Background: Suicide prevention is a major challenge for penal institutions in many countries. The traditional approach relies on the expertise of health professionals and is supplemented by the intervention of other professionals and the inmates themselves. New methods of suicide prevention based on peer support have been developed in recent years. Peer prevention programs rely on the ability of inmates to identify suicide risk. Aims: This study examines perceived suicide risk among inmates and explores possible explanations. Method: 54 inmates and 17 professionals working in prisons responded to a questionnaire. Results: The peer prevention program was found to change inmates’ expectations of support in the event of a suicide crisis. The study also found that the inmates involved in the program tended to underestimate the risk of suicide. The perception of the prevention program and the level of self-consciousness were found to account for the underestimation of suicide risk. Conclusions: Support for inmates involved in suicide prevention programs must take into account their isolation in prison. The training provided to inmates must also consider the biases affecting the assessment of risk.

Abstract. Background: Information on methods of suicide is available online, and access to information on methods of suicide appears to contribute to a small but significant proportion of suicides. There is limited documentation of how methods of suicide are being profiled, as well as what content exists in other languages than English. Aim: We aimed to analyze and compare how methods of suicide are profiled on Danish and English-language websites. Method: We applied a categorization and content analysis of websites describing methods of suicide. Sites were retrieved by applying widely used Danish and English-language search terms. Results: A total of 136 English-language websites and 106 Danish-language websites were included for analysis. Websites were more often categorized as prevention or support sites, academic or policy sites, and against suicide sites than dedicated suicide sites (i.e., pro-suicide sites), or information sites. However, information on methods of suicide was available, and 20.1% and 8.9% of the English and Danish-language sites, respectively, suggested that a particular method of suicide was quick, easy, painless, or certain to result in death. Limitations: Only one author coded and analyzed all websites. A further operationalization of the content analysis checklist is warranted to increase reliability. Conclusion: The websites primarily had a prevention or anti-suicide focus, but information on methods of suicide was available, requiring an increased focus on how to diminish the negative effects of harmful online content.

Patients who commit suicide often deliberately hide their medical history. Given that taking pesticides is one of the most common methods of suicide, other forms of poisoning may be neglected in clinical practice. We report here a case of mixed-pesticide poisoning. The patient was poisoned by oral administration of a coumarin rodenticide in combination with an intramuscular injection of organophosphorus (OP) pesticide. The patient was treated with vitamin K1, cholinesterase reactivators, atropine, ventilator-assisted ventilation, and bedside debridement. Her condition gradually stabilized and she eventually recovered and was discharged. Assessment of the causes of delayed diagnosis and treatment suggests that we need to improve early detection and treatment of acute poisoning. It is especially important to ask about the patient’s medical history, conduct a careful physical examination, and track the clinical symptoms and differential diagnosis of common poisoning. In addition to the three common routes of poisoning—oral, inhalation, and cutaneous mucosal contact—intramuscular injection of OP can also lead to severe poisoning, which manifests as respiratory failure.

Abstract Background: This is an ecological study that analyzes suicides committed in Montenegro during the 2000-2018 period, taking into account gender, age and methods of suicide. Methods: Suicide rates and trends up until 2009 were obtained from the official registers of Bureau of Statistics of Montenegro (MONSTAT) while the later data were obtained from the Department of Interior’s. MONSTAT also provided data on unemployment and average salary. As per statistical methods, descriptive and correlations were calculated. Results: The average crude suicide rate was 21.06, for males 29.93 and for females 12.42. Crude suicide rates were not associated with unemployment rate or average salary. However, the unemployment rate was significantly correlated with lethal methods of suicide, namely suicide by firearm and by hanging. Average net salary was negatively correlated with suicide by firearm. Conclusions: The ratio of males and females who committed suicide was 2.41. In the last three years, this ratio continues to rise in favor of males (reaching 4.29 in 2018). This could be explained by specific cultural features where males are expected to be the main financial contributors to the households. The labor market of Montenegro does not offer adequate opportunities to set and maintain a stable economic situation which puts additional pressure and stress on males.

Intentional pesticide ingestion is a common method of suicide, accounting for up to one-fifth of all suicides worldwide. The importance of intentional ingestion of pesticides was initially recognized in Asia and the Western Pacific but it is also a problem in Latin America, the Eastern Mediterranean, and in Africa. Organophosphate pesticides are responsible for a large proportion of pesticide self-poisonings and for the majority of deaths from pesticide ingestion. Limiting access to pesticides could substantially reduce the global burden of mortality due to suicide. Despite initial enthusiasm for restricting access to pesticides by promoting safe storage of these compounds, the definitive study on this method in Sri Lanka did not find a significant reduction in suicide rates. There is, however, growing evidence that national or regional bans on high toxicity pesticides lead to falls in pesticide-ingestion suicide rates, and overall, suicide rates without substantial substitution of other methods of suicide.

Abstract. Restriction of access to means for suicidal behavior, especially dangerous methods, is a key element in most national suicide prevention strategies. In this paper the rationale for this is discussed, including the fact that suicidal impulses are often brief, that availability of a method may influence both the occurrence and outcome of a suicidal act, and that if a favored means becomes less available it does not always result in substitution by another method. Examples of evidence for the effectiveness of restricting availability of suicidal methods on subsequent suicidal behavior are presented, plus the supporting findings from studies of long-term survivors of serious suicide attempts in which only a minority have gone on to die in subsequent suicide attempts. Finally, factors likely to determine the effectiveness of modifying access to means for suicide are considered, together with the main elements that need to be addressed in evaluation.

In Canada, Bill C-51 was implemented in 1977 to restrict the use of firearms, providing a good opportunity to study the effects of gun control laws in the use of firearms for suicide. The present study examined the use of guns for suicide during the period prior to the bill and during the period after the passing of Bill C-51 to assess the association of the bill with suicide rates. Analysis showed a significant decreasing trend after passage of Bill C-51 on the firearm suicide rate in Canada and the percentage of suicides using firearms. The analysis supports the position that restricting easy access to lethal methods of suicide may assist in reducing suicide.

Abstract Methods of suicide have received considerable attention in suicide research. The common approach to differentiate methods of suicide is the classification into “violent” versus “non-violent” method. Interestingly, since the proposition of this dichotomous differentiation, no further efforts have been made to question the validity of such a classification of suicides. This study aimed to challenge the traditional separation into “violent” and “non-violent” suicides by generating a cluster analysis with a data-driven, machine learning approach. In a retrospective analysis, data on all officially confirmed suicides (N = 77,894) in Austria between 1970 and 2016 were assessed. Based on a defined distance metric between distributions of suicides over age group and month of the year, a standard hierarchical clustering method was performed with the five most frequent suicide methods. In cluster analysis, poisoning emerged as distinct from all other methods – both in the entire sample as well as in the male subsample. Violent suicides could be further divided into sub-clusters: hanging, shooting, and drowning on the one hand and jumping on the other hand. In the female sample, two different clusters were revealed – hanging and drowning on the one hand and jumping, poisoning, and shooting on the other. Our data-driven results in this large epidemiological study confirmed the traditional dichotomization of suicide methods into “violent” and “non-violent” methods, but on closer inspection “violent methods” can be further divided into sub-clusters and a different cluster pattern could be identified for women, requiring further research to support these refined suicide phenotypes.
