The molecular responses to hormonal stimuli in the endometrium are modulated at the transcriptional and post-transcriptional stages. Any imbalance in cellular and molecular endometrial homeostasis may lead to gynecological disorders. MicroRNAs (miRNAs) are involved in a wide variety of physiological mechanisms and their expression patterns in the endometrium are currently attracting a lot of interest. miRNA regulation could be hormone dependent. Conversely, miRNAs could regulate the action of sexual hormones. Modifications to miRNA expression in pathological situations could either be a cause or a result of the existing pathology. The complexity of miRNA actions and the diversity of signaling pathways controlled by numerous miRNAs require rigorous analysis and findings need to be interpreted with caution. Alteration of miRNA expression in women with endometriosis has been reported. Thus, a potential diagnostic test supported by a specific miRNA signature could contribute to early diagnosis and a change in the therapeutic paradigm. Similarly, specific miRNA profile signatures are expected for RIF and endometrial cancer, with direct implications for associated therapies for RIF and adjuvant therapies for endometrial cancer. Advances in targeted therapies based on the regulation of miRNA expression are under evaluation.

Background: Transvaginal sonography (TVS) is a new diagnostic technique used for the evaluation of the female pelvis. The objective of the present study was to study the role of TVS and to assess the diagnostic accuracy in gynecological disorders.Methods: Total number of 100 patients attending OPDs with various complaints was selected by random technique of the study.  All the patients have informed consent and thorough clinical examination including general, systemic and pelvic examination was conducted after taking a detailed history then the patients underwent TVS followed by one of the procedures like fractional curettage, dilatation and curettage and abdominal hysterectomy (with or without conservation of ovaries) or conservative management with regular follow up.Results: The sensitivity of TVS in diagnosing dysfunctional uterine bleeding (DUB) and pelvic inflammatory diseases (PID) was 100% and specificity was 85.1% and 95% respectively. For fibroids and ovarian mass diagnosis, sensitivity was 68.9% and 80.9% respectively and specificity was 100%. Diagnostic accuracy of clinical diagnosis was 70.0% whereas TVS had 94.0% of diagnostic accuracy in diagnosing various gynecological disorders.Conclusions: The final outcome is that TVS examination is an important non-invasive investigation, can be used as important diagnostic method in various gynecological disorders as it has got a high diagnostic accuracy.

Endometriosis is one of the main common gynecological disorders, which is characterized by the presence of glands and stroma outside the uterine cavity. Some findings have highlighted the main role of inflammation in endometriosis by acting on proliferation, apoptosis and angiogenesis. Oxidative stress, an imbalance between reactive oxygen species and antioxidants, could have a key role in the initiation and progression of endometriosis by resulting in inflammatory responses in the peritoneal cavity. Nevertheless, the mechanisms underlying this disease are still unclear and therapies are not currently efficient. Curcumin is a major anti-inflammatory agent. Several findings have highlighted the anti-oxidant, anti-inflammatory and anti-angiogenic properties of curcumin. The purpose of this review is to summarize the potential action of curcumin in endometriosis by acting on inflammation, oxidative stress, invasion and adhesion, apoptosis and angiogenesis.

Cervical cancer is one of the dominant gynecological disorders which has poor prognosis and often diagnosed at advanced stages where it becomes nearly impossible to effectively manage this disorder. MicroRNA-300 (miR-300) has dual role in human tumorogenesis. However, characterization of its regulatory action has not been made in cervical cancer. The molecular role of miR-300 in cervical cancer was thus explored in the present study with prime focus on elucidating its mechanism of action. The results showed significant (P < 0.05) downregulation of miR-300 in cervical cancer. Overexpression of miR-300 in cervical cancer cells inhibited their proliferation in vitro by inducing apoptosis. Cervical cancer cells overexpressing miR-300 also showed decreased rates of migration and invasion. G protein-coupled receptor 34 (GPR34) was found to be the functional regulatory target of miR-300 in cervical cancer. GPR34 was found to be significantly (P < 0.05) overexpressed in cervical cancer tissues and cell lines. Silencing of GPR34 inhibited the growth of the cervical cancer cells. However, overexpression of GPR34 could prevent the tumor-suppressive effects of miR-300 on cervical cancer cells. Collectively, the results of the current study are indicative of the tumor-suppressive regulatory role of miR-300 in cervical cancer and suggestive of the potential therapeutic value of miR-300/GPR34 molecular axis.

The common gynecological problems are Yonidaha, Yonikandu, Yoni paicchilya, Yoni strava, Yoni karkashata, Vamini and Upapluta yonivyapad, etc. The disturbed pattern of menstruation also causes many gynecological problems associated with female health. Ayurveda explains many ways for curing gynecological disorders especially Yoni Roga requiring local therapeutic measures, Sthanik Chikitsa is one of them which offers therapeutic relieves in various Stree Roga. Yoni-Pichu, Yoni Dhoopan, Yonidhawan, Yoni-Lepana, Uttarbasti, Yoni-Varti and Agnikarma, etc. are common approaches of Sthanik Chikitsa which are useful in many gynecological problems. These all approaches of Ayurveda help to maintain good health status of women and relives symptoms of white discharge, itching, burning micturation, foul smell and discharge, etc. These non-surgical and less invasive techniques offers health benefits to retain reproductive health of female and does not imparts severe side effects. Present article explains role of Sthanic Chikitsa in various Stree Roga.

Inflammasomes, intracellular, multimeric protein complexes, are assembled when damage signals stimulate nucleotide-binding oligomerization domain receptors (NLRs). Several inflammasomes have been reported, including the NOD-, LRR- and pyrin domain-containing protein 3 (NLRP3), NLRP1, NLRP7, ice protease-activating factor (IPAF), absent in melanoma 2 (AIM2) and NLR family CARD domain-containing protein 4 (NLRC4). Among these inflammasomes, the NLRP3 inflammasome is the most well-studied in terms of structure and function. Unlike other inflammasomes that can only be activated by a finite number of pathogenic microorganisms, the NLRP3 inflammasome can be activated by the imbalance of the internal environment and a large number of metabolites. The biochemical function of NLRP3 inflammasome is to activate cysteine-requiring aspartate proteinase-1 (caspase-1), which converts pro-IL-1β and pro-IL-18 into their active forms, namely, IL-1β and IL-18, which are then released into the extracellular space. The well-established, classic role of NLRP3 inflammasome has been implicated in many disorders. In this review, we discuss the current understanding of NLRP3 inflammasome and its critical role in gynecological disorders and obstetrical complications.
