Introduction. The mechanisms of generalized periodontitis (GP) development and methods of its treatment remain obscure, so it is important to study changes in the activity of enzymes responsible for maintaining homeostasis, as well as the inclusion of medicines that regulate them into the complex treatment. Aim of research is to study the influence of comprehensive treatment in patients with GP on the dynamics of the activity of indicator blood serum enzymes in different observation periods. Methods. There were examined 29 people with a healthy periodontium and 143 patients with GP aged 19-45 years, somatically healthy, before, immediately after the treatment, after 6 and 12 months. Patients were divided into subgroups with chronic (A) and acute (B) course: IA and IB – the initial degree; IIA and IIB – the I degree; IIIA and IIIB – the II degree. The activity of lactate-dehydrogenase (LDG), arginase and sorbitol dehydrogenase (SDG) in blood serum has been studied. In addition to the basic periodontal therapy, the microalgal medicine Spirulina platensis was prescribed endogenously, and the paste with the same amount of spirulina powder and enterosorbent and 0.05% chlorhexidine bigluconate solution was exogenously prescribed. Results. In patients with IA and IB subgroups, LDG activity has increased in 1.37- and 1.48-times (p1 <0.01; p1 = 0.001). Under the influence of treatment, it has decreased in all patients, especially after 6 months – in 1.33- and 1.50-times (p2<0.001), but after 12 months it has been increased more (p2> 0.05; p2 <0.05). Arginase activity in IA and IB subgroups has reduced in 1.23- and 1.31-times (p1<0.05; p1=0.005). Due to the therapy, it has increased immediately, after 6 and 12 months, respectively in 1.23- and 1.26-times; in 1.21- and 1.25-times, and in 1.20-1.23-times (p2<0.05; p2> 0.05; p1> 0.05). In subgroups IA and IB, SDG activity has increased in 1.15- and 1.17-times (p1> 0.05), and after the treatment it has decreased immediately in 1.14- and 1.16-times (p2 <0.05); later it increased, but differed slightly from the norm (p1> 0.05). LDG activity in subgroups IIA and IIB has increased in 1.38- and 1.54-times (p1 <0.01; p1=0.001). After the treatment in subgroup IIA, it has decreased in 1.21-times immediately and after 6 months (p2<0.005), and a year later it has increased (p2>0.05; p1>0.05); in subgroup IIB it has decreased in 1.33-, 1.39- and 1.24-times (p2<0.05; p2<0.01; p2 <0.05 and p1> 0.05). In subgroups IIA and IIB, arginase activity has reduced in 1.32-times (p1=0.001). Immediately after the treatment in subgroup IIA, it has increased in 1.21-times (p2=0.005), and subsequently decreased (p2> 0.05). In the IIB subgroup, its increasing was 1.31-, 1.27- and 1.25-times (p2 <0.05), and the difference with the norm was insignificant. SDG activity in subgroups IIA and IIB has increased in 1.18- and 1.24-times (p1 <0.05; p1=0.01). After the treatment, it has decreased at all terms in both subgroups similarly: 1.13- and 1.16-times; 1.17- and 1.17-times; 1.12- and 1.10-times (p2 <0.05; p2 <0.05; p2> 0.05) and it differed slightly from normal one. The largest increase in LDG activity was found in subgroups IIIA and IIIB – 1.45- and 1.62-times (p1≤0.001). As a result of therapy immediately, after 6 and 12 months it has decreased in 1.18- and 1.20-times; 1.26- and 1.23-times; 1.13- and 1.15-times (p2 <0.05; p2 <0.05; p2> 0.05; p1> 0.05). In subgroups IIIA and IIIB, arginase activity has reduced in 1.32- and 1.37-times (p1≤0.005). Treatment has increased the indices in group IIIA in 1.22-, 1.22- and 1.18-times (p2<0.05), and in group IIIB it immediately increased in 1.25-times and then decreased (p2 <0.05; p2> 0.05). The activity of SDG in IIIA and IIIB subgroups has increased in 1.31-times (p1=0.001). Under the influence of therapy in subgroup IIIA, it has decreased immediately, after 6 and 12 months in 1.17-, 1.22- and 1.13-times (p2 <0.05; p1> 0.05), and in subgroup IIIB it initially decreased, but after a year it has increased (p2>0.05) and the difference with healthy people became significant. The altered indices of activity of enzymes studied in patients with GP did not exceed the reference values, but showed a violation of the enzyme system, which was regulated by the treatment. Prior to therapy, reliable (p<0.05-0.005) strong correlations were found between these parameters: LDG with SDG (r> 0.71) and arginase with SDG (r> -0.90). After the treatment, they were not found, six months later one correlation has restored, and a year later – both have restored, which indicates the necessity for the maintenance of endogenous therapy after six months. Conclusion. GP is accompanied by significant (p1<0.05-0.001) changes in the enzymes activity in the blood: in LDG and SDG, it is increased, and in arginase – it is reduced. Comprehensive treatment has regulated these disorders, especially immediately and after 6 months (p2<0.05-0.001). The activity of LDG and SDG of the initial and the I degree immediately and after 6 months and arginase at the initial degree after 6 and 12 months became the closest to norm. In the GP of the II degree, the data of healthy people were not achieved, but the difference with them was insignificant (p1> 0.05).

