Climate change which is triggered by a consumptive human culture continues to be a concern for many parties to be tackled. Shoes have become the primary need of urban communities because of their function that cannot be separated from their daily activities. Shoes that are purchased by consumers are usually packaged in cardboard boxes that follow the components of the contents, namely the shoes and accessories inside. In this case, shoeboxes with the right design and function according to consumer preferences will reduce the likelihood that the shoeboxes are thrown into the trash. Although it is not the centre of consumer attention, shoe boxes remain a necessity in the final production process. It is necessary to design and develop a shoebox product that is in accordance with the final function of the shoebox according to consumer interests. This study uses the QFD method which produces quality 1 houses with communication tools in the form of a preliminary questionnaire to 30 respondents and a follow-up questionnaire to 100 respondents. Concept selection was carried out on the eight alternative concepts developed. The shoebox concept chosen to take into account the desires of the consumer will be made with a sporty shoebox standard size of 22.5 x 12.3 x 32.5 cm. The environmentally friendly shoebox can be seen through the installation of plastic straps which are expected to reduce the use of single-use plastics. In addition, the shoebox is made with an innovative design so that users can use it repeatedly as shoe storage or multipurpose box.

Patients with short bowel syndrome (SBS) on long-term home parenteral nutrition (HPN) chronically receive high fluid volumes directly into the right atrium (RA) through the superior vena cava. We retrospectively evaluated cardiac function measured by routine transthoracic echocardiography (TTE) in a population of 26 SBS patients on long-term HPN and compared their data on echocardiograph-derived right heart structure and function, with those of a control group of 26 patients also bearing a central venous catheter (CVC) for other reasons. Results showed that body weight and BMI were significantly higher in the control group. The echocardiographic estimate of RA pressure was higher in HPN patients than in controls (p = 0.01). An increased estimate of RA pressure indicates the need to consider TTE in the follow-up of long-term HPN patients to detect functional impairment early.

SummaryFracture of the central tarsal bone is an uncommon injury in dogs and occurs predominantly in racing Greyhounds. To the authors’ knowledge, this type of fracture has not been described previously in cats. This case report describes a five-year-old Domestic Shorthair cat referred to the Centro Veterinario Luni Mare because of lameness, swelling and signs of pain in the right hindlimb caused by trauma. Clinical examination and diagnostic imaging revealed a right central tarsal bone fracture. Open reduction and internal fixation with a 2.0 mm position screw and two 0.8 mm Kirschner wires were carried out. The last follow-up examination three years postoperatively found the cat in good health with normal range of motion and function, and no signs of lameness in the right hindlimb.

Background: Right ventricular (RV) dysfunction is a significant cause of morbidity and mortality after surgical correction of RVOT obstruction including tetralogy of Fallot (TOF). Transatrial repair avoids a ventriculotomy (in contrast to the transventricular approach) emphasizing maximal preservation of RV structure and function. We have adopted this technique as less traumatic for the right ventricle. This study evaluates the early surgical results of our approach.Methods: Between January 2005 to January 2014, 77 consecutive patients with RVOT obstruction were referred to our unit for surgical therapy. Of these, 14 were unsuitable for repair and underwent aortopulmonary shunting. In the remaining 63 patients (mean age of 2.67±0.38 years), complete transatrial/transpulmonary repair was performed. Previously placed shunts (four patients) were taken down. In all cases, subpulmonary resection and ventricular septal defect (VSD) closure were accomplished transatrially. In 51 patients, the main pulmonary artery was augmented with an autologous pericardial patch.Results: There were 7 (9%) deaths in this series. No patient required permanent pacemaker. Median ICU and hospital stay were 91 hours and 14 days, respectively. At median follow up of 54 (mean 51±12) months, all patients are asymptomatic, with no significant residual lesion.Conclusions: Transatrial/transpulmonary repair of TOF is associated with remarkably low morbidity and mortality in our early experience.

