Abstract Objective: To analyze th e differences of maternal serum soluble endoglin level in pr eeclampsia, molar pregnan cy, and normal pregnancy, and to analyze th e correlation between maternal serum soluble endoglin level with gestational age. Method: This is a cros s-sectional compara tive analytic study involving 18 preeclamps ia cases, 18 molar pr egnan cies, and 18 normal pregnancies. The sample were obtained from Dr. Hasan Sad ikin hospital and six satellite hospitals from January until Mar ch 2013. The comparison of mean seng se rum level of th e preeclampsia,molar pregnancy, and normal pregnancy gro up was calculated using Kruskal Wallis, and the corr elation we re calculated using Rank Spearman. Result: The mean level of seng se rum in pr eeclamp sia gro up was higher (168.79 ngjml) than in molar pr egnan cy (43.47 ngjml) and normal pregnancy (32.38 ngjml). Ther e is no significant difference of se rum seng level be tween molar and norm al pregnan cy, with p value of 0.393 (p>0.05). There is significant differ ences of seng serum level between pr eeclampsia gro up and molar pr egnancy (p=O.OOO), but no significant differ ences between molar and normal pregnancy, p value=0.393 (p >0.05). Ther e is positive correlati on between seng se rum level of norm al pregnancy with gestational age (rs=0 .647 ; p<0.001). Conclusion: Mat ern al se rum seng level in preeclampsia is higher than th e level of which in molar pre gnan cy and norm al pregnancy. Asthma is a chronical disease of the airways characterized by reversible obstruction of the bronchi and airway inflammation. In recent decades, the scientific interest of the vitamin D system and its role in development of asthma and other alergic diseases has been increased. Aims of this study are to mesure and compare the serum level of 25 OHD in asthmatic and healthy children and corelate the level of 25OHD and total IgE in asthmatic children. This prospective study includes 70 children at age 2 to 14, of which 32 are children with diagnosed asthma and 38 healthy children. In both  of the groups the serum level of 25 OHD was measured  and by the results 18 % of the healthy children (C) and 28% of the asthma children (E) had 25OHD  deficiency, 45%  of C and 50% of E were insufficient and 37 % of C / 22% of E were with normal 25 OHD serum level. The mean level of 25OHD in C was 27,83 +/- 10,24 and in E 20,9 ng/ml +/- 10,72. The mean levels in both groups had statistic significant difference with p-value < 0,05. According to age no statistic significant difference was found in both of the groups. There was a statisticaly significant decreased serum level of 25 OHD in asthmatic females.In the examined group (children with asthma) there was a negative linear correlation (association) of the level of 25OHD and total IgE serum level with r=- 0,55  Vitamin D serum level measurements in asthma patients gives the possibility for discovering the connection between its deficiency and development of asthma symptoms.

Objective: To compare the mean hemoglobin levels and frequency of polycythemia in full term neonates after early and delayed cord clamping. Study Design: Randomized Controlled Trial. Setting: KRL General Hospital Islamabad (Labor Room/ Neonatology). Periods: December 2017 to June 2018. Material & Methods: 190 full term neonates were selected and divided into 2 equal groups randomly: Early cord clamping group after delivery and late cord clamping group. Two hours after clamping the venous blood samples were taken for the hemoglobin and hematocrit levels. Mean and standard deviation were calculated for gestational age, birth weight, hemoglobin and hematocrit. Frequency and proportions were calculated for gender and polycythemia. Results: Mean gestational age of the mothers was 39.27 ± 1.50 weeks. Of 190 neonates, 91 (47.9%) were males, 99 (52.1%) were females. Mean birth weight was 3.64 ± 0.72 kg while mean Hb and HCT levels were 16.07 ± 2.30 g/dl and 63.26 ± 5.32% respectively. Keeping cut off value of 13.5 g/dl of Hb to label anemia or no, 35 (18.4%) neonates were anemic in this study. The polycythemia (HCT >65%) was present in 72 (37.9%) of neonates. There was no difference between groups in terms of gender, anemia, gestational age and birth weight (p values 0.663, 0.852, 0.700 and 0.491 respectively). The distribution of polycythemia was different among groups (p value 0.007). The mean hemoglobin level in group A was 15.52 ± 1.90 g/dl while in group B it was 16.62 ± 2.53 g/dl (p value 0.001). Mean Hb levels were statistically not different among some of the groups (gestational age <40 weeks, birth weight <4 kg) while HCT levels are significantly different among male group and category of birth weight >4 kg. Rest of the stratification groups showed significant difference. Conclusion: The delayed cord clamping in neonates results in increased mean hemoglobin and hematocrit levels with increased frequency of polycythemia as compared to early cord clamping.

