Effectiveness of sustained natural apophyseal glides in females with cervicogenic headache: A randomized controlled trial

BACKGROUND: Cervicogenic headache (CGH) is a common condition that results in significant disability. To treat this dysfunction, Mulligan described sustained natural apophyseal gliders (SNAGs) as a manual therapy approach. However, only inconclusive short-term evidence exists for treating CGH with SNAGs. OBJECTIVE: The present study aims to investigate the effect of SNAGs in the treatment of CGH. METHODS: Fourty female patients ranging from 20 to 40 years with CGH were randomly assigned to two groups: 20 in a treatment group and 20 in a control group. SNAGs were applied to the treatment group while the control group received placebo treatment. Both groups received their respective treatment for 20 minutes, alternately three times per week, for a total of 12 times in four weeks. The outcome measures were the Neck Disability Index (NDI) and the Visual Analogue Scale (VAS). Participants were assessed at baseline and at the end of each week. The data was analyzed using SPSS version 20. Independent t-testing was used to reveal changes between groups. One-way ANOVA was used to determine changes within groups. The level of significance was P< 0.05. RESULTS: Twenty participants (100%) in the treatment group and 17 (85%) in the control group had a history of headache aggravation with active movements or passive head positioning. There was no significant difference at baseline (p> 0.05), indicating that both groups were homogeneous at the time of recruitment. The p value (p< 0.05) showed a significant difference in pain and level of disability at three and four weeks (p< 0.05) in patients treated with SNAGs. However, the cervical range of motion (ROM) showed a statistically significant improvement in flexion and extension in the treatment group (p< 0.05) while there was no significant improvement in side flexion and rotation ROM in both groups (p> 0.05). CONCLUSION: This study found that SNAGs were effective in reducing pain and neck disability and improved ROM in females with CGH.

Neck pain found to be most common musculoskeletal condition in office workers with 12-month prevalence. Mechanical neck pain can be classified into two categories in clinical studies mechanical neck pain which has patients without any identifiable anatomic cause and leaves patients having neurological dysfunctions cervicogenic headache and inflammatory disorders. Current research work has total of 150 participants out of which there were 100 females school teachers having mean age of 37.8 years(median 38 years) and 50 male school teachers having mean age 40 years (median 39 years). Participants included in the study were professionally active school teachers of primary, secondary and senior secondary. Scapula position and neck disability was examined as per keiblers lateral scapular slide test and Neck disability index. Total 150 teachers were included in the study 80 teachers having mechanical neck pain (study group) and 70 teachers not having mechanical neck pain (control group) statistically significant difference found in scapula position and neck disability. In the present research, it is found that there is statistically significant difference in scapula position protraction and upward rotation at three different levels as well as there are statistically significant changes observed in neck disability.

Background:Rheumatoid arthritis is a common type of autoimmune arthritis characterized by chronic inflammation. Cervical spine is often affected specially in long lasting diseaseObjectives:Evaluate efficacy of Third occipital nerve Radiofrequency under fluoroscopic guidance to treat refractory cervicogenic headache in RA patients.Methods:The current study was revised and approved from the local ethical committee of Faculty of Medicine; Assiut University, then registered in the clinical trials under the number ofNCT03852355. Inclusion criteria included, Patients who fulfilled the American College of Rheumatology (ACR) (2010) criteria for RA and suffering from upper neck pain and/or headache due to bilateral 3rd occipital nerve involvement, excluding other local cervical spine pathologies was confirmed by MRI and previously failed conservative treatment for at least three months prior to enrollment. Sixty adult patients were randomly assigned to one of the two studied groups Group 1 (RF, n = 30), received bilateral Third occipital nerve Radiofrequency under fluoroscopic guidance or Group 2 (control group, n = 30), received oral prednisolone 10 mg/day. The two groups were then followed-up with neck disability index (NDI), nocturnal neck pain VAS score and headache score every two weeks for three months. Sleep disturbance, sleep disability index were reassessed six months post intervention. Post interventional assessment was done by pain physician who were kept blind to the grouping process.Results:Neck disability index (1ry outcome), Nocturnal pain VAS, and severity of headache showed significant differences during the whole post-interventional study period. The patients in RF group demonstrated significant improvement of pain in comparison to baseline value over the whole six months with p-value < 0.001 as regard to the fore-mentioned three parameters. On the other aspect, the control group patients showed significant improvement in comparison to its baseline value after the 2nd, 12th and 24th weeks only as follows: (0.001,0.003, 0.003 for the NDI) (p values of 0.02,0.01, 0.01 for the nocturnal pain VAS), (0.001 0.009, 0.005 for the headache VAS severity.Conclusion:Radiofrequency of 3rd occipital nerve is effective in treatment of refractory cervicogenic headache in RA.Disclosure of Interests: :None declared

