IMPROVING THE STATISTICAL REPORTING OF MEDICAL ORGANIZATIONS IN THE RUSSIAN FEDERATION

Abstract. All management decisions in the healthcare are based on reliable data from medical accounting and reporting. Aim. Improve reporting forms in order to optimize accounting and reporting. Materials and methods. One of the main statistical forms of medical reporting that makes it possible to analyze indicators of the health status of workers and the activities of medical organizations in occupational medicine in the Russian Federation is the Form No. 30 “Information about a medical organization”. Results. The indicators analysis of the activities of medical organizations in the Russian Federation providing occupational health care was carried out on the basis of the Form No. 30 “Information about a medical organization”. The reporting forms about the examinations of occupational suitability and examinations of the connection between the disease and the occupation in the Russian Federation have been also developed and improved. The data analysis of medical organizations in the system for accounting and analyzing of occupational diseases «Occupational medicine» posted on the web-site http://profstat-irioh.ru is also carried out. Conclusion. It is proposed to improve the reporting documentation in order to optimize accounting and reporting if necessary. It is also proposed to keep statistics on the mandatory preliminary and periodic medical examinations for workers employed in jobs with harmful and (or) hazardous working conditions in the federal statistical form No. 1-zdrav “Information about the health care organizations”.

The relevance of the research is based on the role of special orders of the Ministry of Health about the realization of citizens’ rights to available and qualitative medical care by providing patients with the opportunity to choose a medical organization. Problems arising in connection with the untimely delivery of health care arise from the contradictions in the provisions of regulations that occupy different places in the hierarchy of domestic legislation. The formulation of the research problem is conditioned by the subject of the analysis, which does not only reflect the existing contradiction between departmental orders in the health care system of the Constitution of the Russian Federation and the provisions of current federal legislation, but allows us to resolve the problems of their correlation, on the basis of which a serious transformation of the not yet patient-oriented normative framework in the health care sector should take place. The objective of the study is to formulate proposals for improving the current legislation that defines the rights of citizens in the field of healthcare. Based on the results obtained, conclusions were drawn, according to which it is advisable to adjust certain provisions of departmental regulations, taking into account the rights of patients, which they are endowed with by the Constitution of the Russian Federation and federal laws in the field of health care. Based on the criminological aspect of violations in the normative acts hierarchy by departmental orders, we can speak about the emergence of a new group of corruption risks.

Introduction. In recent years, special attention has been paid to the discrepancy between working conditions in different sectors of the Russian economy and the inadequately low level of registered occupational morbidity (OM). At the same time, as a rule, issues of access to occupational health care that are important for the rural population are not considered due to the lower potential of socio-economic and infrastructural development of rural areas compared to the city. Material and methods. The paper uses updated data on 82 subjects of the Russian Federation on working conditions, the level of OM of agricultural workers, and indicators of occupational health care for the rural population in 2011-2017. Results. The ranking of subjects of the Russian Federation by the level of non-infectious occupational morbidity of agricultural workers was carried out: group I (OM above the national average), group II (OM below the national average), group III (OM is not registered). It is shown that non-detection of occupational diseases in the regions of group III could be due to insufficient provision of rural health care by occupational pathologists (54.2%), low coverage of workers with periodic medical examinations (PME) (76.6%), as well as the absence of occupational health centers (OPC) in 5 territories. of the 18 regions. According to the criteria of availability of occupational pathologists (97.5%) and coverage of PME (95.5%), the highest availability of primary occupational pathology care occurred in the regions of group II. However, the high proportion of occupational diseases identified during self-treatment indicated a formal approach to conducting PME in this group. The most accessible specialized occupational health care was typical for the subjects of the Russian Federation of group I with a high level of OM, on the territory of which the most significant number of OPC functioned with the possibility of extended pre-examination of employees with suspected occupational diseases in a hospital. Conclusion. Along with harmful working conditions, the occupational morbidity of agricultural workers is primarily determined by the availability of specialized occupational health care, namely, occupational health centers’ availability, equipment, and capacity.

Abstract: The Russian Federation is currently undergoing a period of active industrial development, the introduction of innovative technologies and the emergence of new professions. In turn, the development of industry leads to an even greater number of people than before who are engaged in production in contact with harmful labor factors, and therefore increases the risk of developing occupational diseases. Occupational medicine protects the health of employees, and thanks to actively developing legislation, it can effectively monitor the improvement of safety measures and changes in working conditions, in order to ultimately reduce the number of people with occupational pathologies in the population.

Preserving and strengthening the health of the working population is one of the priority tasks of health care in the Russian Federation, including in the Samara region. Up to 39.7% of the country's population works under conditions that do not meet sanitary and hygienic requirements. Unsatisfactory working conditions are the main reason for the development of occupational diseases among the working population in the Samara region. The workers of medical institutions registered 42 acute occupational diseases with permanent disability, including 13 acute occupational diseases with fatal outcomes from the biological factor "new coronavirus infection caused by the COVID-19 virus."