Objective: To understand the correlation between cortisol level and spontaneous abortion in normal pregnancy with gestational age less than 20 weeks. Method: The study design was a correlative analytic prospective study with cross sectional observational approach. This study started from June 2012 until the number of participant sufficiently from Prof. Dr. R.D. Kandou General Hospital. The data then processed SPSS version 2.0, ROC and logistic regression equation model (chance model). Result: The study included 50 cases consisted of 25 spontaneous abortion cases and 25 normal pregnancies with gestational age less than 20 weeks, and got result if cortisol level for 18.52 μg/dl so spontaneously abortion chance for 35.7% and if cortisol level for 24.0 μg/dl would chance for spontaneous abortion for 99.3%. Conclusion: In this study, there was a significant difference in cortisol level in women with spontaneous abortion and < 20 weeks normal pregnancy. Higher cortisol level would increase spontaneous abortion risk. The mean cortisol level in spontaneous abortion was 27.28892 μg/dl. While the mean cortisol level in women with normal pregnancy was 11.7660 μg/dl. Analysis of the correlation between cortisol level and spontaneous abortion gave a cut off point for cortisol level of 19.1 μg/dl with sensitivity of 92% and specificity of 92%. Background: Assessment of accurate gestational age of fetus is very imperative for proper obstetric management of normal as well as IUGR pregnancies. The objective of this study was to evaluate the accuracy of transverse cerebellar diameter as against the conventional parameters of Biparietal diameter, head circumference, abdominal circumference and femur length in normal pregnant mothers between 15 to 40 weeks and in antenatal diagnosis of intrauterine growth retardation.Methods: Study done on 100 women attending antenatal department, 80 were normal pregnancy cases and 20 were clinically suspected IUGR cases. In each patient BPD, HC, AC, FL and TCD was measured. Correlation of TCD was done with other measured parameters as well as with estimated gestational age of fetus.Results: There was no statistically significant difference between the age and parity distributions of two groups It was seen that 18 cases were below the fifth percentile for AC and HC, 19 cases were below the fifth percentile for BPD and 16 cases were below the fifth percentile for FL. In 16 out of 20 cases the TCD values were within the normal range. Only in 4 cases the TCD values were below the 5th percentile. This difference in number of cases below the 5th percentile for BPD, HC, AC, FL and TCD was statistically significant (p-value=0.001). In 20 IUGR cases the gestational age predicted by transverse cerebellar diameter measurements closely correlated with gestational age predicted by last menstrual period.Conclusions: It was found that there is good correlation between TCD and other parameters. Also, TCD shows good correlation with gestational age. It is better than BPD in circumstances like excessive moulding and dolichocephaly. Also, TCD measurement is not fraught with problems as encountered in FL measurement.

AbstrakPreeklampsia-eklampsia sampai saat ini masih merupakan disease of theory. Kelainan yang terjadi pada penyakit ini adalah disfungsi endotel yang dapat disebabkan oleh perubahan kadar profil lipid. Tujuan penelitian ini adalah mengetahui perbedaan rerata kadar profil lipid pada preeklampsia dengan kehamilan normal pada etnik Minangkabau. Penelitian observasional dengan rancangan cross sectional ini dilakukan di RSUP dr. M. Djamil Padang, RS Reksodiwiryo Padang dan UPTD Laboratorium Kesehatan Daerah Sumatera Barat. Subjek terdiri dari 2 kelompok yang diambil secara consecutive sampling dengan jumlah masing-masing 27 orang. Pemeriksaan profil lipid dilakukan dengan metode kalorimetri. Analisis data dilakukan secara uji statistik independent t-test. Hasil penelitian diperoleh rerata kadar total kolesterol pada kelompok preeklampsia dan kehamilan normal adalah 270,19+68,955 mg/dL dan 247,56+44,415 mg/dL dengan nilai p=0,158, rerata kadar trigliserida pada kelompok preeklampsia dan kehamilan normal adalah 296,07±157,993 mg/dL dan 272,89±84,608 mg/dL dengan nilai p=0,504, rerata kadar HDL pada kelompok preeklampsia dan kehamilan normal adalah 51,93±19,882 mg/dL dan 63,33±11,222 mg/dL dengan nilai p=0,012, rerata kadar LDL pada kelompok preeklampsia dan kehamilan normal adalah 159±52,038 mg/dL dan 129,67±38,692 mg/dL dengan nilai p=0,023, rasio perbandingan LDL/HDL pada kelompok preeklampsia dan kehamilan normal adalah 3,86 dan 2,04. Dari hasil penelitian dapat disimpulkan pada kedua kelompok tidak ada perbedaan kadar kolesterol total dan trigliserida, tetapi ada perbedaan kadar HDL dan LDL , rasio perbandingan LDL/HDL lebih tinggi pada preeklampsia daripada kehamilan normal.Kata kunci: preeklampsia, total kolesterol, trigliserida, HDL, LDL, etnik Minangkabau AbstractPreeclampsia-eklampsia still being a disease of theory. The mean levels of triglyceride in preeclampsia and normal pregnancy were 296.07±157.993 and 272.89±44.415 mg/dL with p-value 0.504. The mean level of HDL in preeclampsia and normal pregnancy were 51.93±19.882 mg/dL and 63.33 mg/dL p-value 0.012. The mean level of LDL in two groups were 159±52.038 mg/dL and 129.67±38.692 mg/dL with p-value 0.023 and the mean levels of LDL/HDL ratio were 3.86±3,09 mg/dLin  preeclampsia and 2,08±0,64 in normal pregnancy. It can be concluded that there is no significant difference in total cholesterol and triglyseride between preeclampsia and normal in HDL and LDL and there is significant difference in LDL/HDL ratio between preeclampsia and normal pregnancy at Minangkabau Ethnic.Keywords: preeclampsia, total cholesterol, triglyceride, HDL, LDL, Minangkabau Ethnic

