’ selection of silhouettes that represented their body size preferences (67% normal weight, 68% overweight preferred overweight) and perceptions of a healthy body size (96% normal weight, 94% overweight selected overweight as healthy) did not differ by their weight status. A higher percentage of mothers of overweight than normal weight children preferred overweight child body sizes (70% vs. 48%, P = 0.003). Mothers’ perceptions of a healthy child body size (89% normal weight, 94% overweight selected overweight as healthy) did not differ by the child's weight status. To attain a larger body size, mothers said they could eat or feed the child larger quantities or more frequently and increase consumption of fatty/oily foods and drinks (such as sodas, sweetened yoghurt, and milk), but many cannot afford to do this. Conclusions Malawian mothers had strong preferences for overweight body sizes for themselves and mixed preferences for their children. Their desired strategies for increasing weight indicate that body size preferences may drive food choice but could be limited by cost. ObjectivesTo explore weight perceptions in a large, nationally representative sample of older adults, and the extent to which they differ according to age and perceived health status.SettingEngland.Participants5240 men and women (≥50 years old) participating in the English Longitudinal Study of Ageing (2016/2017).Main outcome measuresWeight perception was self-reported as too heavy, too light or about right.ResultsThe majority of older adults endorsed a weight perception that matched their (objectively measured) body mass index (BMI) classification. However, 1 in 10 (9.9%) older adults classified by BMI as normal weight (18.5–24.9 kg/m2) felt too light, with women at the upper end of the older age spectrum (OR=1.04, 95% CI 1.01 to 1.09), and men (OR=3.70, 95% CI 1.88 to 7.28) and women (OR=2.61, 95% CI 1.27 to 5.35) in poorer health more likely to do so. Almost half (44.8%) of older adults classified as overweight (25–29.9 kg/m2) and 1 in 10 (10.3%) classified as obese (≥30 kg/m2) felt about the right weight, with this observed more frequently among men and women at the upper end of the older age spectrum (OR range 1.04–1.06).ConclusionOlder adults’ perceptions of their own weight generally correspond with traditional BMI cut-offs for normal weight, overweight and obesity. However, a substantial minority ‘underestimate’ their weight status, with those at the upper end of the age spectrum and those in poorer health more likely to do so.

Background: It remain unclear that the association between weight status, weight perception, weight satisfaction and the clustering of physical activity (PA) and dietary behaviors in adolescents. Method: A cross-sectional analysis of National Health and Nutrition Examination Survey and the US Department of Agriculture’s Food Patterns Equivalents 2007–2014 data from adolescents aged 12–17 years (n = 2965) was conducted. Multivariable logistic regression models adjusted for demographic characteristics examined the association between weight status, weight perception, weight satisfaction, and the four created lifestyle groups (healthier behaviors, healthier diet only, physically active only, unhealthier behaviors). Results: Males with obesity were more likely to be in the healthier diet only group than males with a normal weight (OR = 1.90, 95% CI: 1.02, 3.52). Similar patterns were found in males who perceived themselves as being overweight or having obesity (OR = 2.09, 95% CI: 1.09, 3.99) and males with obesity who perceived their weight status accurately (OR = 2.33, 95% CI: 1.12, 4.88). Female respondents who were satisfied with their weight were 59% less likely to be in the healthier diet only group than healthier behaviors group compared with females who were weight dissatisfied (OR = 0.41, 95% CI: 0.23, 0.75). This pattern was not observed in males. Conclusions: Clustering PA and dietary behaviors were associated with weight status and weight perception for males but not females. Weight satisfaction was associated with clustered PA and dietary behaviors for females. These findings are important for obesity prevention policies and programs to better address adolescents’ obesity and reduce health disparities in this population.

Obesity is rapidly increasing worldwide and there is a general misconception regarding body weight perception. The aim of this study was to determine the prevalence of obesity and body weight perception among pregnant women. A total of 315 pregnant women participated in this study. Their Body Mass Index (BMI) was recorded and weight perception was assessed using a questionnaire. About two third (60.6%, n=191) participants in the study had excess weight out of which, 29.8% (n=94) were obese and 30.8% (n=97) were overweight. There was poor agreement between participant’s true body weight category and their perception (k=0.185 (95% CI, 0.119 to 0.258), p<0.001). Majority of participants who were obese (84%, n=79) and overweight (77.4%, n=75) underestimated their weight status. Only a very small percentage of obese (16%, n=15) and overweight 18.6% (n=18) participants had correct perception of their weight. However, the majority (84.3%, n=86) of participants with normal body weight had correct perception regarding their body weight. This study found that most obese and overweight women tend to underestimate their weight status while those of normal weight had correct perception of their body weight. The inaccurate perception of those who are obese and overweight may impact weight gain during pregnancy and affect pregnancy related complications. Education regarding ideal body weight and its benefits needs to be instilled during pre-pregnancy care to achieve better overall outcome.

