Variations in root canal morphology presents a constant challenge for clinicians in their detection and management. Locating and treating these extra canals is important in order to achieve favorable prognosis in endodontic therapy. This review article presents the anatomical variations, frequency of occurrence of aberrant root canal configuration and methods of identification using advanced imaging techniques like CBCT, DOM and loupes.

Aim: The aim of this case report is to present a series of anatomical variations and endodontic treatments in four two-canal mandibular premolar teeth and three rooted three-canal maxillary second premolar teeth with root canal treatment indications identified via clinical and radiographic examinations. The success of root canal treatment is achieved with a thoroughly examined root canal morphology that has been accurately determined radiographically and clinically before adequate shaping, irrigation, and hermetic filling procedures. Root canals that are not found or not adequately disinfected can cause root canal treatment failure and complications, such as pain, swelling, or persistent fistula, also known as flare-up, after treatment. Canal variations in the teeth were detected via periapical radiographs during the root canal instrumentation stage. Methodology: The endodontic treatments of four two-canal mandibular premolar teeth and one triple-rooted three-canal maxillary second premolar with root canal treatment indications were described. Conclusion: To achieve full success in root canal treatment, anatomical variations should be examined in detail before and during treatment, and treatment should be completed with appropriate techniques.   How to cite this article: Dinger E, Bodrumlu E. It is of interest to compile available information on the root canal morphology of primary maxillary molars from known literature. The literature resources used to collect data include Medline/PubMed, The Cochrane Central Register of Clinical Trials, SIGLE and Science Direct. Data consists of type of population, number of teeth per study, number of root canals, canal length and type of root canal configuration. We used data from a total of 13 studies (951 primary maxillary molars). Maxillary molars (1st and 2nd) are dominant for two roots variant. The first molar the mean root length ranges from 7.9mm – 8.1mm. The second molar ranges from 7.2mm-8.5mm. Type I (explain in a phrase) canal morphology is the common variant in both the molars. Data shows that Root Canal morphology shows variations with the diagnostic aid (example micro CT) used and in different ethnic populations.

Introduction. Predictable endodontic treatment depends on the dentist?s knowledge about root canal morphology and its possible anatomic variations. The majority of mandibular canines have one root and root canal, but 15% may have two canals and a smaller number may have two distinct roots. The following clinical reports describe endodontic treatment of mandibular canines with two roots and two root canals. Outline of Cases. Four clinical case reports are presented to exemplify anatomical variation in the human mandibular canine. Detailed analysis of the preoperative radiographs and careful examination of the pulp chamber floor detected the presence of two root canal orifices in all canines. Working length was determined with an electronic apex locator and biomechanical preparation was carried out by using engine driven BioRaCe Ni-Ti rotary instruments in a crown-down manner, followed by copious irrigation with 1% sodium hypochlorite. Definitive obturation was performed using cold lateral condensation with gutta-percha cones and Top Seal paste. The treatment outcome was evaluated using postoperative radiographs. Conclusion. Endodontists should be aware of anatomical variations of the treated teeth, and should never presume that canal systems are simple.

Recognition of anatomical variations is a real challenge for clinicians undertaking therapy regardless of the teeth that are to be treated. The extent of the curvature is one of the most important variables that could lead to instrument fracture. In clinical conditions, two curves can be present in the same root canal trajectory. This type of geometry is denoted as the “S” shape, and it is a challenging condition. This report describes a different clinical and educational scenario where four specialists around the world present different approaches for the treatment of root canals with double curvatures or S-shaped canals. Endodontic therapy is a very nuanced and challenging science and art. The clinical and teaching experience of the authors show different approaches that can be successfully employed to treat challenging teeth having roots with multiple curves. The necessity of precise knowledge of the root canal morphology and its variation is also underlined.

The complex root canal system of maxillary second premolars shows variations among different populations. The aim of this study was to evaluate the root canal morphology of second maxillary premolars in the romanian population. The number of roots, root canals and the root canal configuration were investigated using radiographs in both buccal-oral and mesio-distal incidence. They were categorized using Weine’s classifi cation. Among the 480 teeth, 90.83% had one root and 66.67% one root canal. The frequency of secondary maxillary premolars with two root canals (33.12%) shouldn’t be underestimated. Knowing the variations of the internal root canal morphology of maxillary second premolars in a certain population can lead the clinician to a better approach of the endodontic treatment.

