Masalah yang sering terjadi pada PT X ialah produk cacat yang dapat meningkatkan biaya sehingga terjadi pemborosan. Tujuan dari penelitian ini adalah ini adalah untuk membantu perusahaan dalam mencapai standar kualitas yang sudah ditetapkan dan mengurangi pemborosan yang terjadi karena produk cacat. Metode yang digunakan untuk meningkatkan kualitas pada PT X ialah seven tools. Terdapat 7 alat analisis dalam metode seven tools, yaitu : check sheet, scatter diagram, fishbone diagram, pareto diagram, flow chart, histogram, control chart. Flow chart berfungsi untuk memperlihatkan alur produksi pada PT. X sehingga dapat mempermudah proses analisis selanjutnya. Check sheet dan histogram berfungsi untuk menyajikan data lengkap produk cacat berdasarkan jenis cacatnya. Control chart berfungsi untuk memonitor produk cacat yang keluar dari batas kendali yang sudah ditentukan. Scatter diagram berfungsi untuk memperlihatkan apakah beberapa variabel yang diambil memiliki hubungan. Pareto chart berfungsi untuk dapat memperlihatkan jenis produk cacat mana yang paling dominan sehingga dapat dilakukan tindakan terhadap produk cacat yang dominan tersebut. Fish-bone diagram berfungsi untuk membantu mencari faktorfaktor yang menjadi penyebab terjadinya produk caca

Quality is a factor that can increase product competitiveness. In improving products quality is necessary to control the quality (Quality Control) of the process activities. This study aims to determine the quality control of granulated sugar packaging process by using the seven tool method consisting of check sheet, histogram, Pareto chart, flow chart, scatter diagram, control chart and fishbone diagram. From 30 days observation concluded that the highest type of damage consist of metal detection by 145 bags and torn packaging by 50 bags. Then, from the results of control chart known that out of control process still happen. Both types of damage then revised, and process capability analysis is carried out, so can be concluded that the process is not capable in making the product according to specifications. Therefore, an analysis using fishbone diagram shows that the causes of damage are human, working methods, and machines. From these factors, Standard Operating Procedures can be done as well as designing new machines in packaging process

Tulisan ini menyajikan analisis pengendalian mutu No Good (NG) tertinggi produksi bermasalah di industri pembuat battery carbon dengan metode QC seven tools sebagai upaya peningkatan kepuasan konsumen telah dilakukan. Dalam penelitian ini dilakukan contoh kasus penanggulangan NG inside crack di bagian battery carbon PT. I. Penerapan metode QC seven tools, meliputi: (1) check sheet, (2) stratifikasi, (3) pareto diagram, (4) histogram, (5) control chart, (6) fish bone diagram, (7) scatter diagram. Hasil analisis dan usulan kaizen diperoleh: (1) jenis NG inside crack ada di urutan ke-1 (78%) sehingga menjadi masalah utama yang harus segera ditanggulangi, (2) penyebab NG produksi bermasalah adalah karena operator, mesin dan peralatan, cara kerja dan lingkungan kerja, (3) melalui usulan kaizen dengan menurunkan NG menjadi 50% dapat dihemat biaya inside crack sekitar Rp. 2,5 juta/bulan. Bahasan ini diharapkan menjadi contoh kasus sebagai upaya peningkatan kepuasan konsumen agar produk yang dihasilkan memiliki daya saing di pasar global.Kata kunci: pengendalian mutu, tujuh alat QC, battery carbon

This study aims to demonstrate effectively application of the seven tools as one of the quality management tools to reduce product defects. A case study has been conducted in the production of white body at PT. XYZ, which is located in Semarang. The high percentage of defects is considered as one causes of not attaining the target of the company. Seven tools, which consist of check sheet, stratification, histogram, scatter diagram, control chart, Pareto chart, and cause-and-effect diagram, are used to analyze the causes of the defects. Based on data processing results, there are eight types of defects, namely: tobi, hage, burn back, crolling, saya boro, broken back stamp, hama kake, and nama kire. Recommendations for improvement have been proposed using 5W+1H analysis to improve the company's quality control performance and minimize product defects.

The study shows that there are unpredictable factors influencing loan default in small-scale enterprises in Port Loko Municipality. A fishbone diagram which is a cause an effect tool was used to determine these factors. A brainstorming activity was used to get the views of participants with regard to the Research Question. The research question was to respond to a research objective which was “Are there unpredicted factors influencing loan default in small scale enterprises in Port Loko Municipality in Sierra Leone?”. Reviews of necessary literature were done to aid the study. In the review, matters relating to loan default and possible causes were addressed. It is unfolded that there are some loan defaults that are as a result of the borrowers’ lapses and others that are lender-oriented causes. The population size of one hundred and a random sample size of sixty people were used as participants to carry out the brainstorming activity. The population is comprised of small-scale enterprise owners and workers of credit or Microfinance institutions in the Municipality. Brainstorming participants proved that the death of clients or borrowers, internal insecurity, outbreak of diseases (Pandemic), Natural Disasters, and accident all significantly influence loan default of small-scale enterprises.