This study aims to investigate whether the use of six selected short stories throughout the duration of a 14-week course could enhance students’ reading comprehension achievement at the end of the semester. Out of the six short stories read, three were chosen as in-class assignments known as ‘Personal Reading Logs’ (hereafter, PRLs). One group of semester two Diploma students taking a reading skills course was selected through a convenience sampling method. A pre-test was conducted by having the students answer a past semester reading quiz of which the results would then be compared to their post-test (final reading exam) results. A paired samples t-test revealed no significant difference in the reading scores of the pre-test and the post-test, t (17) = -.265, p > .05. Since the p-value was bigger than 0.05, this indicated that the mean reading score of the post-test (M = 50.556) was not significantly higher than the mean reading score of the pre-test (M = 49.722). Therefore, the null hypothesis which stated that there was no difference in the mean score of the pre-test and post-test was retained. Overall, the result refuted the findings of other studies promoting the effectiveness of using short stories to enhance L2 reading comprehension achievement.

Abstract Background Chronic rhinosinusitis (CRS) is a common inflammation of the nose and the paranasal sinuses. Intractable CRS cases are generally treated with endoscopic sinus surgery (ESS). Although the effect of ESS on CRS symptoms has been studied, the pattern of symptom improvement after ESS for CRS is yet to be investigated. The aim of this study was to determine the magnitude and sequence of symptom improvement after ESS for CRS, and to assess the possible preoperative factors that predict surgical outcomes in CRS patients. Methods This was a longitudinal prospective study of 68 patients who had CRS (with or without nasal polyps). The patients underwent ESS at King Fahd Hospital of the University, Al Khobar, Saudi Arabia. The Sino-nasal Outcome Test-22 (SNOT-22) questionnaire was used for assessment at four time points during the study: pre-ESS, 1-week post-ESS, 4 weeks post-ESS, and 6 months post-ESS. Results The difference between the mean scores recorded for the five SNOT-22 domains pre-ESS and 6 months post-ESS were as follows: rhinologic symptoms (t-test = 7.22, p-value =  < 0.001); extra-nasal rhinologic symptoms (t-test = 4.87, p-value =  < 0.001); ear/facial symptoms (t-test = 6.34, p-value =  < 0.001); psychological dysfunction (t-test = 1.99, p-value = 0.049); and sleep dysfunction (t-test = 5.58, p-value =  < 0.001). There was a significant difference between the mean scores recorded for the five domains pre-ESS and 6 months post-ESS. Rhinologic symptoms had the largest effect size (d = 1.12), whereas psychological dysfunction had the least effect size (d = 0.24). The only statistically significant difference in the SNOT-22 mean scores recorded 4 weeks post-ESS was observed between allergic and non-allergic patients (t = − 2.16, df = 66, p = 0.035). Conclusion Understanding the pattern of symptom improvement following ESS for CRS will facilitate patient counselling and aid the optimization of the current treatment protocols to maximize surgical outcomes and quality of life. The aim of this study was to evaluate the effect of implant surface treatment with carboxyethylphosphonic acid and fibroblast growth factor 2 on the bone–implant interface during the osseointegration period in vivo using an animal model. The present research was carried out in six minipigs, in whose left tibia implants were inserted as follows: eight implants with a standard surface treatment, for the control group, and eight implants with a surface treatment of carboxyethylphosphonic acid and immobilization of FGF-2, for the test group. At 4 weeks after the insertion of the implants, the animals were sacrificed for the histomorphometric analysis of the samples. The means of the results for the implant–bone contact variable (BIC) were 46.39 ± 17.49% for the test group and 34.00 ± 9.92% for the control group; the difference was not statistically significant. For the corrected implant–bone contact variable (BICc), the mean value of the test group was 60.48 ± 18.11%, and that for the control group, 43.08 ± 10.77%; the difference was statistically significant (p-value = 0.035). The new bone formation (BV/TV) showed average results of 27.28 ± 3.88% for the test group and 26.63 ± 7.90% for the control group, meaning that the differences were not statistically significant (p-value = 0.839). Regarding the bone density at the interthread level (BAI/TA), the mean value of the test group was 32.27 ± 6.70%, and that of the control group was 32.91 ± 7.76%, with a p-value of 0.863, while for the peri-implant density (BAP/TA), the mean value of the test group was 44.96 ± 7.55%, and that for the control group was 44.80 ± 8.68%, without a significant difference between the groups. The current research only found a significant difference for the bone–implant contact at the cortical level; therefore, it could be considered that FGF-2 acts on the mineralization of bone tissue. The application of carboxyethylphosphonic acid on the surface of implants can be considered a promising alternative as a biomimetic coating for the immobilization of FGF-2. Despite no differences in the new bone formation around the implants or in the interthread or peri-implant bone density being detected, the biofunctionalization of the implant surface with FGF-2 accelerates the mineralization of the bone–implant interface at the cortical level, thereby reducing the osseointegration period.