Coffee bag is pure coffee packaged in a dip bag. The first advantage of this product is that consumers can enjoy black coffee with the right taste because it already has a one-time dose. Some drinkers who do not pay attention to the rest of the coffee, by consuming coffee bag they will not get the remaining coffee grounds in their drinks. This study have purpose to see the attributes that become preferences and indicators that measure the taste of the best variation of coffee bag that consumers want. The result of this research is that the coffee is dyed based on consumer preference which is coffee that has a bitter taste, an aroma that represents the taste of coffee, a strong mouth weight, and a long-lasting taste. The taste of the coffee bag with the best variation that consumer input is the natural sample, namely 4169 100% natural coffee bag with an average of 3.79. Respondents argued that the natural sample has a strong mouth weight, bitter taste, and the aroma of the sample represents coffee (has a characteristic). Coffee produced from the dry process is usually superior in the body, fruity taste (fruity), more bitter, low acidity.

The factors that affect a consumer when making a purchase decision include product function, quality, and price. Different products might feature similar functional performance although they are designed and manufactured by different vendors. The most decisive factor for a consumer in buying a product is no longer its physical functions. Nowadays, consumers care more about the aesthetics that a product can deliver. Designers should make any extra effort to improve the aesthetic design of their products. Therefore, it is critical for a designer to know the aesthetic tastes of his/her target consumer group in order to create products with the preferred style. Furthermore, the idea of design differentiation needs to be incorporated so that a product can present different aesthetic perceptions to consumers of different tastes. The objective of this study is to investigate consumers’ preference models from their preferred product forms. The aesthetic principles of Symmetry, Minimalism, and Cohesion are applied to the case study of the design of various vase forms. A quantitative approach of evaluating vase forms by the aesthetic principles is proposed. A conjoint analysis of the vase features and attributes was carried out in order to determine their correlation with consumer preferences. Consumers are classified into 6 groups of different aesthetic conceptions by cluster analysis. The consumers’ preference model can be determined by indices including Pearson’s R and Kendall’s tau. This approach provides designers with an effective way of determining the right direction for new form designs and renovation.

ABSTRACT A 51-year-old male presented to the ENT outpatient department with complaints of pain, irritation and foreign body sensation in the throat for 3 months. There was history of being diagnosed to have laryngeal paraganglioma for which the patient had undergone microlaryngeal surgical excision twice during the preceding 9 years. Clinical examination revealed a reddish proliferative mass of about 2 cm involving the laryngeal surface of the epiglottis and extending to the right aryepiglottic fold. Vocal cords were normal in structure and function. With the previous history of endolaryngeal excision of the tumor and its recurrence, the patient underwent laryngofissure approach with median thyrotomy and left lateral pharyngotomy. The tumor was excised in toto and histopathologically confirmed as paraganglioma. Postoperative period was uneventful. Patient is on continued follow-up with no recurrence till date. Main funding source(s): Helios Clinical Research Institute Background/Introduction Patients with severe tricuspid regurgitation (TR) are characterized from severe dilatation and negative remodeling of the right heart chambers causing functional right ventricular (RV) failure and increased mortality and morbidity. The transcatheter tricuspid edge-to-edge repair (TTVR) is a new treatment option associated with symptom improvement and reduced hospitalization. The cardiac magnetic resonance imaging (CMRI) remains the gold standard for evaluating the right ventricular morphology and function and could be a useful in assessing the RV reverse remodeling after TTVR.  The Tricuspid Regurgitation REgistry (TRuE) is an ongoing national prospective register of patients with severe TR that aims to identify morphofunctional parameters of the right heart chambers related to improve outcomes after TTVR. Purpose The present is a subanalysis of patients with serial CMRI examinations recruited in the ongoing prospective TRuE registry. Methods After excluding patients with intracardiac RV leads, seven patients (age 70 ±12 years) with completed thirty-day follow up were included in the present analysis. Retrospective gated steady state free precession cine images were acquired in the long and short-axis views covering the entirety of both ventricles. Aortic and pulmonary flow data were acquired with a flow-sensitive gradient echo sequence. Assessment of left ventricular (LV) and RV volumes was performed manually in the short-axis cine images (Circle Cardiovascular Imaging, Calgary, Alberta, Canada). The TR fraction was calculated as: ({total RV stroke volume (SV) – total pulmonary forward flow}/total RVSV) × 100. All volumes and flow measurements were indexed for body surface area and expressed in ml/m2. The ratio between the diameter of pulmonary artery (PA) and ascending aorta (AA) was used as indirect measurement of pulmonary arterial hypertension (PAH). CMRI parameters assessing TR quantification, RV morphology and function were analyzed at baseline and follow-up with Fisher´s test or paired t-test. Results At thirty days there was a significant reduction of the TR fraction (45% to 20%, p < 0.001), RV dilatation (RV-end diastolic volume (EDV): 106 ± 12ml/m2 to 86 ± 9.4ml/m2, p = 0.002, RV- end sistolic volume (ESV):55 ± 6.5 to 43 ± 6.6ml, p = 0.003) and improvement of function (RVEF: 47 ± 3.6 to 53 ± 2.4 %, p = 0.006). On the left heart side, there was an improvement of LV filling, with increased LVEDV and LVESV and of the LVSV (36 ± 5.3 to 41 ± 3.7ml/m2, p = 0.040), causing an improvement of the cardiac index (2.2 ± 0.9 l/min/m2 to 2.9 ± 0.7 l/min/m2; p = 0.019. Furthermore, the PA/AA ratio improved significantly (0.91 ± 0.15 to 0.81 ± 0.14, p = 0.006). Conclusion TTVR is associated with positive reverse remodeling of the RV with reduction of dilatation, PAH and function improvement.

