One of the SDGs targets in achieving UHC is increasing access to quality essential health services. Puskesmas is the front line in providing essential health services in the community. However, several complaints were still found in the health services provided, which also decreased public access to the Puskesmas. This study aims to determine the quality of outpatient health services at the UPT Puskesmas Ciputat Timur in 2019. This research is an evaluation study using a descriptive-analytical survey method with a quantitative approach with a cross-sectional study design. The sample determination was based on the Probability Proportionate to Size sampling technique with a total sample of 341 samples according to the sample table from Krejcie and Morgan. Data were collected using a questionnaire with two methods of measuring service quality, namely Servqual and Community Satisfaction Index, and using univariate data analysis. The results of this study stated that outpatient satisfaction at the UPT Puskesmas Ciputat Timur was 2.16 (satisfactory) based on the servqual concept and 76.47 (good) based on the concept of community satisfaction index. This study concludes that the quality of outpatient health services at the UPT Puskesmas Ciputat Timur is good, although several items still need to be improved. Suggestions are needed to make information boards for patient registration requirements, increase discipline in-service time, make persuasive stickers or posters, add RM distribution officers, publish IKM results annually through the website and add waiting room seats for patients and their families.

Patient Visits Outpatient and inpatient insurance at Class C Hospitals is increasing from year to year. Increased visits to insurance patients will have an impact on the inpatient and outpatient health services provided. From the increase in patient visits, the data owned by the hospital is increasingly abundant. The data can be used to explore knowledge, find certain patterns. To explore knowledge about Inpatient and Outpatient Insurance patients, data mining clustering techniques are used with the Self Organizing Map (SOM) algorithm using R Studio tools. Clustering technique with the implementation of the Self Organizing Map (SOM) algorithm is a technique for grouping data based on certain characteristics which are then mapped into areas that resemble map shapes. The CRISP-DM method is used in this study to perform the stages of the data mining process. The results obtained from the implementation of clustering with the Self Organizing Map (SOM) algorithm are obtained 2 clusters representing dense areas and non-congested areas. Dense areas are represented by Internal Medicine Clinic, Surgery Clinic, Eye Clinic, Hemodialysis, Melati Room, Orchid Room, Bougenville Room, Flamboyan Room. Non-crowded areas are represented by General Clinics, Dental Clinics, Obstetrics and Gynecology Clinics, Children's Clinics, Mawar Room and Soka Room

Objectives: Increasing care needs for the elderly are an important concern for different countries, especially those with an aging population. It is important for health policy making to have knowledge of the factors affecting the use of health services in the elderly to identify the potential problems and develop appropriate interventions for improving utilization and increasing access to health services. This study aims to investigate the barriers and facilitators of the outpatient health service use in the elderly Methods & Materials: In this systematic review, studies in English published from 1996 to 2019 were searched in Web of Science, PubMed and Scopus databases using PRISMA guidelines and related keywords. After eliminating duplicate and irrelevant articles, the quality of remaining articles was evaluated by two evaluators independently, based on STROBE checklist. Narrative synthesis method was used to combine the data Results: Forty-four eligible studies were included for the review. The determinants of the health service use were divided into three categories of predisposing factors (e.g. age, gender, marital status, ethnicity), enabling factors (e.g. income, insurance coverage, education level, employment status, social network, social support), and need factors (e.g. having chronic disease, self-assessed health status, severity of disease, number of diseases, comorbid diseases, physical disability, unhealthy lifestyle). Findings showed that age >80 years, ethnic minority, being unemployed and retired, low educational level, small and limited social network, and physical disability were the barriers to using outpatient health services, while being female, married, having insurance, social support, having a companion during a disease, having children, high income level, and shorter distance to the health care centers were the facilitators of using outpatient health services in the elderly Conclusion: A group of factors are associated with the outpatient health service use by the elderly. These factors include predisposing, enabling, and need-related factors according to Andersen’s behavioral model of health service use. Interventions to increase the use of health services by the elderly should be based on these factors, and should be taken into account by the policymakers to reduce the burden of health services caused by diseases.