In places of imprisonment of Russia the system of registration and monitoring ofpatients is the same as in civil health care. This system includes data on detection, diagnostic, treatment and dispensary observation of socially significant diseases. The obtained information is summarized with data concerning civil population and then is passed to Ministry of Health of the Russian Federation and profile research institutes within the framework of common federal and sectoral report forms. The systematic medical examinations, intensification of activities of infection control, support of effective treatment and coordination of measures between Ministry of Justice of Russia and Ministry of Health of the Russian Federation, international partners favored dramatic decreasing of indicator of morbidity of contingents ofpenitentiary system (up to 2.7 times during 1999-2015). Nowadays, the epidemic situation on tuberculosis in institutions of the penal executive system can be characterized as stable and controlled and having tendency to amelioration. Overall, as compared with Russian national average indices (2015), in places of imprisonment morbidity is higher: with tuberculosis 19,4 times, HIV-infection - in 26,9 times, syphilis - in 8.9 times. Among primarily registered patients of Federal penitentiary service of Russia in investigative isolation ward s in 2015 were detected 46% with tuberculosis, 84.6% with HIV-infection, 96.6% with syphilis. The number of patients infected with HIV-infection in contrast with tuberculosis has no tendency to decreasing in places of imprisonment. In 2015 percentage of infected individuals in institutions of the Federal penitentiary service of Russia began to increase in the structure of morbidity of HIV-infection in Russia in general. The cause of death of deceased with combined pathology (tuberculosis + HIV-infection) is HIV-infection mainly because almost in 90% HIV-infection has late stages. This indirectly testifies presence ofproblems related to detection of infection socially significant diseases among socially deadapted persons in civil health care.

One of the main factors in the formation of motivation to improve the quality and performance of employees of health care providers is a qualitatively and logically built wage system in a medical organization. The basis for calculating an employee’s salary is the amount of the salary, on the basis of which its other elements are established - compensation and incentive payments, other charges. This article is devoted to the study of regulatory issues related to the determination of the salary of employees of medical organizations. Object of study. The system of remuneration of employees of medical organizations. Subject of study. The salary of employees of medical organizations. Purpose of the study. The article discusses the problems of the current system of remuneration in health care, as well as the image/svg+xml regulations governing the determination of the size of the official salary of employees of medical organizations. The main theoretical position of the article. The salary of medical workers has a high differentiation even between the subjects of one federal district. Federal legislation regarding the approaches to the wage system is advisory in nature. Therefore, the official salary of doctors of one qualification level, approved at the level of each subject of the Russian Federation, may differ by 5 times. It is substantiated that in order to reduce the differentiation in the size of salaries for medical workers, it is necessary to establish the basic parameters that are uniform throughout the Russian Federation, including the sizes of official salaries of employees of medical organizations. It is advisable to use the value of the minimum wage, annually approved by the Government of the Russian Federation in an amount not lower than the living wage, as the minimum size of the official salary. Scope of the research results. The results can be used as the basis for the policy of improving the remuneration system for employees of federal medical organizations.

Significance. Development and implementation of new medical and information technologies in healthcare, development of quality management system and digitalization are prioritized by the Decree of the President of the Russian Federation as of June 6, 2019 No.254 «On Strategy on healthcare development in the Russian Federation for the period until 2025». They are also mentioned in the list of key activities of the Ministry of Health of the Russian Federation in 2019-2024. Modern evidence-based management approaches are necessary to work in the mentioned fields, while the system development requires improvements including improvements in the management process based on best Russian and international practices in modern management in healthcare. Purpose. To assess the possibility of using modern management technologies to increase flexibility and efficiency of the medical organization. Material and methods. Analysis of domestic and foreign literature, normative legal documents for the period from 2015 to 2020 was carried out by means of theoretical analysis and synthesis using databases of the Russian Science Citation Index, Google Scholar, «Garant» reference system, open Internet sources and keywords search in Yandex search system. Results. Based on the analysis and guided by the functional principle, the authors have proposed a three-group classification of modern management technologies in healthcare allowing to structure the management process in a health care organization depending on the objectives. The groups include: 1) technologies that ensure development and implementation of organizational solutions; 2) technologies that ensure treatment and diagnostic process, quality and safety of health care; and 3) technologies for human resource management. Each group of technologies is represented by a set of organizational solutions, the characteristics of which are presented in this study. Conclusion. Application of flexible technologies is determined by the need to cope with changes in the business environment and to adapt the management system to new conditions. Due to objective restrictions related to the normative nature of the facility functioning and existing personnel limitations preventing development of the necessary managerial competencies of the staff, managers of health care organizations may not be able to properly attend to these issues. At the same time, the accumulated successful experience in implementing individual project management tools, the Agile method and other modern management technologies in the international practice suggest their possible replication to improve management processes in health care organizations in Russia. Scope of application. The study results can be used by healthcare managers, heads of health care organizations in developing and implementing managerial decisions, selecting modern tools and management technologies in order to achieve flexibility and efficiency of the institution's performance, as well as by regional authorities in public health in determining priorities for organizational development of health care organizations. The article presents the results of the analysis of health, working conditions and prevalence of adverse production factors, the structure of the detected occupational pathology in the working population of the Russian Federation. The article presents Statistical data on the dynamics of the share of workplaces of industrial enterprises that do not meet hygienic standards, occupational morbidity in 2015-2018 for the main groups of adverse factors of the production environment and the labor process. The indicators of occupational morbidity over the past 6 years in the context of the main types of economic activity, individual subjects of the Russian Federation, classes of working conditions, levels of specialized occupational health care. The role of the research Institute of occupational pathology and occupational pathology centers in solving organizational, methodological and practical tasks for the detection, treatment, rehabilitation and prevention of occupational diseases is shown. The basic directions of activity in the field of preservation and strengthening of health of workers, and also safety at a workplace are defined.

Article is devoted to the analysis of the Order of the Russian Ministry of Health of 19.03.2020 № 198n (an edition of 29.04.2020) “About a temporary order of the organization of work of the medical organizations for implementation of measures for prevention and reduction of risk of spread of a new koronavirusny infection of COVID19” regarding formation of the temporary staff list of the division of the medical organization providing medical care to citizens with a koronavirusny infection of COVID-19 and also the requirement with the staff of this division. The criteria recommended by the Ministry of Health of the Russian Federation for determining the number of rates of medical workers in these divisions, requirements for medical workers of the division have been considered, a practical example of calculation of rates has been given.