Objective: Thyroidectomy is a surgery in which head and neck became extended during operation to facilitate surgical performance. Patients experience neck pain and limitation to the range of motion following surgery. The study’s aim was to assess the effect of neck range of motion exercises on neck disability and pain among patients undergoing thyroidectomy.Methods: A quasi-experimental research design was utilized, the research was carried out at the surgical department in Shebin Emergency Hospital, at Shebin El-Kom District, Menoufia Governorate, Egypt. Sample; 90 adult subjects undergoing thyroidectomy and are welcoming to participate in the current research. Four Instruments for data collection: Instrument (1): Interview questionnaire sheet consists of two parts to assess subjects’ socio-demographic and medical data. Instrument (2): Visual analog pain scale to rate the patients’ level of pain intensity. Instrument (3): Neck disability index to assess neck pain and its effect on patients’ ability to perform daily activities. Instrument (4): Compliance assessment sheet to follow the patient’s adherence with neck range of motion exercise.Results: There was no statistically significant difference found between study and control group socio-demographic characteristics and medical history while there was a statistically significant difference between study and control group concerning neck disability and pain after one week and four weeks post-operative thyroidectomy.Conclusions: Patient compliance with a range of motion exercises significantly improves neck movement in order to be able to carry out daily life activities and reducing pain for the study group in comparison with the control group.Recommendation: Nurses who work in the general surgery department should include a neck range of motion exercises in providing the care and follow up protocol for the patients undergoing thyroidectomy.

Introduction : Upper trapezius myofascial pain syndrom (MPS) is characterized by presence of trigger points in upper trapezius muscle. MPS can cause disability and also has negative economic effect. Purpose : The reserach’s goal is to improve that combination of ultrasound and dry needling was better than combination of ultrasound and hold relax to decrease neck disability index. Methods : This research used experimental methods to study with pre-test and post-test control group design. Number of samples of the experimental group was 17 subjects given three times treatment of combination of ultrasound and dry needling, while in the control group were 17 subjects given three time treatment of combination of ultrasound and hold relax. NDI was used as out come measure. Shaphiro-wilk test was used to test the normality and levene’s test was used to test the homogenity. wilcoxon signed ranked test was used for hipothesis I, t-test related was used for hipothesis II and t-test independent wa used for hipothesis III. Results : The research showed that: (1) There was significant decrease of NDI in the experimental group . values of mean for pre test were 42,04 + 7,33 % and post test were 10,18 + 3,78 %. with p value = 0.000 (p <0.05) (2) there was significant decrease of NDI in the control group. values of mean for pre test were 45,29 + 6,03 % and post test were 22,24 + 5,42 %, with p value = 0.000 (p <0.05) (3) There were significant differences between experimental group and control group comparing with the differences of post test values between the group. The mean of post test values in experimental group showed 10,18 + 3,78 % meanwhile 22,24 + 5,42% in control group, with p value = 0.000 (p <0.05). Conclution : It was concluded that combination of ultrasound and dry needling and combination of ultrasound and hold relax can decrease the neck disability index. Combination of ultrasound and dry needling was better than combination of ultrasound and hold relax to decrease neck disability index in subject with upper trapezius myofascial pain syndrom.

Objective. To ascertain the efficacy and safety of Ganji Formulation (GF) for patients with Hepatocellular carcinoma (HCC) who had undergone surgery. Materials and Methods. A total of 262 HCC patients who had undergone liver resection, local ablation, or transcatheter arterial chemoembolization (TACE) were divided randomly into the treatment group and control group. The former was treated with GF and the later with placebo, both for 6 months. The primary endpoint was overall survival (OS). Second endpoints were disease-free survival (DFS) or time to disease progression (TTP). Results. OS of the treatment group was significantly longer than that of the control group (P < 0.05). Subgroup analysis showed that, for patients who received TACE, the TTP was significantly longer in the treatment group than in the control group (P < 0.05). However, for patients who underwent liver resection or local ablation, there was no significant difference in DFS between the two groups (P > 0.05). Conclusion. GF could improve postoperative cumulative survival and prolong the TTP. This clinical trial number is registered with ChiCTR-IOR-15007349.

<p>Smoking habits are common in schizophrenic patients. Nicotine can suppress negative symptoms and cognitive impairments. The aim of this study was to determine the efficacy of bupropion on cognitive function in schizophrenic patients.<strong> </strong>This study is a double blind randomized controlled trial in a large referral psychiatric university hospital in Iran. Ninety smoker schizophrenic patients were randomly allocated (based on DSM -IV TR criteria) in two groups (46 patients for case group and 44 patients in control group). They get risperidone up to 6 mg/d and bupropion up to 400 mg/d .clinical assessment (Positive and Negative Syndrome Scale (PANSS), Brief psychiatric rating scale (BPRS) were taken in beginning of study, 14<sup>th</sup> and 28<sup>th</sup> days of study. Cognitive assessment (Stroop, Digit Span, and Wechsler, Wisconsin) were taken in begging of study, the days 2<sup>nd</sup>, 7<sup>th</sup>, 14<sup>th</sup>, 28<sup>th</sup>. All data were analyzed by SPSS Ver. 17 with analytic and descriptive tests. Mean age of patients was 37.66±1.01. Mean duration of disorder was 11.63±.98 years. The scores were significantly lower at the day 28<sup>th</sup> compared to the beginning of the study in both groups in Wechsler, Stroop color word , Stroop word , Stroop color , BPRS, PANSS p value ≤0.05 .The difference between the two treatments was not significant as indicated by the effect of group, the between-subjects factor<strong> </strong><strong>p </strong>value ≥0.05. In this study, the side effects were examined and there was no significant difference between the two groups p value ≥0.05.<strong> </strong>Augmentation of bupropion to routine treatment improves cognitive symptoms of schizophrenia in abstinence of tobacco.</p>