Abstract Background Patient satisfaction is an essential indicator in medical practise and research. To monitor the health and well-being of adult populations and the ageing process, the World Health Organization (WHO) has initiated the Study on Global AGEing and Adult Health (SAGE), compiling longitudinal information in six countries including China as one major data source. Objective The objective of this study was to identify potential predictors for patient satisfaction based on the 2007–10 WHO SAGE China survey. Methods Data were analysed using random forests (RFs) and ordinal logistic regression models based on 5774 responses to predict overall patient satisfaction on their most recent outpatient health services visit over the last 12 months. Potential predictor variables included access to care, costs of care, quality of care, socio-demographic and health care characteristics and health service features. Increase of the mean-squared error (incMSE) due to variable removal was used to assess relative importance of the model variables for accurately predicting patient satisfaction. Results The survey data suggest low frequency of dissatisfaction with outpatient services in China (1.8%). Self-reported treatment outcome of the respective visit of a care facility demonstrated to be the strongest predictor for patient satisfaction (incMSE +15%), followed by patient-rated communication (incMSE +2.0%), and then income, waiting time, residency and patient age. Individual patient satisfaction in the survey population was predicted with 74% accuracy using either logistic regression or RF. Conclusions Patients’ perceived outcomes of health care visits and patient communication with health care professionals are the most important variables associated with patient satisfaction in outpatient health services settings in China.

Abstract The hospital marketing mix is ​​a set of marketing tools used by hospitals in influencing and knowing the response of service buyers. Dr. Abdoer Rahem Situbondo Regional General Hospital experienced a decrease in the number of patient visits over the past three years from 2014 to 2016, respectively 59,537, 53,720 and 47,699. This shows that patient loyalty is reduced. This study aimed to analyze the marketing mix association with the decision process of choosing outpatient health services at Dr. Abdoer Rahem Situbondo Regional General Hospital. The type of research used is analytic with cross-sectional design. Samples of 94 people were taken by systematic random sampling technique and analyzed using the contingency coefficient test with a confidence level of 95%. The results of this study were that type of employment, product mix, price mix, and promotion mix had weak association with the decision process of choosing outpatient health services. While variables that were not associated with the decision process of choosing outpatient health services were age, gender, education level, monthly income and place mix. The conclusion of this study was that the hospital can focus on improving product mix, price mix, and promotion mix.    Abstrak Bauran pemasaran rumah sakit merupakan seperangkat alat pemasaran yang digunakan rumah sakit dalam mempengaruhi dan mengetahui tanggapan pembeli pelayanan. RSUD dr. Abdoer Rahem mengalami penurunan  jumlah kunjungan pasien lama selama tiga tahun terakhir dari tahun 2014 hingga 2016 secara berturut turut 59.537, 53.720 dan 47.699. Hal ini menunjukkan loyalitas dari pasien berkurang. Penelitian ini bertujuan untuk menganalisis hubungan bauran pemasaran dengan proses keputusan memilih pelayanan kesehatan rawat jalan RSUD dr. Abdoer Rahem Situbondo. Jenis penelitian yang digunakan adalah analitik dengan design cross sectional yang. Sampel sebanyak 94 orang yang diambil dengan teknik systematic random sampling dan dianalisis menggunakan uji koefisien kontingensi dengan tingkat kepercayaan 95%. Hasil penelitian ini adalah ada hubungan yang lemah antara jenis pekerjaan, bauran produk, bauran harga, dan bauran promosi dengan proses keputusan memilih pelayanan kesehatan rawat jalan. Sedangkan variabel yang tidak berhubungan dengan proses keputusan memilih pelayanan kesehatan rawat jalan adalah umur, jenis kelamin, tingkat pendidikan, pendapatan per bulan dan bauran tempat. Kesimpulan penelitian ini adalah pihak rumah sakit dapat berfokus pada pembenahan bauran produk, bauran harga, dan bauran promosi.

In recent years Public hospital services are becoming a burden in many large cities. Many patients are needed to be served, but it was constrained by the number of doctors. Even though the resources are limited, the efficiency of public hospital services is important. The patient’s queues can be shortened by service quality enhancement of outpatient. This study has applied Business Process Reengineering (BPR) method to improve the efficiency of public hospital services. Igrafx software is used to visualize the improvement design. The result has suggested a solution to accelerate the outpatient health services of a public hospital in Jakarta.
