Human dirofilariasis is a parasitic disease, transmissible helminthiasis caused by parasitizing nematodes of the genus Dirofilaria, most often D. repens and D. immitis which occurs with a predominant lesion of the skin (63%), conjunctiva (50%), mucous membranes, subcutaneous tissue and less often – internal organs. The most frequent incidence of dirofilariasis is observed in regions with a warm, humid climate, and at temperatures below 14℃, dirofilariae stop developing. Human cutaneous dirofilariasis on the European continent is most often diagnosed in representatives of southern and eastern Europe. There are confirmed cases of dirofilariasis on the territory of the Republic of Belarus and neighboring countries. Quite often, this disease affects the maxillofacial region. The purpose of the work was to initiate infectious alertness when planning operations for the removal of benign neoplasms in the maxillofacial region and inform medical specialists about the possibility of developing dirofilariasis of the specified localization, its clinical manifestations and objective diagnostic methods. Material and methods. The object of observation was a 34-year-old female patient R. with maxillofacial dirofilariasis. The subject of the study is the medical record of an inpatient patient R. Results and discussion. At the pre-hospital stage, patient R. underwent a general clinical examination. All indicators of the tests were within the age norm. According to the results of radiation examination methods, no data confirming the parasitic etiology of the disease was obtained. Taking into account the presence of a rounded formation about 1.0-1.2 cm in diameter which is slightly displaced relative to the underlying and surrounding soft tissues, the patient was offered surgical treatment. During the operation, a rounded self-moving helminth was extracted from the formation, about 11.0 cm long and 0.15 cm in diameter. After the helminth was identified, the patient was diagnosed with dirofilariasis. This should be taken into account in the diagnostic and therapeutic aspects when working with patients who have tumor processes in the maxillofacial region. Conclusion. The presented clinical observation shows that in the practice of a dental surgeon and a maxillofacial surgeon, it is increasingly possible to deal with the localization of helminth in the maxillofacial region which simulates a neoplasm which must be taken into account when conducting diagnostics, differential diagnosis and preoperative preparation. The results of the described case are aimed at developing infectious alertness when planning surgical interventions to remove benign neoplasms in the maxillofacial region, developing a rational diagnostic scheme and differential diagnosis for patients with suspected dirofilariasis and other parasitic lesions of the maxillofacial region

This article analyzes domestic and foreign sources, regulatory documents of the Russian Federation, in order to determine the etiological factors of oral cancer, assess the statistical data on morbidity and mortality from malignant neoplasms of the maxillofacial region in the Penza region. The results of changing the routing, an interdisciplinary approach to treating patients with malignant neoplasms of the maxillofacial region in the Penza region are presented. The epidemiology, clinical manifestations, main diagnostic methods, routing, treatment and rehabilitation of patients with head and neck oncology are considered. The results of treatment of patients with oral cavity and lower lip cancer using photodynamic therapy (PDT), both alone and in combination with radiation and chemotherapy, are analyzed. The study showed that PDT of tumors of the lower lip and buccal mucosa in mono-mode made allowed achieving 100% efficiency. After the completed treatment, all participants in the experiment were provided with a complex of rehabilitation, including nutritional support, psychological assistance, exercise therapy, training in correct swallowing tactics, percutaneous endoscopic gastrostomy, and a course of oral cavity massage. An interdisciplinary approach to treating malignant neoplasms of the maxillofacial region is the most correct in modern practice.

As life expectancy increases, older people require strict individualization of management and treatment, taking into account all age and pathophysiological characteristics of a particular patient, a more attentive approach to the features of clinical manifestations of diseases, the presence of comorbid pathology. Timely diagnosis and comprehensive treatment of postmenopausal patients with benign ovarian tumors is a debatable issue, since none of the existing diagnostic methods of research in practice provides sensitivity and specificity equal to or at least approaching 100% of the indicator. The progressive aging of the population raises the question of whether it is necessary to operate on menopausal patients with benign ovarian tumors of small size (up to 5 cm), given the low percentage of malignancy of these formations and the high risk of deterioration in the patients quality of life after surgical interventions.

