Introduction: With the advent of newer radiographic diagnostic procedures of the maxillofacial region, there is a drastic increase in the use of Ionizing radiation which further leads to increased chance of radiation hazards among the patients and the health care workers. In addition to the diagnostic information extracted, the radiation exposure carries the potential to induce carcinogenesis in the exposed individual. However, the amount of Radiation exposure in dentistry is significantly low but it is still harmful owing to the requirement of repeated radiographic examination during the dental treatment. Therefore, to ensure minimum and inevitable exposure during dental treatment, it is necessary to follow principles of radiation protection and safety. Recommendations: Several studies in the literature have revealed that the attitude and knowledge of the dental professionals regarding radiation safety is not up to the mark. Henceforth, there is a necessity of implementing certain basic guidelines regarding radiation safety and protection. Further state dental councils must advocate new and interesting methods of education regarding the same and should introduce strict rules and penalties for this spectrum of field. Conclusion: This present short commentary is to familiarize the dental practitioner regarding the methods to minimize the risk of the radiation hazards. Further this article will also educate the dental practitioners regarding the pathogenesis of Radiation effects during Radiation therapy of head and neck region along with pertinent management protocols.

The determination of the physicochemical forms of radioiodine in the gas-air environment of an industrial nuclear reactor is necessary to solve related problems – technological control and radiation safety. In the technological context, the results obtained make it possible to adequately assess the efficiency of purification of emissions of radioactive iodine isotopes, the choice of instruments and methods for controlling emissions. In the context of radiation safety, research results make it possible to correctly assess the radiation effects on the environment and humans, substantiation of emission standards for the atmosphere and confirmation of the safety of operation of an industrial reactor installation. The research method is based on the difference in the deposition of radioiodine on a set of one AFA-RMP aerosol filter and six filters of the AFA-SI type, which makes it possible to separately determine the 131I aerosol, easily and hardly sorbed form. It has been shown that the non-purified gas-aerosol mixture mainly contains radioactive iodine in the form of gaseous hardly adsorbed compounds. For 131I, the most probable percentage in volumetric activity of hardly adsorbed, easily adsorbed compounds and iodine aerosols was obtained. Based on the data obtained, an assessment of dose loads was carried out taking into account the annual emissions of the reactor installation and weather conditions. A conservative approach to assessing the radiation exposure of 131I emissions is 47 times higher than the assessment taking into account its physicochemical forms.

SummaryThe curative effectivity of external or internal radiotherapy necessitates exposure of normal tissues with significant radiation doses, and hence must be associated with an accepted rate of side effects. These complications can not a priori be considered as an indication of a too aggressive therapy. Based on the time of first diagnosis, early (acute) and late (chronic) radiation sequelae in normal tissues can be distinguished. Early reactions per definition occur within 90 days after onset of the radiation exposure. They are based on impairment of cell production in turnover tissues, which in face of ongoing cell loss results in hypoplasia and eventually a complete loss of functional cells. The latent time is largely independent of dose and is defined by tissue biology (turnover time). Usually, complete healing of early reactions is observed. Late radiation effects can occur after symptom-free latent times of months to many years, with an inverse dependence of latency on dose. Late normal tissue changes are progressive and usually irreversible. They are based on a complex interaction of damage to various cell populations (organ parenchyma, connective tissue, capillaries), with a contribution from macrophages. Late effects are sensitive for a reduction in dose rate (recovery effects).A number of biologically based strategies for protection of normal tissues or for amelioration of radiation effects was and still is tested in experimental systems, yet, only a small fraction of these approaches has so far been introduced into clinical studies. One advantage of most of the methods is that they may be effective even if the treatment starts way after the end of radiation exposure. For a clinical exploitation, hence, the availability of early indicators for the progression of subclinical damage in the individual patient would be desirable. Moreover, there is need to further investigate the molecular pathogenesis of normal tissue effects in more detail, in order to optimise biology based preventive strategies, as well as to identify the precise mechanisms of already tested approaches (e. g. stem cells).

