Increasing requirements of medical aid given to children with infectious-inflammatory diseases of the urinary system stipulate the necessity to improve its quality using evidence-based therapeutic-diagnostic and organization technologies. The aim of the work – to substantiate, develop the improved model of the specialized nephrology care for children with infectious inflammatory diseases of the urinary system at the regional level. The official statistical data have been studied (2006 to 2017); information-analytical and statistical methods have been used. A bacteriological study (2009–2016) of urine samples was carried out for 3089 children (0–17 years old) in the Chernivtsi region. They formed the foundation for substantiation and development of an improved functional-organizational model of the system. In addition to the existing and functionally changed elements contains new elements: regional/inter-regional center of specialized medical aid to children with infectious-inflammatory diseases of the urinary system. Implementation of the elements of the suggested improved model in a part of a rational approach in distribution of functions concerning medical observation of patients at the stages of giving medical aid enabled to make the period of hospitalization of nephrological patients 11,40% shorter and an average period of treatment of patients with infectious-inflammatory diseases of the urinary system 2,93% shorter.

The purpose of the study was to substantiate and develop the improved model of the specialized nephrology care for the children with infectious inflammatory diseases of the urinary system at the regional level. Materials and methods. The official statistical data have been studied (reports on the state of medical care for children in the Chernivtsi region and data from the Center of medical statistics of the Ministry of Healthcare from 2006 to 2017), information-analytical and statistical methods have been used. The modern etiological structure of uropathogens – urinary tract infection pathogens among the children of the Chernivtsi region (2009-2016) was studied. Clinical and laboratory examination of 3,089 children (0-17 years old) was conducted in the region; the regional spectrum of sensitivity to antibacterial drugs was determined among the main groups of urinary tract infection pathogens; their age, gender and administrative-territorial differences among the children's population of the region are analyzed. Results and discussion. During 2012-2017 there was a significant increase (by 23.0%) in the incidence of infectious and inflammatory groups of the urinary system diseases among children of 0-14 years old, while among adolescents there was simultaneously an intensive decrease in indicators (by 40.0%) compared to 8.7% in 2006-2011. The ratio of indicators and their dynamics suggests that the growth of sick adolescents is largely due to the insufficient effectiveness of medical care, while children of 0-14 years old is due to their low and insufficient prevention. The data formed the foundation for substantiation and development of an improved functional-organizational model of the system. In addition to the existing and functionally changed elements the model contains new elements: regional/inter-regional center of specialized medical aid for children with infectious-inflammatory diseases of the urinary system. Conclusion. Implementation of the elements of the suggested improved model in a part of a rational approach in distribution of functions concerning medical observation of patients at the stages of providing medical aid enabled to make the period of hospitalization of nephrological patients by 11.40% shorter and an average period of treatment of patients with infectious-inflammatory diseases of the urinary system by 2.93% shorter. Efficiency of implementation of certain elements of the suggested model with its positive evaluation by independent experts and its compliance with the strategy of branch reforms enables to recommend the improved functional-organization model of providing medical aid for children with infectious-inflammatory diseases of the urinary system at the regional level to be introduced into the health care system of Ukraine

In a child, cystitis (non-specific microbial inflammation of the mucous membrane of the bladder) is considered to be a dangerous disease; the prolongation of the process is usually associated with a delayed diagnosis. The aim of this work was to analyze the health status of the child population of the Chernivtsi region, especially the dynamics of the prevalence and incidence of cystitis. The official statistical data have been studied (reports on the state of medical care for children in the Chernivtsi region and data from the Center of Medical Statistics of the Ministry of Healthcare from 2006 to 2017); information-analytical and statistical methods have been used for the purpose of this study. Attention should be drawn to the significantly high prevalence of cystitis among children aged 15–17 years, especially in the Chernivtsi region as during period I (8.7±0.6 vs. 4.3±0.3 in Ukraine) and II (11.7±1.0 and 5.7±0.4, respectively, per 1000 people). Moreover, over the years, the growth of indicators acquires intensity, while this process is more than twice as pronounced in Chernivtsi. Thus, the growth rate was 65.0% in 2006–2011 and 90.3% in 2012–2017 vs. 27.2% and 32.8% in Ukraine, respectively. The identified data indicate the need to provide specialized care to children with infectious and inflammatory diseases of the urinary system of the Chernivtsi region and the need to improve regional clinical routes of patients with infectious and inflammatory diseases of the urinary system.

The aim: To compare the effectiveness and sensitivity of prenatal and postnatal diagnostics in the diagnosis of congenital malformations of the urinary system in the Slovak Republic. Materials and methods: Data of postnatal sonographic screening of congenital developmental malformations of the urinary system in Slovak Republic including 2017 were identified and updated using a questionnaire survey, 38,496 newborns were involved. Statistical data on the proportion of prenatal diagnosis for the years 1995, 2000, 2005, 2008 and 2013-2016 were provided from the National Register of Congenital Defects. The chi2-test and t-test were applied to assess the sensitivity differences. Results: The study showed a low sensitivity of prenatal diagnosis with its maximum in 2016, reaching 32.3% and a minimum in 2005-2008 (8.0 – 8.4%). The sensitivity of postnatal diagnostics for selected years has always been a stable indicator and reaches 99.6%. Conclusions: Available statistical data confirm that prenatal diagnostics of congenital developmental malformations of the urinary system in the Slovak Republic is not perfect. Our work underlines the importance, or we should rather say inevitability of postnatal ultrasound screening for congenital developmental anomalies in the kidneys.

