ABSTRACT Objectives To evaluate the effects of submucosally administered platelet-rich plasma (PRP) on the rate of maxillary canine retraction. Levels of soluble receptor activator of nuclear factor-κb ligand (sRANKL) and osteoprotegerin (OPG) in the gingival crevicular fluid (GCF) were also measured over 2 months. Materials and Methods This split-mouth trial involved 20 sites in 10 subjects randomly assigned to PRP (experimental) side and control side. After alignment, the freshly prepared PRP was injected submucosally distal to the experimental side maxillary canine, and retraction was performed using NiTi closed-coil springs (150 g) on 0.019 × 0.025-inch stainless steel wire. The rate of canine movement was assessed using digital model superimposition at 0, 30, and 60 days. The OPG and sRANKL were assayed using enzyme-linked immunosorbent assay from GCF collected at 0, 1, 7, 21, 30, and 60 days. Results Twenty sites were analyzed using paired t test. The rate of tooth movement increased significantly by 35% on the PRP side compared with the control side in the first month (P = .0001) and by 14% at the end of the second month (P = .015). Using the Mann–Whitney U test, OPG levels were found to be significantly decreased on the 7th (P = .003) and 30th day on the PRP side (P = .01), while sRANKL became detectable by the third week postinjection on the PRP side (P = .069). Conclusions Submucosal injection of platelet-rich plasma significantly increased tooth movement during the 60-day observation period. Local injection of PRP significantly altered the levels of OPG and sRANKL in GCF.

Objective: To compare the rate of accelerated tooth movement in canine retraction with micro-osteoperforation on one side and control on the other. Study Design: Quasi experimental study. Place and Duration of Study: Orthodontics department, Armed Forces Institute of Dentistry, Rawalpindi Pakistan, from Jul 2018 to Jan 2019. Methodology: A total of 30 patients were inducted. After alignment and extraction of maxillary first premolars, canine retraction was started with closed NiTi coil spring on both sides of the maxillary arch. Micro-osteoperforation was done on the right side and other side was a control side. The distance between the lateral incisor and the canine was measured on both sides before micro-osteoperforation. The same measurements were recorded after three weeks of retraction. The difference between pre and post retraction measurements was recorded. The difference in the rate of canine retraction between both modalities was compared using independent sample t-test. Results: The study included 17 males (56%) and 13 females (43%). The mean rate of tooth movement at experimental side of maxilla was 1.6 ± 0.52 mm and on control side was 0.66 ± 0.31 mm (p<0.05). Conclusion: Micro-osteoperforation was an effective, comfortable, and safe procedure to accelerate tooth movement (1.6 times in accordance with this study). It significantly shorten the duration of orthodontic treatment.

Objective: To evaluate and compare the leptin levels in gingival crevicular fluid (GCF) and rate of canine retraction using an elastomeric chain and nickel–titanium (NiTi) closed coil spring. Design: In vivo, clinical study. Setting: Orthodontic department at ITS Centre for Dental Studies and Research, Ghaziabad, India. Participants: Twenty-seven patients reported to the Department of Orthodontics at ITS Centre for Dental Studies and Research. Methods: Twenty-seven patients were included in the study in which canine retraction was carried out with an elastomeric chain and NiTi closed coil spring on either side of the arch applying 150 g of force on both sides. GCF samples were collected before the commencement of canine retraction, on the first, seventh and 21st day after application of force and were analysed for leptin levels by the ELISA technique. Impressions for the study model were taken at baseline and after 21 days to measure the rate of tooth movement bilaterally with an electronic digital calliper. Results: The results obtained within both groups showed a significant decrease in leptin levels from baseline to 21 days ( P = 0.0001). There was no significant difference in leptin levels between both groups, but leptin levels decreased more in the NiTi closed coil spring group. The rate of tooth movement is not significantly different between both groups, but space closure is faster in the NiTi closed coil spring group. Conclusions: Leptin levels in GCF showed a significant decrease during canine retraction using an elastomeric chain and NiTi coil spring; there was no significant difference in the rate of tooth movement between both the groups.

