Objectives: Increasing care needs for the elderly are an important concern for different countries, especially those with an aging population. It is important for health policy making to have knowledge of the factors affecting the use of health services in the elderly to identify the potential problems and develop appropriate interventions for improving utilization and increasing access to health services. This study aims to investigate the barriers and facilitators of the outpatient health service use in the elderly Methods & Materials: In this systematic review, studies in English published from 1996 to 2019 were searched in Web of Science, PubMed and Scopus databases using PRISMA guidelines and related keywords. After eliminating duplicate and irrelevant articles, the quality of remaining articles was evaluated by two evaluators independently, based on STROBE checklist. Narrative synthesis method was used to combine the data Results: Forty-four eligible studies were included for the review. The determinants of the health service use were divided into three categories of predisposing factors (e.g. age, gender, marital status, ethnicity), enabling factors (e.g. income, insurance coverage, education level, employment status, social network, social support), and need factors (e.g. having chronic disease, self-assessed health status, severity of disease, number of diseases, comorbid diseases, physical disability, unhealthy lifestyle). Findings showed that age >80 years, ethnic minority, being unemployed and retired, low educational level, small and limited social network, and physical disability were the barriers to using outpatient health services, while being female, married, having insurance, social support, having a companion during a disease, having children, high income level, and shorter distance to the health care centers were the facilitators of using outpatient health services in the elderly Conclusion: A group of factors are associated with the outpatient health service use by the elderly. These factors include predisposing, enabling, and need-related factors according to Andersen’s behavioral model of health service use. Interventions to increase the use of health services by the elderly should be based on these factors, and should be taken into account by the policymakers to reduce the burden of health services caused by diseases.

The basis for the establishment of integrated health service post for elderly (elderly Posyandu)  was to improve the welfare of the community, especially the elderly. This study aimed to determine the factors that were related to the utilization of  elderly Posyandu in the working area of  Penurunan Public Health Center Bengkulu. This research was conducted in elderly Posyandu in the working area of  Public Health Center Bengkulu in August 2017. This type of research was an analytical survey using a cross sectional design. The population in this study were elderly working area of Penurunan Public Health Center Bengkulu. The sampling technique in this study used proportional random sampling and obtained a sample of 88 elderly people. Data collection in this study used primary data obtained from direct interviews with respondents. Data analysis techniques were carried out by univariate and bivariate analysis with Chi-Square test through the SPSS program. The results were obtained : there were 51 people (58.0%) who did not utilize the elderly posyandu. There were 43 people (48.9%) elderly with good knowledge. There were 45 people (51.1%) elderly who had a Favorable attitude and there were 49 people (55.7%) elderly who received family support. This study showed that there was a significant relationship between knowledge, attitudes, and family support with the utilization of elderly Posyandu in working area of Penurunan Public Health Center Bengkulu. It is expected that the elderly can play an active role in following                  the elderly posyandu activities so that the elderly are more independent and productive and the health status of the elderly can be monitored. Keywords: attitude, elderly posyandu, family support, knowledge,

Sehati Integrated Health Service Center (Posyandu) is one of four Integrated Health Service Center (Posyandu) for the Elderly at the Wonorejo Health Center. During this time, the Integrated Health Service Center (Posyandu) for the Elderly has not been maximized because of inadequate human resources. This study aims to describe the implementation of the operational work plan in Sehati Integrated Health Service Center for Elderly programs in Wonorejo Health Center works area. The type of research was qualitative with a phenomenological approach. The research informants were the chief of the Health Center, senior program person in charge and cadres. Observation, interviews, and documentation are used to collect the data. The techniques to analyze data were data collection, reduction, data presentation, and verification. The results showed that there was no background for the role of the PKK due to the lack of human resources however the person in charge was right and supporting resources namely equipment for the Integrated Health Service Center for Elderly activities were available. There had been no role of the PKK due to lack of human resources, cadres were still less active and there were still elderly people who didn’t come to the Integrated Health Service Center due to lack of motivation from the elderly family are the Conclusion of this research. The person in charge of the implementation of the Integrated Health Service Center for the elderly was appropriate namely the chief of RT and the leader of the Integrated Health Service Center for the elderly cadres. Supporting resources such as the equipment for the activities of the Integrated Health Service Center for the elderly programs are already available at the moment. Keywords: posyandu; elderly; cadres ABSTRAK Puskesmas Wonorejo memiliki 4 Posyandu Lansia, salah satunya adalah Posyandu Sehati. Pelaksanaan Posyandu lansia belum maksimal karena SDM kurang memadai. Penelitian ini bertujuan untuk menggambarkan pelaksanaan rencana kerja operasional program posyandu lansia sehati wilayah kerja Puskesmas Wonorejo Samarinda. Jenis penelitian adalah kualitatif dengan pendekatan fenomenologi. Informan penelitian adalah kepala puskesmas, pemegang program lansia dan kader. Teknik pengumpulan data melalui observasi, wawancara dan dokumentasi. Teknik analisis data yaitu pengumpulan data, reduksi, penyajian data dan verfikasi. Hasil penelitian yaitu latar belakang belum ada peran dari PKK dikarenakan kurangnya SDM, penanggung jawab sudah tepat dan sumber daya pendukung yaitu peralatan untuk kegiatan posyandu lansia sudah tersedia. Kesimpulan belum ada peran dari PKK dikarenakan kurangnya SDM, kader masih kurang aktif dan masih ada lansia yang tidak datang ke posyandu karena kurangnya motivasi dari keluarga lansia tersebut. Penanggung jawab pelaksanaan posyandu lansia sudah tepat yaitu ketua RT dan ketua kader posyandu lansia. Sumber daya pendukung, yaitu peralatan kegiatan program posyandu lansia sehati sudah tersedia. As the trend of aging in rural China has intensified, research on the factors affecting the health of the elderly in rural areas has become a hot issue. However, the conclusions of existing studies are inconsistent and even contradictory, making it difficult to form constructive policies with practical value. To explore the reasons for the inconsistent conclusions drawn by relevant research, in this paper we constructed a meta-regression database based on 65 pieces of relevant literature published in the past 25 years. For more valid samples to reduce publication bias, we also set the statistical significance of social support to the health of the elderly in rural areas as a dependent variable. Finally, combined with multi-dimensional social support and its implications for the health of the elderly, meta-regression analysis was carried out on the results of 171 empirical studies. The results show that (1) subjective support rather than objective support can have a significant impact on the health of the elderly in rural areas, and there is no significant difference between other dimensions of social support and objective support; (2) the health status of the elderly in rural areas in samples involving western regions is more sensitive to social support than that in samples not involving the western regions; (3) among the elderly in rural areas, social support for the older male elderly is more likely to improve their health than that for the younger female elderly; and (4) besides this, both data sources and econometric models greatly affect the heterogeneity of the effect of social support on the health of the elderly in rural areas, but neither the published year nor the journal is significant. Finally, relevant policies and follow-up studies on the impact of social support on the health of the elderly in rural areas are discussed.