The research purpose was to analysis effect of giving mahkota dewa fruits (Phaleria macrocarpa) extract to epithelialization in incision wound of white rats (Rattus norvegicus). The method was randomized posted-only control group design. There were 30 male rats (Rattus norvegicus) grouped on control and treatment group. Control group divided into three groups (KK1, KK2, KK3) and also treatment group divided into three groups (KP1, KP2, KP3). Control group just given CMC 1% peroral without mahkota dewa fruits extract, the treatment group given mahkota dewa fruits extract 22.5 mg/kg body weight. The data was analyzed by Kruskall Wallis. Based on Kruskall Wallis test, obtained result that there was a significant difference (p<0.05) epithelialization variable with p value p = 0.000 between control and treatment group. And based on One-way Anova test, obtained result that there was a significant difference (p<0.05) with p value p =0.000 between control and treatment group. Abstract Background: Considering the increase in drug resistance over time to Helicobacter pylori treatment relying on the anti-inflammatory and antibacterial effects of atorvastatin to increase the success rate of H. pylori eradication, we examined the effect of adding atorvastatin to standard treatment of H. pylori eradication.Results: A total of 186 symptomatic patients who had been diagnosed with Helicobacter pylori infection and tested for H. pylori eradication were examined by a pathological response or positive urea breath test. Patients who received atorvastatin in addition to standard treatment were also identified based on a table of random numbers. Standard treatment included a 240mg bismuth subcitrate tablet, a 40mg pantoprazole tablet, a 500mg metronidazole tablet, and 2 capsules of 500mg amoxicillin, all taken BID for 14 days. After 4 weeks of treatment, all patients underwent stool testing for H. pylori fecal antigen. If the test was positive, the request was considered a failure of treatment, and if the test was negative, it was considered a successful eradication of H. pylori. The clinical trial registration code for this study is IRCT20190823044589N1. The eradication rate of H. pylori was 80% in the control group and 80.9% in the intervention group, which did not show a statistically significant difference between the two groups (P-value=0.971).Conclusion: Adding atorvastatin to 4-drug regimen of PPI, bismuth subcitrate, amoxicillin, and metronidazole as the first line of treatment for H. pylori eradication is ineffective.Trial registration: IRCT, IRCT20190823044589N1. Background: Temporomandibular disorder (TMD) is a set of disorders involving the masticatory muscles, the temporomandibular joint (TMJ) and/or the associated structures. Objectives: To evaluate the occurrence of cervical and scapular instability in subjects with TMD. Methods: A total of 22 patients participated in the study, being 11 of them with TMD, selected using the RDC/TMD criteria, and 11 in the control group. The stabilization capacity of the neck muscles was evaluated through StabilizerTM and the muscles of the shoulder girdle through specific tests. Cervical mobility data from both groups were provided using the accelerometer while for cervical disability was used the Neck Disability Index (NDI) questionnaire. Results: Cervical instability was higher in the TMD group (20.36 ± 3.2) than in the control group (28.54 ± 0.8), revealing significant difference (p= 0.03). The highest percentages of scapular stabilization tests were found in subjects with TMD, (n= 9; 81.81%) when compared with control subjects (n= 5; 45.45%). The NDI results showed that the TMD group presented mild cervical incapacity (11.18 ± 2) and the control presented no disability (2.27 ± 0.4; p= 0.001). Conclusion: Cervical disability, and cervical and scapular instability were more frequent in subjects with TMD.

Introduction: Every woman who undergo Sectio Caesaria (SC) surgery for delivery their babies often complain about pain after the surgery and it become very distrubing. The effect of the pain cause mobilisation limitation, interrupting bonding attachment between mother and child, and postponning breastfeeding inititation. This study conduct EFT or tapping as the pain management in women post SC surgery.The aim of this study is to identify the effect of EFT or tapping for reducing pain after SC surgery. Methods: Quasy experimental with pre-post non randomized control group design was used in this study. The data been colleted using Numeric Rating Scale (NRS) to measure the pain in participants before and after EFT or tapping treatment. The number of participants was 30 mothers with 15 women in control and treatment group respectively. The t-test was used to analize the effect of the EFT. Results: The mean of pain scale before treatment in control group was 5.20 dan 6.20 for the treatment group. The pain scale after treatment in control group was 5.00 and 4.27 in treatment group. There was a significant difference between the reducing pain scale in treatment group with p value = 0,000 (α = 0.05). Discussion: EFT or tapping can be one of many non farmacologic treatment as complementer therapy to reduce pain after SC surgery. Keywords: post SC pain, EFT, pain scale decline, non farmacologic therapy