Context: An excess of the soluble receptor, fms-like tyrosine kinase 1 (sFlt-1) may contribute to maternal vascular dysfunction in women with preeclampsia by binding and thereby reducing concentrations of free vascular endothelial growth factor and placental growth factor (PlGF) in the circulation. The putative stimulus for increased sFlt-1 during preeclampsia, placental hypoxia due to poor perfusion, is common to both preeclampsia and idiopathic intrauterine growth restriction. However, the latter condition occurs without maternal vascular disease. Objective: We asked whether, as with preeclampsia, sFlt-1 is increased and free PlGF is decreased in villous placenta and maternal serum of normotensive women with small-for-gestational-age (SGA) neonates. Study Design: This was a case-control study using banked samples. Groups of women with SGA neonates (birth weight centile &lt; 10th) and women with preeclampsia were matched to separate sets of normal pregnancy controls based on gestational age at blood sampling (serum) or gestational age at delivery (placenta). Results: sFlt-1 levels were higher in preeclamptics than controls (serum, P &lt; 0.0001; placental protein, P = 0.03; placental mRNA, P = 0.007) but not increased in SGA pregnancies. PlGF was lower in both preeclampsia (serum, P &lt; 0.0001; placental protein, P = 0.05) and SGA (serum, P = 0.0008; placental protein, P = 0.03) compared with their controls. PlGF in preeclampsia and SGA groups did not differ. Conclusions: These data are consistent with a role for sFlt-1 in the maternal manifestations of preeclampsia. In contrast to preeclampsia, sFlt-1 does not appear to contribute substantially to decreased circulating free PlGF in SGA pregnancies in the absence of a maternal syndrome.

Background & Method: We conducted a double blinded study at Index Medical College Hospital & Research Centre, Indore. The sample size was determined to be minimum of 120 cases as based upon previous years admission due to acute bronchiolitis. Initially, 146 cases were included in the study out of which 23 cases dropped out of the study after giving consent by guardian for participation in the study as they left against medical advice from the hospital. Result: The mean difference of CSS between 0 minutes to 60 minutes of nebulisation between groups in all cases was 0.4 ± 0.6, between 60 minutes and 4 hours was 0.8 ± 0.6, between 4 to 8 hours was 0.7 ± 0.6, between 8-12 hours was 0.6 ± 0.4, between 12-24 hours was 1.6 ± 0.9 and between 24-48 hours was 1.9 ± 0.9.The mean values and resultant p-value of ANOVA of various nebulising agents used for improvement in CSS shows significant association between various nebulising agents used along with improvement in CSS at the end of assessment at 48 hours of treatment. Conclusion: This study was conducted to establish the efficacy of each nebulisation agent (i.e.  adrenaline, 3% hypertonic saline and normal saline) currently used and compare the outcomes as there is not enough evidence amongst Indian population on level of efficacy of each drug in causing improvement in symptoms and signs in various severities of bronchiolitis in early childhood. Comparison of significant improvement in mean difference in CSS at various intervals in all cases compared between groups by post hoc test revealed non-significant difference (p-value 0.700) between 3% hypertonic saline and normal saline.