190 Background: Provider burnout is a challenge adversely affecting the quality, safety, and cost of health care. We measured burnout among pediatric oncology providers in the St. Jude Affiliate network and used a Plan-Do-Study-Act (PDSA) improvement cycle to address one of the factors contributing to burnout. Methods: Within the framework of the American Society of Clinical Oncology (ASCO) Thematic Quality Training Program (QTP) we sent the Mini Z 2.0 Survey to 47 pediatric oncology providers. Applying a fishbone diagram and Pareto chart we analyzed potential causes of provider burnout. Based on the analysis, we used a PDSA approach to address documentation of oral chemotherapy adherence for children with acute lymphoblastic leukemia to mitigate burnout among providers. Results: The burnout survey response rate was 44.6%. Burnout was identified in 42.9% of providers. Documentation in the electronic medical record (EMR) was cited as the second most common contributor to burnout, and it was this issue we chose to address based on a priority matrix. We improved the completeness of oral chemotherapy documentation from a baseline of 13% compliance to 87% compliance within 3 months. The improved compliance was achieved by standardizing the documentation process in the EMR for content and location. Conclusions: The EMR was one of the contributing factors in the burnout survey of the pediatric oncology providers in the St. Jude Affiliate network. A PDSA improvement model to improve clinical research documentation was successful in addressing one of several contributing factors to provider burnout.

In a construction project, there must be construction waste. The present study aims to find out the quantity, the source, and the factors leading to construction waste. The present study aims to find out the quantity, the source, and the factors resulting in construction waste in Gran Sungkono Langoon Project by using Pareto Method and Fish Bone Diagram. This study also aims to categorize the construction waste in Grand Sungkono Langoon Project based on its type by using Pareto Method and Fish Bone diagram. The present study employed descriptive study with qualitative analysis. It was done by analyzing and evaluating construction waste. The result of the study was in the form of data table on construction waste in structural construction using Pareto method and Fish Bone Diagram in Grand Sungkono Langoon Project. The study found that the most significant amount of material waste in Grand Sungkono Langoon project is steel bar (Rp.229.009.000), total amount of construction waste was 0.92% or equal to Rp 263.090.000 from total project cost of Rp. 24.204.280.000. The source of material waste comes from piles, casting, steelwork and formwork, and the factors resulting in construction waste were the condition of the pile, this can be caused by careless unloading process. Besides, not all piles get into the ground for its condition is different at each point. Regarding reinforcement, it turned into waste because of cutting mistake. The category of construction waste in a structural project is casting, steelwork, and formwork.

PT. XYZ adalah perusahaan manufaktur yang menjalankan perusahaan mereka dengan memproduksi plastik. Produksi hanya akan berjalan jika konsumen membuat permintaan atau permintaan itu atau biasanya kita menyebutnya dengan syarat stok. Secara umum, perusahaan ini telah melakukan pengecekan kualitas terhadap kualitas produk. Dan kualitas produk ini diperiksa oleh jaminan kualitas dan departemen kontrol kualitas. Namun, terkadang beberapa cacat masih ditemukan pada produk tersebut. Tujuan dari penelitian ini adalah untuk menganalisis jumlah dan jenis cacat serta mencari solusi bagaimana menguranginya. Dengan ini, kami berharap kualitas produk akan tetap stabil dan keuntungan perusahaan akan meningkat. Metode yang digunakan adalah Statistical Process Control (SPC). SPC adalah alat yang digunakan untuk menganalisis cacat dalam suatu masalah. Alatalat dalam Kontrol Proses Statistik adalah stratifikasi, lembar periksa, histogram, diagram kontrol, diagram sebar, diagram pareto dan diagram tulang ikan.   PT. XYZ is a manufactur company that run their company by produce plastic. Production will only going if the consument make demand or request of it or usually we called it by make to stock term. Generally, this company has done the quality check to the product quality. And this product quality check by quality assurance and quality control department. However, sometimes some defect is still be found in that product. The objective of this research is to analyze the amount and the type of the defect and also find the solution how to reduce it. With this, we hope that product quality will keep stable and company’s profit will increase. The method that being used is Statistical Process Control (SPC). SPC is a tool that be used to analyze the defect in a problem. The tools in Statistical Process Control are stratification, check sheet, histogram, control chart, scatter diagram, pareto diagram and fishbone diagram.

In order to survive in a competitive market, improving quality and productivity of Product or processis a must for any company. And in case of pharmaceutical industry, effective quality assurance policyimplementation is the most important goal. The main objective of this paper is to apply the Total QualityManagement (TQM) tools in a pharmaceutical company and prove its advantage. Here Control Chart,Fishbone diagram, EWMA were used and the control limits were improved. This activity resulted in moreperfect control limit recommendations which actually capable of reducing the material cost. The TQMperspective views quality as the pivotal purpose of the organization. In the present scenario there are various methods are available to improve and control the product quality of any organization but it is necessary to continue monitoring and measuring the process so that product quality can be maintain. Through this paper try to examined the various defects during capacitor manufacturing process by using fishbone and Pareto analysis. To analyze the various defects, three months data has been collected from October 2018 to December 2018 and analyze all the defects through fishbone and Pareto chart. The outcomes of the research show that the maximum problem was arises from the capacitor’s winding and welding process and have to mainly focus on those area so that defects can be controlled up to maximum extend. To minimize all the defects, prepare fishbone diagram which shows all the root causes of defects and afterward analyze through pareto chart. Now follow some suggestive action to reduces the defects and improve the overall quality of the capacitors.
