The reproductive system is exposed to various adverse factors and harmful effects at all stages of functioning of an organism. Against the background of declining levels of women's reproductive health in recent years, there has been a clear tendency towards an increase in the number of patients with pathology of puberty. The low level of somatic and reproductive health before pregnancy causes a high incidence of complicated labors, which leads to a deterioration in the development of subsequent generations.The purpose of the study. To provide clinical characteristics of women who had menstrual dysfunction in puberty, which were based on the study of clinical and anamnestic data, laboratory and instrumental research methods, analysis of risk factors, clinical course, treatment effectiveness and dispensary observation.Materials and methods. According to the nature of the disfunctions, the main group of women (n = 210) who had menstrual disorders in puberty was divided into 3 subgroups (n = 70): the first - women with primary oligomenorrhea and timely average menarche, the second - with late menarche, the third - with pubertal bleeding. The control group included women (n = 70) with the correct rhythm of menstruation in puberty.Results. The clinical prospective study analyzed the social status of women with reproductive dysfunction in puberty, the frequency and structure of existing concomitant somatic diseases, the duration of the menstrual cycle, the nature of diseases of the female genital organs; the assessment of the comprehensive treatment of revealed disorders, which allowed to obtain a favorable reproductive prognosis. The majority of surveyed women (53.3%) planned a pregnancy, which occurred in 75.9% from among of planning ones.Conclusions. The conduction of clinical and anamnestic analysis of the reproductive function of women with menstrual dysfunction in puberty will increase the effectiveness of the treatment of identified disorders and will allow developing algorithms for management of such women during planning and during pregnancy.

The control group included women (n=70) with the correct rhythm of menstruation in puberty. Results. During a clinical prospective study, the social status of women with impaired reproductive function in the puberty was analyzed, the frequency and structure of existing concomitant somatic diseases, the duration of the menstrual cycle, the nature of diseases of the female genital organs; an assessment of the complex treatment of the revealed violations was made, which allowed to obtain a favorable reproductive prognosis. Most of the women examined (53.3%) planned a pregnancy, which occurred in 75.9% of those planning. Conclusion. A study of the clinical and anamnestic analysis of the reproductive function of women with menstrual irregularities in the puberty period will increase the effectiveness of the treatment of identified disorders and develop algorithms for the management of such women during planning and during pregnancy. Deviations in the formation of reproductive function, and subsequently menstrual irregularities from the age of menarche, ovarian dysfunction and steroidogenesis disorders can be clinically manifested in remote periods after their direct action. The nature and depth of morphofunctional disorders in this case depends on the degree of maturity of the reproductive system, the initial functional state of the regulation centers (hypothalamus, pituitary gland) and ovaries, and the duration of the action of adverse factors. The accumulated specific research experience indicates that in addition to the pituitary gonadotropin hormones, cytokines (interleukins), which can simulate ovarian function and play an important role in ovulation, are of great importance in the normal functioning of the ovaries. The objective: аccording to laboratory and instrumental methods of research, to study the characteristics of hormonal status and the relationship of ovarian hormones with interleukins (IL) in women with menstrual dysfunction in the puberty. Materials and methods. According to the nature of the violations, the main group of women (n=90) who had a pathology of menstrual function in the puberty period was divided into 3 subgroups (n=30): the first – women with primary oligomenorrhea in the anamnesis, the second – with a late age menarche, the third – with puberty bleeding. The control group included women (n=30) with the correct rhythm of menstruation in puberty. Results. A clinical study found a decrease in the concentration of progesterone in women with a history of primary oligomenorrhea, a decrease in estradiol in the preovulatory period in women with late menarche. When assessing the concentration of follicle-stimulating hormone in the subgroups, an increase was noted in patients with puberty bleeding compared with the control group, due to the low level of antimuller hormone in the blood serum. The correlation between the indicators of these hormones and interleukins (IL-4, IL-8) indicates the influence of cytokines on folliculogenesis and the formation of the corpus luteum. Ovarian reserve parameters are not reduced. Conclusion. An analysis of the hormonal and cytokine status in women with menstrual dysfunction in the puberty allows us to draw an analogy between the processes of ovulation and the inflammatory response. The cytokines produced by certain immune cells are signaling molecules that affect cell proliferation and apoptosis of ovarian cells, folliculogenesis, hormone secretion and thus play an important role in ovulation. Therefore, the immune system may be an additional local regulator of ovarian function. Estradiol and progesterone are of great importance in the secretory transformation of the endometrium, especially during the «implantation window». ObjectiveTo evaluate the efficacy of a psychoeducational manual as first intervention when treating BED.MethodIt was conducted a 8-week controlled trial (before-after) without a control group. Twenty BED patients were enrolled at an outpatient service specialized in eating disorders (PROATA).After being submitted to a psychiatric evaluation and responding to the following instruments: MINI-PLUS, QEWP-R, BES, BDI, HAM-A, SF-36, and BSQ, the subjects signed a consent term and they received explanations about the manual. After eight weeks the patients were re-evaluated by trained examiners.The main outcome criterion was the BED evolution (evaluated through QEWP-R and BES). The secondary criteria were evaluated through the other instruments plus a semi-structured interview.ResultsMost patients presented a slight improvement of BED symptoms, although these symptoms are still present. The secondary criteria show that the most relevant aspect was that the information presented in the manual allowed the patients to acknowledge they had a condition and to see themselves with less guilt and prejudice, making it easier to look for a more comprehensive treatment.ConclusionsThere are indications that using the psychoeducational manual can reduce the symptoms, and that it can be useful in identifying the condition and allowing the patients to look for treatment. Also, since the instrument can be used as part of a stepped-care program, a broader, controlled and randomized study enrolling a larger number of patients must be conducted in order to better clarify the results.

