Background Caries process is not a static one, but is dynamic with interspersed periods of demineralization and remineralization of enamel, intimately related and occurs episodically based upon the presence of cariogenic bacteria in dental plaque and the availability of refined carbohydrates for fermentation to organic acids.Objectives Early enamel caries could be reversed with avoidance of frank cavitation. The main objective of this study is to check whether enamel demineralization can be prevented by using the remineralizing agents.Method Forty freshly extracted human central incisors were selected and stored in saline at normal temperature. A window of 3 X 3mm enamel was created and all the specimens were then randomly divided into 4 groups of 10 each. Group I- teeth received no treatment, Group II- teeth treated with Acidulated Phosphate Flouride gel, Group III- teeth treated with Tooth Mousse Plus, Group IV- teeth treated with Remin+. Samples in all the groups were kept in artificial saliva for 24 hours and subjected to modified Ten Cate’s solution at an acidic pH of 3.5 for 10 days. The samples were sectioned and subjected to SEM evaluation.Result Scanning Electron Microscope(SEM) images showed decrease in pore volume of the enamel in all the treatment groups compared to the control group indicating increase in resistance to demineralization in acidic pH.Conclusion The three groups of remineralizing agents, Acidulated Phosphate Fluoride gel, Tooth Mousse Plus and Remin + showed significant increase in fluoride content and negligible increase in calcium content indicating there is remineraliztion.Kathmandu University Medical Journal Vol.12(1) 2014: 16-20

Background The aim of this study was to evaluate the cytotoxic effects of 9 different soft denture liners on the viability of L-929 mouse fibroblast cells at different incubation periods by storing them in artificial saliva (AS) with different pH levels. Methods 96 disk samples from each lining material were prepared and divided into 4 groups: GI: No treatment; GII: Stored in artificial saliva with pH 3 for 21 days; Group III: Stored in artificial saliva with pH 7 for 21 days; and Group IV: Stored in artificial saliva with pH 14 for 21 days. The cytotoxicity of the extracts to cultured mouse fibroblasts (L-929) was measured by MTT (tetrazolium salt 3-[4,5-dimethylthiazol-2-yl]-2,5-dipHnyltetrazolium bromide) assay. Data were analyzed using 1-way analysis of variation (ANOVA). Results It was found that for the pH 3 values of New Truliner, Trusoft, Mollosil Plus, Dentusil, TDV, and HydroCast®; for the pH 7 values of Ufi Gel P and Elite plus; and for the pH 14 values of HydroCast®, there was a noncytotoxic effect during both the 24-hour and 48-hour incubation periods. In the control group 48-hour incubation period, HydroCast®, TDV, Mollosil, 24-hour incubation period Elite plus, for pH 3 values; Elite Plus 24-hour incubation period, for pH 7 values Trusoft 48-hour incubation period there was a moderately cytotoxic effect. Conclusions This in vitro study revealed that storage in artificial saliva with different pH levels can affect the cytotoxicity of soft lining materials.

Abstract Background This in-vitro-study aimed to evaluate the potential of different fluoride gels to prevent gastroesophageal reflux induced erosive tooth wear. Methods Surface baseline profiles of a total of 50 bovine enamel specimens [randomly assigned to five groups (G1–5)] were recorded. All specimens were positioned in a custom made artificial oral cavity and perfused with artificial saliva (0.5 ml/min). Reflux was simulated 11 times a day during 12 h by adding HCl (pH 3.0) for 30 s (flow rate 2 ml/min). During the remaining 12 h (overnight), specimens were stored in artificial saliva and brushed twice a day (morning and evening) with a toothbrush and toothpaste slurry (15 brushing strokes). While specimens in the control group (G1) did not receive any further treatment, specimens in G2–5 were coated with different fluoride gels [Elmex Gelée (G2); Paro Amin Fluor Gelée (G3); Paro Fluor Gelée Natriumfluorid (G4); Sensodyne ProSchmelz Fluorid Gelée (G5)] in the evening for 30 s. After 20 days, surface profiles were recorded again and enamel loss was determined by comparing them with the baseline profiles. The results were statistically analysed using one-way analysis of variance (ANOVA) followed by Tukey`s HSD post-hoc test. Results The overall highest mean wear of enamel (9.88 ± 1.73 µm) was observed in the control group (G1), where no fluoride gel was applied. It was significantly higher (p < 0.001) compared to all other groups. G2 (5.03 ± 1.43 µm), G3 (5.47 ± 0.63 µm, p = 0.918) and G4 (5.14 ± 0.82 µm, p > 0.999) showed the overall best protection from hydrochloric acid induced erosion. Enamel wear in G5 (6.64 ± 0.86 µm) was significantly higher compared to G2 (p = 0.028) and G4 (p = 0.047). Conclusions After 20 days of daily application, all investigated fluoride gels are able to significantly reduce gastroesophageal reflux induced loss of enamel.

