Each new stage in the development of endoscopy was characterized by the creation of more sophisticated equipment and the expansion of diagnostic capabilities. The 21st century was marked by the transition to robotic remotely controlled endoscopic systems, the use of digital or electronic endoscopic technology in children. The modern level of endoscopy in pediatrics includes a high definition of the image obtained, a morphological study of biopsy specimens and a full range of endosurgical procedures. The paper presents the experience of introducing, teaching, preparing, conducting, analyzing and evaluating the results, features and nuances of use in children of therapeutic and diagnostic enteroscopy of a new generation: video capsule endoscopy and double-balloon enteroscopy. The modern approach to intraluminal diagnosis and endoscopic treatment of children with digestive system pathologies is substantiated.

Abstract Background Despite an increasing number of available mental health apps in the bipolar disorder field, these tools remain scarcely implemented in everyday practice and are quickly discontinued by patients after downloading. The aim of this study is to explore adherence characteristics of bipolar disorder patients to dedicated smartphone interventions in research studies. Methods A systematic review following PRISMA guidelines was conducted. Three databases (EMBASE, PsychInfo and MEDLINE) were searched using the following keywords: "bipolar disorder" or "mood disorder" or “bipolar” combined with “digital” or “mobile” or “phone” or “smartphone” or “mHealth” or “ehealth” or "mobile health" or “app” or “mobile-health”. Results Thirteen articles remained in the review after exclusion criteria were applied. Of the 118 eligible studies, 39 did not provide adherence characteristics. Among the selected papers, study length, sample size and definition of measures of adherence were strongly heterogeneous. Activity rates ranged from 58 to 91.6%. Conclusion The adherence of bipolar patients to apps is understudied. Standardised measures of adherence should be defined and systematically evaluated in future studies dedicated to these tools.

Introduction: In patients with acute large vessel occlusion, the definition of penumbral tissue based on T max delay perfusion imaging is not well established in relation to late-window endovascular thrombectomy (EVT). In this study, we sought to evaluate penumbra consumption rates for T max delays in patients treated between 6 and 16 hours from last known normal. Methods: This is a secondary analysis of the DEFUSE-3 trial, which included patients with an acute ischemic stroke due to anterior circulation occlusion within 6-16 hours of last known normal. The primary outcome is percentage penumbra consumption defined as (24 hour infarct volume-core infarct volume)/(Tmax volume-baseline core volume). We stratified the cohort into 4 categories (untreated, TICI 0-2a, TICI 2b, and TICI3) and calculated penumbral consumption rates. Results: We included 143 patients, of which 66 were untreated, 16 had TICI 0-2a, 46 had TICI 2b, and 15 had TICI 3. In untreated patients, a median (IQR) of 48% (21% - 85%) of penumbral tissue was consumed based on Tmax6 as opposed to 160.6% (51% - 455.2%) of penumbral tissue based on Tmax10. On the contrary, in patients achieving TICI 3 reperfusion, a median (IQR) of 5.3% (1.1% - 14.6%) of penumbral tissue was consumed based on Tmax6 and 25.7% (3.2% - 72.1%) of penumbral tissue based on Tmax10. Conclusion: Contrary to prior studies, we show that at least 75% of penumbral tissue with Tmax > 10 sec delay can be salvaged with successful reperfusion and new generation devices. In untreated patients, since infarct expansion can occur beyond 24 hours, future studies with delayed brain imaging are needed to determine the optimal T max delay threshold that defines penumbral tissue in patients with proximal anterior circulation large vessel occlusion.