The aim of the study was to decrease or to eliminate upper limb lymphostasis with the use of comprehensive physical antiedematous therapy (CPhAT) in patients after the radical treatment of breast cancer. Material and methods. The work was based on data about 30 patients. There are presented the CPhAT technique, the results of the treatment and ways to evaluate the effectiveness of the treatment. To evaluate the results of the treatment, we used the technique developed by ourselves, consisting in immersing first the healthy upper limb, then the affected upper limb in a vessel with a disinfectant. The degree of edema (lymphostasis) and the evaluation of the effectiveness of treatment were determined according to the difference of the displaced fluid (patent for invention No. 2169358). The method is ultra-precise in comparison with a centimeter tape. Results. The developed comprehensive treatment of the upper limb lymphostasis allows achieving the complete regression of stage I-II edema, and volume decrease of III-IV stage edema in 83% patients. Using the developed complex treatment of edema of the upper limb prevents its recurrence in the next three years in 90% patients.

Relevance. The success of treatment of periodontal diseases directly depends on the patient’s response to the therapy, therefore, the characteristics of the person’s personal characteristics can affect both the effectiveness of treatment and the prevention of relapse of the disease.Purpose. To study the features of the internal picture of the disease in the process of complex treatment of adult patients suffering from chronic generalized periodontitis.Materials and methods. The generally accepted comprehensive treatment of chronic generalized periodontitis in 69 middle-aged and elderly men was carried out taking into account the personal response of patients Solovyov «Psychosensory-anatomical-functional maladaptation syndrome».Results. When patients were discharged from the hospital, there was a difference in the phenomena of maladaptation among the elderly and middle-aged: in middle-aged people, sufficient adaptation to the conditions of existence was determined; in elderly people, due to the existing comorbid pathology, a state of maladaptation was diagnosed, which was caused by the presence of complaints of defects in the dentition.Conclusion. In elderly people, as soon as possible after completion of treatment in a hospital, dental rehabilitation cannot be considered completed, which requires the adoption of organizational measures for their timely provision of dentures.