The woman’s health and its integral component – reproductive health – are largely determined by the conditions of her development in the womb, during the neonatal period, childhood, and adolescence. The article provides an analysis of modern research on the relationship of reproductive dysfunction since its inception, where there is a clear tendency to increase the number of patients with puberty pathology. The low level of somatic and reproductive health before pregnancy causes a high frequency of complicated births, leading to a deterioration in the development of subsequent generations. The woman’s reproductive system is the most dynamic biological object, which is extremely sensitive to adverse external and internal factors by the emergence of new adaptive reactions that acquire, under certain conditions, the properties of the pathological process. At the same time, many issues of the pathogenesis of fertility disorders have not been fully studied and remain controversial. Accordingly, not only the development of informative methods for assessing the state of the reproductive system, but also the problem of restoring reproductive health are becoming increasingly important. In such conditions, to improve the indicators of female reproductive health at all stages of its formation, it is necessary to improve and optimize the provision of gynecological and obstetric care. Aim. To determine the possible damaging effects of ecologically unfavorable environmental factors (radiation and chemical) on the health of descendants. Methods. The cross-sectional study included 232 women of the Altai territory, who were divided into three groups: the first group included the descendants of persons who were in the area of radiation exposure, the second one - the descendants of chemical workers, the third group was the control group. As part of the expedition trip to the study areas, the collection of clinical and anamnestic data, the study of extragenital, gynecological morbidity, laboratory and functional methods of research were carried out. Results. The data were obtained confirming the most damaging effect on reproductive health in the descendants of persons who were in the area of radiation exposure. It was the first group where the high frequency of extragenital and gynecological pathology was revealed. At the same time, the reproductive function of women, closely related to functional changes in the immune system, was disordered. The results of the study of a number of parameters of the immune system revealed changes in the functioning of cellular immunity, increase in the level of proinflammatory cytokines (tumor necrosis factor), mainly in the descendants of persons who were in the area of radiation exposure, in contrast to the descendants of workers of chemical production and control group. Conclusion. A comparative assessment of some health parameters of descendants of persons exposed to radiation and chemical factors showed that the most damaging effect on the reproductive health of descendants is caused by radiation factor.