Within the embryonic early psychosis field in the early 1990s, the conceptualisation and definition of an at-risk or ultra-high-risk (UHR) mental state for psychosis was a breakthrough which transformed the clinical and research landscape in psychiatry. Twenty-five years later, we have a new evidence base that has illuminated the neurobiology of the onset phase of psychotic disorder, delivered Cochrane level 1 evidence showing that the onset of full-threshold sustained psychotic disorder can be at least delayed, and is paving the way to a new generation of transdiagnostic research. Here, we document the contribution of the UHR approach to understanding the underlying mechanisms of psychosis onset as well as the long-term outcomes. Particularly, we highlight that psychosis onset can be delayed in those meeting UHR criteria and that these criteria have a higher valence for subsequent psychotic disorders and some valence for persistent non-psychotic syndromes. Critiques have helped to identify some of the limitations of this paradigm, which are acknowledged. These include evidence that psychotic disorders can emerge more acutely and from other, as yet undefined, precursor states. Rather than defending, or alternatively questioning the value of, the UHR approach, we propose a broader, transdiagnostic staging model that is consistent with the pluripotent and variably comorbid trajectories for mental disorders. This approach moves beyond psychosis to capture a wider range of subthreshold symptoms and full-threshold disorders, thus enhancing prediction for the emergence and progression of a range of mental disorders, as well as providing new avenues for early intervention and prevention.

This paper provides a new generation of a markup language by introducing the Freestyle Markup Language (FML). Demands placed on the language are elaborated, considering current standards and discussions. Conception, a grammatical definition, a corresponding object graph and the bi-directional unambiguous transformation between these two congruent representation forms are set up. The result of this paper is a fundamental definition of a completely new markup language, consolidating many deficiency-discourses and experiences into one particular implementation concept, encouraging the evolution of markup.

The article deals with the problematic issues of pre-trial investigation of criminal offenses, under investigation by the units of the National Police of Ukraine, have not been studied in the scientific literature. The author of the article points out the problems of the legal status of a head of an inquest body, the author's definition of this term is given and proposed changes to the current criminal procedural legislation, which should eliminate legal gaps. Considered the inconsistencies of the current criminal procedural legislation in terms of regulating the order of attachment for seizure of the property during the pre-trial investigation of criminal offenses. Analyzed judicial practice on this issue and proposed changes to legislation according to the needs of practice. The drawbacks are noted in establishing the terms of pre-trial investigation of criminal offenses, which are caused by the need to conduct psychiatric and other forensic examinations, it is proposed to provide in the Criminal Procedure Code of Ukraine for an inquiry period of 2 months from the moment a person is informed of suspicion. The author's specified legal inconsistencies in the issue of regulating the conduct of a search during an inquiry, it is indicated that there is a need to standardize Articles 234–236 of the Criminal Procedure Code. Due to fact that the activities of the inquest bodies of the National Police of Ukraine are noticeably inefficient, it is proposed to introduce a shortened inquiry procedure in order to implement the principle of economy in the criminal process and saving resources used during the pre-trial investigation.The current procedural form of criminal investigation in Ukraine provides for the need to conduct a full range of investigative and procedural actions in a short time, even for obvious criminal offenses, when the suspect unequivocally pleads guilty and compensates for the damage, which has negative consequences. The introduction of an abbreviated procedure for inquiry is possible only for a clearly defined range of criminal offenses, the legislation must approve guarantees to ensure the rights of suspects from law enforcement abuses and the criteria under which an abbreviated form of inquiry is impossible. The study of the possibility of implementing an abbreviated order of inquiry is of practical importance and is an important area for further study.

Abstract When researchers evaluate adult outcomes for individuals with intellectual and/or developmental disabilities (IDD), the perspective of families is not always considered. Parents of individuals with IDD (N  =  198) answered an online survey about their definition of a successful transition to adulthood. Content analysis was used to describe themes and ideas present in the responses. Rather than focusing only on developmental tasks of adulthood, such as living independently, being competitively employed, and maintaining friendships, responses reflected a more varied and dynamic view of success in adulthood, taking into account the fit between the person with IDD and his or her environment. As services are developed and implemented for adults with IDD, it is important to consider the full range of goals that families have for their son or daughter's successful transition to adulthood.