The purpose of this study was to determine the variations in the number of canals and its morphology in permanent mandibular first molars using cone-beam computed tomography (CBCT) in a subset of Indian population. A total of 350 CBCT scans were collected and evaluated in all three orthogonal planes to determine the number of root canals and study their anatomic variations using Vertucci’s classification.: The total number of roots and root canals, the root canal configuration, the incidence, and the correlations between left- and right-side occurrences and between males and females were analysed. Statistically significant differences were evaluated using the chi-square test with SPSS 16.0 for Windows (SPSS, Chicago, IL), with significance set at p < 0.05.: The results of this study showed that predominantly, two thirds of the subjects had type VI mesial canal configuration and two-thirds had type I distal canal configuration. Also there was significant variance in the canal anatomy across gender and age of the subjects.: CBCT is a clinically useful tool in understanding the variations in root canal morphology, which guides in the assessment of the canal morphology aiding in successful root canal treatment.

Aim : To investigate the number and. patterns of. root canals. of mandibular. first. molars.. Methods.: Descriptive observational study to investigate the canal morphology using clearing technique. The duration of the study was one year from 1st February 2020 to 1st February 2021.Non purposive sampling technique was used and 200 mandibular first molar were collected. Data collected was analysed using the SPSS Vr 10. Study variables include Number and pattern of the roots in mandibular teeth. Descriptive statistics were used; frequencies of the root canals and their canal pattern were calculated. Results: In the mesial root, 14 teeth had a one root canal,186 possessed double root canals were observed. Distal root of 160 teeth possessed one canal, 40 teeth possessed two root canals with type 1 pattern mostly observed. Conclusion: Like other populations Pakistani population may have a diverse root canal system in the mandibular molars that ultimately affect endodontic therapy. Background: The internal anatomy is a complex system and root canal morphology varies between teeth, especially in maxillary second premolars. Objective: The purpose of this present study is to identify the variations in root and root canal morphology of the upper second premolar. Methods: A retrospective study on 197 maxillary second premolars, which have had an endodontic treatment in Ajman University in the Academic Year 2018/2019. Intraoral periapical (IOPA) radiograph was reviewed for identifying the number of root canals. Results: The incidence of two canals in the maxillary second premolars was very high 78.7%, and the internal anatomy is complex and highly variable. Conclusion: A straightforward message to the clinicians to be careful in their evaluation and to take care whenever an upper maxillary second premolar is to be treated endodontically.

The present study was conducted to determine the number of roots and morphology of the root canal system of permanent mandibular first molars (M1) in a Sri Lankan population. Sample of 529 M1 teeth was used. The number of roots was examined and the lengths of the mesial and distal roots were measured to the nearest 0.01 mm. Vacuum injection protocol was used to inject China ink into the root canal system, making it transparent. Root canal morphology was recorded using Vertucci’s classification. Presence of furcation canals, position of lateral canals, intercanal communications, level of bifurcation, and convergence of the root canal system were recorded. M1 showed three roots in 4.1% of the sample. Commonest root canal morphology of the mesial root was type IV and the distal root was type I. The level of bifurcation of the root canals was commonly observed in the cervical one-third of the root while convergence was observed in the apical one-third in both roots. Prevalence of three rooted mandibular first molars is less than 5%. Mesial root showed the most variable canal morphology. Prevalence of furcation canals was 1.5% while that of middle mesial canals was 0.2%.

Root canal treatment of tooth with aberrant root canal morphology is very challenging. So thorough knowledge of both the external and internal anatomy of teeth is an important aspect of root canal treatment. With the advancement in technology it is imperative to use modern diagnostic tools such as magnification devices, CBCT, microscopes, and RVG to confirm the presence of these aberrant configurations. However, in everyday endodontic practice, clinicians have to treat teeth with atypical configurations for root canal treatment to be successful. This case report presents the management of a mandibular first molar with six root canals, four in mesial and two in distal root, and also emphasizes the use and importance of Cone Beam Computed Tomography (CBCT) as a diagnostic tool in endodontics.
