Background: Currently there is a new world health crisis which is urgent due to the spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), a novel coronavirus. The elderly can be presented as a group of patients at high risk for developing covid-19 with progressive and rapid clinical deterioration including neurological manifestations. Many reports have emerged showing that SARS-CoV-2 infection has an impact on neurological function, and even causes serious neurological damage. Purposes: to investigate the possible correlation between aging process to the severity of neurological deterioration experienced in COVID-19 infection specifically by elderly people. Methods: literature searching was conducted through PubMed, MedLine and Embase. Results: Regardless of age, the infection of SARS-CoV-2 to both central and peripheral nervous system could lead into a range of neurological deteriorations, in which also ranging in forms and severity. Conclusion: In elderly patients, despite of comorbidities, the natural degeneration process is likely being a risk factor of more severe neurological symptoms of Covid-19 infection.

AbstractAccording to the World Health Organization (WHO), viral diseases continue to rise, and pose a significant public health problem. Novel coronavirus disease (COVID-19) is an infectious disease caused by SARS-CoV-2. The pathogenesis and clinical manifestations of COVID-19 is close to Amraz-e-Wabai (epidemic diseases) which was described by Hippocrates, Galen, Aristotle, Razes, Haly Abbas, Avicenna, Jurjani etc. Presently, there is no specific or challenging treatment available for COVID-19. Renowned Unani Scholars recommended during epidemic situation to stay at home, and fumigate the shelters with aromatics herbs like Ood kham (Aquilaria agallocha Roxb.), Kundur (Boswellia serrata Roxb), Kafoor (Cinnamomum camphora L.), Sandal (Santalum album L), Hing (Ferula foetida L.) etc. Use of specific Unani formulations are claimed effective for the management of such epidemic or pandemic situation like antidotes (Tiryaqe Wabai, Tiryaqe Arba, Tiryaqe Azam, Gile Armani), Herbal Decoction (Joshandah), along with Sharbate Khaksi, Habbe Bukhar, Sharbate Zanjabeel, Khamira Marwareed, Jawarish Jalinus, and Sirka (vinegar). Such drugs are claimed for use as antioxidant, immunomodulatory, cardiotonic, and general tonic actions. The study enumerates the literature regarding management of epidemics in Unani medicine and attempts to look the same in the perspective of COVID-19 prevention and management.

The coronavirus disease 2019 (COVID-19) pandemic is a general health crisis and has irreversible impacts on human societies. Globally, all people are at risk of being exposed to the novel coronavirus through transmission of airborne bioaerosols. Public health actions, such as wearing a mask, are highly recommended to reduce the transmission of infectious diseases. The appropriate use of masks is necessary for effectively preventing the transmission of airborne bioaerosols. The World Health Organization (WHO) suggests washing fabric masks or throwing away disposable masks after they are used. However, people often use masks more than once without washing or disposing them. The prolonged use of a single mask might—as a result of the user habitually touching the mask—promote the spread of pathogens from airborne bioaerosols that have accumulated on the mask. Therefore, it is necessary to evaluate how long the living components of bioaerosols can be viable on the masks. Here, we evaluated the viability of airborne Bacillus subtilis (B. subtilis) in bioaerosols filtered on woven and anti-droplet (non-woven) face masks. As a simulation of being simultaneously exposed to sand dust and bioaerosols, the viability rates of bioaerosols that had accumulated on masks were also tested against fine dust and airborne droplets containing bacteria. The bioaerosols survived on the masks immediately after the masks were used to filter the bioaerosols, and the bacteria significantly proliferated after one day of storage. Thereafter, the number of viable cells in the filtered bioaerosols gradually decreased over time, and the viability of B. subtilis in bioaerosols on the masks varied, depending on the mask material used (woven or non-woven). Despite the reduction in viability, bioaerosols containing living components were still found in both woven and anti-droplet masks even after six days of storage and it took nine days not to have found them on masks. The number of viable cells in bioaerosols on masks significantly decreased upon exposure of the masks to fine dust. The results of this study should provide useful information on how to appropriately use masks to increase their duration of effectiveness against bioaerosols.

