Optimization of complex therapy of inflammatory diseases of women pelvic organs

Inflammatory diseases of women pelvic organs in reproductive period reach 60% among outpatients and 30% among inpatient gynecological patients. Aim. The aim of the study was to evaluate therapy effectiveness of women pelvic organs deseases with Galavit as part of combination therapy compared to standard treatment. Materials and methods. The study included 120 patients with diagnosis of inflammatory diseases of pelvic organs, randomized to 2 equal groups (n=60). In the main group, the patient in complex therapy received Galavit, according to the scheme: for 5 days, 100 mg intramuscularly 1 time per day, then 100 mg every 72 hours (No. 20); in the control group - only standard treatment. Results. Improvement of clinical condition (reduction of severity, normalization of body temperature, reduction of pain syndrome) in patients of the main group was observed 2 times faster than in patient of control group. Quickly resolve the signs of inflammation by ultrasound in the main group. The number of leukocytes, ESR, LII in the main group normalized faster than in patient of control group. On the background of treatment complications of inflammatory diseases of women pelvic organs was observed in 6 (5%) patients of the main group and 9 (7.5%) patients of the control group; aftercare of patients of the main group with ineffectiveness of therapy was achieved without tubectomy. The duration of treatment in the study group was 8±2.6 days us 10±3.9 days in the control group. Conclusion. The inclusion of Galavit in the complex therapy of inflammatory diseases of women pelvic organs accelerates relief of inflammation symptoms and reduces frequency of complications, which allows to preserve reproductive function. Galavit can be recommended for treatment inflammatory diseases of women pelvic organs.

The article describes the general principles of manual lymphatic drainage correction. The authors evaluated the effectiveness of its application in the complex therapy of recreational rehabilitation of athletes with myofascial pain syndrome (MPS).Objective: to evaluate the effectiveness of general manual lymphatic drainage correction in the treatment of athletes with MPS.Materials and Methods: a total of 30 rugby players were examined. All athletes from the main group received physiotherapy, general lymphatic drainage massage. Patients from the control group underwent the same procedures and general massage without lymphatic drainage correction. The course of therapy included 6 procedures.Results: tensoalgometric parameter values in athletes from the main group increased by more than 5 times after lymphatic drainage correction. In sportsmen from the control group, these parameter values decreased.Conclusion: the application of manual lymphatic drainage contributes to the active restoration of myotatic reflex of physical muscles and regress of pain syndrome in athletes.

Goal of research - the study aims to examine the osteopathic profi le of children with dysarthria and to develop recommendations for osteopathic correction of somatic dysfunctions in 2-3 year old children presenting this pathology.Materials and methods. 30 2-3 year old children with the symptoms of dysarthria took part in the research. All the children were divided into 2 groups: the control group of 15 children received standard treatment, and the experimental group of 15 children received both standard and osteopathic treatment. The dysarthria severity and the osteopathic profi le were evaluated with account of the number of somatic dysfunctions at global, regional, and local levels.Results. The osteopathic correction was shown to have a positive effect on dysarthria severity. The study established a correlation between the dysarthria severity in children and the number of somatic dysfunctions at the local level.Conclusion. The study suggests using osteopathic correction of somatic dysfunctions in the complex therapy of dysarthria in children.

Relevance: Modern endonasal surgery allows to remove polyps from all affected paranasal sinuses, following the principles of minimal invasiveness. Minimal traumatic of surgical intervention gives the best results, accompanied by less progression of the disease. Minimal invasiveness of surgical intervention and its obligatory combination with postoperative medical treatment are common practice in many countries. One of the drugs that actively effect the restoration of the mucous membrane of the nasal cavity and paranasal sinuses in the early postoperative period is Nazomer, which includes sodium hyaluronate and dexpanthenol in saline solution. The purpose of the study is to investigate the effectiveness of the drug Nazomer in patients with polyposis rhinosinusitis after endoscopic polyposynosotomy. Results and discussion: The main group consisted of 30 patients with polyposis rhinosinusitis, who were prescribed Nasomer in addition to standard treatment in the postoperative period. The control group included 30 patients who underwent standard treatment in the postoperative period. As criteria for clinical efficacy, data from endoscopic examination of the nasal cavity and indicators of anterior rhinomatometry, measured by the «Optimus» device, were selected. In the main group, the index of nasal breathing according to rhinomatometry was better than the results of patients in the control group on the 3rd and 5th day of the postoperative period by 26% and 24%, respectively. Conclusion: The use of the drug Nazomer in the postoperative period in patients with polyposis rhinosinusitis contributes to more active restoration of respiratory function of the nasal cavity compared with the control group, according to rhinomatometry, up to 26%. Based on the results obtained, the drug Nazomer is an effective anti-inflammatory and regenerative agent in the postoperative period in patients with polyposis rhinosinusitis.