Background: To examine associations of adolescents’ weight status perception and weight control intentions with dietary intakes. Methods: Cross-sectional data from adolescents aged 10–15 in the National Health and Nutrition Examination Surveys, 2005–2014 (n = 4940). Adolescents responded to questions regarding weight perception and if they were trying to change their weight. Intakes of calories, protein, carbohydrate, fat, saturated fat, sugar, and fiber were assessed using 24-h dietary recalls. Multivariable linear regression estimated associations of intakes with weight perception and weight control intentions. Results: The majority of adolescents perceived their weight as “about right”; however, 45% and 46% of boys and girls, respectively, reported trying to change their weight. Weight perception was not associated with intakes, with the exception of lower sugar (−13.65 g, 95% CI: −23.06, −4.23) and higher percent calories from protein (1.01%, 95% CI: 0.16, 1.87) in boys with overweight/obesity who perceived themselves as overweight, as well as lower percent calories from saturated fat (−1.04%, 95% CI: −2.24, −0.17) among girls with normal weight who perceived themselves as overweight. Weight control intentions were associated with intakes in boys only. Compared to boys who never tried to lose weight, boys who tried to lose weight consumed fewer calories (−188.34 kcal, 95% CI: −357.67, −19.01), a lower percent of calories from fat (−1.41%, 95% CI: −2.80, −0.02), and a greater percent of calories from protein (1.48%, 95% CI: 0.41, 2.55). Conclusions: Despite perceiving weight as “about right”, many adolescents reported trying to change their weight, which was associated with some dietary intakes. Efforts may be necessary to educate adolescents on healthy nutrition and weight management behaviors.

Background and Objectives: Overweight and obesity are highly prevalent in western countries and growing problems in developing countries. Weight loss behaviors are highly prevalent among adolescents, and body weight perception motivates weight control practices. The aim of this study is to investigate the relationships between actual body weight, body weight perception, and weight control practices among adolescents. Material and methods: A questionnaire-based survey was used to collect data on anthropometric measurements, demographic information, weight perception and weight control practices from a sample of 200 male and female students (89 boys and 111 girls) aged between13 and19 year old. The association between weight loss attempts and socio demographic factors, weight status and the weight perception were analyzed. Multivariable logistic regression analyses were performed to examine the predictors of misperception and its association with weight-related behaviors. Cohen's kappa was calculated to analyze the accuracy of body weight perceptions. Results: This study revealed that prevalence of underweight was higher in girls than boys. In this study, 8% were underweight including 3.5% severely underweight, 23.5% were overweight, 6% were obese and the rest of the participants (59%) were of normal weight. Only 61.5% respondents perceived their weight correctly. Perception of 21.5% adolescents was underestimated and 16.5% were overestimated than their actual weight status. Almost one third of the respondents were in weight control interventions. Almost 11% boys and 33% girls attempted weight loss activities. The gender biasness was seen in weight control (statistically significant ,p value =0.003). Body weight perception was found significant (p value, 0.001) in relation to weight control behavior. Conclusion: Body weight perceptions are not in agreement with actual weight in adolescents. This discrepancy is more marked in females who use a variety of weight control behaviors. These behaviors are motivated more apparently by perceived weight rather than actual body mass index.

Background: Young females tend to overestimate their weight status, which might induce unhealthy weight loss intentions and behaviours. This study aimed to examine weight perception measured by visual and verbal descriptions and its correlation with weight loss intentions among female nursing students. Methods: A cross-sectional survey was conducted among 600 female nursing students from four medical colleges in Shanghai, China. The participants rated perceptions of their weight by selecting a silhouette from the female Photographic Figure Rating Scale (PFRS) and one of the following verbal descriptions: “very underweight”, “slightly underweight”, “normal”, “overweight” or “obese”. Weight loss intentions were measured using the question “How often do you want to lose weight?”. Body mass index (BMI) was calculated from self-reported height and weight. Data were analysed using univariate and ordinal logistic regression analyses. Results: The accuracy of weight perceptions measured by verbal descriptions and visual descriptions was 44.50% and 55%, respectively. In females with underweight BMI (n = 135), 88.15% and 49.63% accurately classified their weight using visual descriptions and verbal descriptions, respectively. These females were more likely to overestimate (53.83% vs. 14.50%) and less likely to underestimate (1.67% vs. 30.50%) their weight when using verbal descriptions than when using visual descriptions. For verbal descriptions, weight overestimation was associated with weight loss intentions (odds ratio, 1.80; 95% confidence interval, 1.25–2.60). However, for visual descriptions, the two variables were not associated. Conclusions: A mismatch occurred between weight perceptions measured by the two methods and BMI status among female nursing students. Compared with verbal descriptions, visual descriptions had higher weight perception accuracy. However, weight overestimation measured by verbal descriptions was more likely to be associated with stronger intentions to lose weight than that of visual descriptions. These findings suggest that methodological discrepancies should be taken into account when measuring weight perception in future studies.
