Objective: To explore the effect of enalapril combined with hydrochlorothiazide and indapamide on hypertension and heart failure. Methods: 80 patients with hypertension and heart failure admitted to our hospital from January 2019 to January 2020 were selected as the research subjects, and they were divided into two groups with random number table method, 40 cases each. The control group was given conventional treatment regimens, including enalapril and hydrochlorothiazide; the observation group replaced hydrochlorothiazide with indapamide based on the above therapies. The efficacy and systolic blood pressure, diastolic blood pressure and left heart ejection fraction (LVEF) of the two groups were compared. Results: After treatment, the effective rate of the observation group was 92.50% (37/40) higher than that of the control group 75.00% (30/40). The systolic and diastolic blood pressure were lower than those of the control group, and the LVEF was higher than that of the control group. The difference was statistically significant (P<0.05). Conclusion: Enalapril combined with indapamide is effective in the treatment of hypertension with heart failure, which can help lower blood pressure, reduce heart load, increase cardiac output, reverse ventricular remodeling, and delay disease progression.

<strong>Objective:</strong> to study a clinical treatment of pulmonary arterial hypertension induced right heart failure. <strong>Method:</strong> A total of 60 cases of pulmonary heart disease right heart failure patients were randomly divided into observation group and control group in which 30 cases for each group. <strong>Results:</strong> In the observation group of 30 cases, 14 cases produce an effect, 15 cases is effective, 2 cases is invalid and the total effective rate is 96.6%. In the control group of 30 cases, 8 cases produce an effect, 17 cases is effective, 5 cases is invalid and the total effective rate is 83.3%. <strong>Conclusion:</strong> Dobutamine can alleviate symptoms of heart failure, reduce heart failure patients’ fatality rate and improve prognosis.

The effects on mild to moderate essential hypertension of 20 mg timolol and 5 mg bendroflumethiazide daily were compared in a randomized double-blind, crossover study in twenty-seven out-patients with pre-treatment diastolic blood pressure ranging from 100 to 130 mm Hg. After a four-week placebo period, a six-week treatment period for each drug followed, with one week on placebo before the crossover. All the patients then received a combination tablet containing the two drugs for another six weeks, using the same doses, except in those patients whose diastolic blood pressure had fallen to 90 mm Hg or below with either timolol or bendroflumethiazide alone, in whom the daily doses were halved. The lying diastolic blood pressure in the group of patients receiving timolol before bendroflumethiazide was 108 mm Hg on placebo, 90 mm Hg on timolol, 94 mm Hg on bendroflumethiazide and 86 mm Hg on the combination tablet. In the group of patients receiving bendroflumethiazide before timolol, it was 109 mm Hg on placebo, 95.5 mm Hg on bendroflumethiazide, 94 mm Hg on timolol and 84 mm Hg on the combination tablet. Of the twenty-seven patients, eleven achieved a diastolic blood pressure of 90 mm Hg or below with timolol, ten with bendroflumethiazide and twenty-three with the combination tablet. Two patients could not tolerate timolol, one because of bradycardia and the other because of bronchospasm, but in the rest, tolerance of all treatment regimens was excellent.

Objective: To investigate the effect of Chinese massage combined with warm acupuncture on knee osteoarthritis. Methods: 60 patients with knee osteoarthritis who were treated in our hospital from January 2017 to October 2019 were selected as the research subjects, and they were randomly divided into 2 groups of 30 patients each. The control group was treated with oral western medicine, and the observation group was treated with massage combined with warm acupuncture. The clinical efficacy and WOMAC index were compared between the two groups. Results: The total effective rate of the observation group was higher than that of the control group, and the WOMAC score was lower than that of the control group. The difference was statistically significant(P<0.05). Conclusion: Chinese massage combined with warm acupuncture is effective in treating knee osteoarthritis It can effectively alleviate the symptoms of pain and joint stiffness in patients, and improve joint mobility, which is worth for clinical promotion.