ABSTRACT Objective To investigate the effect of local injection of platelet-rich plasma (PRP) on the rate of orthodontic tooth movement. Materials and Methods Sixteen female patients were randomly allocated in a split-mouth study design to receive PRP injections with CaCl2 activating solution on one side (intervention side) while the other side received CaCl2 injection only (control side). Canine retraction was performed on 0.017 × 0.025-inch stainless steel archwire applying 1.5 N retraction force. PRP and CaCl2 injections were done at 0, 3, and 6 weeks. The duration of the study was 4 months. Data were collected from digitized models. Assessment of pain accompanying the procedure was done using a visual analogue scale. Results The rate of canine retraction was faster on the intervention side in the first 2 months, with a statistically significant difference in the first month (P = .049). On the other hand, the rate was statistically significantly slower on the intervention side in the third month following cessation of PRP injections (P = .02). Pain increased following injections on both sides. Conclusions PRP showed a positive potential to accelerate the rate of tooth movement when injected in the first 2 months. Repeated injections of PRP to maintain a steady rate of accelerated tooth movement warrant further investigation.

Introduction. Various procedures are available today to enhance tooth movement, with relative success rates, one among them being Micro Osteo Perforation (MOP). Our aim is to assess the rate of tooth movement and interleukin 1β levels in gingival crevicular fluid levels (GCF)after MOPs. Methodology. A group of 22 patients were selected, who required first premolars extraction  and were designated for the split mouth study with equal allocation. MOP was performed on the right side, three vertical MOPs were given using 1.2 X 8 mm mini implants with the gap of 2 mm between them and surgical depth of the implant insertion was 5 mm. The left side was used as control. The individual canine retraction was initiated with placement of 150 grams on 19*25 SS wire. The case was evaluated at different timelines for canine retraction and Interleukin 1β levels. Results. There was a significant difference in the rate of canine movement at 45 days (1.42 mm) and end of canine retraction on experimental side(2.61 mm). The GCF levels were raised at T1 and T2 intervals on the side of MOP and were statically significant (p=0.00). Conclusion. MOP is a minimally invasive procedure which accelerates the tooth movement by 20%. The increase in IL 1βlevels indicates a higher rate of chemical interaction on the surgically assisted side. For enhanced efficiency MOPs should be repeated every 6-8 weeks.

ABSTRACT Aim Orthodontic treatment is routinely carried out in patients with the purpose of correcting various forms of dental malocclusions. Retraction of the canines can be achieved either individually or along with incisor. Pentraxin-3 (PTX-3) is regarded as the true independent indicator of disease activity. Hence, we undertook the present study to assess and compare the level of PTX-3 in patients undergoing canine retraction with active tieback and Nickel–Titanium (NiTi) coil spring. Materials and methods The present study included assessment of 25 patients that underwent canine retraction as a part of fixed orthodontic treatment. In the maxillary right and left quadrant, active tieback and NiTi coil spring were used respectively. Gingival crevicular fluid samples were collected 1 hour prior to the commencement of orthodontic canine retraction procedure followed by collection at following time intervals: 1 hour, 1, 7, and 14 days after the start of canine retraction procedure. Enzyme-linked immunosorbent assay kit was used for analysis of the samples as per manufacturer's instructions. All the data were recorded and compiled. All the results were analyzed by Statistical Package for the Social Sciences software. Results Nonsignificant difference was observed in the values of PTX-3 at baseline and 1 hour. While comparing the mean PTX-3 values between different time intervals in both the study groups, significant difference was observed. Conclusion In comparison to the active tieback, NiTi coil exhibited faster space closure rate. Clinical significance In patients undergoing orthodontic treatment, PTX-3 is associated with periodontal remodeling under the effect of orthodontic forces. Background and Objectives The average orthodontic treatment time for extraction therapy is 31 months. One of the main disadvantages of orthodontic treatment is time. Alveolar corticotomies have been used in conjunction with orthodontics to reduce the treatment time by increasing the rate of tooth movement. Concerns about the possible risks of corticotomy procedure have led to the modification of this technique. Germeç et al. reported a case treated by their modified corticotomy technique and noted reduced treatment time without any adverse effects on the periodontium and the vitality of teeth with their new conservative corticotomy technique. This study was undertaken to clinically evaluate the efficacy of the aforesaid technique. Materials and Methods A split-mouth study design was carried out to compare the rate of maxillary canine movement with and without modified corticotomy facilitated orthodontic treatment in 10 patients requiring maxillary first premolar extractions. The modified corticotomy procedure was performed on the maxillary arch unilaterally. The upper arch was immediately activated bilaterally after surgical procedure using equal orthodontic forces for retraction of the maxillary canines. The amount of tooth movement was recorded at an interval of every month till the completion of canine retraction. The rate of canine movement on experimental and control site was compared. The patients were followed for 6 months to check the occurrence of undesired effects such as root resorption, periodontal damage and loss of vitality of teeth on the experimental side. Results Higher mean velocity was observed in canines with modified corticotomy facilitated retraction compared to conventionally retracted canines; with the difference in mean velocity between the two groups was found to be clinically significant as well as statistically significant (P < 0.001). Interpretation and Conclusion The results suggested that modified corticotomy technique serves as an effective and safe way to accelerate orthodontic tooth movement, without adversely affecting the periodontium, root resorption, and the vitality of the teeth, as concluded by clinical and radiographic examination.