Introduction. Back pain is not only a high prevalence, but also a costly health problem. That is why the search for new and the optimization of existing methods of dorsopathies treatment acquire an important clinical and medico-social significance. In recent years, the medical community has increased the interest in non-drug methods of treatment, including osteopathy. Previous studies have substantiated the possibility of effective application of osteopathic correction methods in the treatment of patients with dorsopathies. A specific object of the osteopath′s work is somatic dysfunction (SD). And if earlier SD was perceived from the standpoint of purely biomechanical disorders, then in recent years there have been serious changes in understanding the heterogeneity of this state nature. The biomechanical, rhythmogenic and neurodynamic components are conventionally distinguished in the structure of SD. One of the neurodynamic disorders types is the violation of motor stereotypes, the so-called static-dynamic disorders, revealed through special dynamic tests and manifested in the functional impossibility of building of active movement at various levels. At the same time, the generally accepted biomechanical approaches do not allow to completely eliminate violations of the statodynamic stereotype.The goal of research — the study was to research the effectiveness of the osteopathic approach using in combination with kinesitherapy for correction of the statodynamic stereotype violations in patients with dorsopathy at the cervicothoracic level.Materials and methods. A controlled randomized prospective study was conducted on the basis of a private rehabilitation center «Ezramed-Clinic» in Omsk in the period from February 2019 to December 2019. 52 patients with a diagnosis of dorsopathy at the cervicothoracic level were observed. 12 people were knocked out during the study in accordance with the exclusion criteria. As a result, 40 patients participated in the study. Depending on the applied treatment method, the patients were divided by simple randomization into two groups (main and control), each of which consisted of 20 people. Both groups of patients received osteopathic correction three times with a frequency of receptions 1 time in 7–10 days. The main group of patients additionally independently performed daily for 10 minutes a set of exercises aimed to restore the disturbed motor stereotypes (normal synkinesis). The control group of patients additionally independently performed a set of exercise therapy for the cervical spine every day for 10 minutes. All patients, regardless of the group, underwent an osteopathic examination before and after treatment with the formation of an osteopathic conclusion; the severity of pain syndrome and the volume of active movements in the cervical spine were assessed. The severity of the pain syndrome and the range of active movements were assessed before and immediately after treatment, as well as 3 months after the first session.Results. The use of osteopathic correction in conjunction with kinesitherapy (both special exercises and a complex of exercise therapy) in patients with a diagnosis of dorsopathy at the cervicothoracic level leads to a statistically significant increase in the range of motion in the cervical spine in the sagittal and frontal planes (p<0,05). The combination of osteopathic correction together with exercise therapy in patients of the control group led to a statistically more significant increase (p<0,05) in the range of motion in the cervical spine in the frontal plane after treatment compared with the results of the main group. However, 3 months after treatment, the indicators in patients of both groups did not have statistically significant differences.The combination of osteopathic correction in conjunction with kinesiotherapy in the form of special exercises for the motor stereotype correction in the main group of patients with diagnosed dorsopathy at the cervicothoracic level led to a statistically significant (p<0,05) decrease in the intensity of the pain syndrome at the 2nd session.Conclusion. In order to increase the effectiveness of treatment, osteopathic correction of somatic dysfunctions in patients with dorsopathy at the cervicothoracic level can be supplemented with complex kinesitherapy, both in the form of traditional exercise therapy, and in the form of special exercises for correction of altered motor stereotype.