To explore the application of natural convalescent factors combined with exercise intelligence management in blood pressure control of patients with hypertension, 102 patients with hypertension who were admitted from January 2017 to August 2019 were selected as the research subjects. According to the odd-even number method, they were divided into two groups with 51 cases in each group. The control group was treated with natural convalescent factor therapy alone, and the observation group was treated with natural convalescent factor combined with motor intelligence management. The application effects of the two groups were compared. Before sports intelligence management, the levels of systolic blood pressure (SBP) in control group and observation group were (145.45 ± 8.44) mmHg (1 mmHg = 0.133 kPa) and (146.55 ± 8.37) mmHg, respectively; the diastolic blood pressure (DBP) levels of the control group and the observation group were (98.47 ± 3.48) mmHg and (98.94 ± 3.48) mmHg, respectively, with no statistical significance ( P > 0.05 ). After the exercise intelligence management, the SBP levels of the control group and the observation group were (132.76 ± 4.48) mmHg and (130.06 ± 2.48) mmHg, respectively. The DBP levels of the control group and the observation group were (85.48 ± 5.38) mmHg and (83.47 ± 3.35) mmHg, respectively. The difference was statistically significant ( P < 0.05 ). The scores of each index of quality of life in the observation group were higher than those in the control group, and the differences of physical function and psychological/mental scores were significant. The scores of physical function in the two groups before administration were (48.36 ± 1.69) and (48.74 ± 1.62), and the differences were not statistically significant ( P > 0.05 ). After management, the physiological function scores of the two groups were (40.32 ± 1.33) and (32.15 ± 1.54) and the difference was statistically significant ( P < 0.05 ). There were no significant differences in the psychological (30.75 ± 1.26)/mental scores (30.26 ± 1.48) between the two groups before management ( P > 0.05 ), but there were significant differences in the psychological (25.30 ± 1.02)/mental scores (18.76 ± 1.36) between the two groups after management ( P < 0.05 ). The combination of natural convalescent factors and intelligent exercise management can effectively control the blood pressure level and improve the quality of life of patients with hypertension, and the clinical application effect is good.

Objective: To explore the clinical application effects of perioperative nursing intervention in the treatment of patients with cerebrovascular intervention. Methods: 176 patients with cerebrovascular intervention in our hospital from October 2018 to March 2020 were selected and randomly divided into 2 groups, with 88 cases in the control group and 88 cases in the observation group. Comparative analysis of blood pressure and incidence of complications was performed. Results: During the operation, the systolic blood pressure of the observation group increased by (11.85±1.66) mmHg, and the diastolic blood pressure was (4.63±0.45) mmHg. The control group patients’ systolic blood pressure increased by (22.12±1.98) mmHg and the diastolic blood pressure was (8.36±3.69) mmHg, the two results were significantly different. The blood pressure of the patients in the control group fluctuated more; and the incidence of complications in the observation group is lower than that of the control group. The observation group’s incidence of hematoma, low back discomfort, dysuria, and insomnia and dreaminess was 2.27%, 2.27%, 1.14%, 0%, respectively, with total incidence of 5.68%; in the control group, the incidence of hematoma, low back discomfort, dysuria, insomnia and dreaminess were 6.81%, 5.68%, 3.41%, 4.55% respectively, total incidence being 20.45%. The difference between observation group and control group is obvious. Conclusion: Through perioperative nursing intervention, the recovery speed of patients can be improved, and the incidence of postoperative complications can be reduced. The mental state of the patient before the operation was adjusted, the emotions were calmed, the patient's compliance was improved, and the patient's resistive emotions were reduced. At the same time, if patients can get good nursing care after surgery, the probability of postoperative complications is reduced, so that patients have better results in surgery, and its clinical application is worthy of promotion.

The population in this study wereelderly aged over> 60 years at 55 UPT Panti Wredha Budhi Dharma Yogyakarta. The method oftaking in this study uses total sampling. The sample in this study were 55 elderly. Data analysis wasused to determine the difference in blood pressure before and after laughing therapy with a ratio datascale that was using Pairs T-TestResult: There is an effect of laughing therapy on blood pressure in the elderly at UPT Panti WredhaBudhi Dharma Yogyakarta marked with a significant value of 0.000 (P <0.05)

The aim of the study was to determine the effectiveness of giving hydrotherapi foot soak in hypertensive patients. The study was conducted with one group pretestposttest design without a control group by measuring blood pressure (pretest) before being given an intervention in the form of foot soak hydrotherm against 15 people with hypertension in Banjar Sri Mandala, Dauhwaru Village, Jembrana Subregency. Then do the blood pressure measurement again (posttest) after finishing the intervention. After the study was completed, the results showed that before the hydrotherapi foot bath intervention, there were 0% of patients classified as normal, 13.32% in prehypertension category, 60.08% in hypertension category I and 26.60% in hypertension category II. After the intervention was given, there were 13.32% of the patients classified as normal, 66.68% in the prehypertension category, 20.00% in the first category of hypertension and no patients belonging to the second grade hypertension category. There is a decrease of 20-30 mmHg for systolic blood pressure and 0-10 mmHg for diastolic blood pressure after intervention. Based on the results of the study it can be concluded that the hydrotherapi foot bath is effectively used to reduce blood pressure in hypertensive patients in the Banjar Sri Mandala, Dauhwaru Village, Jembrana District.