<p>Ground based active remote-sensing instruments have proved its potential through its high quality observations of thermodynamic profiles. In this study, thermodynamic profiles obtained from the temperature Raman lidar (TRL) and the water-vapour differential absorption lidar (DIAL) of the University of Hohenheim (UHOH) are assimilated into the Weather Research and Forecasting model data assimilation (WRFDA) system through a new forward operator for absolute humidity and mixing ratio developed in-house.<br>Thermodynamic DA was performed either with the deterministic 3-dimensional variational (3DVAR) DA system or with the hybrid 3DVAR-Ensemble Transform Kalman Filter (ETKF) approach. We used data of the High Definition of Clouds and Precipitation for advancing Climate Prediction (HD(CP)2 project Observation Prototype Experiment (HOPE). The WRF model was configured for a central European domain at a convection permitting resolution of 2.5 km spatial grid increment and 100 levels in the vertical with fine resolution in the planetary boundary layer (PBL). The assimilation experiments were conducted in a rapid update cycle (RUC) mode with an hourly update frequency. The hybrid 3DVAR-ETKF DA system was incorporated with an adaptive inflation scheme using a set of 10 ensemble members each with the same configuration as the previous experiments for the 3DVAR.  We will present the results of three DA experiments. In the first experiment (CONV_DA), or the control run, only assimilation of the conventional observations was carried out with 3DVAR DA. The second experiment (QT_DA) was a 3DVAR DA assimilating WVMR and temperature together in addition to the conventional dataset. The third experiment (QT_HYB_DA) assimilated WVMR and temperature together in addition to the conventional dataset with Hybrid DA.<br>The WVMR RMSE with respect to the WVDIAL reduced by 41 % in 3DVAR and still reduced to 51 % in QT_HYB_DA compared to CONV_DA. Although temperature RMSE with respect to TRL increased by 5 % in QT_DA, RMSE significantly reduced to 47 % in QT_HYB_DA compared to CONV_DA. The correlation between the temperature and WVMR variables in the background error covariance matrix of the 3DVAR, which is static and not flow-dependent, limited the improvement in temperature. Flow-dependency in Hybrid DA improved the error correlations.<br>We also present results of a collaborative effort with the Raman lidar for meteorological observation (RALMO) from the MeteoSwiss and the Atmospheric Raman Temperature and Humidity Sounder (ARTHUS) using even finer model resolution. The initial results of the new study will also be presented here.</p>

A central theme of this book is the three-level approach to CASE environment design and construction that distinguishes conceptual issues (the services) from implementation issues (the mechanisms) and stresses the need for a design context (the process) that the CASE environment must support. Previous chapters have discussed this theme, as well as provided insight into a conceptual model that identifies and classifies the services that might be found in a CASE environment. However, regardless of the service model or conceptual approach selected, environment builders must eventually face the bottom-line decision of how to actually carry out tool integration. The choices they face in terms of potential mechanisms are numerous and include a full range of selections that provide varying degrees of effort and integrated capability. In this chapter, we first consider the properties of integration by continuing the discussion of integration as a relationship (see Section 2.2). In this discussion (Section 5.2), we highlight properties that are addressed by various integration mechanisms. Later sections of this chapter focus on the specific relationship between two particular classes of integrating mechanisms: those based on the sharing or transfer of data between tools (data integration), and those based on synchronization and communication between tools (control integration). In Section 2.2 we introduced the concept put forward by Thomas and Nejmeh that integration can be considered by defining the properties required of the relationships between different environment components. Their definition of integration is exclusively a conceptual view, and is independent of the particular technology being used to implement that integration.

While the concept of utilizing learning objects has been addressed in instructional design for some time, slightly different definitions of the term “learning object” are found in the literature. For example, the Institute of Electrical and Electronics Engineers (IEEE) (2005) defines a learning object as “any entity, digital or non-digital, which can be used, re-used or referenced during technology supported learning.” Wiley (2000) similarly defines a learning object as “any digital resource that can be reused to support learning” (p. 7). Barritt and Alderman (2004) state a working definition of learning objects as “an independent collection of content and media elements, a learning approach (interactivity, learning architecture, context), and metadata (used for storage and searching)” (pp. 7-8). Merrill (1996) uses a different term, a “knowledge object” that consists of a set of predefined elements, each of which is “instantiated by way of a multimedia resource (text, audio, video, graphic) or a pointer to another knowledge object” (p. 32).