Introduction. Diagnosis of Mortoan neuroma in some cases does not cause much difficulty and is based on typical signs, but in some cases it presents certain difficulties. Therapeutic tactics for this disease involves the use of conservative or surgical treatment.Aim. Study of the results of differential diagnosis of Morton’s neuroma in the practice of an outpatient surgeon.Material and methods. A study of the results of diagnostics and differential diagnostics in 15 patients with chronic foot pain at the age of 28 to 46 years was conducted. Among them, there were 14 women and 1 man. Differential diagnostics for foot pain syndromes allowed us to distinguish three groups of patients. The first group included 7 patients with Morton’s neuroma. The second group included 4 patients with plantar fasciitis and the third group-4 patients with arthritis, synoviitis of the metatarsophalangeal joints. To conduct differential diagnostics, we used anamnesis, physical examination, and data from instrumental diagnostic methods.Results. Diagnosis of Morton’s neuroma was based on the use of standard research methods (anamnesis, data from a physical instrumental examination of the foot). It should be noted the importance of ultrasound examination of the soft tissues of the foot on the plantar surface for the diagnosis of Morton’s neuroma. This allows for differential diagnosis with foot diseases such as plantar fasciitis and metatarsophalangeal joint synovitis, which may show similar symptoms.Conclusion. Morton’s neuroma is not a rare disease of the foot, which in the initial stages of the disease has a blurred clinical picture, similar to other diseases of the foot. This can lead to diagnostic errors, but the use of modern diagnostic methods, including sonographic methods, allows in most cases to establish an accurate diagnosis and choose an appropriate treatment strategy.

Aortic esophageal fistula (AEF) is a rare but extremely life-threatening condition requiring immediate surgical treatment. The mortality rate among such patients may exceed 60 %, including after surgical treatment. Etiological and pathogenetic mechanisms of AEF are complex and various, but in most cases, they are associated with chronic aortic diseases. The second group of etiological factors of AEF includes trauma of esophagus wall by foreign bodies, malignant neoplasms of the esophagus or mediastinum. AEF may also occur as a result of surgical interventions on the aorta and esophagus. The difficulties of early diagnosis are primarily associated with non-specific clinical manifestations of AEF and the lack of unified protocol for the examination of patients with the first-time upper gastrointestinal tract bleeding. The described reasons in some cases can lead to an inaccurate diagnosis, which entails a loss of time. Among the instrumental diagnostic methods, the most informative is the combination of esophagogastroduodenoscopy and computed tomography of the chest with intravenous contrast enhancement. Each of these methods has its advantages and disadvantages, and allows to identify a number of direct and indirect signs of pathological communication between the aorta and the esophagus.

ABSTRACT Objective: to report the characteristics, evolution and outcome of patients with primary umbilical endometriosis. Methods: an observational and descriptive study of patients with primary umbilical endometriosis diagnosed between 2014 and 2017. The clinical variables evaluated were age, clinical picture, lesion characteristics, diagnostic methods, treatment and recurrence. Results: six patients diagnosed with primary umbilical endometriosis aged 28 to 45 years were operated on during the study period. They had lesions ranging from one to 2.5cm in diameter, violet in five patients and erythematous-violaceous in one. The duration of the symptoms until diagnosis ranged from one to three years and in all the cases studied the diagnosis was made through the clinical manifestations and confirmed by histopathological analysis. No case was associated with neoplastic alterations. All patients evaluated had pain and umbilical bleeding in the menstrual period. Conclusion: umbilical endometriosis is an uncommon disease and should be included in the differential diagnosis of women as umbilical nodules. The treatment of choice is the total exeresis of the lesion.

Background. Lipomas are benign neoplasms arising from fat tissue, with an incidence in the upper extremity of 1%-3,8%. There is scarce literature on the outcomes of the treatment of lipomas in this region. The objective of this study was to assess clinical manifestations and outcomes of surgery for upper extremity lipomas. Material and methods. The sample comprised 40 patients, including 26 women (65%) and 14 men (35%), at a mean age of 37 years, with lipomas located in the upper limbs. Treatment outcomes were assessed at a mean of 4.2 years after surgery in 27 patients from this group. Results. Most of the tumours (29 cases, 73%) were located in the forearm and arm, with 11 patients (27%) presenting with lipomas of the metacarpus and wrist. In 35 patients (87%), lesions were located superficially, within subcutaneous tissue, whereas in 5 (13%) they were located more deeply, in the metacarpus and in the forearm muscles. The follow-up assessment was conducted as telephone interviews in 27 patients. No recurrence was noted. Seven patients (26%) complained of mild tenderness of the postoperative scar. Conclusions. 1. Lipomas are moderately frequent benign lesions occurring in the upper limb. 2. Surgical treat­ment is effective and the recurrence rate is very low.