Background: Cardiovascular diseases are the leading cause of death in patients with chronic renal failure. When a patient undergoes dialysis, making AVF or AVG causes cardiovascular events. Understanding the relationship between complications: hypertension, heart failure, AVF or AVG (formation time, position, diameter) helps us monitor, detect, prevent and treatment of complications to limit the risk of death in patients with dialysis. Objective: Relationship between cardiovascular diseases and anatomosis of arteriovenous fistular in patients with regularly hemodialysis at Cho Ray Hospital. Methods: A cross-sectional study was conducted at Cho Ray Hospital from 2015 to 2016. The survey some cardiovascular diseases are done by clinical examination, tests for diagnostic imaging such as X-ray, electrocardiogram and echocardiogram: heart and diameter of anastomosis AVF, AVG. Results: The study population included 303 patients with chronic renal failure who were dialysis. Of which, patients aged 25-45 accounted for the highest proportion (43.9%). The proportion of male and female patients was similar (48.5% and 51.5% respectively). The mean value of systolic blood pressure on patients made AVF, AVG less than 12 months is higher than patients made AVF, AVG over 12 months, and there is negative correlation (r = -0.43) between AVF, AVG and systolic blood pressure (p <0.05). The mean value of diastolic blood pressure on patients made AVF, AVG less than 12 months is lower than patients made AVF, AVG over 12 months, and and there is positive correlation (r = -0.43) between AVF, AVG and diastolic blood pressure (p <0.05) (p <0.05). The prevalence of patients with heart failure made AVF, AVG over 12 months is higher than that of the under 12 months group, there is a negative correlation (r = - 0.43) between AVF, AVG diameter and EF index (p <0.05). Conclusion: It is important to note the diameter of anastomosis AVF, AVG in patients with chronic renal failure dialysis to limit cardiovascular complications, especially heart failure. Oxidative stress (OS) is increased in chronic diseases, including cardiovascular (CV), but there are few data on its effects on the heart and vessels. The isoprostanes (IsoP) are bioactive compounds, with 8-iso-PGF25a being the most representative in vivo marker of OS. They correlate with the severity of heart failure (HF), but because data regarding OS levels in different types of HF are scarce, our study was aimed to evaluate it by assessing the urinary levels of 8-iso-PGF2aand its correlations with various biomarkers and parameters. Our prospective study included 53 consecutive patients with HF secondary to ischemic heart disease or dilative cardiomyopathy, divided according to the type of HF (acute, chronic decompensated or chronic compensated HF). The control group included 13 hypertensive patients, effectively treated. They underwent clinical, laboratory - serum NT-proBNP, creatinine, uric acid, lipids, C reactive protein (CRP) and urinary 8-iso-PGF2a and echocardiographic assessment. HF patients, regardless the type of HF, had higher 8-iso-PGF2a than controls (267.32pg/�mol vs. 19.82pg/�mol, p[0.001). The IsoP level was directly correlated with ejection fraction (EF) (r=-0.31, p=0.01) and NT-proBNP level (r=0.29, p=0.019). The relative wall thickness (RWT) was negatively correlated with IsoP (r=-0.55, p[0.001). Also 8-iso-PGF25a was higher by 213.59pg/�mol in the eccentric left ventricular (LV) hypertrophy subgroup comparing with the concentric subgroup (p=0.014), and the subgroups with severe mitral regurgitation (MR) and moderate/severe pulmonary hypertension (PAH) had the highest 8-iso-PGF2a levels. Male sex, severe MR, moderate/severe PAH, high LV mass and low RWT values were predictive for high OS level in HF patients.Eccentric cardiac remodeling, MR severity and PAH severity are independent predictors of OS in HF patients.

AbstractObjectivesTo evaluate the serum levels of the serine proteinase inhibitor kallistatin in women with preeclampsia (PE).MethodsThe clinical and laboratory parameters of 55 consecutive women with early-onset PE (EOPE) and 55 consecutive women with late-onset PE (LOPE) were compared with 110 consecutive gestational age (GA)-matched (±1 week) pregnant women with an uncomplicated pregnancy and an appropriate for gestational age fetus.ResultsMean serum kallistatin was significantly lower in women with PE compared to the GA-matched-controls (27.74±8.29 ng/mL vs. 37.86±20.64 ng/mL, p<0.001); in women with EOPE compared to that of women in the control group GA-matched for EOPE (24.85±6.65 ng/mL vs. 33.37±17.46 ng/mL, p=0.002); and in women with LOPE compared to that of women in the control group GA-matched for LOPE (30.87±8.81 ng/mL vs. 42.25±22.67 ng/mL, p=0.002). Mean serum kallistatin was significantly lower in women with EOPE compared to LOPE (24.85±6.65 ng/mL vs. 30.87±8.81 ng/mL, p<0.001). Serum kallistatin had negative correlations with systolic and diastolic blood pressure, creatinine, and positive correlation with GA at sampling and GA at birth.ConclusionsSerum kallistatin levels are decreased in preeclamptic pregnancies compared to the GA-matched-controls. This decrease was also significant in women with EOPE compared to LOPE. Serum kallistatin had negative correlation with systolic and diastolic blood pressure, creatinine and positive correlation with GA at sampling and GA at birth.