OBJECTIVE: This in vitro study was designed to evaluate the shear bond strength (SBS) of orthodontic metal brackets bonded by direct and indirect techniques. METHODS: Thirty healthy human maxillary premolar teeth were used. The teeth were divided into three groups of 10 teeth each: Group I - indirect bonding with SondhiTM Rapid-Set system (3M/Unitek), Group II - indirect bonding with TransbondTM XT adhesive system (3M/Unitek) and Group III - direct bonding with TransbondTM XT adhesive system (3M/Unitek). After bonding and obtaining the specimens for the study, the specimens were subjected to SBS testing in a universal testing machine (Emic, model DL-500). The Kolmogorov-Smirnov test was applied to ascertain that the data had a normal distribution and the Bartlett test to check whether there was homogeneity of variance. One-factor analysis of variance was performed and, subsequently, Tukey's test for paired means. A 5% significance level was adopted. RESULTS: The results of Group I were 67.6 (N) and 5.9 (MPa); Group II, 68.9 (N) and 6.1 (MPa) and Group III (control), 92.5 (N) and 8.1 (MPa). CONCLUSION: It can therefore be concluded that the means for Group III were significantly higher compared with Groups I and II in both Newton (N) and Megapascal (MPa) values. The means attained by the indirect bonding technique used in Groups I and II, however, exhibited no statistically significant differences.

Background. This in vitro study aimed to evaluate the microshear bond strength (μSBS), microhardness and morphological characteristics of primary enamel after treating with sodium fluoride (NaF) and acidulated phosphate fluoride (APF). Methods. Forty-eight primary canines were cut into mesial and distal sections and assigned to five groups randomly: group 1 (immersed in saliva as a control), group 2 (treated with NAF and immersed in saliva for 30 minutes), group 3 (treated with APF and immersed in saliva for 30 minutes), group 4 (treated with NAF and immersed in saliva for 10 days), and group 5 (treated with APF and immersed in saliva for 10 days). Composite resin (Filtek Z250) was bonded on the specimens (n=15) for measuring the μSBS. After storage in 37°C artificial saliva for 24 hours, µSBS and Vickers hardness tests (10 readings) were performed. The data were analyzed using one-way ANOVA and Kolmogorov-Smirnov, Levene’s and Tukey HSD tests (P<0.05). Morphological analysis of enamel and modes of failure were carried out under a scanning electron microscope (SEM) on two remaining specimens. Results. Significant differences in μSBS were only noted between groups 2 and 4 (P=0.024). Group 3 showed a significant decrease in hardness after storage in artificial saliva (P<0.001), with a significantly lower hardness than the other groups (P<0.001). The SEM observations showed irregular particles in groups 3 and 5; uniform, smooth and thin coats were seen in groups 2 and 4. Conclusion. Fluoride therapy with NaF and APF gels prior to restorative treatments had no adverse effects on the microshear bond strength.

Anaphylaxis is the most common allergic reaction triggered by allergens such as insect poisons, food, and drugs through skin contact, injection, or inhalation. In vitro previous research showed that strawberries fruit have activity as antioxidant, anticancer, anti-inflammation, and anti-allergic. The research aimed to determine the antianaphylaxis strawberry fruit extract in mice (Balb/C strain) with ovalbumin-induced.  Twenty-four Balb/C strain mice were divided into six groups (n=4). Group I and II as a normal and control group. Group III till VI as a treatment group was given cetirizine dose 0.042 mg/20 g BW and strawberry extract doses 0,68; 1,36; and 2,72 mg/20 g BW, respectively. This research showed that 70 % of ethanol extract of strawberries fruit have antiallergic activity in response to active cutaneous anaphylaxis. 70% ethanol extract of strawberries doses 2.72 mg/20 g BW had similar antiallergic activity compare with cetirizine. The conclusion of this study showed that strawberries fruit extract could be developed as an alternative medicine to anti-anaphylaxis or anti-allergic.

