The aim of the work was to compare the condition of periodontal tissues in patients with rheumatoid arthritis who live in ecologically favorable and environmentally unfavorable territories. There was performed a dental examination of 21 patients with rheumatoid arthritis who live in environmentally favorable territories and of 21 patients with rheumatoid arthritis who live in environmentally unfavorable areas. There was given the index evaluation of the condition of periodontal tissues, the level of oral hygiene. It has been determined that the course of generalized periodontitis in patients with rheumatoid arthritis who live in ecologically contaminated territories is more difficult than in a group of patients who live in environmentally favorable territories.

In recent decades, both in the world and in our country, thyroid gland diseases are not only widespread, but also have a tendency to constant growth. Currently, more attention is paid to diseases of the thyroid gland that are associated with endemic foci of iodine defi ciency in soil and water. The aim of the study – to investigate the state of periodontal tissues in individuals with primary hypothyroidism.Materials and Methods. We conducted a survey of 86 people with primary hypothyroidism, 16 of them with manifestation and 70 with subclinical forms. A comparison group was 45 people without thyroid gland diseases. Index assessment of periodontal tissues included the defi nition of a simplifi ed oral hygiene index of the oral cavity of the Green-Vermillion (Oral Hygiene Index-Simlifi ed, Green-Vermillion, 1964), the degree of gingival infl ammation was determined by Parma (C. Parma, 1960, Papillary-Marginal Alveolar Index)), the state of periodontal tissues in the complex periodontal index (KPI) (1987), destructive changes in the bone tissue of the alveolar appendix were determined by the Fux index. Results and Discussion. The article presents the results of the index estimation of periodontal tissues in individuals with primary hypothyroidism. An analysis of the conducted index evaluation found that they had a poor level of hygienic state of the oral cavity. This was manifested by the development of infl ammatory and, to a lesser extent, infl ammatory and dystrophic lesions of periodontal tissues, as evidenced by papillarymarginalalveolar (PMA) and complex periodontal indices (KPI).Conclusions. Even more signifi cant changes in these indicators were found in individuals with a manifestation of hypothyroidism. So, we can conclude that in persons with primary hypothyroidism metabolic processes ofthe organism are violated, which leads through a number of pathophysiological changes to the activity of parodontopathogenic microfl ora and the development of infl ammatory and infl ammatory and dystrophic periodontal diseases.

The aim of this work was to determine the values of plaque accumulation indices and oral hygiene indices, to conduct a comparative assessment before the start of multimodality therapy in patients with mild and moderate parodontitis and after its completion among the adult population of the Chuvash Republic. The study of index evaluation values of 146 patients aged 33–62 years with a diagnosis "chronic generalized parodontitis of mild and moderate severity" was conducted. The age range was determined by the criteria for dividing the age groups according to WHO data – early adult (25–35 years), middle adult (36–45 years), late adult (46-60 years). The examination was carried out according to standard clinical methods of simplified oral hygiene index OHI-s (according to Green – Vermillion), dental plaque hygiene index (PI) Silness-Loe (1964), evaluation of the state of periodontal tissues PI (by Russel). The authors present the criteria for evaluating the indices before the start of standard treatment and after its completion for all the studied patients and in groups with mild and moderate severity of chronic generalized parodontitis. The authors give as well a comparative analysis of the patients’ index assessment depending on the severity of the disease, the dynamics of the infection resolution in groups with different degrees of of parodontitis severity.

Aim. To reveal the rate and severity of clinical manifestations of common dental diseases and dentofacial anomalies in professional athletes Methods. The effect of intense physical exertion and overtraining syndrome on the state of the organs and tissues of the dental system were examined in 200 professional athletes aged 1825 years between 2014 and 2016. Five groups where each has the same number of athletes (n=40) included: team sport athletes (volleyball, handball), martial arts athletes (wrestling, boxing), gymnasts, swimmers, athletes. The control group consisted of 40 individuals who were not professionally involved in sports. The state of the oral cavity, in particular periodontal tissues, was judged by changes in the following indicators: prevalence of dentofacial anomalies, the incidence of dental caries, state of the oral cavity hygienic using by simplified oral hygiene index (OHI-S) ( Green J.С., Vermillion J.R., 1963), degree of gums bleeding by using the MuhlemannCowell bleeding index, degrees of severity of periodontopathy by using the papillary-marginal alveolar index (RMA) [Massler, Schour (1949) in Parma modification (1960)]. Results. According to the values of the simplified oral hygiene index the worst state of the oral cavity hygienic was in wrestlers (2.120.022 points) and boxers (2.030.029) compared to the control group (1.620.026, p=0.049 and p=0.001, respectively). Catarrhal gingivitis and generalized chronic periodontitis are most often diagnosed in the groups of wrestling (82.56.01%), gymnastics (77.56.60%), boxing (70.07.25%) and swimming (70.07.25%). Dentoalveolar anomalies were more often detected in boxers (77.5% of cases, p=0.001) and in gymnasts (34.0%, p=0.365) compared to the control group. In these groups, diastema was also more often diagnosed. Conclusion. Periodontal inflammation indicators were higher in wrestlers and gymnasts groups compared to the dental status of all other groups of athletes; caries and maximum level of dental anomalies are more common for boxers.

