Work of the clinical expert department of the ophthalmosurgical clinic in the context of a pandemic of the novel coronavirus disease

Changes in work of the clinical expert department of the Khabarovsk branch of the S. Fyodorov Eye Microsurgery Federal State Institution (the Khabarovsk branch) in the context of a pandemic of the novel coronavirus disease were analyzed in the article. The difficulties that have appeared in all sections of the work performed are described, both medical and expert: the time for checking the quality of filling out medical documentation has increased due to the identification of a larger number of defects; the time for medical control and discharge of patients from the hospital has increased due to the impossibility of accumulating patients in one place and the need to maintain social distance; difficulties arose in working with experts from insurance companies due to the lack of direct communication (medical records are checked outside the Khabarovsk branch). The concern of people about the possibility of carrying out surgical treatment in conditions of coronavirus infection was expressed in an increase in the information load of the «Question - Answer» section on the website of the Khabarovsk branch and, accordingly, on the doctors in the clinical expert department working with it. The aim of the study was to research the hyperbaric oxygenation effect on the condition of patients in the course of complex treatment aimed at rehabilitation after suffering from COVID-19 pneumonia. Results: the impact of hyperbaric oxygenation on the body of patients who have suffered from the novel coronavirus infection provides a positive dynamics and improves their quality of life. Conclusion: in the process of rehabilitating patients who have undergone the new coronavirus infection, it is necessary to use various physiotherapeutic measures, including hyperbaric oxygenation.

Purpose. Analysis of organization and results of medical work in the Khabarovsk branch of the S. Fyodorov Eye Microsurgery Federal State Institution (Khabarovsk Branch) in the context of a pandemic of the novel coronavirus disease (COVID-19) in 2020. Material and methods. The analysis of regulatory documents adopted in the Russian Federation, Khabarovsk Krai, and orders of the Director of the Khabarovsk Branch to ensure epidemiological wellbeing in organization of treatment process was carried out. Results. The developed complex of anti-epidemic measures to ensure epidemiological wellbeing at provision of emergency and planned ophthalmological care to residents of the Far Eastern Federal District of Russia made it possible in 100% of cases to fulfill the state task for the provision of high-tech medical care. The total volume of medical care for ophthalmological diseases in the Khabarovsk Branch as a whole decreased by 18,9%. It should be noted that the same trend was observed in all branches of the S. Fyodorov Eye Microsurgery Federal State Institution. Conclusion. Despite the problems that COVID-19 brought to work of the Khabarovsk Branch, it still fulfilled its mission, as defined by S. Fyodorov - to bring light and goodness to people living in the Far East of Russia. The first experience of organizing specialized care for patients with the novel coronavirus infection COVID-19 on the basis of the leading multidisciplinary hospital of the Ministry of Defense of Russia FSBI 3rd Central Military Clinical Hospital named after A. A. Vishnevsky. Aims: to scientifically substantiate and develop organizational and methodological recommendations for re-profiling a multidisciplinary hospital to create a multifunctional medical center for the treatment of the new coronavirus infection COVID-19. Based on the regulatory and organizational and methodological documents (Orders and Resolutions of the Government of the Russian Federation, orders of the Ministry of Health of the Russian Federation, directives and orders of the Main Military Medical Directorate of the Ministry of Defense of the Russian Federation), a set of measures was taken to create a multifunctional medical center for the treatment of the new coronavirus infection COVID-19. Algorithms for organizational, diagnostic and therapeutic resource-saving technologies, as well as standard operating procedures for assisting patients with the novel coronavirus infection COVID-19, have been developed and tested. Within the framework of the medical association of the Federal State Budgetary Institution 3 CVKG im. A. A. Vishnevsky, the Ministry of Defense of Russia created a system for the provision of specialized medical care to patients with the new coronavirus infection COVID-19 with the established routing and provision of specialized medical care with elements of high-tech medical care, including extracorporeal membrane oxygenation, biological anti-cytokine therapy. Conclusion. The accumulated experience in the work of the newly created center for the treatment of patients with the new coronavirus infection COVID-19 can be used for further medical activities of medical organizations of military and civilian health care.