Background: According to the World Health Organization (WHO), an elderly person is someone who has entered the age of 60 years and above whose percentage tends to increase along with decreasing morbidity of the elderly. Geriatric Neurology itself is the study of a group of neurological diseases in the elderly. Based on 2012 data, the proportion of elderly population in Indonesia is 7.59%. This is accompanied by an increase in the dependency ratio of the productive age population to the non-productive age population. The results of the 2013 Basic Health Research showed an increase in the prevalence of neurological disease in the elderly which was the main cause of death in 15 regencies in 2011. It is to find out the 5 most neurological diseases in the elderly, clinical characteristics and outcomes of geriatric neurology patients treated at the Central Hospital of Sanglah.Methods: It is a retrospective study using no control by looking at medical records in geriatric neurology patients aged 60 years and over who visited the Central Hospital of Sanglah in the March to May 2019 period.Results: A total of 200 patients consisted of 111 men (55.5%) and 89 women (44.5%) who were divided into 5 most geriatric neurology diseases which included 1. ischemic stroke, 2. bleeding stroke, 3. brain tumor, 4. injury head, and 5. epilepsy. The number of deaths was 16, of which 7 cases were caused by sepsis and pneumonia.Conclusions: Stroke, tumors, head injuries and epilepsy are the dominant neurological diseases found. The focus of care and supervision of elderly neurology patients is expected to be further enhanced to prevent side effects that will extend the patient’s treatment period in the hospital which will automatically increase the burden on the hospital itself.

Introduction: The clinical manifestations of chronic subdural hematoma (CSDH) are often confused with other medical entities in the elderly, making their early diagnosis difficult or difficult. Early diagnosis is important, since its prognosis is directly associated with the preoperative neurological state, thus resulting in a worse vital and functional prognosis. Objectives: Report through a literature review the clinical manifestations of CSDH in the elderly population. Methods: Literature review, with the search terms: “Signs and Symptoms”, “Chronic Subdural Hematoma”, Aged, Diagnosis and Prognosis. In which PubMed, Lilacs, Scielo, Cochrane and TripDataBase data platforms were used. The inclusion criteria were: original studies published in any language. Articles in which full reading was prevented were excluded. With the application of the inclusion and exclusion criteria, 110 articles were included in the study. Results: Clinical presentation depends on the location, volume of the hematoma, rapid growth, the location of the CSDH, whether unilateral or bilateral, and the clinical conditions of the patient. Because the forms of clinical presentation of CSDH are variable, it is necessary that health professionals linked to the elderly (geriatrician, psychiatrist, general practitioner) have knowledge of this clinical entity. Conclusions: The recognition of classic forms as well as the identification of risk factors in the elderly favors the timely diagnosis and treatment of CSDH in the elderly population.

The World Health Organization has declared that the spread of tuberculosis is a global emergency. Despite the implementation of strong tuberculosis-control initiatives by WHO, this highly infectious disease continues to affect all vulnerable populations, including the elderly population. Adverse social factors and poor living conditions also affect the elderly much more than the young. The clinical characteristics of tuberculosis in older adults can be unusual and may be confused with age-related illnesses. Various factors related to old age can also cause complications in the diagnosis, treatment, and disease outcomes for tuberculosis patients. The contributory factors may be poor memory, deafness, mental confusion, or impairment of speech. In addition, therapy for tuberculosis in elderly individuals is challenging because of the increased incidence of adverse drug reactions. Hence, understanding the impact of these substantial aspects will help to overcome the problem of tuberculosis in the elderly population.

Health crisis situations generate greater attention and dependence on reliable and truthful information from citizens, especially from those organisations that represent authority on the subject, such as the World Health Organization (WHO). In times of global pandemics such as COVID-19, the WHO message “health for all” takes on great communicative importance, especially from the point of view of the prevention of the disease and recommendations for action. Therefore, any communication must be understandable and accessible by all types of people, regardless of their technology, language, culture or disability (physical or mental), according to the World Wide Web Consortium (W3C), taking on special relevance for public health content. This study analysed whether the WHO is accessible in its digital version for all groups of citizens according to the widely accepted standards in the field of the Internet. The conclusion reached was that not all the information is accessible in accordance with the Web Content Accessibility Guidelines 2.1, which implies that there are groups that are, to some extent, left out, especially affecting the elderly. This study can contribute to the development of proposals and suggest ways in which to improve the accessibility of health content to groups especially vulnerable in this pandemic.