ABSTRACT Aim Aim of this study was to compare class II composite restoration using flowable composites as lining with various thickness and curing techniques by evaluating cervical marginal microleakage. Materials and methods Fifty intact molars, each prepared with two box-only class II cavities, were randomly divided into five groups: Group I, P60 filling alone; group II, ultrathin flowable composite lining (0.5-1mm) cocured with overlying composite; group III, thin lining (1-1.5) cocured with overlying composite; group IV, ultrathin lining (0.5-1 mm) precured and group V, thin lining (1-1.5) precured. The teeth were then thermocycled for 1500 cycles (between 5 and 60 °C) and immersed in dye for 24 hours. Cervical microleakage was measured as the extent of dye penetration. Results Cocured specimens (groups II and III) showed least microleakage and control specimens (group I) showed maximum microleakage. On comparison of curing techniques, cocured specimens (groups II and III) showed less microleakage than precured (groups IV and V). On comparison of lining thickness and type of curing, group IV showed less microleakage than group V. Groups II and III were statistically not significant. Conclusion It was concluded from the results that ultrathin cocured flowable composite lining specimens improved the marginal sealing with decreased microleakage. Clinical significance In this study, the application of additional flowable composite lining with various thicknesses presented different influences in marginal quality of class II box only composite restorations. A new technique applying an ultrathin flowable composite lining with cocuring technique improved the marginal sealing with decreased microleakage. BACKGROUND AND OBJECTIVES: This study evaluated and compared the antimicrobial efficacy and minimal inhibitory concentration (MIC) of chlorhexidine 0.12% and extracts of aloe vera and mushroom against Streptococcus mutans and lactobacillus. MATERIALS & METHODOLOGY: The agar disc diffusion and broth micro dilution method was used to check the antimicrobial activity of 0.12% chlorhexidine and aqueous extracts of two medicinal plants. The test samples were divided as follows- Streptococcus mutans, group I: chlorhexidine, group II: Aloe vera and group III: Mushroom. Lactobacillus, Group I: chlorhexidine, group II: Aloe vera extract and group III: Mushroom extract. The zone of inhibition and MIC values were tabulated and the data was statistically analyzed using ANOVA and Bonferroni post- hoc tests. RESULTS: Chlorhexidine shows maximum antibacterial action against Streptococcus mutans and lactobacillus followed by mushroom extract and aloe veraextracts with the zone of inhibition of (20.3mm/ 24.13), (17.03/ 16.05), (14.09/14.26) respectively. Both the extracts had MIC value of 80µg/ ml and chlorhexidine was 40µg/ml. CONCLUSION: Within the limitations of this in- vitro study, it can be concluded that all the herbal extracts tested in this study demonstrated antibacterial activity against MS and lactobacillus. Chlorhexidine showed maximum antibacterial action against Streptococcus mutans and lactobacillus followed by mushroom extract and then aloe veraextracts with the zone of inhibition of (20.3mm/24.13), (17.03/ 16.05), (14.09/14.26) respectively. Herbal products have potent antimicrobial activity that can be looked at as an alternative to chlorhexidine. However, further in vitro and long-term in vivo studies are recommended to confirm and correlate the findings of this study clinically.