The objective: was to optimize the treatment of patients with post-stress disorders of menstrual function by conducting pathogenetic complex therapy from the perspective of an interdisciplinary approach. Materials and methods. 54 women aged 19-37 years with psychogenic amenorrhea (PA) – the main group – were examined. The control group included 25 practically healthy patients aged 18 to 35 years. Results. The variants of hormonal disorders in this pathology are determined, the analysis of the results of electroencephalography and the study of individual and reactive anxiety in patients with PA are performed. The presence of three pathogenetic variants of hormonal disorders was established, which became the criterion of the proposed differentiated pathogenetic treatment. Conclusion. Features of the pathogenesis of psychogenic amenorrhea, the complexity of neuroendocrine and hormonal disorders indicate the need for an interdisciplinary approach in the examination, treatment and rehabilitation of this category of patients with the involvement of related specialists – a neurologist and psychotherapist. The problem of postpartum pyo-inflammatory diseases requires the introduction of new approaches to its solution and optimization of diagnostic and therapeutic developments. The objective: was to determine the optimal time for manual vacuum aspiration of the uterus in postpartum endometritis, correction of therapeutic tactics and control of patients in the framework of the method of fluorescence spectroscopy. Materials and methods. The main group – 170 women with postpartum pyo-inflammatory diseases, control group – 40 women with uncomplicated course of the postpartum period. Their blood serum was examined using the method of fluorescence spectroscopy, in particular after manual vacuum aspiration of the uterus. Results. Fluorescence spectra were studied for 40 women of control group and 170 women of the main group, among them in 44 (25.8%) – in the dynamics during the treatment after manual vacuum aspiration of the uterine cavity. In 93.2% of the patients in the main group after it an increase in fluorescence intensity and max displacement in the short-wave region were recorded, which was a reliable sign of improving the condition of the patients. Conclusion. The use of the method of fluorescence spectroscopy in postpartum endometritis after manual vacuum aspiration of the uterus significantly helps to improve the monitoring of the process of treatment and, if necessary, to correct it in order to recover the patients in time. Results of randomized prospective study with participation of 94 patients aged from 20 to 70 years with monosegmental lumbar spine lesions are presented. Minimum invasive surgical interventions were performed in 55 patients from the main group. Control group consisted of 39 patients in whom decompressive-stabilizing operations via conventional posteromedian approach with skeletization of posterior segments of vertebral column were performed. Average size of operative wound in open interventions more than 10 times exceeded that size in minimum invasive interventions and made up 484 ± 56 and 36 ± 12 sq.cm, respectively. Mean blood loss was 326.6 ± 278.0 ml in the main group and 855.1 ± 512.0 ml in the comparative one. In the main group no one patient required substitution hemotransfusion, while in 13 patients from the comparative group donor erythrocytic mass and/or fresh-frozen plasma were used to eliminate the deficit of blood components. Intensity of pain syndrome in the zone of surgical intervention by visual analog scale in the main group was lower than in comparative group. In the main and comparative groups the duration of hospitalization made up 6.1 ± 2.7 and 9.7 ± 3.7 bed days, respectively. In no one patient from the main group complications in the zone of operative wound were noted. Three patients from the comparative group required secondary debridement and in 1 patient early deep operative wound suppuration was observed. Application of low invasive surgical techniques for the treatment of patients with degenerative lumbar spine lesions enabled to perform radical surgical treatment with minimal iatrogenic injury. The method possessed indubitable advantages over the conventional open operations especially intraoperatively and in early postoperative period.

Chronic apical periodontitis (CAP) is characterized by tissue inflammation around the tooth tip. Unstable outcomes of current treatments against CAP dictate the need for novel therapeutic techniques and medications. The aim of this study was to analyzed the effects of hirudotherapy on the treatment course in patients with CAP. Forty-one study participants aged 25 to 40 years were divided into the main group (20 patients) and the control group (21 patients). Pain level and the gingival index (GI) were measured in all study participants. During the first visit, pain scores did not differ significantly between the control (5.81 ± 0.65) and the main (5.75 ± 0.92) groups. During the second visit, pain was almost unnoticeable in the main group patients (1.05 ± 0.34), whereas pain scores were higher in the control group (4.10 ± 0.7). Our findings suggest a positive effect of hirudotherapy used in combination with standard treatment regimens.

The effectiveness of the original technique of hemorrhoidectomy with lateral ultrasonic dissection in the cutting mode was analyzed from a clinical standpoint and studied according to pathomorphological changes in tissues during its use. A comparative assessment of the immediate results of treatment as well as pathomorphological changes in the tissue of removed hemorrhoids in two groups of patients was performed. The main group included 30 patients in whom the original hemorrhoidectomy technique with lateral ultrasound dissection in the cutting mode was used. The control group consisted of 30 patients who underwent Milligan Morgan hemorrhoidectomy using electrocoagulation. No significant difference was found between these groups in terms of the age and sex structure of patients, as well as the main clinical characteristics of the disease. In a comparative analysis of the immediate treatment results, the intensity of the pain syndrome and the incidence of postoperative complications were lower in the main group than in the control group. In the assessment of pathomorphological changes, the depth of coagulation necrosis and the severity of necrobiotic changes in the underlying layer after hemorrhoidectomy with lateral ultrasonic dissection in the cutting mode were significantly less than those after Milligan Morgan surgery using electrocoagulation. A significant difference was also revealed in the timing of tissue regeneration. The formation of active granulations in the postoperative wound area in the main group was noted on postoperative days 1416, while this process developed no earlier than on postoperative day 20 in the control group. As a result, complete epithelialization of wounds after hemorrhoidectomy with lateral ultrasound dissection in the cutting mode occurred much earlier, i.e., postoperative days 2630. A similar process after the Milligan Morgan hemorrhoidectomy using monopolar coagulation was completed only on postoperative days 3638. Thus, the use of the original technique of lateral ultrasound dissection provides a more sparing effect on tissues, which is the key to a favorable course of the postoperative period and rapid rehabilitation of patients.

First results of application of cultivated autologous stromal bone marrow cells for the stimulation of distraction bone regenerates' reparative regeneration in 8 patients aged from 3.7 to 16 years with congenital lower extremity length discrepancy (main group) are presented. In 24 children with similar pathology aged from 2.5 to 14 years (control group) no cellular technology was used. In the main group duration of treatment reduces to 4.5-6.5 months versus 7.5- 11 months in the control one. That enabled to avoid potential complications related to the long-term fixation of the extremity in the distraction apparatus.