Novel coronavirus disease 2019 (COVID-19) is a respiratory tract infection that was first identified in China. Since its emergence in December 2019, the virus has rapidly spread, transcending geographic barriers. The World Health Organization and the Centers for Disease Control and Prevention have declared COVID-19 as a public health crisis. Data regarding COVID-19 in pregnancy is limited, consisting of case reports and small cohort studies. However, obstetric patients are not immune from the current COVID-19 pandemic, and obstetric care will inevitably be impacted by the current epidemic. As such, clinical protocols and practice on labor and delivery units must adapt to optimize the safety of patients and health care workers and to better conserve health care resources. In this commentary, we provide suggestions to meet these goals without impacting maternal or neonatal outcomes. COVID-19 has spread worldwide causing an unprecedented public health crisis. After the World Health Organization declared a pandemic in March 2020, the number of confirmed cases and deaths has continued to increase. This situation may be prolonged until an effective, tested, and safe treatment is available. COVID-19 can occur at any age. However, the maximum confirmed cases and deaths have occurred among the elderly. Particularly, the mental and physical health of the elderly aged above 60 and classified as high-risk groups is more vulnerable than other age groups, requiring more attention. Strong social restraint, social distancing, and quarantine measures to prevent the COVID-19 spread have raised concerns about their mental health. Therefore, it is crucial to analyze and identify the psychological concepts and protective factors that support and constitute these guidelines and strategies and prepare practical suggestions and guidelines to protect the mental health of the elderly during COVID-19. These discussions will facilitate a deeper understanding and expansion of these guidelines and strategies. Therefore, this study explores factors—including pandemic-induced stress, self-integration, self-efficacy, and resilience—in order to prepare practical and detailed suggestions and guidelines using studies that considered these factors, including coping with COVID-19-induced stress, social support, and physical activity.

With the increase in aging all over the world, and the elderly population nearly tripling from 524 million (8% of the world's population) in 2010 to 1.5 billion (16% of the world's population) in 2050, we will face new challenges and opportunities in providing healthcare. In 2050, it is estimated that Europe will see an increase of 70% in elderly population aged over 65 years, and 170% in those aged over 80 years (World Health Organization (WHO), 2011). It is vital to respond to the needs of this emerging population and the consequent rise in chronic diseases, especially dementia and mental health disorders, which will overload the healthcare system, as well as raise health and social costs, and demand new policies from national governments (World Health Organization, 2012). We urgently need to know how to organize healthcare for elderly people in the years to come.

Introduction: The emergence of a novel coronavirus, SARS-CoV-2, an etiolating agent of coronavirus disease (COVID-19), has become a pandemic of global concern. Considering the huge number of morbidity and mortality worldwide, the World Health Organization, on 11th March 2020, has announced an unprecedented public health crisis. This virus is a member of plus sense RNA viruses that can show a high rate of mutations. The ongoing multiple mutations in the structural proteins of coronavirus drive viral evolution, enabling them to evade the host immunity and rapidly acquire drug resistance against COVID-19. In the present study, we focused mainly on the prevalence of mutations in the four types of structural proteins like S (spike), E (envelope), M (membrane), and N (nucleocapsid) that are required for the assembly of a complete virion particle. Further, we estimated the antigenicity and allergenicity of these structural proteins to design and develop a potentially good candidate vaccine against SARS-CoV-2. Methods: In the present in silico study, envelope protein was found highly antigenic followed by nucleocapsid, membrane, and spike protein of SARS-CoV-2. Results: Consequently, in this study, we detected 987 mutations from 729 sequences of Asia in October 2020 and compared them with China's 1st Wuhan isolate sequence as a reference. Spike showed the highest mutations with 807 point mutations among the four structural proteins, followed by nucleocapsid with 151 mutations, while envelope showed 19 and membrane only 10 point mutations. Conclusion: Taken together, our study revealed, variation occurring in the structural protein of SARS-CoV-2 might be altering their structure and functions, and envelope protein appears to be a promising vaccine candidate to curb coronavirus infections.