Abstract Background Self-care and professional care of implants may prove difficult for elderly people who require nursing care. However, the actual state of care and problems remains unknown. In this study, we investigated the actual state of implant problems in elderly people living in their own home or in a nursing home who received visiting dental treatment. Methods We mailed questionnaire survey forms to 2339 representatives or specialists who were members of the Japanese Society of Oral Implantology, the Japanese Society of Gerodontology or the Japan Prosthodontic Society. We narrowed down the respondents to those who provided visiting dental treatment, and analyzed the actual state of implants observed during visiting dental treatment (type, care, problems, countermeasures, etc.). Results Of the 924 dentists who responded to the questionnaire survey, 291 (22%) provided visiting dental treatment. While the majority of implant types encountered in the previous 12 months were root-form implants, there were still a certain number of blade and subperiosteal implants. Daily implant care involved mostly cleaning with a toothbrush + auxiliary tools. The most frequent implant problems encountered in the past were difficulty in cleaning and peri-implantitis. Medication and antiphlogistic treatment were most frequently adopted as countermeasures to implant problems, followed by observation. When we classified the results into those for the dentists who provided implant treatment and those for the dentists who did not, we found that many of the dentists who did not provide implant treatment opted for observation or medication, while those who provided implant treatment also implemented removal of superstructure, retightening of screws, repair and so forth. Conclusions We found that many of the implant troubles encountered by dentists who provided visiting dental care were difficulty in cleaning or peri-implantitis, and that the actions taken against these troubles varied depending on the experience of the dentist performing the implant treatment. Our study also revealed that dentists who provide visiting dental care need to acquire knowledge and skills of implant treatment, to have actions prepared in case they encounter such cases, or to closely coordinate with dentists who specialize in implants.

Objective: To assess the knowledge about different mandibular nerve block techniques of fresh dental graduates, dental surgeons and specialists to attain mandibular nerve block for dental treatment in Multan. Design of the Study: It was a cross-sectional survey. Study Settings: This study was carried out at Department of Dentistry at Multan Medical & Dental College, Nishtar Institute of dentistry (Nishtar Medical University) and Bakhtawer Amin Dental College from August 2019 to August 2020. Material and Methods: The study involved 220 respondents. The questionnaire was administered to fresh dental graduates (house officers) and dental surgeons in 3 dental colleges of Multan, and was collected on the same day. House officers who did not return the forms were reminded personally and contacted on the phone thrice over the following week. Results of the Study: The frequency of respondants who could administer Gow-Gates without supervison was 20.9% (n=46). Participants from NID, Multan had the highest frequency (35.3%) with the lowest in BIMDC (16.6%)and MMDC(20.6%). The differences were non-significant (p=0.19). Significantly higher frequency of male respondants (10%, n=22) claimed they could administer Gow-Gates without supervison compared to female respondants (5.4%, n=12, p=0.012, Chi-Square). Training for Gow-Gates had been received by 12.7% (n=28) of the House Officers and dental surgeons. The Highest frequency of training was in NID, Multan (5.4%, n= 12, p= 0.016, chi-square). Conclusion: The majority of the dental practitioners of Multan used IANB as their primary LA technique and intraligamental injections as a supplemental LA technique in their clinics to attain mandibular molars anesthesia for general dental procedures like fillings, root canals and extractions. The different types of injury from external sources of ionizing radiation are briefly described and the manifestations of radiation injury in the fetus, the infant and the child are reviewed. If radiation therapy for malignant disease is excluded from consideration, it is found that x-ray damage to skin, other tissue destruction, and growth impairment are relatively minor radiation hazards compared to the dangers of leukemia and other malignancy and of radiation-induced genetic damage. Consideration is given to somatic radiation injury as largely an individual problem and genetic injury as a population problem. This point of view is reflected in the differing recommendations concerning "permissible doses" for individuals and for populations. Medical radiation represents a major and presumably growing source of exposure to individuals and to the population in the United States. Various ways are shown in which excessive diagnostic exposure to x-rays, especially in children, can be reduced without interference with requirements of diagnosis. Control of avoidable radiation can be accomplished by combined attack from different fronts. The more important approaches to radiation safety are discussed. They include attention to technical detail, personnel monitoring, maintenance of radiation records, radiation safety through legislative control and public education. The responsibility of the physician in promoting public understanding of radiation hazards is emphasized.