Lymphangiomas have a dysontogenetic origin, that is, they occur during the development of the fetus and are treated as an abnormal development of the endothelium of proliferating lymphatic vessels and manifest clinically, most often immediately after the birth of a child or in infancy. According to data published earlier by the staff of our department, among benign tumors of soft tissues of maxillofacial area, dermoid cysts (32.6%) take first place. The second place is given to hemangiomas (26.0%); lymphangiosis of such localization are quite rare, accounting for only 3% of the total number of tumors of soft tissues of the face and neck, which causes certain features of tactical approaches to choice and scope of diagnostic measures. In general, lymphangioma, a malformation of the lymphatic system, accounts for 9% of all soft tissue germline tumors and is diagnosed immediately after birth in 65-85% of children, with their superficial location. When they are located in the maxillofacial region, aesthetic deficiencies or functional impairments can be quite pronounced. The features of their topographic-anatomical location in deep fiber spaces and in the area of the floor of the oral cavity cause considerable difficulties in the conduct of diagnostics, the choice of tactical operating techniques and methods of treatment, because the postoperative period is accompanied by a high probability of complications. The presented content of the medical history indicates that the diagnosis of deep-seated and bulky lymphangiomas in young children is rather difficult. Especially, when it comes to festering and combined with acute infectious somatic diseases, it can provoke the occurrence of the inflammatory process directly in the lymphangioma. Attention is drawn to the fact that the frequency of diagnostic errors at the prehospital stage is 75%. Basically, the children were sent to hospital with an incorrect diagnosis, although they were under the supervision of district pediatricians for a long time and repeatedly looked around at a pediatric surgeon. The purpose of our study was to study the peculiarities of clinical manifestations and the diagnosis of volumetric, deep-seated lymphangiomis of the maxillofacial focal infiltration in a child of infancy. Routine diagnostic methods, such as needle biopsy and modern computed tomography, make it possible to unify the diagnostic process to a great extent and decide on the optimal option for operative access and the volume of surgical intervention. There are also certain difficulties in the choice of pharmacological drugs and their dosage when it comes to the combination of a diffuse purulent process with acute manifestations of the pathology of infectious origin. This category of children is subject to dynamic observation to exclude the possibility of recurrence of lymphangioma in conditions of its incomplete necrotization with timely involvement of measures.

Rhinitis can be of various etiologies. More often it is an infectious process (bacterial, viral) or clinical manifestations of an immediate allergic reaction. Other factors that provoke the development of rhinitis, for example, irritating substances, a number of drugs, hormonal imbalance, neurovegetative dysfunction, viruses are known. Doctors of various specialties are familiar with allergic rhinitis (AR) in their practice. In a number of cases there can be some difficulties in the differential diagnosis of rhinitis, the correct diagnosis and the appointment of adequate therapy. The COVID-19 epidemic (“coronavirus disease 2019”) has already been imprinted in the global history as an emergency of international importance. Scientific medical communities continue to study the characteristics of this epidemic, which is regarded by WHO as a pandemic. In a short period (from January 2020), there has been accumulated enough information about the etiology, features of the clinical manifestations of this disease, diagnostic methods have been developed, and clinical approaches and treatment regimens continue to be developed. The pandemic coincided with successive pollen seasons of trees, grasses and weeds. At the beginning of the pollen season, doctors were concerned about the possible difficulty in differential diagnosis of allergic rhinitis, manifestations of coronavirus and other viral infections. In the process of monitoring patients, more and more data has been accumulated, indicating the features of the clinical characteristics of COVID-19, influenza, acute respiratory infections, seasonal allergic rhinitis (rhinoconjunctivitis). Undoubtedly, information is still being accumulating and enriching day by day. AR is not only the most common form of rhinitis, but one of the most common diseases in children and adults. It is known about the pronounced negative effect of AR symptoms on the patient’s quality of life: normal daily activity, cognitive functions, mood, sleep. The severity of AR symptoms varies from minimal clinical manifestations to severe. Therapy should be directed towards the general control of AR symptoms. Over the past decades, the concept of phenotyping (definition of a disease by its clinical manifestations) and endotyping (based on the pathobiological mechanisms of the disease) has been developed. The belonging of AR in a particular patient to a certain phenotype plays a key role in choosing the most effective therapy and requires a personalized approach to treatment. The article provides frequently asked questions regarding AR treatment during the COVID-19 pandemic. The role of intranasal corticosteroids (InHCS), which are the most effective drugs in AR therapy, is discussed, due to their pronounced anti-inflammatory effect and influence on all stages of the pathogenesis of the disease. Mometasone furoate nasal spray has the widest indications among all InHCS drugs registered in our country.