When interacting on a learning task, which is typical of several academic situations, individuals may experience two different motives: Understanding the problem, or showing their competences. When a conflict (confrontation of divergent propositions) emerges from this interaction, it can be solved either in an epistemic way (focused on the task) or in a relational way (focused on the social comparison of competences). The latter is believed to be detrimental for learning. Moreover, research on cooperative learning shows that when they share identical information, partners are led to compare to each other, and are less encouraged to cooperate than when they share complementary information. An epistemic vs. relational conflict vs. no conflict was provoked in dyads composed by a participant and a confederate, working either on identical or on complementary information (N = 122). Results showed that, if relational and epistemic conflicts both entailed more perceived interactions and divergence than the control group, only relational conflict entailed more perceived comparison activities and a less positive relationship than the control group. Epistemic conflict resulted in a more positive perceived relationship than the control group. As far as performance is concerned, relational conflict led to a worse learning than epistemic conflict, and - after a delay - than the control group. An interaction between the two variables on delayed performance showed that epistemic and relational conflicts were different only when working with complementary information. This study shows the importance of the quality of relationship when sharing information during cooperative learning, a crucial factor to be taken into account when planning educational settings at the university.

<p><b>Aim:</b> The goal was to determine the effectiveness of the posterior pericardiotomy technique in preventing the development of early and late pericardial effusions (PEs) and to determine the role of anxiety level for the detection of late pericardial tamponade (PT).</p><p><b>Materials and Methods:</b> We divided 100 patients randomly into 2 groups, the posterior pericardiotomy group (n = 50) and the control group (n = 50). All patients undergoing coronary artery bypass grafting surgery (CABG), valvular heart surgery, or combined valvular and CABG surgeries were included. The posterior pericardiotomy technique was performed in the first group of 50 patients. Evaluations completed preoperatively, postoperatively on day 1, before discharge, and on postoperative days 5 and 30 included electrocardiographic study, chest radiography, echocardiographic study, and evaluation of the patient's anxiety level. Postoperative causes of morbidity and durations of intensive care unit and hospital stays were recorded.</p><p><b>Results:</b> The 2 groups were not significantly different with respect to demographic and operative data (<i>P</i> > .05). Echocardiography evaluations revealed no significant differences between the groups preoperatively; however, before discharge the control group had a significantly higher number of patients with moderate, large, and very large PEs compared with the pericardiotomy group (<i>P</i> < .01). There were 6 cases of late PT in the control group, whereas there were none in the pericardiotomy group (<i>P</i> < .05). Before discharge and on postoperative day 15, the patients in the pericardiotomy group showed significant improvement in anxiety levels (<i>P</i> = .03 and .004, respectively). No differences in postoperative complications were observed between the 2 groups.</p><p><b>Conclusion:</b> Pericardiotomy is a simple, safe, and effective method for reducing the incidence of PE and late PT after cardiac surgery. It also has the potential to provide a better quality of life.</p>

The objective: the developing of a system of preventive measures and principles of pregravid preparation for women with somatoform disorders and violation of autonomic homeostasis (VAH), in which observed changes in reproductive health (CRH) in the shape of states after undergoing artificial abortion, infertility and uterine fibroids. Patients and methods. Conducted clinical and epidemiological studies in the population of women of fertile age (WFA) allowed identifying for further examination of women with CRH in the form of state after undergoing artificial abortion, infertility and uterine fibroids in women with VAH. Further women were examined, in particular the conducted clinical and instrumental methods of research; determined the state of autonomic homeostasis and psycho emotional health of the biotopes of the organism, immunity; analyzed the hormonal regulation of the menstrual cycle; performed genetic studies and determined the morphofunctional state of reproductive system. Results. The most frequent complications during pregnancy in women who had CRH in history in the form of abortions, infertility and uterine fibroids and in the background of the PAF, there was a threat of interruption of pregnancy (often in I and II trimester – 56%) and preterm delivery (21%). The study of the catamnesis of further reproductive health found that in the case of well-conducted therapeutic measures in women undergoing artificial abortion, had infertility and uterine leiomyoma in the background of VAH, restore reproductive function, and in 82% of cases occurred a pregnancy. Conclusion. The the provided study of reproductive health, and state of various organs and systems of fertile aged women with somatoform disorders and violations of the autonomic homeostasis allowed to develop preventive measures for these women and pregravid preparation with the inclusion to the therapy Magnesium and vitamins (Magne-В6®). The machine learning algorithms (artificial intelligence) was used for this purpose, the model has high prognosis accuracy. The aim of the model applying is prioritize diagnostic and preventive measures to minimize reproductive system diseases complications and preserve workers’ health and efficiency.