Introduction: Cavo-tricuspid isthmus (CTI) dependent atrial flutter (AFL) is one of the most common atrial arrhythmias involving the right atrium (RA) for which radiofrequency catheter ablation has been widely used as a therapy of choice. However, there is limited data on the effect of this intervention on cardiac size and function. Hypothesis: CTI dependent ablation for patients with AFL will improve tricuspid valve function, biatrial enlargement, and ventricular function. Methods: A retrospective study was conducted on 468 patients who underwent CTI dependent ablation for clinical typical AFL at a single institution between 2010 and 2019. After patients with congenital or rheumatic heart disease, heart transplant recipients, or those without baseline echocardiogram were excluded, 211 patients remained in the analysis. Echocardiographic data were analyzed at baseline prior to ablation, and at early follow-up within 1-year post-ablation. Follow-up echocardiographic data was available for 130 patients. Results: Of the 211 patients with CTI-AFL, 200 had typical counterclockwise flutter. The mean age was 64.2±12.1 years old with 12% (n=26) female. The average left ventricular (LV) ejection fraction (EF) significantly improved on follow-up echo (45.55±14.26 to 49±14.4%, p=0.0075), of which 63 (48%) patients had an improvement in EF of 5% or more and 19 (25%) patients had an increased EF of 20% or more. The prevalence of moderate to severe tricuspid regurgitation (TR) was 24% (n=50) at baseline and 18% (n=22) at follow-up with no significant difference (p=0.27). However, 30% (n=39) of the patients had one grade or more improvement in their TR within the 1-year follow-up. Echocardiography also showed improvement of RA size in 48.6%, and left atrial (LA) size in 48.2% of the patients. Conclusions: Patients who underwent CTI dependent AFL ablation showed an improvement in cardiac size and function at follow-up evaluation. Although longer-term results are unknown, these findings suggest that restoration of sinus rhythm from atrial flutter is associated with improvement in TR severity, RA size, LA size, and LVEF.

The lumbar vertebrae are the most common site for fracture incident because of its high mobility. The spinal cord injury usually happened as a result of a direct traumatic blow to the spine causing fractured and compressed spinal cord. A 38-year-old man presented with lumbar spine’s compression fracture at L2 level. In this patient, decompression laminectomy, stabilization, and fusion were done by posterior approach. The operation was successful, according to the X-Ray and patient’s early mobilization. Pneumothorax of the right lung and pleural effusion of the left lung occurred in this patient, so consultation was made to a cardiothoracic surgeon. Chest tube and WSD insertion were performed to treat the comorbidities. Although the patient had multiple trauma that threat a patient’s life, the management was done quickly, so the problems could be solved thus saving the patient’s life. After two months follow up, the patient could already walk and do daily activities independently.