Background. The risk of chronic gingivitis is increased in patients who undergo orthodontic treatment. It is known that the gingivitis is closely correlated with the level of hygiene.Objective. The study involved 123 orthodontic patients with chronic catarrhal and hypertrophic gingivitis that developed during the first two months of active orthodontic treatment. We chose VITIS ORTНODONTIС (DENTAID INTERNATIONAL, Spain) that contains active ingredients we need to support healthy state of oral cavity. All studied patients were 12-15 years old.Methods. For dental examination we used health indices OHI-S according to Greene-Vermillion (1964). Gums bleeding was determined according to a modified SBI index by Muhlemann (1971), inflammation of the gingival margin was assessed by PMA index Parma (1960).Results. Therapeutic treatment consisted of the following: all patients underwent correction of oral hygiene, removal of dental plaque. Vitis ORTHODONTIC was prescribed due to the manufacturer's recommendations: rinse 15 ml for 30 seconds after normal procedures of oral hygiene. Eating or drinking is not recommended during the next 30 minutes after using this product. The results proved a high anti-inflammatory efficacy of the treatment schemes.Conclusions. The complex therapy of early manifestations of inflammation in periodontal had a positive effect on the subjective feelings of patients and health performance rate, gum inflammation and bleeding.

Introduction. The study of the main dental statuses begins with the determination of indicators of the state of quality of oral hygiene and periodontal tissues. Given these factors, dentists should choose the most suitable for use in the appropriate age-sex and professional groups, taking into account the existing harmful factors of the external and internal environment, and occupational hazards. The objective was to determine the hygienic and dental status in different population groups by studying the parameters of the main dental diseases and to draw a parallel between them and the level of hygienic knowledge in these groups, for the correction of hygiene and preventive measures as the main way to prevent these diseases. Methods and materials. The study involved 307 people who were not burdened with general somatic pathology, of which 6 age groups were formed. Dental status was determined using the Green-Vermillion hygiene index, PMA and Mühlemann and Son periodontal indices. In order to study the level of hygienic knowledge and skills of the population, a survey was conducted using a Questionnaire and its assessment by the index of hygienic knowledge of S. B. Ulitovskiy.Results. As a result of the study, a decrease in the Green – Vermillion index was found in group 1 from (2.13±0.38) to (1.07±0.27), in group 5 – from (2.83±0.14) to (0.75±0.09); in group 6, the index at the beginning of the study was (3.47±0.12), and by the end, it decreased to (1.93±0.05), which reflects an increase in the cleansing efficiency in various groups of the population. The positive dynamics of the anti-inflammatory efficiency was observed in the 1st group from (16.81±3.52) to (8.15±1.64) %; in the 5th group, indicators has decreased by 3 times, from (18.86±0.65) to (6.53±0.77) %, in the 6th group – from (19.24±0.43) to (7.01±1.19) %. As a result of the study, it was found that the state of periodontal tissues in the 6th group according to the index of bleeding Mühlemann and Son was (9.21±2.33) %, and by the end of the study reached (5.49±1.63) %, in the 5th group – from (11.29±1.98) to (5.31±1.62) %; in the 2nd group, the indicators for the index of bleeding decreased from (11.67±0.69) to (5.00±0.28) %. In the groups, there was a decrease in the survival of hygienic knowledge, which determined the need for dental education among the population in different age groups.Conclusion. Application of individual hygienic programs of prevention of dental diseases on the basis of consistently performed hygienic procedures using various means of oral hygiene confirms the effectiveness of the activities taking into account personal hygiene indicators and dental status.Authors declare no conflict of interest. The authors confirm that they respect the rights of the people participated in the study, including obtaining informed consent when it is necessary, and the rules of treatment of animals when they are used in the study. Relevance. Adverse environmental conditions and man-made disasters are not considered as air, soil and groundwater oversaturated environmental pollutants and toxicants. They cause disturbances in biochemical reactions and metabolic processes in childhood and at a young age. The aim of this study was to determine the effect of polymorphism of the detoxification gene of the first phase Cyp1A1 (A1506G) and the second phase genes GSTM1, GSTT1, on hard tissue indices of permanent teeth, periodontal indices and oral hygiene indices in children living in areas of different anthropogenic stress. Materials and methods. We studied the polymorphism of the CYP1A1 gene (A1506G), polymorphisms GSTM1, GSTT1 in children living in areas of different anthropogenic stress. We evaluated the state of hard tissues of teeth, periodontal tissues and oral hygiene. Findings. The results of a study of the dental status of children indicate a negative effect of polymorphisms of the GSTM1, GSTT1 genes on the state of hard tissues of teeth, periodontal tissues and oral hygiene, especially as a result of adverse environmental factors. To reduce the likelihood of adverse factors provoking the development of major dental diseases, it is necessary to develop prevention methods based on the study of hereditary predisposition to them.