Annotation. The population of the Kyiv region are losing the largest number of life potential years in Ukraine due to cardiovascular diseases (CVD), which determines the special medico-social significance of CVD in this region. The aim of the work is to determine the modern features of the incidence and prevalence of CVD among the adult population of the Kyiv region. The analysis of the incidence and prevalence of CVD among the adult population of the Kyiv region was carried out in dynamics for 2010–2019 and compared with Ukraine on the basis of statistical data from the Center for Medical Statistics of the Ministry of Health of Ukraine and the Kyiv Regional Center of Medical Statistics using statistical methods of dynamic and territorial analysis. It has been determined that in the Kyiv region more than a million residents suffer from CVD. In 2019 the incidence rate of CVD was 446.2, and the prevalence of CVD was 7114.8 per 10 thousand of the adult population. The epidemic of CVD in the Kyiv region is much more spread than in Ukraine. Myocardial infarctions in the region are diagnosed 1.2 times, and strokes 1.5 times more often than in the country, and the prevalence of ischemic heart disease is 1.5 times higher than in Ukraine. The tendency to increasing the prevalence (by 8.5%) and incidence (by 5.7%) of CVD in the Kyiv region during 2010–2017 and the absence of the possibility of further objective monitoring of the CVD morbidity due to the cancellation of the state registration system of morbidity were determined. Unfavorable dynamic trends in CVD morbidity in Kyiv region and a significant excess of regional indicators of averages in Ukraine indicate the need to improve the system of prevention and medical care for patients with CVD in Kyiv region, on which our further research will be focused.

Aim. To ground the personified approaches when organizing risk-oriented medical observation of animal breeding workers. Materials and methods. The total capillary blood analysis with calculation of percentage of peripheral blood cellular composition was carried out, allergization index (AI) and immunoreactivity index (IRI) was assessed, the total number of microorganisms per 1 m3 of the air in the working zone was determined, the total microbial count (TMC) was calculated. Results. The standard periodic medical examination of workers is sufficient with TMC value less than 500 CFU/m3, absence of chronic infectious inflammatory diseases (CIID), AI value less than 1.08 standard units, IRI value less than 13 standard units. When having a mean level of microbiological risk, CIID detection ranging from 1 to 3, AI value less than 1.08 and IRI value less than13 standard units, it is recommended to add a survey to the previous stage, using special questionnaire (attitude to work, working conditions, fatigability degree, susceptibility to allergy etc.). When having TMC value more than 2500 CFU/m3, CIID detection from 4 to 5, AI value 1.08–1.3 standard units, IRI value 13.1–15.7 standard units, an additional study of the upper airways microflora as well as immune status using tests of the first level should be realized; further recommendations on treatment-and-preventive measures in outpatient conditions should be given; with TMC value more than 5000 CFU/m3, presence of CIID – 6 and more, AI value 1.4–1.5 standard units, IRI value 15.8–18.3 standard units, it is recommended to perform immunological studies using tests of the second level with determination of susceptibility to antimicrobial preparations in hospital conditions of Occupational Pathology Center. Conclusions. The method of personified risk-oriented medical observation of persons, working in animal breeding is offered.

The article presents the results of the research of etiological structure of urinary tract infection among children. Out of the total number of urological and nephrological sick children`s research, it is defined that 283 microorganisms selected from the urine were found. 63.2 % of the total number of children in nephrological department is related to only urological diseases. In recent, there has been an open trend to increase a frequency of diseases of the urinary system. Over the past five years, the number of diseases of the genitourinary system in urban children in Kazakhstan has increased to 65 %.The review shows that inflammatory diseases of kidneys and urinary tract take the second place after acute respiratory diseases of upper breathing tract. In this work, it is established that a causative agent of urinary system infection among children is pathogenic, as well as relative-pathogenic microorganisms. As it is shown, the leading etiological agent of uroinfection is Staphylococcus epidermidis (13.9 %), Escherichia coli (12.2 %), Enterobacter cloacae (7.8 %). In some cases, streptococcus are excreted. Also, there were dominant pathogens of nephroinfection among children such as Escherichia coli (21.1 %), Enterococcus faecalis (9.6 %), Staphylococcus epidermidis (20.1 %). Some types of Candida fungi in the formation of the urinary tract and kidney infection (mostly C.albicans (9.5 %), C.kruseae (8.1 %), C.glabrata (5.6 %) pathogens) are observed in the study of urine in children. During the general research, it was found that the etiological agents of urinary system among children consist of a huge microbiological spectrum, especially among children living in urban areas.

Proteolytic systems of tissue and blood plasma take part in the processes of adaptation and protection of an organism, as well as in development of pathological reactions. We have studied the activity of kallikrein, kallikreinogen, angiotensine transforming enzyme, α1-protease inhibitor, and α2-macroglobulin in healthy children in different age periods, in newborns with hypoxic-ischemic encephalopathy, in children with infectious-inflammatory diseases of the urinary system, obesity, and metabolic syndrome.The activity of the kallikrein-kinin system was increased in all the groups against the background of the decreased inhibitory activity of blood plasma of different intensity, which can be used for the prediction of the course of diseases.