ABSTRACT Aim By using the finite element method (FEM), this study aimed to evaluate the effect of different corticotomy formats on the distribution and magnitude of stress on the periodontal ligament (PDL) during retraction of the maxillary canine. Materials and methods A geometric model of the left hemi-jaw was created from computed tomography scan images of a dry human skull and loads were administered during distalization movement of the canine. Three trials were performed: (1) without corticotomy, (2) box-shaped corticotomy and perforations in the cortical bone of the canine (CVC) and (3) CVC and circularshaped corticotomy in the cortical bone of the edentulous space of the first premolar. Results There was no difference in stress distribution among the different corticotomy formats. Conclusion Different corticotomy formats used to accelerate orthodontic tooth movement did not affect stress distribution in the PDL during canine retraction. Clinical significance From a mechanical perspective, the present study showed that the stress distribution on the PDL during canine retraction was similar in all the corticotomy formats. When using the Andrews T2 bracket, the PDL presented the highest levels of stress in the middle third of the PDL, suggesting that the force was near the center of resistance. Also, as bone weakening by corticotomies did not influence stress distribution, the surgical procedure could be simplified to a less aggressive one, focusing more on inflammatory cellular stimulation than on bone resistance. A simpler surgical act could also be performed by most orthodontists in their practices, enhancing postoperative response and reducing patient costs. ABSTRACT Objectives To explore the expression of miR-34a and its effect on expression of matrix metalloproteinases (MMPs) during orthodontic tooth movement (OTM). Materials and Methods Twenty patients, age 12–18 years old, who underwent orthodontic treatment were enrolled. The expression of miR-34a and MMPs (MMP-1, MMP-2, MMP-3, MMP-8, MMP-9, and MMP-14) were detected in gingival crevicular fluid by enzyme-linked immunosorbent assay (ELISA) and polymerase chain reaction at different time points. The miR-34a mimics or inhibitors were transfected into human periodontal ligament (hPDL) cells, and the MMP expression was measured by ELISA. Results The miR-34 expression in GCF on both the tension and pressure sides after orthodontic treatment were significantly downregulated, while the levels of MMPs were significantly upregulated compared with baseline level. The levels of miR-34 and MMPs returned to baseline level 3 months after orthodontic treatment. The expression of miR-34 was negatively correlated with the expression of MMP-2, MMP-9, and MMP-14. After transfection with miR-34, the MMP-2, MMP-9, and MMP-14 expression by hPDL cells were significantly downregulated compared with miR-control and miR-34 inhibitor. Conclusions Downregulated miR-34 expression was positively correlated with MMP-2, MMP-9, and MMP-14 expression. The miR-34a transfection into hPDL cells inhibited expression of MMPs. The results suggest that miR-34a is involved in expression of MMPs during OTM.

Abstract Objective: To investigate the effects of application of vibratory stimuli, using an electric toothbrush, on the rate of orthodontic tooth movement during maxillary canine retraction. Methods: A split-mouth study was conducted in 28 subjects (mean age = 20.8 years; ranging from 18 to 24 years) whose bilateral maxillary first premolars were extracted with subsequent canine retraction. On the Vibration side, light force (100 g) was applied to the canine for 90 days, in combination with vibratory stimuli provided by an electric toothbrush; only orthodontic force was applied to the canine on the non-vibration side. Amount of canine movement was measured monthly. Related to electronic toothbrush usage, a diary was provided to each patient for recording discomfort during experimental period, having 100-mm visual analogue scale (VAS). The paired t-test was used to assess the differences in amount of tooth movement between canines of the vibration and non-vibration sides. Results: The amount of tooth movement was similar for canines on the vibration side and on the non-vibration side (mean 0.81 ± 0.10 mm and 0.82 ± 0.11 mm, respectively, p> 0.05). Plaque accumulation was minimal in any subject throughout the study. No subject reported discomfort as a result of using the electric toothbrush. Conclusions: This study demonstrates that application of vibratory stimuli using an electric toothbrush, in combination with light orthodontic force, do not accelerate orthodontic tooth movement.