Purpose: Harmonization and improvement of the system for regulating the internal radiation exposure of workers and the basic requirements for ensuring radiation safety with international requirements and recommendations. Material and methods: Issues related to the development of approaches to regulation and monitoring of workers for internal radiation exposure in the process of evolution of the ICRP recommendations and the national radiation safety standards, are considered. The subject of analysis is the standardized values: dose limits for workers and permissible levels as well as directly related methods of monitoring of workers for internal radiation exposure, whose purpose is to determine the degree of compliance with the principles of radiation safety and regulatory requirements, including non-exceeding the basic dose limits and permissible levels. The permissible levels of inhalation intake of insoluble compounds (dioxide) of plutonium-239 are considered as a numerical example. Results: Based on the analysis of approaches to the regulation and monitoring of workers for internal radiation exposure for the period from 1959 to 2019, it is shown that a qualitative change in the approach occurred in the 1990s. It was due to a decrease in the number of standardized values by introducing a single dose limit for all types of exposure: the effective dose E, which takes into account the different sensitivity of organs and tissues for stochastic radiation effects (WT), using the previously accepted concepts of the equivalent dose H and groups of critical organs. From the analysis it follows that the committed effective dose is a linear transformation of the intake, linking these two quantities by the dose coefficient, which does not depend on the time during which the intake occurred, and reflects certain exposure conditions of the radionuclide intake (intake routes, parameters of aerosols and type of radionuclide compounds). It was also shown that the reference value of the function z(t) linking the measured value of activity in an organ (tissue) or in excretion products with the committed effective dose for a reference person, which is introduced for the first time in the publications of the ICRP OIR 2015-2019, makes it possible to standardize the method of measuring the normalized value of the effective dose. Based on the comparison of the predicted values of the lung and daily urine excretion activities following constant chronic inhalation intake of insoluble plutonium compounds at a rate equal annual limit of intake (ALI) during the period of occupational activity 50 years it was shown that the modern biokinetic models give a slightly lower level (on average 2 times) of the lungs exposure compared to the models of the previous generation and a proportionally lower level (on average 1.4 times) of plutonium urine excretion for the standard type of insoluble plutonium compounds S. However, for the specially defined insoluble plutonium compound, PuO2, the level of plutonium urine excretion differs significantly downward (on average 11.5 times) compared to the models of the previous generation. Conclusion: With the practical implementation of new ICRP OIR models, in particular for PuO2 compounds, additional studies should be carried out on the behavior of insoluble industrial plutonium compounds in the human body. Besides, additional possibilities should be used to determine the intake of plutonium by measuring in the human body the radionuclide Am-241, which is the Pu-241 daughter. To determine the plutonium urine excretion, the most sensitive measurement techniques should be used, having a decision threshold about fractions of mBq in a daily urine for S-type compounds and an order of magnitude lower for PuO2 compounds. This may require the development and implementation in monitoring practice the plutonium-DTPA Biokinetic Model.

Both the description of and the operational experience with the radiation protection system at the research reactor WWR-M are presented. The list of the factors regarding the radiation hazards during the reactor routine operation is given and the main activities on the radiation safety provision are established. The statistical information for the staff exposure, the radioactive aerosol releases and the external radiation monitoring is shown. The preliminary considerations on the system upgrading for the decommissioning are presented.

The dimensions of patient-centred care include not only clinical effectiveness and patient safety, but, importantly, the preferences of patients as consumers of healthcare services. A total of 249 participants were included in the study, with a balanced population proportional representation by age, gender, ethnicity and geographic region of New Zealand. An online questionnaire was used to identify participants’ decision-making process, and what factors and barriers for participants to seek dental treatment. Cross-tabulations, Spearman correlation analysis and Pearson Chi-Square analysis were used for the statistical analyses. Three most common reasons for visit were check-up (77%), clean (57%) and relief of pain 36%). A desire to treat a perceived problem was the most common encouraging factor to seek dental care. Cost was the most common barrier to seeking dental services. The majority of participants attended a private practice (84%), with convenience of location and referral from professionals the most likely to influence their choice. Participants felt the most important trait a dental practitioner could demonstrate was to discuss treatment options with them before any treatment. Dental check-up, teeth cleaning and relief of pain were the most common reasons for patients to choose dental services. Cost and ethnicity of the consumers had a significant impact on how dental services were perceived and sought. Dental practitioners may need to reorientate how they express value of oral health practice, not just in regard to communication with patients, but also with government funding agencies.

Concerning radiation safety must meet several radiation protection requirements which include justification or utilization of nuclear power, dose limitation, optimization of protection, and radiation safety. The purpose of this study was to determine the optimization of the exposure factor selection and determine the radiation dose received by the patient based on the selection of exposure factors on the thorax examination. The author observed the use of exposure factors on chest radiographic examination with a sample of 60 people. The design of this study uses the correlational method. The independent variables in this study are age, body weight, object thickness, kV, and mAs. Dependent variable exposure to radiation dose on chest examination. The conclusion of this study is the dose exposure level is influenced by age, object thickness, weight, and use of exposure factors which will then be obtained by the DRL (Diagnostic Reference Level) value.

Neuropathic pain is a common phenomenon that affects millions of people worldwide. Maxillofacial structures consist of various tissues that receive frequent stimulation during food digestion. The unique functions (masticatory process and facial expression) of the maxillofacial structure require the exquisite organization of both the peripheral and central nervous systems. Neuralgia is painful paroxysmal disorder of the head-neck region characterized by some commonly shared features such as the unilateral pain, transience and recurrence of attacks, and superficial and shock-like pain at a trigger point. These types of pain can be experienced after nerve injury or as a part of diseases that affect peripheral and central nerve function, or they can be psychological. Since the trigeminal and glossopharyngeal nerves innervate the oral structure, trigeminal and glossopharyngeal neuralgia are the most common syndromes following myofascial pain dysfunction syndrome. Nevertheless, misdiagnoses are common. The aim of this review is to discuss the currently available diagnostic procedures and treatment options for trigeminal neuralgia, glossopharyngeal neuralgia, and myofascial pain dysfunction syndrome.