The novel coronavirus infection (COVID-19) pandemic has become an unprecedented threat to the lives of those affected and has increased the burden on the healthcare system in most countries. The severity of COVID-19 is not always predictable. However, comorbid pathology and advanced age are risk factors for a severe course of the disease with the development of multiple organ failure. When treating COVID-19, clinicians place particular emphasis on nutritional support as part of intensive care. Correction of nutritional status is also important in the rehabilitation of patients with the syndrome after intensive care. The aim of the review was to show the need for a differentiated approach to nutritional support for patients with COVID-19 during treatment and rehabilitation. From more than 200 originally selected literature sources from various databases (Scopus, Web of science, RSCI, etc.), 49 publications were selected for analysis, mostly published over the past 5 years. Earlier articles were used if they were still relevant to clinicians. The review provides methods for assessing the nutritional status of patients with COVID-19. It is emphasized that patients with comorbid pathology and elderly patients often develop malnutrition, which progresses over the disease. The article discusses in detail the issues of correction of malnutrition, depending on the severity of the disease and the stage of treatment. The effectiveness of rehabilitation is enhanced with adequate nutritional support. Current approaches to providing COVID-19 patients with nutrients and energy include a gradation of nutritional support prescription depending on the severity of the disease. Particular attention is paid to the gradual achievement of target protein and energy levels, as well as the preferential use of the enteral method of delivery of food components. Continuity of nutritional status correction at the outpatient, inpatient and rehabilitation stages improves the quality of care for patients with COVID-19.

In December 2019, a unique coronavirus infection, SARS-CoV-2, was first identified in the province of Wuhan in China. Since then, it spread rapidly all over the world and has been responsible for a large number of morbidity and mortality among humans. According to a latest study, Diabetes mellitus, heart diseases, Hypertension etc. are being considered important risk factors for the development of this infection and is also associated with unfavorable outcomes in these patients. There is little evidence concerning the trail back of these patients possibly because of a small number of participants and people who experienced primary composite outcomes (such as admission in the ICU, usage of machine-driven ventilation or even fatality of these patients). Until now, there are no academic findings that have proven independent prognostic value of diabetes on death in the novel Coronavirus patients. However, there are several conjectures linking Diabetes with the impact as well as progression of COVID-19 in these patients. The aim of this review is to acknowledge about the association amongst Diabetes and the novel Coronavirus and the result of the infection in such patients.

<b><i>Background:</i></b> Pediatric endocrine practices had to rapidly transition to telemedicine care at the onset of the novel coronavirus disease 2019 (COVID-19) pandemic. For many, it was an abrupt introduction to providing virtual healthcare, with concerns related to quality of patient care, patient privacy, productivity, and compensation, as workflows had to change. <b><i>Summary:</i></b> The review summarizes the common adaptations for telemedicine during the pandemic with respect to the practice of pediatric endocrinology and discusses the benefits and potential barriers to telemedicine. <b><i>Key Messages:</i></b> With adjustments to practice, telemedicine has allowed providers to deliver care to their patients during the COVID-19 pandemic. The broader implementation of telemedicine in pediatric endocrinology practice has the potential for expanding patient access. Research assessing the impact of telemedicine on patient care outcomes in those with pediatric endocrinology conditions will be necessary to justify its continued use beyond the COVID-19 pandemic.

A review article is based on current foreign sources. The level of cytokines in the peripheral blood can be increased in many diseases, but in some cases there may be an excess of their normal concentration in tens, hundreds or more times with the development of a peculiar clinical picture, which is based on a systemic inflammatory reaction. In the literature this condition has received the figurative name «cytokine storm», which highlights an extremely violent reaction of the immune system with an unknown (often unfavorable) outcome. Close attention of the scientific world and the public to the problem of extremely high levels of cytokines in the peripheral blood (hypercytokinemia) was drawn due to the high frequency of the cytokine storm in the novel coronavirus infection.

The novel coronavirus infection (SARS-CoV-2), which first appeared in Wuhan, China in December 2019, has been declared a global pandemic by WHO. COVID-19 affects people of all age groups. The disease in children is usually asymptomatic or mild compared to adults, and with a significantly lower death rates. Data on kidney damage in children with COVID-19, as well as the effect of coronavirus infection on the course of diseases of the genitourinary system, are limited, the risks of contracting a new coronavirus infection in children with significant health problems, including those with chronic kidney disease, remain uncertain. The pandemic has affected the activities of surgeons treating diseases of the urinary system in children. Since the prospects for the end of the pandemic are vague, it is necessary to formulate criteria for selecting patients who can and should be provided with routine care in the pandemic. The purpose of this review is to highlight the features of the clinical manifestations and treatment of children with COVID-19, occurring against the background of previous renal pathology or complicating its course.

In late 2019, a new viral infection appeared in China, which spread around the world, causing a pandemic. The causative agent of the new coronavirus infection COVID-19 is the SARS-CoV-2 coronavirus. The review presents modern data on the epidemiology, pathogenesis and course of the novel coronavirus infection COVID-19 in children. Chinese, American and European scientists have described a variety of cutaneous manifestations in children with COVID-19. The article provides a literature review of the cutaneous manifestations of COVID-19 coronavirus infection in children. During our own observation of 301 patients with coronavirus infection COVID-19 caused by SARS-CoV-2 at the Bashlyaevs Children Hospital in Moscow from May 17 to November 16, 2020, it was revealed that 39 (13 %) patients had skin manifestations. The article presents a classification of skin manifestations characteristic of COVID-19. A brief description of each group is given.