Background:Interest in highly specialized tissue cytokines contributed to the discovery of new biologically active molecules. Nesfatin-1 (NF) - discovered in 2006 as an anorexigenic factor. NF-1 is believed to be involved in the regulation of energy homeostasis by regulating appetite and water intake. The role of NF-1 in the pathogenesis of inflammatory diseases is poorly understood. Recently, studies have found a relationship between an increased level of NF-1 and inflammatory markers in various pathologies.Objectives:Study of the level of nesfatin-1 in the blood serum of healthy people, determination of the correlation between the level of NF-1 with the severity of clinical symptoms and classic markers of inflammation in patients with RA.Methods:120 persons were examined: 90 patients with RA and 30 healthy people. All patients underwent a complete clinical and laboratory examination. Plasma NF-1 levels were determined using commercial test systems (RaiBiotech, cat # EIA-NESF) according to the manufacturer’s instructions. Patients with various forms of RA were comparable in age to the group of healthy individuals. Statistical processing of clinical examination data was carried out using the “STATISTICA 10.0 for Windows” software package. Quantitative data were processed statistically using the parametric Student’s t-test, qualitative data using the non-parametric chi-square test. The significance of differences between groups was determined using analysis of variance. The results were considered statistically significant at p <0.05.Results:The average level of NF-1 in blood serum in healthy individuals was 31.79 ± 3.21 ng / ml (M ± σ). The level of normal NF-1 values in healthy individuals, defined as M ± 2σ, ranged from 25.3 to 37.83 ng / ml. There was no significant difference in the levels of circulating NF-1 and BMI in healthy individuals and patients with RA (p> 0.05). The inverse relationship of a lower level of NF-1 with an increase in BMI was not significant.Group 1 (66 patients with RA) with increased serum NF-1 levels (> 37.83 ng / ml), and group 2 (44 patients) with normal values (<37.83 ng / ml). A high level of NF-1 was characteristic for patients with high activity according to DAS28, RF seropositive, ACCP-positive, with extra-articular manifestations, who had been ill for 10 years or more. A reliable relationship between the level of NF-1 in the blood serum and laboratory parameters of RA activity - ESR, CRP, was shown, and secondary synovitis was more common. Our data show a direct correlation between the NF-1 level of the pro-inflammatory markers of RA.Conclusion:The positive correlation between the level of NF-1 and classical markers of inflammation, such as CRP and ESR, confirms the involvement of NF-1 in the pathophysiology of inflammation in RA. This is also evidenced by the correlation of a high level of NF-1 in the blood serum with a more severe clinical picture of RA. It is known that NF-1 can promote the release of pro-inflammatory cytokines such as interleukin-8 (IL-8), interleukin-6 (IL-6), and macrophage inflammatory protein-1a (MIP-1a) in the chondrocytes of RA patients.It is necessary to further study the role of NF-1 in the pathogenesis of systemic inflammatory reactions and the possibility of targeting pro-inflammatory cytokines, the possibility of regulating the level of NF-1 by drugs.References:[1]Kvlividze T.Z., Zavodovsky B.V., Akhverdyan Yu.R. Aims: To investigate the differences in disrupted rhythms between healthy people and patients with functional gastrointestinal disorders (FGIDs) and their associations with mood disorders during the coronavirus disease 2019 (COVID-19) pandemic. Methods: The rhythm scales were composed of subscales 1 and 2 for the assessment of life-work and entertainment rhythms, respectively; Zung’s Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS) were used to assess mood disorders. Results: A total of 671 patients with FGIDs and 4373 healthy people successfully participated. The scores of subscales 1 and 2 for patients with FGIDs were significantly higher than those for healthy people ( p < .005). The SAS and SDS scores, their prevalence rates were significantly higher than those for the healthy group (all p < .001). Health status, current occupation, life-work rhythm, SDS, and SAS were independent related factors of FGIDs. The score of life-work-entertainment rhythm was significantly positively correlated with SDS and SAS (both p < .001). Conclusion: Disrupted rhythms in patients with FGIDs under the COVID-19 pandemic were more frequently and significantly positively associated with mood disorders.

Objective.The current study attempted to identify and characterize distinct CP subgroups based on their level of dispositional personality traits. The secondary objective was to compare the difference among the subgroups in mood, coping, and disability.Methods.Individuals with chronic pain were assessed for demographic, psychosocial, and personality measures. A two-step cluster analysis was conducted in order to identify distinct subgroups of patients based on their level of personality traits. Differences in clinical outcomes were compared using the multivariate analysis of variance based on cluster membership.Results.In 229 participants, three clusters were formed. No significant difference was seen among the clusters on patient demographic factors including age, sex, relationship status, duration of pain, and pain intensity. Those with high levels of dispositional personality traits had greater levels of mood impairment compared to the other two groups (p<0.05). Significant difference in disability was seen between the subgroups.Conclusions.The study identified a high risk group of CP individuals whose level of personality traits significantly correlated with impaired mood and coping. Use of pharmacological treatment alone may not be successful in improving clinical outcomes among these individuals. Instead, a more comprehensive treatment involving psychological treatments may be important in managing the personality traits that interfere with recovery.

During modeling of esophageal alkali burns 1 and 2 degrees in immature rats were observed violations of various metabolic systems, melanin administration helped to normalize the biochemical parameters. It was shown that under the esophageal burns there occurred violations of protein (total protein, albumin), nitrogen metabolism (creatinine, urea) metabolism of carbohydrate (glucose) and lipids (cholesterol), changed the activity of enzymes (GOT, GPT, CC), violated fluid and electrolyte balance (K +, Na +, Cl-, Ca2 +, Mg2 +, phosphates), while the introduction of melanin contributed to the normalization of these indicators. The most effective dose were of 0.5 mg/ kg and 1 mg/kg. There were found anti-toxic properties of melanin, which may be a promising drug for the normalization of metabolism in patients with burns of esophagus and prevent its complications.


A comprehensive clinical and laboratory analysis of the use of the phytoremedy «Parodontocide» in the complex treatment of chronic periodontitis was carried out. «Parodontocide» contains sage oil, peppermint oil, sage oil, clove oil, allantoin, phenyl salicylate and it demonstrates a pronounced anti-inflammatory and repairing effect. It was well tolerated by the majority of patients, practically causing no adverse reactions and complications in comparison with other medications. That's precisely why «Parodontocide» can be used in a comprehensive treatment program for mild chronic periodontitis and in patients with intolerance to synthetic antiseptics as a part of monotherapy.