The objective: to evaluate the results of planned open surgical interventions (POSI) in pregnant women with primary symptomatic chronic vein disease (PSCVD). Materials and methods. The study included 457 pregnant women operated on a routine basis with PSCVD. The patients underwent ultrasound duplex angioscanning (USDA) of the veins of the lower extremities, inguinal canals, and iliac veins. The severity of the clinical manifestations of PSCVD was evaluated on a VCSS scale. With the help of the circadian visual-analog scale, the dynamics after the operative pain were studied. The effect of POSI on uterine tone and cardiac function of the fetus was investigated using cardiotocography. When forming the results for POSI, data from USDA, the severity of clinical and cosmetic manifestations of PCVD, the need for active prevention of thrombophlebetic, thromboembolic and hemorrhagic complications, minimization of surgical childbirth were taken into account. The results of treatment were studied in 2 groups of patients. Patients of the first group were treated with conservative therapy, the second group – POSI. Parametric indicators, which were presented as mean and standard error M(SD), were used for statistical data processing of the study, and their reliability was estimated using Student’s t-test. The species was determined to be significant at p<0.05. Results. 495 POSI were performed at 28-38 weeks gestation in an obstetric hospital (perinatal center), by a vascular surgeon from the staff of the center, who was familiarized with the peculiarities of working with this contingent of patients. Surgery was performed under local anesthesia in 346 (75.49%) pregnant women at one extremity, in 111 (24.51%) – at two in one session. According to the results of the study, it was noted that in the first group 33.7% of patients had a positive clinical result due to the reduction of pain syndrome, while 67.5% of patients had an increase and spread of varicose transformation in the area of saphenous and/or non-saphenous veins with spread of pathological venous reflux in the distal direction. 78% of patients had the need to administer prophylactic doses of low molecular weight heparins, both during pregnancy and in postpartum period. Out of 126 pregnant women with pronounced varicose veins of the external genital organs and perineum in 36 (28.6%) deliveries were performed by caesarean section. In 9 (3%) patients there was an acute thrombophlebitis of the subcutaneous veins of the lower extremities, which required 5 pregnant women to undergo urgent surgery when the inflammatory process had spread to the middle and above along the femoral vein of the large subcutaneous vein. Pregnant women of the second group with PSCVD on the basis of obstetric hospital (perinatal center) POSI were performed in the organization, which laid the ideology of the FTS program, strictly individual indications for intervention in the optimal terms of pregnancy, multidisciplinary management of patients, due to this in 93% of operated patients regression of clinical manifestations of the disease was noted, whch contributed to the correct and safe delivery of pregnancy with 100% absence of preterm birth, abnormalities in fetal development, pregnancy course, negative impact on obstetric and somatic condition of the pregnant woman. No hemorrhagic, thrombophlebic, thromboembolic complications were noted. Patients in the postoperative period did not require medical support, as during the period of pregnancy, during delivery and in the postpartum period. There were no indications for surgical delivery. 2.4% of patients experienced complications of post-operative wounds in the form of cheese, which had no effect on pregnancy and was eliminated before delivery. In the postoperative period, if necessary, it was recommended to use elastic compression class garments 1–2. Conclusion. POSI made at PSCVD in optimal terms of pregnancy in a specialized obstetric hospital by a vascular surgeon in strictly individual indications is safe, both for the fetus and the pregnant woman. It is promising to further study the results of POSI in pregnant women with PSCVD to introduce it into the arsenal of treatment of this pathology. Dystonia refers to an involuntary, repetitive, sustained, painful and twisting movements of the affected body part. This movement disorder was first described in 1911 by Hermain Oppenheim, and many studies have been conducted to understand the mechanism, the diagnosis and the treatment of dystonia ever since. However, there are still many unexplained aspects of this phenomenon. Dystonia is diagnosed by clinical manifestations, and various classifications are recommended for the diagnosis and the treatment. Anatomic classification, which is based on the muscle groups involved, is the most helpful classification model to plan the course of the treatment. Dystonias can also be classified based on the age of onset and the cause. These dystonic syndromes can be present without an identified etiology or they can be clinical manifestations of a neurodegenerative or neurometabolic disease. In this review we summarized the differential diagnosis, definition, classifications, possible mechanisms and treatment choices of dystonia.