The susceptibility to the development and progression of inflammatory periodontal diseases, which depends on genetic and external factors (smoking, stress, oral hygiene), varies widely. In the development of these diseases, an important role is played not only by the presence of periodontal pathogenic microorganisms, but also by the presence of congenital or acquired immunodeficiency, immunoregulatory defects. The immune system plays a key role in the physiological and pathological processes of periodontal tissues. In this regard, IL17, produced by CD4+ Th cells, which has both Pro-inflammatory and protective activity, is of particular interest in the pathogenesis of periodontitis. The aim of study was to identify the relationship between polymorphic loci of the IL-17A (rs2275913) and MMP-1 (rs1799750) genes and clinical manifestations of chronic periodontitis in petrochemical workers. Dental examination was performed in 92 ethylene oxide production workers with chronic periodontitis and 74 patients with chronic periodontitis who did not come into contact with chemical factors (control group). Genotyping of polymorphisms rs2275913 of the IL17A gene and rs1799750 of the MMP1 gene was performed by allele-specific real-time polymerase chain reaction (PCR). Hygienic assessment of the degree of air pollution of the working area with harmful substances was carried out by gas chromatography according to the guidelines for the determination of harmful substances in the air № 5098-89, № 3119-84. When comparing the results of studies of both groups, there were no statistically significant differences in the frequency distributions of allelic variants and genotypes of the IL-17A and MMP-1 genes. The AA/AG genotypes of the IL-17A gene were associated with an increased risk of severe disease compared to the GG genotype in workers in the main group (OR=6.1; 95% CI 1.33-28.5; p=0.021) and in the control group (OR=7.26; 95% CI 1.34-39.25; p=0.016). Carriers of the A allele in the control group increased the risk of severe chronic periodontitis by 2.4 times compared to carriers of the G allele (OR=2.41; 95% CI 1.19-4.87; p=0.014). During the dental examination of employees of the ethylene oxide plant, the clinical course of periodontal diseases was more severe in comparison with the control group, and the number of patients with severe periodontitis was twice as high. It was found that the AA/AG genotypes of the IL-17A gene and the carrier of the A allele are associated with increased susceptibility to the development of severe chronic periodontitis. The association between the MMP-1 gene polymorphism and the risk of severe forms of chronic periodontitis has not been established. A risk factor for the development of inflammatory periodontal diseases in employees of the petrochemical complex is a complex of harmful production factors.

With dental interventions, bacteremia often develops, which is called transient. Its essence is that after laziness of teeth and dental deposits, curettage, endodontic treatment, a microflora appears in the blood, which is far from indifferent to patients, especially those suffering from diseases of the heart, blood vessels, and kidneys. According to Li I terature, transient bacteremia often occurs during interventions in patients with periodontal inflammation. At the same time, a number of authors believe that the frequency of bacteremia and its severity are influenced not only by the state of the periodontal tissues, but also by the volume of intervention.