ABSTRACT Background Orthodontic treatment has gained popularity since beginning of era of dentistry. Now a day, everyone is conscious about their appearance, smile and function. During orthodontic treatment use of brackets and adhesives are common. The bonding of brackets will cause demineralization which requires the fluoridation. So the study has been undertaken to analyze the pattern of fluoride release by commercially available adhesive bonding material for the prevention of demineralization. Aim To evaluate and compare the clinical significance of quantity and pattern of fluoride release from three commercially available adhesives. Objectives To assess the pattern of fluoride release and quantity, to reduce the decalcification of enamel around orthodontic brackets and bands during treatment and to prevent further use of topical fluoride both office and self-use agents for prevention of demineralization/for remineralization. Materials and methods The comparison of quantity and pattern of fluoride release study involved commercially available bonding adhesives. They are: Group I—resin reinforced glass Ionomer light cure material (OrthoLC), Group II—fluoride releasing composite resin material (Excel) and Group III— conventional composite (Relay-a-bond) evaluated on 78 freshly extracted premolar teeth divided into three groups consisting 26 specimens in each group. The prepared specimens were stored in artificial saliva at 37°C in an incubator for subsequent fluoride analysis using ORION ion selective electrode coupled with ionalyzer 901. Fluoride analysis made at 24 hours intervals for first 3 consecutive days and thereafter at the end of 10th, 17th, 24th and 31st day of bonding. The data obtained were tabulated and interpreted by statistical analysis using ‘t’ test and one-way analysis of variance (ANOVA). Observations and Results The quantity of fluoride release in groups I and II was significant even at the end of 31st day. The one-way AVOVA showed intra and inter group significance in the quantity of fluoride release. But group III with zero fluoride release with significant decalcification on enamel which requires external use of topical fluorides. The pattern of fluoride released was 3.06 ppm for group I and 2.01 ppm for group II and was declined sharply after 24 hours; and continued to decline in subsequent weeks. Mean quantity of fluoride release by group I was 15.08 ppm were as group II was 9.02 ppm over the test period of 31 days. At the end of 31st day the group I bonding adhesive was releasing considerable amount of fluoride compared to group II whereas group III was nil. At all the periods inter and intra group mean values were highly significant. And group III acted as base line or control group as it was non fluoride releasing material. Conclusion Both the fluoride releasing adhesive bond material are useful to reduce the risk of demineralization and further prevent the usage of topical fluoride application and reduce cost and clinical visiting time for both patient and clinician. ABSTRACT Background Smear layer is a negative factor which prevents adhesion of the filling material to the dentinal walls. Recent advances in dental research have incorporated lasers as a potential adjunct in root canal treatment by removing the smear layer before filling the root canal system, enhancing the adhesion of sealers to dentin and improving the sealing ability. Aim To evaluate the microtensile bond strength of AH-Plus resin-based sealer to dentin after treatment with 980 nm diode and 1,064 nm neodymium-doped:yttrium aluminum garnet (Nd:YAG) laser in vitro. Materials and methods Thirty specimens prepared for three groups namely group I (control), group II (980 nm diode–lased specimens) and group III (Nd:YAG–lased specimens). One tooth from each group was observed under scanning electron microscope for evaluation of intracanal root dentin morphology. Remaining specimens were used for making microsections by hard tissue microtome. Specimens for groups II and III were lased with 980 nm diode and 1,064 nm Nd:YAG laser. AH Plus sealer was applied onto specimens and mounted onto Instron universal testing machine for microtensile bond strength testing. Results were subjected to statistical analysis using one-way analysis of variance (ANOVA) and Tukey's test. Results Group III Nd:YAG had maximum mean microtensile bond strength values (11.558 ± 0.869), followed by group II diode (9.073 ± 0.468) and group I control (6.05 ± 0.036). Statistically significant differences were seen among all the groups. SEM analysis shows removal of smear layer in both groups II and III. Conclusion Both Nd:YAG and diode laser were more effective than control group in improving the microtensile bond strength of AH Plus sealer to dentin. Clinical significance Lasers have the potential to increase the adhesiveness of root canal sealer to dentin surface, thereby improving the quality of root canal obturation. This in vitro study evaluated the surface microhardness of human enamel submitted to bleaching with 10% carbamide peroxide (CP) containing calcium or fluoride. Ninety-eight dental blocks (5 x 5 mm²) with polished enamel surfaces were randomly assigned to 7 treatment groups (n=14), as follows: without bleaching and storage in artificial saliva (control); 10% CP; 10% CP + 0.05% calcium; 10% CP + 0.1% calcium; 10% CP + 0.2% calcium; 10% CP + 0.2% fluoride; and 10% CP + 0.5% fluoride. During 14 days, enamel surfaces were daily exposed to a 6-h bleaching regimen followed by storage in artificial saliva. Surface microhardness was measured before (baseline), during (7th day), immediately after bleaching (14th day) and 1 week post bleaching. Data were analyzed by two-way ANOVA and Tukey's test (p<0.05). All treatments reduced SM significantly during the bleaching cycle (7th day), immediately after bleaching (14th day) and 1 week post bleaching, compared to baseline and to the unbleached control group. In conclusion, in spite of the addition of calcium and fluoride, all bleaching treatments affected the enamel surface microhardness.
